1.Bacterial Biofilm Formation on Indwelling Double J Stent Observed by Scanning Electron Microscopy

OBJECTIVE To observe the bacterial biofilm formation on indwelling double J stent in the human body.METHODS A prospective study of 42 cases with double J stent indwelling in the human body after operation was conducted.The stent was divided into 4 segments then detected microbiologicaly in a separate manner and observed with scanning electron microscope for the presence of bacterial biofilm.RESULTS From 42 cases with douple J stent,only one case was without any bacteria in four segments.20 cases(47.6%) had the same bacteria in four segments.The bacterial positive rate of each segment was pelvic segment(81.0%),upper ureter segment(64.3%),lower ureter segment(71.4%) and bladder segment(83.3%);totally 18 kinds and 126 bacterial strains were flund.the most popular pathogens were Escherichia coli(23.0%),Pseudomonas aeruginosa(15.9%),corynebacterium(15.1%) and Staphylococcus epidermidis(13.5%).On the surface of unused double J stent,there was uneven but clean,without any attachment and encrustation,but on the surface of double J stent indwelling in the human body,the inflammatory affachment and encrustation were observed as well as bacterial biofilm,encapsulated in the abundant fibrous membrane.CONCLUSIONS Our study found that bacterial biofilm can be encrustated on the surface of double J stent indwelling in the human body,it may be one of the reasons for catheter-associated urinary tract infection.
2.Correlation and significance of combined detection of D-Dimer,cTnI and NT-ProBNP in acute coronary syndrome

Objective To explore the significance of combined detection of D-Dimer,cardic troponin I(cTnI)and N-terminal pro-brain natriuretic peptide(NT-ProBNP)in acute coronary syndrome and the correlation between them.Methods 143 patients with acute coronary syndrome were selected as the observation group,and 40 CAG negative people as the control group.The difference between the two groups was compared and the correlation was analyzed.According to different diagnostics,patients in the observa-tion group were separated into 3 groups,unstable angina pectoris,non ST segment elevation myocardial infarction and ST segment elevation myocardial infarction.The correlation of D-Dimer,cTnI and NT-ProBNP with the severity of coronary artery disease was analyzed.Results The levels of D-Dimer,cTnI and NT-ProBNP in the observation group were higher than those in the control group,and the difference was statistically significant(P < 0.05 ).The severity of coronary artery disease had positive correlation with the test results (P <0.05).Conclusion The combined detection of D-Dimer,cTnI and NT-ProBNP could help to diagnose the acute coronary syndrome and the test result has a positive correlation with the severity of acute coronary syndrome.

4.Development of national secondary reference materials of urea and creatinine in frozen human serum

Objective:To develop a national secondary reference material of Urea and Creatinine in frozen human serum as a standard for metrological traceability.Methods:According to JJF1343-2012 "General and Statistical Principles for Characterization of Reference Materials" and JJF 1006-1994 " Technical Norm of Primary Reference Material ", the homogeneity, stability, and commutability were evaluated;Using the JCTLM recommended methods, the value of the reference materials was assigned through collaboration with 6 accredited reference laboratories from Guangdong Provincial Hospital of Chinese Medicine, Beijing Aerospace General Hospital, Shenzhen Mindray Bio-Medical Electronics, Maccura Biotechnology, Beijing Leadman Biochemistry, and Zhejiang MedicalSystem Biotechnology. Uncertainty components including inhomogeneity, stability and value assignment were evaluated.Results:The results of one-way analysis of variance of homogeneity for the reference materials showed P>0.05, and the stability evaluation was less than the critical value of the t-test. The measured values were in the 95% confidence interval in the four conventional detection systems for commutability, and the certified values and expanded uncertainties were urea:(14.7±0.3) mmol/L ( k=2),Cr:(313.9±14.5) μmol/L ( k=2). Conclusion:The prepared secondary reference materials of urea and creatinine had promising homogeneity, stability, and commutable, the values of urea and creatinine concentration in reference materials were accurate and reliable.
5.Adjustable "paper clip" techniques suturing the dorsal vein complex in radical prostatectomy

【Objective】 To investigate the efficacy of the adjustable "paper clip" techniques in the suture of dorsal vein complex (DVC) and retention of urethral function in robot-assisted laparoscopic radical prostatectomy (RALRP). 【Methods】 A total of 30 cases of prostate cancer treated with RALRP were enrolled, all of which used the adjustable "paper clip" techniques. During operation, the DVC was sewed with barbed suture, and then a reverse suture was made through two sides of the prostatic ligaments. A Hem-o-lock was used to fasten the suture, which would be flexible to control the degree of tightness for the ligature. Perioperative and follow-up data of urinary continence and symptoms were collected and analyzed. 【Results】 All operations were successful. The estimated blood loss was (123.3±80.7) mL, 53.6% patients recovered continence in 1 month, and the continence rate increased to 92.9% and 96.3% at month 3 and 6. 92.9 of patients had no risk of incontinence 3 months after surgery. 【Conclusion】 The adjustable "paper clip" techniques have advantages in reducing blood loss, maintaining clear surgical field, preserving urethral function, and improving urinary continence.
