1.Health survey analysis of workers in athermal power plant

Objective To explore the health of the thermal power plant worker in the noise,under the dust environment.Methods The oecupatiorml health inspection to the workers was carried.Results In 90 workers,41 were unusual,the detection rate is 45.6%.And the loss of hearing 11 people,the detection rate is 14.3%,other unusual 32 people,the detection rate is 35.6%.Conclusion We sheuld strengthen to the noise nuisance control,uses the effective protection of hearing measure,prevents occupational disease.
2.Clinical application and biocompatibility of implants for internal fixation of spinal tuberculosis

OBJECTIVE:To investigate the development of implants for internal fixation of spinal tuberculosis,and to evaluate the clinical effect and biocompatibility. METHODS:Articles were retrieved from CNKI and Medline database from 1998 to 2008 with the of "spinal tuberculosis,thoracic spine,internal fixation,bone fusion,biocompatibility" in both Chinese and English. Among 50 retrieval articles,20 were included in the final evaluation according to inclusion and exclusion criteria. This study was designed to summarize therapy development of spinal tuberculosis and selection of implants for internal fixation,and to investigate clinical development of spinal tuberculosis therapy,types of implantation,biocompatibility of implants,and complications. RESULTS:Spinal tuberculosis treated by debridement bone graft and internal fixation characterized by reconstructing spinal stability,enhancing bone graft fusion ratio,reducing relapse rate,and preventing loss of corrected angle degree,thus it became a tendency for treatment of spinal tuberculosis. Implants which were implanted in focus of spinal tuberculosis were safe because they could not cause infection diffusion or formation of tuberculose focus in the local region. Otherwise,the implants had a good biocompatibility to host. Individualized program was chosen according to lesion degree and site in order to thoroughly clean focus,reconstruct spinal stability,and provide a stable environment for rehabilitation. Implantation of autologous ilium and titanium-net cancellated bone were widely used on clinic,and both of them had an excellent fusion ratio. Artificial bone did not have immunological rejection to organism,and it was gradually degraded and provided sufficient bone source. CONCLUSION:Spinal tuberculosis treated by debridement bone graft and internal fixation is an effective method to treat severe spinal tuberculosis. At present,autologous ilium and titanium-net cancellated bone are frequently used for clinical internal fixation,because both of them have good biocompatibility. Artificial bone characterizes by well clinical effect and biocompatibility,but the long-term effect and safety need to be further studied.
3.Percutaneous biliary drainage for treatment of malignant biliary obstruction

Objective To introduce the technique of percutaneous biliary drainage for treatment of malignant biliary obstruction and to eveluate its therapeutic effect. Methods Percutaneous biliary drainage was performed in 47 patients with malignant biliary obstruction. The biliary obstruction occurred in the common bile duct of 10 patients, in the common hepatic duct for 13, at the conjuction of right and left hepatic ducts in 24. The inner outer draining catheter, the metallic stent or plastic endoprosthesis was placed to drain the bile internally if the obstruction could be passed with guide wire after percutaneous transhepatic cholangiography, or the outer drainage could only be carried out. Results The internal drainage was done with stent in 15 patients and with plastic endoprosthesis in 3, the inner outer drainage in 15, and the external drainage in 14. The serum total bilirubin decreased from 514.1?204.3?mol/L before the procedure to 238.4? 142.8 ?mol/L one week after the procedure( P
4.Normal meniscus and discoid meniscus injuries:Classification and tissue repair under an arthroscopy

BACKGROUND:Meniscus plasty and prosthesis under an arthroscopy are widely used in treating discoid meniscus or normal meniscus injury.The normal shapes of meniscus are maintained by excising medial partial tears and suturing the lateral partial tearsOBJECTIVE:Injury classification and special features of discoid meniscus and normal meniscus were observed,and corresponding trearing methods were used,to discuss the therapeutic effect of repair.METHODS:Totally 260 cases of meniscus injury in knee ioint were selected from the First Aftiliated Hospital of Xinxiang Medical University from September 2003 to June 2008.They were divided into normal meniscus injury group(n=215),and discoid lateral meniscus injury(n=45).People with complete clinical data before operation and following data,confirmed by arthroscopy were selected.Meniscus injury in 260 cases was examined by arthroscopy,and plasty,or partial excision or incomplete resection or full resection was performed according to the injury types and surgical circumstance.RESULTS AND CONCLUSION:All subjects were followed for half a year to 2 years The knee function was evaluated by the Tenger criterion.Four tear types of Discoid meniscus were as follows:73.3% horizontal,15.5% complex,4.4% radial,and 2.2% longitudinal.Five tear types of normal meniscus were as follows:47.9% longitudinal,28.8%radial,1 1.6%horizontal,6 5% transverse and 5.1% complex.In 215 cases of normal meniscus,there were 165 knees with excellentfunction,35 with good,8 with fair and 7 with poor.The excellent and good rate was 93.0%.In 45 cases of discoid meniscus.there were 25 knees with excellent function,16 with good,3 with fair and 1 with poor.The excellent and good rate was 91.1%.Discoid meniscus is different from normal meniscus in anatomical feature and histological structure,therefore,the arthroscopic surgery for discoid meniscus is also different.The most function of meniscus should be remained and the degeneration of knee ioint should be delayed.Meniscus plasty should be performed in all discoid meniscuses except the patients with severe tear of meniscus.
5.Pollution Status of Microcystin in Dianshan Lake and Poyang Lake in China

Objective To investigate the pollution status of algae and microcystin(MC)in Dianshan Lake,Shanghai City and Poyang Lake,Jiangxi Province.Methods Water samples were collected from different sites of the above lakes,as the pretreatment of intracellular and extracellular microcystins was finished,MC-LR concentration was detected by ELISA.Results The proportion of Cyanobacteria in Dianshan Lake was 38.8% and 68.2% respectively,the mean content of MC was 281.78 and 1 145.77 pg/ml respectively.In Poyang Lake,the proportion of Cyanobacteria was 48.9% and 73.7% respectively,the mean content of MC was 80.53 and 405.65 pg/ml respectively.In the same water-blooming lake,MC content was significantly lower in July than that in October(P
6.Clinical report on arthroscopic reconstruction of anterior cruciate ligament with LARS artifical ligament

[Objective]To explore the operating methods and rudimental clinical effects of reconstruction of anterior cruciate ligament(ACL) with the ligament advancement reinforcement system(LARS) artificial ligaments under arthroscope.[Method]Since June of 2006,15 patients(11 males and 4 females,with an average age of 26.8 years)with acute ACL rupture have been treated with the LARS artificial ligaments.The results of preoperative Arto-scan of all patients suggested discontinuation of ACL.The average score of Lysholm on knee joint was 50 points.The operations were completed under arthroscope.Bone tunnels of the femur were located at the central point of arc on the medial surface of ectocondyle of 140 degree.Bone tunnels of tibia were located between the medial and lateral condyle on the tibial plateau、2-4 mm to summit line of fossa intercondylica.The diameter of the bone tunnels was 7.5 mm while that of LARS ligament and hollow crushing screw were 8 mm.[Result]All 15 patients were available for follow-up with an average of 18 months.No postoperative complications such as infection,ligament rupture,ligament loosening occurred.The clinical results were graded as being excellent in 8 patients,good in 5 and fair in 2 according to the Lysholm's classification.The excellent and good rate was 86.6%.[Conclusion]The anatomical reconstruction and the stability of the knee joint can be obtained after ACL reconstruction with LARS artificial ligaments.The operation under arthroscope has advantages of less-injury,rapid recovery and satisfactory curative effect.
7.Role of MSCT,routing X-ray and CT in forensic identification of fractures

Objective To compare MSCT with routing X-ray, CT and appraise their applications to fractures in forensic identification. Methods366 cases which had got routing X-ray or CT examination were examined by thin slice scanning with MSCT and all of the data were processed with MPR, see-through and SSD method. Accurate diagnosis rate and misdiagnosis & underdiagnosis rate were calculated and analyzed by Chi-square test. ResultsThe accurate diagnosis rates of MSCT in nasal fracture, rib fracture, limb or joint fracture, orbital fracture and cranial fracture were higher than that of X-ray or routing CT (P0.05). Objective To investigate the feasibility of osteoarthritis (OA) model in rabbits by injuring medial meniscus so as to understand the role of interleukin-1 beta (IL-1β) and matrix metalloproteinase-13 (MMP-13) in pathological mechanism of articular cartilage and synovial fluid of OA model in rabbits.Methods We randomly divided 40 New Zealand white rabbits into experimental group (n =30) and control group (n =10).Pathological changes in articular cartilage of the femoral condyle were scored at weeks 2,6 and 12 after surgery.We detected the expressions of IL-1β and MMP-13 by immunohistochemistry.The cell fractions of IL-1β and MMP-13 were recorded by ELISA.Results The articular cartilage score and HE staining significantly differed at various time points of gross and pathological observation between control group and experimental group (P< 0.05).Immunohistochemistry showed that IL-1β was expressed in both groups and that the cell fraction differed significantly at weeks 6 and 12 (P<0.05),but not at week 2 between the two groups.MMP-13 protein expression was not detected in articular condrocytes in control group,but was detected in experiment group with a significant difference (P<0.05).IL-1β expression was consistent in articular cartilage and synovial fluid.Conclusion The reasonable rabbit animal OA model could be established by knee meniscus injury caused by surgical method.Expressions of IL-1β and MMP-13 change obviously in the pathomechanism of OA.Further clinical studies are needed to determine whether they can be used as markers in early diagnosis of OA.
9.The relationship study between the changes of the frangible lymphocyte and the degree of destroyed liver function in fulminant viral hepatitis

Objective To study the changes and the significance of the frangible lymphocyte in patients with fulminant viral hepatitis and investigate the relationship between the degree of frangible lymphocyte and the severe of fulminant viral hepatitis. Methods 32 cases confirmed with fulminant viral hepatitis, 66 cases with chronic hepatitis have been studied. 15 cases children from 3 to 5 years old and 46 cases adults have been used with control groups. 100?l lymphocytes (1?106/ml) and 100?l tumor cells(1?105/ml) mixed with 100?l self-serum at 37℃ for 5min to 30min. The fragmental lymphocyte can defined as a frangible lymphocyte. The lymphocyte attached with tumor cell is undestroyed anything can be regarded as negative frangible lymphocyte. Less than 30 percent of lymphocyte attached with tumor cells is destroyed will be conformed as low-grade frangible lymphocyte, between 30 percent to 60 percent regard as mid-grade, and more than 60 percent as severe-grade mode. Results The equiform frangible lymphocyte is detected in fulminant viral hepatitis and other groups like that in SARS in 2003 years. There is none of children have frangible lymphocyte. Only 3 person’s lymphocytes show severe-grade frangible and 9 people display mid-grade in 46 adult individuals. Contrarily There is high of 46.9 percent of cases with fulminant viral hepatitis show severe-grade frangible lymphocytes, only 4 cases is negative and 3 cases is recovered in one months of them. The frangible lymphocytes in fulminant viral hepatitis are significantly lower than those in chronic hepatitis patients not only in mid-grade frangible but in severe type. There is no significant recovery of frangible lymphocyte from acute to recover in fulminant viral hepatitis, but the degree of the lymphocytes are closely relate to the state of an illness. Conclusion The percent of frangible lymphocytes in children is lowest among adult people, chronic hepatitis and fulminant viral hepatitis. The proportion of frangible lymphocytes in fulminant viral hepatitis is highest, and significantly higher than that in chronic hepatitis. The phenomenon of frangible lymphocytes may be an important characteristic of an innate immunology reaction of the body.
10.Isolation and purification of neohesperidin reference substance from Fructus Aurantii

Objective To isolate and purify high-purity neohesperidin from Fructus Aurantii extract.Methods Fructus Aurantii extract was treated by macroporous resin and extracted by n-butanol, then concentrated and some solid was precipitated.After crystallizing the precipitate three times, neohesperidin was obtained.The concentration of n-butanol extract was optimized, which was of great importance in the whole preparation.Results The yield of neohesperidin was 48.6% and purity of neohesperidin was up to 98.7% in Fructus Aurantii extract.Conclusion High-purity neohesperidin could be prepared feasibly and economically by proposed method.
