2.Thirty-Nine Children with Refractory Systemic Juvenile Idiopathic Arthritis Treated by Glucocorticoid

Objective To investigate clinical characteristics of refractory systemic juvenile idiopathic arthritis(JIA)and the efficiency of glucocorticoid in therapy on this kind of disease.Methods Thirty-nine children with systemic JIA were divided into low dose group 0.5-1.0 mg/(kg?d)and high dose group 1.0-1.5 mg/(kg?d).And the efficiency was observed by change of active index after 10 and 20 days.Results The effective power was 58.8% and 72.7% after 10 days,respectively.After 20 days,the power was 76.5% and 90.9%,respectively.The power in high dose group was significantly higher than that in low dose group.It had no difference in statistical analysis for efficiency of 2 kind of glucocorticoid dosage to control fever,but it had obvious difference to control arthralgia,arthrocele,erythrocyte sedimentation rate(ESR),C-reactive protein(CRP).Conclusion Glucocorticoid therapy is very effective to control the activity of disease in patients with systemic JIA.
3.Development of a Multiplex PCR-Microarray Method for Detection of Important Enteropathogen

Objective: To establish a multiplex PCR-microarray method for detecting important enteropahogens.Methods: Uniplex and multiplex PCR were performed to obtain the best primer sets for identifying the target bacteria at species and multi-species level.Fluorescent dyes were mixed into PCR reaction to determine whether it can affect the efficiency of amplification.To improve the efficiency of microarray,a 35 pairs primer-labeling system was optimized based on the hybridization results to find the best combination to avoid false negative results.Results: Specific PCR products were all obtained using species-specific primer sets.More preferential amplification may happen when more primer pairs were added to the reaction.The hybridization results showed a positive association between the efficiency of multiplex-PCR and signal intensity.Conventional PCR yielded more products than fluorescent dyes labeled PCR.Thirty-five primers were divided into three different combinations to label target respectively,hybridization results showed a high specificity.Conclusion: Mixing fluorescent dyes into PCR may reduce the efficiency of amplification and hybridization,but may have no effect on the analysis of hybridization results.The hybridization efficiency of microarray depends on the amplification efficiency of multiplex PCR.For microarray target labeling,three primer sets could be used to avoid negative hybridization led by preferential amplification of multiplex-PCR.It indicates that the multiplex PCR-microarray method is an attractive diagnosis tool for the high-throughput identification of enteropathogenic organisms especially for multiple causative agents and epidemiological investigations.
4.The quality assessment of the interventional radiololgy publications in Chinese journal of radiology using the randomized controlled trials

Objective To assess the quality of reporting randomized controlled trials published in Chinese journal of radiology from 2000 to 2005.Methods A manual search was performed and 22 checklists of CONSORT statements and other self-established criteria were applied.Results Six volumes and 72 issues were investigated.There were total trials of 236 in 2186 literatures,and finally 3 randomized controlled trials(RCTs)(1.27%)were identified.In the 3 RCTs,there were 3 trials with methods of randomization,1 with endpoints measurement,1 with multi-centre,but without the prior calculation of sample size,blind methods,statistically probability,participant flow,compliance and negative results.Conclusion The quality of reporting randomized controlled trials of interventional radiology has been improved,but it did not meet fully the CONSORT statement.

7.Effects of radicular dentin treatments and luting materials on the bond of quartz fiber posts: scanning electron microscope study

Objective To investigate the effects of radicular dentin treatments of sodium hypochlorite (NaOCl) and ethylenediaminetctraacetic acid (EDTA) on the regional root canal bending interface of quartz fiber posts using 2 luting materials with SEM analysis. Methods Nine intact maxillary central incisors were sectioned and endodontically treated. Standardized post space preparations and acid etch were performed . All specimens were randomly divided into 3 groups(n =3). D.T. LIGHT posts were placed into the root canal using one of three radicular dentin treatments(0. 9% NaCl for 60 s, 10% NaOCl for60 s, 17% EDTA for 60 s followed by 5.25% NaOCl for 60 s) in combination of one of two luting materials (DuoLink, LuxaCore) respectively (factorial design). Cervical, middle, apical sections of each teeth are used for SEM study and spectroscopy of dispersion energy (EDS) microanalysis. Results With the radicular dentin treatment with 10% NaOCl alone or with 17% EDTA followed by 5.25% NaOCl, longer and increased number of penetration of resin tags into the dentinal tubules were observed at the resin-dentin interfaces, and adhesive lateral branches could be found easily. EDS microanalysis showed increase in the infiltration behavior of the luting cement. Conclusions Radicular dentin treatments provide good resin infiltration, which can produce a three-dimensional interlocking micronetwork of resin tags in the dentin tubules with multiple lateral branches that penetrate the intertubalar dentin, thus positively influence the adhesion between dentin and the luting materials.

9.Epidemiological and etiologic characteristics of ocular fungal infection in Haikou.

OBJECTIVETo analyze the epidemiological and etiologic characteristics of ocular fungal infection in Haikou.
METHODSThe samples of corneal, conjunctivae, anterior chambers, lacrimal sacs were taken from the outpatients and inpatients in the Department of Ophthalmology for fungal culture. The positivity rates of fungal culture were statistically analyzed to describe the seasonal and genus distribution of the fungal infections.
RESULTSA total of 683 patients were involved in this study, and the total positivity rate of fungal culture was 27.96%. The positivity rate was 33.42% in male patients and 21.52% in female patients (P<0.01). The positivity rates were 31.42%, 11.32%, 10.81%, 21.28% and 29.17% in the cornea, conjunctivae, anterior chambers, lacrimal sacs and other locations (P<0.01), and were 34.88%, 22.58%, 11.76%, 4.11%, 21.43% and 13.33% in farmers, workers, teachers, government officers, students and personnel of other occupations (Chi2=39.550, P=0.001), respectively. In terms of age, the rate was 32.30% in 21-40 years group, 31.72% in 41-60 years group, 17.12% in over 60 years group and 26.61% in below 20 years group, showing significant differences (P<0.01). The rates were higher in August, September, October, December, November, all above 30%, but lower in March. Nine genera were identified, including Candida (49.21%), Aspergillus (19.37%), Fusarium (9.42%), mucor (5.76%), Actinomyces (5.24%), Penicillium (3.66%), Saccharomyces (2.62%), non-spore group (1.57%), Alternaria (1.57%), and other genera (1.57%). Among the 94 strains of Candida, 63 (67.02%) belonged to Candida albicans and 14 to Candida tropicalis (14.89%).
CONCLUSIONOcular fungal infection occurs more often in male patients and in the cornea. The common pathogen of ocular fungal infection is Candida, which is different from the etiological characteristics in other areas possibly in association with the tropical climate.

10.Effect of alfacalcidol soft capsules on immune function in patients with chronic obstructive pulmonary disease

Objective To explore the effect of alfacalcidol soft capsules on immune function in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 172 COPD patients were random-ly divided into the trial group ( n=86 ) and the control group ( n=86 ) . They all accepted the conventional treatment, but the patients in the trial group were given the treatment of alfacalcidol soft capsules 0.25μg? d -1 for 6 months.The difference of immune function ( CD3 +, CD4 +, CD8 +and CD4 +/CD8 +), pulmonary function [forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC, diffusion capacity for carbon monoxide of lung/alveolar volume ( DLCO/VA ) ] and acute exacerbations of COPD during 6 months follow -up were compared between the two groups.Results The contrations of CD3 +, CD4 +, CD8 +and CD4 +/CD8 +after treatment in the trial group were significantly higher than that before treatment and control group (P<0.05).The FEV1,FEV1/FVC,DLCO/VA after treatment in the trial group were significantly higher than that before treatment and control group(P<0.05).There were no significant difference of CD3 +,CD4 +, CD8 +, CD4 +/CD8 +, FEV1, FEV1/FVC, DLCO/VA before and after treatment in the control goup(P>0.05).The rate of acute exacerbations of COPD in the trial group (9.30%)was sig-nificantly lower than that in the control group ( 25.58%) during 6 months follow-up.Conclusion Alfacalcidol soft capsules could improve the pulmonary function and reduce acute exacerbations by improving immune function of patients with COPD.
