1.Phenylpropanoids from roots of Berberis polyantha.

The chemical constituents were systematically separated from the roots of Berberis polyantha by various chromatographic methods, including silica gel column chromatography, HP20 column chromatography, polyamide column chromatography, reversed-phase C_(18) column chromatography, and preparative high-performance liquid chromatography. The structures of the compounds were identified by physicochemical properties and spectroscopic techniques(1D NMR, 2D NMR, UV, MS, and CD). Four phenylpropanoids were isolated from the methanol extract of the roots of B. polyantha, and they were identified as(2R)-1-(4-hydroxy-3,5-dimethoxyphenyl)-1-propanone-O-β-D-glucopyranoside(1), methyl 4-hydroxy-3,5-dimethoxybenzoate(2),(+)-syringaresinol(3), and syringaresinol-4-O-β-D-glucopyranoside(4). Compound 1 was a new compound, and other compounds were isolated from this plant for the first time. The anti-inflammatory activity of these compounds was evaluated based on the release of nitric oxide(NO) in the culture of lipopolysaccharide(LPS)-induced RAW264.7 macrophages. At a concentration of 10 μmol·L~(-1), all the four compounds inhibited the LPS-induced release of NO in RAW264.7 cells, demonstrating potential anti-inflammatory properties.

2.Problems to be solved in the development from the perspective of future application scenarios of telesurgery.

Telemedicine, which integrates medicine, communication, engineering, information and other disciplines, is a hot emerging cross field in recent years. With the development of telecommunication technology and surgical robot, telesurgery is regarded as the "crown pearl" in telemedicine and has attracted more and more attention. As an extension of traditional surgery, telesurgery greatly extends the connotation and concept of surgery and embodies the great leap forward development of surgical technology. Despite the current limitations such as network delay, transparency of remote robot operation and team construction of surgeons, telesurgery has still formed a variety of innovative application scenarios and achieved rapid development in China in recent years. In view of the uneven distribution of medical resources in China and the epidemic of COVID-19 in the world, this paper puts forward the possible problems and solutions in the development of telesurgery, and looks forward to the feasibility of telesurgery technology in process of shifting the focus of medical and health care down to the community level, channeling resources accordingly.

Patrinia villosa, regarding its functions in clearing heat and detoxification and eliminating carbuncles and pus, is widely used as a traditional medicinal herb that contains rich nutrition and substances such as various amino acids, vitamins, and soluble su-gar, and it is also an edible wild herb in Chinese folk tradition for 2 000 years. In 1973, Japanese scholars firstly separated three iridoids from Japanese P. villosa, and by 2021, chemical components such as flavonoids, iridoids, organic acids, triterpenoids, phenylpropanoids, and steroids have been found, which have multiple pharmacological effects, including antioxidant, antitumor, anti-diarrhea, antibacterial, sedative, and liver protection capabilities. Studies indicate that flavonoids, saponins, phenylpropanoids, and triterpenoids in P. villosa are vital substances for its pharmacological activities. However, the quality of this medicinal material cannot be controlled due to the unclear records in ancient books in the past dynasties and different drug use habits in different places, and thus its circulation is chaotic. At present, researchers have used flavonoids, organic acids, phenylpropanoids, triterpenoid saponins, and other compounds to conduct studies in this regard. Therefore, on the basis of the existing literature resources, we comprehensively summarize the chemical constituents, pharmacological activities, and quality control of P. villosa to further provide a reference for the safety and effectiveness of clinical drug use and lay a foundation for the follow-up experimental research.

4.Study on Impedance of Implantable Cardiac Pacemaker in Unipolar/Bipolar Pacing Mode by in Vitro Experiment.

The unipolar/bipolar pacing mode of pacemaker is related to its circuit impedance, which affects the battery life. In this study, the in vitro experiment scheme of pacemaker circuit impedance test was constructed. The human blood environment was simulated by NaCl solution, and the experimental environment temperature was controlled by water bath. The results of in vitro experiments showed that under the experimental conditions similar to clinical human parameters, the difference between the circuit impedance of bipolar mode and unipolar mode is 120~200 Ω. The results of the in vitro experiment confirmed that the circuit impedance of bipolar circuit was larger than that of unipolar mode, which was found in clinical practice. The results of this study have reference value to the optimization of pacing mode and the reduction of pacemaker power consumption.

OBJECTIVE:
To analyze clinical feature and information of medication to explore the risk signals of preparations containing Psoraleae Fructus (BGZP) related with hepatobiliary adverse drug reactions (ADR), in order to reinforce pharmacovigilance.
METHODS:
A retrospective study was conducted based on hepatobiliary ADR related with BGZP from the China Adverse Drug Reaction Monitoring System in years from January 2012 to December 2016. Serious and general ADRs were analyzed and assessed.
RESULTS:
There were 355 cases of hepatobiliary ADR related to BGZP. Both the amount of cases and the proportion of serious ADR showed an increasing growth by years (P<0.05). It was found that 10.43% of 355 cases may be involved with irrational drug use, including overdose, repeated medication, and combination of multiple drugs. There were 190 cases which used BGZP (non-combination), and they were mainly for common in diseases caused by abnormal immune activation (accounting for 40.53% of the total cases). Especially at the age group with the most cases with age of 41-50 years, the cases associated with immunological diseases of female were obviously more than that of male (P<0.05). The latency of hepatobiliary ADR related to BGZP ranged from 1 to 386 days, and the median latency was 27.5 days, along with the range of cumulative dose (0.45-520.02 g) as well as the daily dose (0.09-2.64 g/d) after the conversion.
CONCLUSIONS
Cases of hepatobiliary ADR related to BGZP showed significant individual differences, and there was no correlation between drug usage duration and dosage and the occurrence of hepatobiliary ADR. It may be similar with idiosyncratic drug-induced liver injury, and recommended that BGZP should be used with more caution under monitoring liver function, especially in female patients with immunological diseases.

ObjectiveTo understand the mechanism of β-amyloid (Aβ)-induced neurotoxicity, this study aimed to investigate the effects and possible pathways of Aβ on apoptosis-related nuclear transcription forkhead protein O3a (FoxO3a) and postsynaptic density protein 95 (PSD95). MethodsPC12 cells and neurons were treated with gradient concentrations (5, 10, 20 μmol/L) of Aβ25-35 for 24 h, and the alterations of FoxO3a and PSD95 were detected by RT-PCR and Western blot. Immunofluorescence assay was used to detect the level of PSD95 in PC12 cells and the subcellular localization of FoxO3a in the cells. Then in rats injected with Aβ25-35 and APP/PS1 transgenic mice, the changes of PSD95 and FoxO3a in brain tissue were investigated. Western Blot was used to detect the effects of Aβ on the phosphorylation of FoxO3a and protein kinase B (AKT) in vitro and in vivo. ResultsCompared with cells in the normal control group, the protein levels of PSD95 in the cells treated with Aβ25-35 at the dosage of 20 μmol/L were down-regulated to (45.09±1.61)% (P=0.054 0). Correspondingly, the protein levels of FoxO3a were increased to (228.70±20.44)% (F=17.48, P=0.021 0) when the cells were treated with 20 μmol/L of Aβ25-35. In the primary cultured neurons, similar results were obtained. In addition, the results of immunofluorescence showed that Aβ25-35 promoted the nuclear translocation of FoxO3a. The residence time of Aβ25-35-injected group was (24.35 ±1.29) s (F=2.843, P=0.098) and the number of crossings was 2.53±0.49 (F=3.459, P=0.014 9) of rats in the water maze test. There was significant difference between the CTL group and Aβ25-35-injected group (P<0.05). The RT-PCR assay showed that the mRNA level of PSD95 in the hippocampus of rats treated with Aβ25-35 was decreased to (58.40±8.28)% (F=1.193, P=0.010 1) of that in the CTL group, and the mRNA expression of FoxO3a was increased to (140.90±7.45)% (F=2.378, P=0.049 6). In the brain tissue of 7-month-old APP/PS1 transgenic mice, the mRNA and protein levels of PSD95 were down-regulated to (60.89 ±1.53)% (F=20.05, P=0.008 8) and (59.63±13.55)% (F=8.496, P=0.044 5). Meanwhile the mRNA and protein levels of FoxO3a were up-regulated to (172.4±4.87)% (F=2.351, P=0.000 4) and (235.00 ± 39.03)% (F=2.754, P=0.032 0), respectively. After treatment with 20 μmol/L Aβ25-35 for different times (5, 10, 20, 40 min), the phosphorylation of FoxO3a and AKT in PC12 cells was decreased with time. The levels of phosphorylated AKT and FoxO3a in the brain tissue of APP/PS1 transgenic mice were decreased to (65.75±3.51)% (F=6.362, P=0.023 6) and (46.62 ± 9.64)% (F=8.562, P=0.007 9) when compared with mice in the control group. ConclusionsAβ can up-regulate the expression of nuclear transcription FoxO3a and down-regulate the change of PSD95 in both in vitro cell models and in vivo animal models. The possible mechanism is to reduce the phosphorylation level of FoxO3a and increase the expression of FoxO3a by dephosphorylating AKT.

Objective To explore the clinical characteristics of autoimmune disease with dual seropositive antibodies of leucine-rich glioma inactivated 1(LGI1)and contactin-associated protein 2(Caspr2).Methods The clinical data of seven patients with dual seropositive LGI1 and Caspr2 antibodies who were admitted to the Neurology Department of Peking Union Medical College Hospital from July 2014 to December 2017 were retrospectively analyzed.Results Central,peripheral and autonomic nervous systems were all involved in the seven cases;100%(7/7)presented with insomnia,myokymia,neuropahic pain and hyperhydrosis;71%(5/7)showed memory decline or psychiatric and behavioral symptoms;57%(4/7)had urinary hesitation or constipation;and 43%(3/7)had seizure.Electromyography showed 100%(6/6) of the patients had prolonged afterdischarges following normal M waves and/or abnormal spontaneous firing.Electroencephalography revealed slow waves or basic rhythm slowing in 71%(5/7)of patients.Electrocardiography showed sinus tachycardia,axis deviation,and prolonged QT intervals in 71%(5/7)of patients.One patient died from arrhythmia before immunotherapy.One died from pulmonary infection after immunotherapy.Improvement with immunotherapy was documented in the other five cases.No relapse was noted during the 1-2-year follow-up.Conclusions Autoimmune disease with dual seropositive antibodies of LGI1 and Caspr2 can diffusely affect the central,peripheral,and autonomic nervous systems.The possibility of this disease should be considered in patients with acute and subacute onset of neuropsychiatric symptoms,especially in patients with accompanying insomnia,myokymia,and hyperhydrosis.
