1.Management of congenital cataract with different technique: phacoemulsification I/A and 23-gauge system

AIM: To evaluate the efficacy and safety of two different surgical techniques on congenital cataract on children.METHODS: Twenty-two children (1-3 years old) with congenital cataract were randomly divided into two groups (group A and group B). With group A (10 patients, 20 eyes), we applied 23-gauge (23G) trans corneal limbus vitrectomy system to complete lens cortex gettering, posterior capsulotomy and anterior vitrectomy;With group B (12 patients, 24 eyes), we used the phacoemulsification I/A to complete lens cortex gettering, and performed anterior vitrectomy with anterior vitreous cutting instrument. After that, the differences in intraoperative and postoperative complications between two groups were compared. RESULTS:In group A, the width of corneal limbal incision was 0.6mm, the incision was self-sealing, and the anterior chamber was stable and iris did not prolapse during the surgery. In group B, the width of corneal limbal incision was 3mm, anterior chamber was unstable and intraoperative iris prolapse occurred in 14 eyes (58%). And the incision need to be stitched up after surgery. In the postoperative follow-up of 6-24 months (an average of 14 months), we found that corneal neovascularization did not occur in group A, while in group B, corneal neovascularization occurred in four eyes (17%); Other complications, such as posterior capsular opacification,retinal detachment, glaucoma, hypotony or endophthalmitis did not occur in either group.CONCLUSION: The 23G trans corneal limbus vitrectomy system used in pediatric cataract surgery is safer and more effective than phacoemulsification I/A. It is promising in treatment of congenital cataract on children.
2.Bufadienolides from venom of Bufo bufo gargarizans.

Twelve compounds were isolated from the venom of Bufo bufo gargarizans. On the basis of their physical and chemical properties and spectral data, their structures were identified as resibufagenin (1), bufotalin (2), desacetylcinobufagin (3), 19-oxodesacetylcinobufotalin (4), cinobufotalin (5), 1beta-hydroxylbufalin (6), 12alpha-hydroxybufalin (7), bufotalinin (8), Hellebrigenin (9), telocinobufagin (10), hellebrigenol (11) and cinobufagin-3-hemisuberate methyl ester (12), respectively. Compounds 7 and 12 are new natural products.

3.Effects of bacillus bifidus supplementation on the immunity in very-low-birth-weight infants.

OBJECTIVETo study the effect of bacillus bifidus supplementation on the immunity in very-low-birth-weight (VLBW) infants.
METHODSFifty VLBW infants who were hospitalized in the neonatal intensive care unit were equally randomized into observed and control groups. The observed group received bacillus bifidus for 14 days after birth in addition to the conventional management, which was applied in the control group. The clinical indicators and relevant immunological parameters in the peripheral blood were observed.
RESULTSThe times required for brine enema was significantly fewer in the observed group than in the control group (P<0.05), while the incidence of diarrhea showed no significant difference (P>0.05). In the observed group, the proportions of CD4(+) T lymphocytes and the CD4(+)/CD8(+) ratio increased, whilst the proportion of CD3(+) and CD8(+) T lymphocytes showed no significant change. The levels of immunoglobulin A in the peripheral blood increased in the observed group, while the levels of immunoglobulin M and immunoglobulin G were not statistically changed in the observed group when compared with the control group.
CONCLUSIONSBacillus bifidus can improve gastrointestinal symptoms of VLBW infants and have a positive effect on their immunity.

4.The antalgic and antiphiogistic function and mechanism of RGDT plaster.

OBJECTIVETo study the antalgic and antiphlogistic functions and mechanism of ronggudingtong (RGDT) plaster (traditional Chinese medicine).
METHODSThe painful models were established with hot plate test or acetic acid writhing and the inflammatory models were established with daubing dimethylbenzene on auricle or injecting formaldehyde in toe or synovial envelope to study the antalgic and antiphlogistic functions of RGDT Plaster. The total protein and leukotriene B4(LTB4) in inflammatory exudate were detected to investigate the antalgic and antiphlogistic mechanism of RGDT plaster. The mice were randomly divided into different groups (n = 11), on the basis of drug using, the indexes of pain threshold, swelling degree were observed. Sixty-six mice were used to establish gasbag synovitis model and randomly divided into normal control group,model control group, positive control group (Voltaren gel 0.8 mg/d)and low/medium/high dosage RGDT plaster treating groups(30 mg/d, 60 mg/d, 120 mg/d).
RESULTS30 mg/d, 60 mg/d,120 mg/d RGDT plaster could upgrade the pain thresholds, remit auricular and foot swelling (P < 0.05, P < 0.01), and degrade total protein and LTB4 in inflammatory exudates (P < 0.05, P < 0.01).
CONCLUSIONRGDT plaster has some antalgic and antiphlogistic functions, and one of the mechanisms is depressing synthesis of LTB4.

Objective To study the role of hematopoietic growth factor(HGF)of placenta chorionic villus in fetal hematopoiesis during embryo ontogeny by observation of the appearance time and the content changes with the fetal growth, which was compared with HGF in cord blood. Methods Thirty embryo villus (2 g each) and 30 cord blood (2 mL each) were collected separately from early pregnant stage(6- 8 weeks), middle pregnant stage(16-22 weeks)and late pregnant stage (37-42 weeks). 6.Glycated hemoglobin A1C and diabetes mellitus in critically ill patients

BACKGROUND:Hyperglycemia has been detected in many critically ill patients in the department of emergency medicine. But its mechanism and prognosis have not been well elucidated. In this study, we measured the serum level of glycated hemoglobin A1C (HbA1c) in critically ill patients to evaluate the effects of hyperglycemia on the prognosis of the patients.METHODS:A total of 826 critically ill patients, who had been treated at the Department of Emergency Medicine of Chaoyang Hospital during October 2006 and November 2007, were divided into a diabetes mellitus group (n=184) and a non-diabetes mellitus group (642) according to whether they had diabetes mellitus. Fasting glucose and HbA1c were measured in all patients. Those in the diabetes mellitus group were further assigned to a drug therapy subgroup and a non-drug therapy subgroup; the serum level of HbA1c and its relationship with short-term outcome were evaluated.RESULTS:Fasting glucose increased in 78.8% of the patients (88.6% in the diabetes mellitus group, and 75.9% in the non-diabetes mellitus group,P<0.05), and HbA1c was elevated in 45.5% of the patients (78.3% in the diabetes mellitus group, and 36.1% in the non-diabetes mellitus group, P<0.01). Fasting glucose, HbA1c and 28-day mortality were improved more significantly (P<0.01) in the drug therapy subgroup than in the non-drug therapy subgroup. The 28-day mortality was more significantly different in patients with fasting blood glucose >8.33 mmol/L than in those with fasting blood glucose <8.33 mmol/L.CONCLUSIONS:Hyperglycemia of critically ill patients could not totally attribute to stress response, especially in those who have no history of diabetes mellitus. Prognosis of hyperglycemia may vary among critically ill patients.
7.Based on infrared spectrum to study the changes in the intrinsic chemicalcomponents in Trichosanthes before and after steaming

Objective To analyze the change in infrared(IR)spectral information and to screen out the classification of major compounds affecting information difference in IR spectra of Trichosanthes before and after steaming. Methods The similarity between the original IR fingerprint and the first derivative IR fingerprint of Trichosanthes and their steamed products was calculated by using the Computer Aided Similarity Evaluation System. The principal component analysis(PCA)model and the partial least squares dis-criminant analysis(PLS-DA)model of IR spectral data of Trichosanthes before and after steaming were established by using SIMCA-P 11 statistical software for PCA and PLS-DA,and the classification of major compounds affecting information difference in IR spectra of Trichosanthes before and after steaming was selected by 3D scatter plot,load 3D scatter plot and variable important in project(VIP) value. Results The similarity between the original IR fingerprint and first derivative IR fingerprint of Trichosanthes and their steamed products were 0.9165 and 0.2832. Seven VIP>1 spectral peaks were screened out by using SIMCA-P 11 statistical software,of which,the absorption peak of 1456 cm-1 was νc=c,the absorption peak of 1726 cm-1 was νc=o and the VIP values were 1.6290 and 1.4256 respectively. Conclusion The categories of compounds of Trichosanthes before and after steaming did not change,but the chemical components changed. Compounds of Trichosanthes before and after steaming affect the difference in IR spectral information may mainly contain C=C-C=C or C=O or both of them.
8.Analysis of the changes in the chemical components in Fructus Trichosanthes, before and after the processing of steaming,based on the PCA and PLS-DA algorithms

Objective To analyze the changes in the chemical components in Fructus Trichosanthes before and after the pro?cessing of steaming,so as to explore the material basis of the pharmacodynamic changes between Trichosanthes and steamed Tricho-santhes.Methods The peaks matching data of Fructus Trichosanthes and steamed Fructus Trichosanthes were obtained by using the similarity evaluation system for chromatographic fingerprints of traditional Chinese materia medica.The principal component analysis (PCA)model and the partial least squares discriminant analysis(PLS-DA)model for the analysis of the Fructus Trichosanthes and steamed Fructus Trichosanthes data were established using the SIMCA-P 11 statistical software for PCA and PLS-DA,from which the score chart,load chart and Variable Importance(VIP)value were obtained,so as to identify the main different components in Fructus Trichosanthes and steamed Fructus Trichosanthes.Results The PCA(R2X=0.96,Q2=0.552)model and PLS-DA(R2Y=0.917,Q2=0.579)model were established,and 8 chromatographic peaks with significant difference in peak area were selected.Among them,two of the chromatographic peaks were assigned to be 5-hydroxy methyl furfural and vanilla acid,and 5-hydroxy methyl furfural had the largest VIP value.In addition,an unknown component was also found in the steamed Fructus Trichosanthes,which was generated in the process of steaming and needed to be identified in future studies.Conclusion The content of some chemical components in Fruc?tus Trichosanthes were changed after the process of steaming,and the processing of steaming also caused the formation of an unknown chemical component.5-Hydroxy methyl furfural and vanillic acid seem to be a likely choice for exploring the material basis of the phar?macodynamic changes in Fructus Trichosanthes after the processing of steaming.
9.Research of autophagy activity between rat bone marrow mesenchymal stem neural differentiation.

OBJECTIVETo study the autophagy activity between rat bone marrow stem cells (BMSCs) neural differentiation in order to explore the mechanism involve in this process.
METHODSBMSCs were passed by 3 generation, then was induced with the revulsant 2% (DMSO) + 200 µmol/L (BHA), NSE expression was detected by immunocytochemical stain, the mRNA expression of autophagy associated genes L3B, Beclinl, Atg5, Atg7, Atg10 were detected by RT-PCR, the autophagy protein LC3B was examined by Western blot and flow cytometry analysis.
RESULTSBMSCs were passed by 3 generation, the purity of BMSCs could reach more than 90%, the morphology of cells were like fibroblasts, after the revulsant 2% DMSO + 200 µmol/L BRA induced, cells were extended long neurites, like nerve cells, positive rate of NSE staining was (83±5) %, RT-PCR results showed that the expression of autophagy associated genes LC3B, Beclinl, Atg5, Atg7 Atg0 were rised after BMSCs neural differentiation, Western blot analysis showed that the LC3B-II protein expression was increased after neural differentiation and the MFI of L3B was highten by flow cytometry.
CONCLUSIONAutophagy is increased after rat BMSC neural differentiation.

10.Determination of the interaction kinetics between meloxicam and β-cyclodextrin using the quantitative high-performance affinity chromatography coupled with mass spectrometry.

The association rate constant and dissociation rate constant are important parameters of the drug-cyclodextrin supermolecule systems, which determine the dissociation of drugs from the complex and the further in vivo absorption of drugs. However, the current studies of drug-cyclodextrin interactions mostly focus on the thermodynamic parameter of equilibrium constants (K). In this paper, a method based on quantitative high performance affinity chromatography coupled with mass spectrometry was developed to determine the apparent dissociation rate constant (k(off,app)) of drug-cyclodextrin supermolecule systems. This method was employed to measure the k(off,app) of meloxicam and acetaminophen. Firstly, chromatographic peaks of drugs and non-retained solute (uracil) on β-cyclodextrin column at different flow rates were acquired, and the retention time and variance values were obtained via the fitting the peaks. Then, the plate heights of drugs (H(R)) and uracil (H(M,C)) were calculated. The plate height of theoretical non-retained solute (H(M,T)) was calculated based on the differences of diffusion coefficient and the stagnant mobile phase mass transfer between drugs and uracil. Finally, the k(off,app) was calculated from the slope of the regression equation between (H(R)-H(M,T)) and uk/(1+k)2, (0.13 ± 0.00) s(-1) and (4.83 ± 0.10) s(-1) for meloxicam and acetaminophen (control drug), respectively. In addition, the apparent association rate constant (k(on,app)) was also calculated through the product of K (12.53 L x mol(-1)) and k(off,app). In summary, it has been proved that the method established in our study was simple, efficiently fast and reproducible for investigation on the kinetics of drug-cyclodextrin interactions.
