1.Relationship of serum estradiol levels with age and lesion severity in women with knee osteoarthritis

BACKGROUND:The pathogenesis of knee osteoarthritis is stil not very clear, and there is a lack of early diagnosis methods. Because it is more common in middle-aged women, it is suggested that the pathogenesis of knee osteoarthritis may be closely related to serum estradiol levels in the human body. OBJECTIVE:To explore the correlation between the level of serum estradiol and the severity of osteoarthritis in female patients with knee osteoarthritis. METHODS:Seventy-six female patients with knee osteoarthritis admitted in the First Affiliated Hospital of Guangxi Medical University from February 2012 to December 2013 were enroled, and divided into premenopause group (n=42) and postmenopause group (n=34). According to Kelgren-Lawrence (K-L) grading, both two groups were subdivided into K-L 0, II and IV groups. The knee function was evaluated with Hospital for Special Surgery (HSS) Knee Score Standard in USA, the serum estradiol levels were detected with electrochemiluminescence immunoassay. Then, the correlation analysis was performed. RESULTS AND CONCLUSION: The serum estradiol levels of the premenopause group and postmenopause group were decreased significantly with the increase of K-L grade and age (P < 0.05). Under the same K-L grade, the serum estradiol levels in the postmenopausal patients were significantly lower than those in the premenopausal patients. Linear regression analysis showed that there was a negative relationship between age and serum estradiol level (R2=0.222,P < 0.05), and there was a positive correlation between HSS scores and serum estradiol levels (R2=0.415,P < 0.05). The severity of knee osteoarthritis in female patients shows a negative correlation with the level of serum estradiol but a positive correlation with age.
2.The effects and safety of closed versus open tracheal suction system: a meta analysis

Objective To evaluate the effects and safety of closed tracheal suction system(CTSS)versus open tracheal suction system (OTSS) for mechanically ventilated patients.Methods All randomized controlled trials (RCTs) comparing CTSS with OTSS for mechanically ventilated patients home and abroad were identified via manual and computer retrieval.All related data were extracted.Meta analysis was conducted using the statistical software RevMan 5.1 on the basis of strict quality evaluation with the methods recommended by the Cochrane Collaboration.Results Fifty-one related papers were found and 12 RCTs involving 1205 patients in CTSS group and 1179 patients in OTSS group were included.The results of meta analysis showed that CTSS was associated with a significant reduction in the duration of mechanical ventilation (WMD =-0.73,95% CI-1.07--0.40,P<0.0001),but the incidence of ventilator associated pneumonia and microbial colonization,mortality and length of ICU stay exhibited no difference between the two groups (P > 0.05).However,compared with OTSS,CTSS reduced the incidence of arrhythmia (RR =0.23,95% CI 0.07-0.74,P =0.01) and minimized the disturbance to heart rate (WMD =-1.97,95% CI-3.03--0.91,P =0.0003),mean arterial pressure (WMD =-2.01,95% CI-3.02--1.01,P < 0.0001) and oxygen saturation (SpO2) (WMD =-1.00,95% CI-1.14--0.86,P < 0.000 01).Conclusions Compared with OTSS,CTSS could reduce disturbance to respiratory and circulatory system by sputum suction and shorten the duration of mechanical ventilation.However,CTSS has no advantage in prevention of ventilator associated pneumonia or microbial colonization,nor does it shorten the length of ICU stay or improve the outcome of mechanically ventilated patients.

Both Chinese GB nutrient agar and American FDA standard method agar are commonly used for counting bacteria in food inspection. A number of our experiments showed that the count of bacteria by FDA standard method agar is 23.9% higher than that by GB nutrient agar. The present C8 medium is an improved medium based on the composition of the two media mentioned above The result of our comparison experiment showed that the C8 medium used for counting bacteria was 35.8% higher than that by GB nutrient agar and 9.5% higher than that by FDA standard method agar respectively.

5.Efficacy of dexmedetomidine versus midazolam for sedation in critically ill patients: a Meta-analysis

Objective To systematically review the efficacy of dexmedetomidine or midazolam for sedation in critically ill patients. Methods We searched the PubMed, EMBaes, Cochrane Library, Wanfang Database,CNKI and VIP for all randomized controlled trials (RCTs) about the efficacy of dexmedetomidine versus midazolam for sedation in severe cases. The quality of the studies was evaluated by the method recommended by Cochrane Collaboration. Meta-analysis was conducted using the Cochrane Collaboration's RevMan 5.0 software. Results Six RCTs involving 613 patients were included in our Meta-analysis. The results of Meta-analysis showed that the length of ICU stay was significantly shorter in group dexmedetomidine than in group midazolam. There were no significant differences in the duration of mechanical ventilation, incidences of bradycardia, hypotension and delirium and mortality rate between the two groups. Conclusion Dexmedetomidin can shorten the length of ICU stay and is beneficial for the outcome in critically ill patients.
6.Effects of 14-3-3σ on UVB-induced radiation damage in HaCaT cells

Objective To explore the role of 14-3-3σ in cell cycle arrest,proliferation inhibition of HaCaT cells after UVB exposure.Methods Crystal violet assay was used to determine the viable density of HaCaT,HaCaTKD cells after being irradiated with UVB of different doses (10,20,30,40,50,60 and 80 mJ/cm2) for 48 h.After HaCaT and HaCaTKD being treated with 30 mJ/cm2irradiation,cell growth and cell cycle distribution were detected by CCK-8 assay and PI staining combined with flow cytometry,respectively.Western blot was used to evaluate the protein expression of 14-3-3σ,Cdc2,Cdc25c and Cyclin B1.Results After 48 h,the survival rate of both HaCaT and HaCaTKD decreased in a dosedependent manner.Especially,HaCaTKD cells had drastically low proliferation rate compared with normal HaCaT at 10 mJ/cm2 (t =8.83-49.63,P < 0.05).The proliferation rate of HaCaTKD cells was significantly lower than that of HaCaT cells (F =32.89,P < 0.05).After treatment with 30 mJ/cm2 UV irradiation,the ability of proliferation in normal HaCaT cells was recovered after 24 h while there was no proliferation in HaCaTKD cells within 72 h after the same treatment.The distribution of cell cycle has little change in HaCaTKD (P > 0.05).UVB treatment led to cell cycle arrest from 6 to 18 h in HaCaT cells(t =7.41,9.22,9.16,P <0.05)while no cell cycle arrest could be observed in the HaCaTKID cell.Western blot detection indicated that the expression of 14-3-3σ in HaCaT cells was upregulated(t =5.42-9.57,P <0.05)while the Cdc25c and Cyclin B1 proteins were downregulated in both HaCaT and HaCaTKD cells (t =3.95-11.21,P <0.05).Specifically,Cdc2 protein decreased in HaCaT cells(t =4.93-5.37,P < 0.05)but there was no change in HaCaT~ cells.Conclusions 14-3-3σ protein affects the proliferation and cell cycle of HaCaT cells after UVB irradiation.14-3-3oσ co-activates the expression of Cdc2,Cdc25cand Cyclin B1 to mediate UVB-induced G2/M arrest in HaCaT cells.
7.Prognostic observation of surgery or endovascular embolization for high grade intracranial aneurysms after interim conservative treatment

Objective To observe the prognostic effects of the patients with intracranial saccular aneurysm (Hunt-Hess grade Ⅳ- Ⅴ)first treated conservatively for 12 hours and then with surgical treatment and endovascular treatment. Methods The clinical data of 32 patients with intracranial saccular aneurysm (grade Ⅳ,n = 24 and gradeⅤ,n = 8)Hunt-Hess grade Ⅳ-Ⅴadmitted from January 2012 to January 2014 were analyzed retrospectively. Sixteen of them were treated conservatively for 12 hours in hospital,and then they were treated with surgery or embolization (postpone surgery group)and 16 underwent emergency surgery or embolization (immediate surgery group). The neurological prognosis of the patients was evaluated at 1,3 and 6 months before and after treatment. Results There was no significant difference in Glasgow outcome scores between the postpone surgery group and immediate surgery group at 1 month after treatment (3. 7 ± 1. 4,3. 8 ± 1. 2;t = 1. 372,P > 0. 05);there was no significant difference in Rankin prognostic scores at 3 months after treatment (3. 7 ± 1. 7,3. 6 ± 1. 5;t = 1. 361,P > 0. 05);But there was significant difference in prognostic scores at 6 months after treatment between the two groups (3. 5 ± 1. 5, 4. 2 ± 1. 8;t = 2. 234,P < 0. 05). Conclusion Patients with high grade cystic aneurysmal subarachnoid hemorrhage can be treated with conservative treatment. After 12 hours,endovascular embolization or surgical treatment may be performed. The prognosis at 6 months is better than emergent direct surgery or embolization.

Objective To study the influence of excisional biopsy on detection rate of sentinel lymph node biopsy through methylene blue in breast cancer.Methods A review of 76 patients with clinical stage T1-2N0M0 breast cancer who had a successful sentinel lymph node biopsy at the Affiliated Nanhai Hospital of Southern Medical University between July 2009 to July 2014 was analyzed.There were 37 patients received sentinel lymph node biopsy directly.And 39 patients received sentinel lymph node biopsy after a excisional biopsy to primary lesion.We compare the difference of detection rate of sentinel lymph node between the two groups.Results Seventy cases had been found sentinel lymph node successfully in a total of 76 patients and the detection rate is 92.1%.There were 34 cases had sentinel lymph node biopsy in successfully in group of excisional biopsy and the detection rate is 87.2%.There were 36 cases had sentinel lymph node biopsy in successfully in group of needle core biopsy and the detection rate is 97.3%.This difference,however,was not statistically significant between the two groups (P > 0.05).Conclusions The influence of excisional biopsy on detection rate of sentinel lymph node biopsy through methylene blue in breast cancer is not significant.
