1.Effect of Shengmai Chenggu Prescription in Preventing Endothelial Cells from Endotoxin -Induced Damage

Objective To observe the effect of Shengmai Chenggu Prescription (SCP) on the damage of endothelial cells (EC) induced by endotoxin. Methods EC obtained from rabbit's aorta were cultured and were treated with endotoxin and serum containing SCP respectively. Histological changes and function of the cultured cells were observed under light microscope and electron microscope and with MTT method . Results Endotoxin could result in the pathologic injury of cultured EC and inhibit the proliferation of the cells. Serum containing SCP could increase the activity of EC and promote its proliferation. Conclusion SCP can protect endothelial cells from damage induced by endotoxin.
2.Histological and enzymohistochemical observation of experimental muscle sprain

Method We used biomechanical methods in controlling strain of anterior tibial muscles of rabbits .Objective To observe the histological and enzymohistochemical change after sprain.Result Experimental model of muscle sprain could be made with yield load which was about 128% of the body weight extracting anterior tibial muscle of the rabbits.Some of the muscle fibers broke near the junction between muscle and the tendon. Conclusion Fibrosis of endomysium and scar formation at the injury might be an important cause of frequent recurrence of muscle sprain.
3.The effects of sodium hyaluronate on matrix metalloproteinase-1, -3 and tissue inhibitor of metalloproteinase-1 mRNA expression in cartilage and synovium of osteoarthritis rabbits

Objective To observe the influence of intra-articular injection of sodium hyaluronate (HA) on mRNA expression of matrix metalloproteinase (MMP) -1, -3 and tissue inhibitor of metalloproteinase (TIMP)-1 in cartilage and synovium of osteoarthritis (OA) . Methods Sixteen white rabbits underwent unilateral anterior cruciate ligament transection and were divided into 2 groups randomly 5 weeks after transection. Experimental group rabbits received 0.3 ml of intra-articular 1% HA injection once a week. Animals in control group were treated under the same condition using saline. At death, 10 weeks following surgery, histological changes of articular cartilage of medial femoral condyle were evaluated by microscopy. The mRNA expression of MMP-1, MMP-3 and TIMP-1 in cartilage and synovium was analyzed using reverse transcription-polymerase chain reaction (RT-PCR) . Results Cartilage degradation in experimental group was significantly less severe than that in the control group. The expression of MMP-3 mRNA in synovium was significantly suppressed in experimental group compared with the control group (0.40?0.10 vs 0.62?0.13). HA treatment had no effect on MMP-3 expression in cartilage. No significant difference of MMP-1 and TIMP-1 expression in cartilage and synovium was found between experimental group and control group. Conclusion HA can alleviate the degeneration of articular cartilage of OA. One possible mechanism of the therapeutic effect of HA may be inhibition of expression of MMP-3 in synovium during early stage of OA.

Objective:To study the interaction between the SARS-S1 protein and the SARS-sensitive cells.Methods:The SARS-S1 expression in the E.coli was purified and the anti-SARS-S1 was produced.After incubate with the SARS-S1 protein,cells was staining by the mouse-anti-SARS-S1 antibody and the anti-mouse antibody labeled by FITC.The result was determined by FACS.Results:After incubate with the SARS-S1 protein,different cell line have a different reaction to the SARS-S1 antibody.Conclusion:The SARS-S1 protein expressed by E.coli has a specific binding ability to the SARS-sensitive cells.
5.Diagnostic and staging value of orbital structures quantitative measurement with 3T magnetic resonance imaging in Graves’ophthalmopathy

Objective To evaluate the diagnostic and staging value of orbital structures quantitative measurement with 3T mag-netic resonance imaging in patients with Graves’ophthalmopathy (GO).Methods Twenty-three GO patients (patient group)and eighteen healthy volunteers (contrast group)were enrolled.Quantitative measurement of orbital structures including exophthalmos values,fatty tissue thickness of the inner side of eye balls,cross-sectional areas of extraocular muscles and signal intensity ratios of extraocular muscles to the ipsilateral temporal muscles (SIR values),and they were compared between two groups.Correlations be-tween the quantitative values and clinical active score (CAS)were accessed.Receiver operating characteristic (ROC)analysis was performed to evaluate the most relevant quantitative parameter and its diagnostic value in discriminating active from inactive GO pa-tients.Results There were significant differences between the GO group and contrast group regarding to all the quantitative parame-ters (P <0.05).Strong correlation was found between SIR values and CAS (r =0.730,P <0.001 ).Significant difference of SIR was found between patients with active GO and inactive GO (P =0.002),and a cut off value of 3.25 might be the critical threshold value,with diagnostic sensitivity of 81.8% and specificity of 83.3% respectively.Conclusion Quantitative measurement of orbital structures with 3T MR imaging could provide assistant in diagnosing and staging of GO.
6.The value of conventional MRI combined with diffusion weighted imaging in the differential diagnosis of orbital lymphoproliferative disorders

Objective To investigate the diagnostic value of conventional MRI combined with DWI in the differential diagnosis of orbital lymphoproliferative disorders (LPD). Methods As a retrospective study, 42 patients were enrolled, including 23 orbital lymphoma and 19 benign orbital LPD confirmed pathologically or clinically. These patients underwent conventional MR, DWI and contrast?enhanced MRI examination of orbit. Qualitative MR imaging features [location, margin, patient ratio of involved quadrants (≥2), signal intensity,“flow void sign”, sinusitis] and quantitative features [ADC and contrast enhanced ratio of the lesion to temporal muscle (CER)] were evaluated. Chi?square test and t test were used for the analysis of qualitative and quantitative features between lymphoma and benign orbital LPD groups, respectively. Multivariate Logistic regression analysis was employed to identify the significant variable for predicting malignant LPD. ROC analysis was used to evaluate the ability of the diagnostic models established based on identified variables. Results “Flow void sign”(9 lymphomas and 17 benign LPDs) and sinusitis (5 lymphomas and 13 benign LPDs) were statistically significant (P<0.05). The MR imaging features [location, margin, patient ratio of involved quadrants (≥2), and signal intensity] had no significant difference between the two groups (P>0.05). Both ADC [(0.79±0.09)×10?3 mm2/s of lymphoma, (1.29±0.35)×10?3 mm2/s of benign LPD] and CER (1.43±0.17 of lymphoma, 1.79±0.31 of benign LPD) between the two groups had significant difference (t=-6.630, -8.257;P<0.01). Multivariate logistic regression analysis showed that ADC value, CER and“flow void sign”were significant variables for predicting malignant orbital LPD (P<0.05). ROC curve showed that ADC value was the most significant single variable in differentiating orbital LPD [threshold value, 0.74 × 10- 3 mm2/s; areas under ROC curve (AUC), 0.97; sensitivity, 87.0%; specificity, 94.7%]. Combination of ADC and CER could further improve the specificity in differentiating benign from malignant orbital LPD (AUC, 0.90;sensitivity, 78.3%;specificity, 100.0%, accuracy 88.1%). Conclusions ADC, CER and“flow void sign”on T2WI were significant variables for predicting malignant orbital LPD. ADC value was the most significant single variable. Combination of ADC and CER could further improve the specificity in differentiating benign from malignant orbital LPD.
7.Predictive value of preoperative gastric fund volume on postoperative gastroparesis

Objective:To study the relationship between gastric fundus size and postoperative gastroparesis and to find effective ways to prevent postoperative gastroparesis in high-risk patients.Methods:We retrospectively reviewed the clinical data of 276 gastric cancer patients undergoing radical gastrectomy from 2015 to 2016. The gastric fundus volume/total gastric volume (FV/TV) ratio was measured by computed tomography (CT) and comparative study between the gastroparesis group and the non-gastroparesis group was carried out in terms of postoperative gastroparesis. Receiver operating characteristic (ROC) curve analysis was used to evaluate the predictive performance. Single-factor and multiple-factor analyses were performed to filter clinically significant predictive factors of gastroparesis. Then, we increased the sample size to 304 patients whose FV/TV ratio was >19.4%. The different surgical methods and perioperative management of these patients were analysed. The chi-square test and logistic regression analysis were performed to identify effective independent factors for preventing gastroparesis.Results:The FV/TV ratio in the gastroparesis group was significantly higher than that in the non-gastroparesis group ( P<0.05). A cut-off value of 19.4% was selected by ROC curve analysis, at which the FV/TV ratio had a sensitivity of 76.2% and a specificity of 53.7%. In 304 patients in the second retrospective study, the incidence of gastroparesis was 9.2%. Gastroparesis was significantly reduced in patients with residual gastric size <1/3 ( P<0.05) and early postoperative gastrointestinal decompression ( P<0.05). Conclusions:The FV/TV ratio can effectively predict the risk of postoperative gastroparesis preoperatively. Small residual stomach and early postoperative gastrointestinal decompression are effective measures to prevent gastroparesis in high-risk patients.
8.Survival analysis of gastric cancer patients during 2005-2010 in Zhejiang Province, China.

OBJECTIVETo conduct a survival analysis of gastric cancer patients according to the data of population-based cancer registry during 2005-2010 in Zhejiang Province in order to provide information for prognosis assessment and control of this disease.
METHODSThe deadline of the last follow-up of 26, 536 patients was December 31st, 2012. Cumulative observed survival rate (OSR) and expected survival rate were calculated by life table and Hakulinen method.
RESULTSthe 1-, 3-, and 5-year OSR were 58.51%, 39.07%, and 33.08%, and the 1-, 3-, and 5-year relative survival rates (RSR) were 60.24%, 42.90%, and 39.03%, respectively. The 1-, 3-, and 5-year relative survival rates of males vs. females were 60.49% vs. 59.65%, 42.88% vs. 42.96%, and 38.76% vs. 39.64%, respectively, statistically with non-significant differences (χ(2) = 0.13, P > 0.05) between them. The 5-year OSR and RSR of urban patients were 39.15% and 46.30%, and the 5-year OSR and RSR of rural patients were 30.81% and 36.32%, with statistically significant differences between them (P < 0.05). The 15-44 age group had a better relative survival rate.
CONCLUSIONSThe survival rate of gastric cancer patients in Zhejiang Province is low. Work of cancer prevention and control should be strengthened. The wide variation in gastric cancer survival rates between urban and rural patients indicates that priority should be given to rural areas in allocating medical and public health resources.

9.Survival rate and risk factors of mortality among first-ever stroke patients

Objective To describe the survival status and to analyze the factors associated with mortality on first-ever stroke patients.Methods The first-ever stroke patients registered in 2009 were collected from "Zhejiang provincial information system for NCDs' surveillance and management".Survival status and the cause of death through active and passive follow-up programs,were collected.Kaplan-Meier methods were used for survival description.Monovariant and multivariant Cox' s proportional hazard regression models were used to analyze risk factors on mortality.Results A total of 78 189 patients,who suffered from cerebral infarctions (ICD-10:I63),intracerebral haemorrhages (I61),subarachnoid haemorrhages (I60) and unspecified strokes (I64),accounted for 61.65%,30.42%,2.32% and 5.62%,were recruited.33 265 cases died during the period of this study.27 147 cases were stroke related,accounted for 81.61%.6 122 cases died on the same day,with one-day case fatality as 7.83% and the overall 28-day case fatality as 21.01%.The survival rates from one-year to four-year were 72.04%,68.92%,66.27% and 64.29%,respectively.The four-year survival rates of I63,I61,I60 and I64 were 80.06%,50.15%,71.80% and 21.41%,respectively.Effect of the model showed that the risk factors associated with mortality were age,gender,educational level,the diagnosis and quality of the hospitals on treatment,hypertension and the types of stroke incidences.Age had interacted with gender (P<0.001).Results from the ‘single effect’ showed that males had higher risk than females in those younger than 75 years old,but vice versa in those older than 75 of age.Conclusion Patients appeared very high risk of death in both acute and sub-acute phases.Factors including age,gender,educational level,both quality on diagnosis and treatment of the hospitals,clinical types of hypertension and stroke etc.were at risk,associated with prognosis of the disease.
10.Intelligent flow detection in hospital microservices platform security operation and maintenance management system based on genetic algorithm optimized LightGBM algorithm

A novel data detection algorithm is proposed to improve the data detection efficiency of the operation and maintenance management system for the hospital microservices platform.Based on the multiple characteristics of the platform data,the algorithm constructs the overall framework of the operation and maintenance management system.By combining the parameter optimization ability of genetic algorithm and the rapid detection ability of LightGBM algorithm,the effective detection of the flow data in the operation and maintenance management system is realized.The effectiveness of the model is verified through a control test,and the results show that the proposed method performs the best in intelligent flow detection,achieving accuracy of 0.981 0,recall rate of 0.68 and F1 value of 0.77 which are all higher than those of the traditional methods.
