BACKGROUND:. Until now modern science cannot explain clearly aboutthe mechanism of acute alcoholic liver injury. Besides such methods as givingup drinking, or symptomatic treatment, there is no specific therapy and remedy. Traditional Chinese Medicine (TCM) has obvious advantages for this: Itcannot only improve the clinical symptoms, but also adjust organism immunefunction, and has very good development future. OBJECTIVE: To study the effects of radix bupleuri and radix scutellariae incompatibility with the ratio of different dosage (2:1, 1:1, 1:2) on acute alcoholicliver injury by regulating the system of endocrine-immunity network in rats. DESIGN: Groups are divided randomly and experiments are made underblank conditions. SETTING: TCM Clinical Laboratory of Pharmaceutical School, Henan University of TCM. MATERIALS: From September to November 2002 in TCM Clinical Laboratory of Pharmaceutical School, Henan University of TCM, the general condi tion of the animals were as follows: 96 healthy Wistar rats were divided into 8groups: blank group, model group, radix bupleuri group, radix scutellariaegroup, xiao-chaihu group, radix bupleuri-radix scutellariae 2:1 group, radix bupleuri-radix scutellariae 1:1 group, radix bupleuri-radix scutellariae 1:2 group. METHODS: ① According to the ratio of 2:1, 1:1, 1:2, radix bupleuri andradix scutellariae were taken respectively (480 g, 240 g; 360 g, 360 g; 240 g, 480 g) to produce the water decocting nedicine. Except the blank group(duplicated by saline of the same volume through consecutive gastric infusion), the other groups are duplicated through consecutive gastric infusionby 56 degrees of Er-Guotou White Spirit (7 mL/kg) two times a day for tendays until the model-making is finished. From the first day, the medicine wasgiven two times a day. The blank group and the model group were given thedistilled water of the same volume. The other groups were given the properwater decocting medicine. The dosage in xiao-chaihu group was 10 g/kg andin the other was all 12 g/kg. ② 16 hours later after the last time when thewhite spirit was given, blood was taken from stomach aorta to produceserum. Liver, spleen, chest gland, adrenal gland were quickly removed. Thecalculating formula of an organ index: An organ index= The weight of an organ (g)/The weight of a body (g) ×100%. The calculating formula of vitamin C content in adrenal gland (mg/g): Content of vitamin C= [(The Avalue of the testing tube-The A value of the blank tube)/ (The A value ofthe standard tube-The A value of the blank tube)] ×density of the standardtube (6 mg/L)×dilution times of the sample before tested/protein content(g/L). ③ Comparison between the groups was shown by using variancesimilar test and single-factor variance analysis.MAIN OUTCOME MEASURES: Index of rats' liver, spleen, chest gland, and adrenal gland (on the right side); the determination of vitamin C content in rats' right adrenal gland.RESULTS: During the experiment, 3 rats in the model group died. Tworats died in radix scutellariae group and radix bupleuri-radix scutellariae group respectively, In each of the other groups only one rat died. The condition of the number of the rats which were used for analyzing results:There were 12 in blank group; 9 in model group; 10 in radix bupleuri group, radix bupleuri-radix scutellariae 1:1 group respectively; 11 in each of the other groups. ① Comparison of index of rats in liver, spleen, chest gland, adrenal gland (right side) between different groups: There was no significant difference between the indexes of rats' liver in different groups.The index of spleen, chest gland, adrenal gland in the model group were all greatly less than in blank group (P ＜ 0.05-0.01). The index of spleen in the radix bupleuri group wcre greatly higher than in the model group, it could make the reduced spleen index return to normal (P ＜ 0.05); The index of chest in radix scutellariae group, radix bupleuri-radix scutellariae 1:1 group, radix bupleuri-radix scutellariae 1:2 group and xiao-chaihu group were greatly higher than in model group (P ＜ 0.05); The index of adrenal gland in radix bupleuri-radix scutellariae 1:2 group and xiao-chaihu group were greatly higher than in model group (P ＜ 0.05). ② Comparison of vitamin C content in rats' right adrenal gland between the groups: Compared with blank group, the vitamin C content of adrenal gland in the model group had no significant difference. While that of radix bupleuri group, xiao-chaihu group, radix bupleuri-radix scutellariae groups were greatly less than that of model group (P ＜ 0.05-0.01), especially radix bupleuri-radix scutellariae 1:1 group had the most significant influence (P ＜ 0.01).CONCLUSION: Interfering functional injuries of liver, spleen and suprarenal gland is one of the mechanisms of acute alcoholic liver injury.Generally speaking, bupleuri-radix scutellariae 1:2 group has the greatest effect on the testing index.

Objective To observe the effect of electroacupuncture(EA) on hippocampal structure in splenic asthenia pedo-rats.Methods A total of 15 SD male rats were randomly assigned to normal control group(n=5),model group(n=5) and EA group(n=5).Splenic asthenic syndrome model was established by intragastric administration of rhubarb and intraperitoneal injection of Reserpine for 14d.EA(1mA,3Hz/15Hz) was applied to bilateral "Zusanli"(ST 36) and "Sanyinjiao"(SP 6) for 20min,once a day for 14 days.The cellular structure of hippocampus was observed by light microscope and transmission electron microscope.Results Optical microscopic observation showed that in normal control group,the cellular nucleus was distinct,and the granular cell layer well-arranged and tight.In model group,the intracellular space was widened,and the granular cell layer was out of order in the arrangement.In EA group,the cellular nucleus and the granular cell layer were nearly normal.Results of the electronic microscope showed that cells in model group had a karyopyknosis with irregular appearance and clear incisure,and some of them presented dissolving and necrotic phenomena;and those in EA group were milder in injury,had nearly-normal nucleus with visible nucleoli and relatively-intact nuclear membrane.Regarding the cellular plasma,in comparison with rich normal organelles of control group,the mitochondria in model group were swelling,with vague,dissolved and broken cristae,while in EA group,majority of the organelles were well-kept,and slightly dissolved mitochondrial cristae found.In regard to the synaptic structure,in comparison with control group,synaptic apomorphosis and swelling mitochondria were found in model group.While in EA group,milder swelling and hydropic degeneration were seen.Different from the distinct pre-and post-synaptic membrane and synaptic vesicles of control group,the synaptic clefts and vesicles of model group were vague in the profile;while those in EA group were nearly-normal.Conclusion Electroacupuncture can effectively relieve splenasthenic syndrome induced pathohistological changes of neurons of the hippocampus in the rat.

Objective:To screen and characterize the common epitopes recognized by monoclonal antibody 3A8,which binds several kinds of Gram negative bacteria,which were recognized by monoclonol antibody 3A8 agaisnt different strains of LPS.Methods:Phage clones binding to 3A8 were screened from C7C phage displayed peptide library,the attached phage clones were eluted by LPS2630(O111:B4).After 3 rounds of panning,the positive phage clones were identified by ELISA,and the amino acid sequences of these positive clones were deduced from DNA sequences.In order to predict how these peptide mimics interaction with 3A8,peptides(A2 and A5) based on conserved sequences of positive clones were synthesized and conjugated with KLH for further study.The binding of A2-KLH and A5-KLH to 3A8 were identified by ELISA.Results:15 of 33 phage clones were identified as positive clones and shown specific binding with 3A8.LPS2630 potently inhibited the binding of phage clone to 3A8.In analysis of the amino acid sequence,there were seven kinds of sequences containing highly hydrophilic residues,and Ser Pro Pro/Pro X Pro was the conserved sequence.The peptides A2 and A5 could bind to 3A8 specially.Conclusion:The conserved sequences containing Ser Pro Pro/Pro X Pro are obtained,which are recognized by mAb 3A8 against broad spectrum Gram negative bacteria and several strain LPS.The synthetic peptides based on the conserved sequence can bind to 3A8,indicating that the peptides can mimic a common epitope of Gram negative bacteria,which be expected as candidate epitope for vaccinization.

Objective: We present our preliminary experience of using a homemade transumbilical single-port access for performing transumbilical single port access (SPA) laparoscopic surgery for gastrointestinal disease and its perioperative nutritional management.Methods: During Nov.2009-Dec.2009,five patients received SPA surgery and nutritional management following the protocol of fast track surgery.A patient with stomach GIST received partial gastrectomy,a patient with ascending colon lymphoma received right hemicolectomy,a patient with rectal cancer received anterior resection,a patient with jejunum GIST received partial small bowel resection,and a patient with early gastric cancer received distal subtotal gastrectomy (D1+α). Results: Transumbilical SPA surgery were successfully done in five patients without conversion.The mean length of incision was 3 cm,the mean time for operation was 138 (60-240)min,and the mean blood loss was 50 (10-100)ml.All patients could mobilize and orally drink on the 1st day after operation,have soft diet the 3st days after operation,their mean postoperative hospital stay was 4 days.No complication occurred.Mean body weight loss was 2.6 kg on discharge compared with at admission.Conclusion: Transumbilical SPA surgery with the protocol of fast-track surgery had advantages including shorter incision,less trauma,faster recovery,and less body weight loss.

Objective To compare the image quality and adverse effects among different concentrations of nonionic iodinated con-trast media in hysterosalpingography (HSG).Methods 99 female infertile patients were recruited in this study for HSG and were averagely divided into Group A,B and C.The application of contrast media were as follows:iomeprol injection(400 mg I/mL)for Group A,iopamidol injection (370mg I/mL)for Group B,iohexol injection(300 mg I/mL)for Group C.The image quality was in-dependently evaluated by two doctors using a 3-point scale (1 -3 score)and adverse effects were recorded.Results (1 )Scores of image quality:all images met diagnostic desire with (2.55±0.5 1)score in Group A,(2.42±0.50)score in Group B,(2.21±0.42) score in Group C.There were statistically significant differences among goups(H =7.790,P =0.022).Kappa values were 0.693 in Group A,0.687 in Group B,0.672 in Group C.(2)Adverse effects:4 cases in Group A(12.12%),3 cases in Group B(9.09%), 2 cases in Group C(6.06%),which showed no statistically significant differences(χ2 = 0.733,P =0.693).Conclusion The three kinds of concentrations of nonionic iodinated contrast media can all be applied in HSG.The higher of iodinated concentration,the better of image contrast.

Objective To compare the results of surgical resection with radiofrequency ablation in the treatment of primary hepatocellular carcinoma with type Ⅰ portal vein tumor thrombus.Methods The clinical data of 30 patients with primary hepatocellular carcinoma with type Ⅰ portal vein tumor thrombus treated between January 2006 and December 2009 were analyzed retrospectively.Patients were divided into two groups according to the treatment.Result The treatments were successfully carried out in the surgical resection group (n=15) and the radiofrequency ablation group (n =15).There was no perioperative death.Both surgical resection and radiofrequency ablation improved the overall survival and recurrence-free survival.The survival outcome of radiofrequency ablation was similar to surgical resection.Conclusion Both surgical resection and radiofrequency ablation were safe and effective for patients with primary hepatocellular carcinoma with type Ⅰ portal vein tumor thrombus and improved the overall survival and recurrence-free survival.

Objective To study the effect of nickel-titanium stent(NTS) and consequent anti-allergy dexamethasone therapy on macrophage cells reactivity to lipopolysaccharide (LPS) from gram-negative bacterium .Methods The macrophage cell line Raw 264 .7 and dexamethasone-pretreated Raw264 .7 were co-cultured with NTS for 4 days ,and stimulated with LPS for 24 hours .The surface marker CD molecules of CD80 ,CD86 and FasL were detected with flowcytometr method ,the supernant cytokine production of proinflammatory cytokines IL-6 and TNF-αwas valued with ELISA method ,and intracellular inflammatory signal pathway acti-vation of NF-κB ,GAS ,ISRE and STAT3 was checked with signal molecule specific promoter lunciferase analysis .Results The stent pre-treatment improved LPS-mediated CD80 expression ,suppressed FasL production ,decreased IL-6 secretion and NF-κB ac-tivation ,the results have statistical significance (P<0 .05) .The dexamethasone treatment improved stent-mediated up-regulated ex-pression of CD80 ,FasL and TNF-α,and suppressed the activation of intracellular inflammatory signal pathway of NF-κB ,ISRE and STAT3 ,the results have statistical significance(P<0 .05) .Conclusion NTS inhibit macrophage cells Raw264 .7 react to TLR4 ag-onist LPS ,and dexamethasone treatment improved the function .

Objective · To investigate the clinical effect of intravitreal injection of Conbercept in the treatment of diabetic macular edema (DME).Methods · Eleven patients (17 eyes) with DME (inflammatory type) received intravitreal injection of Conbercept monthly. After the first treatment, the patients were treated on demand. Follow-up after treatment for more than 6 months, the best corrected visual acuity (BCVA), central retinal thickness (CRT),diminished rate of DME and result of fundusfluorescein angiography(FFA) of DME eyes before and after treatment were compared. Results · During a follow-up of 7-29 months [(12±7) months], the injections were given 1-10 times [(4±3) times]. The results showed that the treatment effects on improving BCVA (logMAR) and diminishing of CRT were significant (t=7.306, P=0.001; t=5.272, P=0.000). The diminished rate of DME of our patients was 76.5%. Conclusion · Intravitreal injection of Conbercept in the treatment of DME is effective on reducing macular edema and improving visual acuity.

This study was aimed to analyze the correlation between CT perfusion parameters, e. g. perfusion (PF), blood volume (BV), peak enhancement image (PEI), time to peak (TTP), and the microvessel density (MVD) of cervical cancer. CT perfusion scans were carried out in 31 patients with cervical cancer. After their surgical resections, we obtained 31 cases of cervical cancer specimens, and 15 cases of adjacent normal cervical specimens as control group, then these 46 specimens were used in MVD detection through immunohistochemical CD34 staining. The MVDs of the tumor group were significantly higher than those in the control group, and the former seems to increase with the clinical stages with a positive correlation. The PF and PEI values have positive correlations with corresponding MVD values. The PF value of squamous carcinoma group has correlation with corresponding MVD value. The PF and PEI values of poorly differentiated group have correlations with corresponding MVD values. The PF, PEI and BV values of II-III stage group are positively correlated with corresponding MVD values. The PF value of lymph metastasis or non-lymph metastasis group both have correlations with corresponding MVD values. We found that CT perfusion parameters were positively correlated with corresponding MVD values in cervical cancer. Our results suggest that CT perfusion is a new method which could reflect tumor angiogenesis, histological malignant grade, invasion, and clinical stage. It will help us determine the tumor staging and prognosis.

Objective To investigate the value of bedside lung ultrasound B-line score in the diagnosis of acute heart failure (AHF). Methods A retrospectively analysis was conducted. The adult patients presenting with acute dyspnea in intensive care unit (ICU) of Affiliated Hospital of Nanjing University of Traditional Chinese Medicine from January 2016 to June 2017 were enrolled. An 8-zone lung ultrasound was performed and plasma B-type natriuretic peptide (BNP) level was tested in all patients. AHF was determined as the final diagnosis by two experienced ICU doctors according to the diagnostic criteria of AHF. Patients were divided into two groups: AHF group and non-AHF group. The levels of BNP and B-line score were compared between the two groups, and the diagnostic value of BNP and B-line score in AHF was evaluated. Results Fifty-six patients were included in this study, with 32 of men and 24 of women,and with an average age of 77.3±8.8. Thirty-six patients were diagnosed as AHF. The level of BNP and lung ultrasound B-line score in AHF group were higher than those in non-AHF group [BNP (ng/L): 1 640.4±1 078.4 vs. 236.9±124.9,B line score: 12.8±5.3 vs. 5.4±1.8, both 1 < 0.01]. There was a strong correlation between elevated BNP levels and an increased B-lines score (R2 = 0.712, 1 = 0.000). The receiver operating characteristic curve (ROC) showed that when the cut-off of lung ultrasound B-line score was 8.5, AHF could be discriminated from dyspnea caused by other diseases (sensitivity was 77.8%, specificity was 95%, positive likelihood ratio was 15.56, negative likelihood ratio was 0.23).The area under the ROC curve (AUC) of lung ultrasound B-line score was 0.917 [95% confidence interval (95%CI) =0.847-0.987, 1 = 0.000], slightly lower than that of plasma BNP [0.979 (95%CI = 0.951-1.008)]. Conclusion Lung ultrasound B-line score was highly specific, but moderately sensitive for identifying patients with AHF.
