1.Free superficial iliac circumflex artery skin flap: the clinical application and management of donor site defects.

OBJECTIVETo investigate the clinical application of free superficial iliac circumflex artery skin flaps, as well as the management of donor site defects.
METHODS17 free superficial iliac circumflex artery skin flaps were applied for the traumatic defects or deformities on face, neck, foot, hand, ankle and lower leg, respectively. The donor site defects were closed directly or covered by paraumbilical island flaps.
RESULTSThe 17 flap size ranged from 5 cm x 3 cm to 19 cm x 14 cm. 16 flaps survived completely except 1 flap with partial necrosis, which was closed by free skin graft. The donor site defects were closed directly in 10 cases, and covered by paraumbilical island flaps in 7 flaps without no flap necrosis. The abdomen had a good appearance.
CONCLUSIONSGood appearance can be achieved with free superficial iliac circumflex artery skin flaps for the defects on face, neck, foot, hand, ankle and lower leg. Paraumbilical island flap can be used for the donor site defects.

3.Safety analysis of postoperative cyclophosphamide combined with taxane chemotherapy in patients with triple-negative breast cancer and chronic renal failure

Objective:To explore the safety of cyclophosphamide combined with taxane chemotherapy in triple-negative breast cancer patients with chronic renal failure and the management strategy of complications.Methods:Data of 8 patients with triple-negative breast cancer and chronic renal failure admitted to our hospital from Jun 2016 to Dec 2019 were retrospectively analyzed.Results:Eight patients received standard cyclophosphamide combined with taxane (TC regimen) chemotherapy after operation, 5 of which received docetaxel 75 mg/m 2 + cyclophosphamide 600 mg/m 2, and 3 received albumin paclitaxel 260 mg/m 2+ cyclophosphamide 600 mg/m 2, during chemotherapy, only leukopenia, hair loss and gastrointestinal reactions occurred in grades 3 to 4, and the incidence was 25%, 25% and 12%, respectively. Four patients adjusted the dosage due to adverse reactions. One patient quit, the remaining patients successfully completed 4 cycles of chemotherapy. The average serum creatinine before chemotherapy was (498±63) μmol/L, and after chemotherapy, it was (518±61) μmol/L ( t=-2.335, P>0.05). Conclusions:Combined with chronic renal failure is not a contraindication to postoperative chemotherapy for patients with breast cancer. It is safe to choose standard TC regimen and adjust the dose of adjuvant chemotherapy for patients with triple-negative breast cancer.
4.Clinical efficacy of Paroxetine combined with mid-frequency electrical pulse acupoint stimulation for premature ejaculation.

OBJECTIVETo investigate the clinical value of Paroxetine combined with mid-frequency electrical pulse acupoint stimulation (EPAS) in the treatment of premature ejaculation (PE).
METHODSTotally 69 PE patients were equally assigned to receive oral Paroxetine 20 mg/d, mid-frequency EPAS, or oral Paroxetine 10 mg/d combined with mid-frequency EPAS (P + EPAS) , all for 8 weeks. We obtained the intravaginal ejaculation latency time (IELT) and Chinese Index of Premature Ejaculation (CIPE-5) scores of the patients before and after treatment, and compared adverse reactions among the three groups of patients.
RESULTSOne patient of the Paroxetine group gave up treatment because of abdominal pain and nausea. Compared with the baseline, the patients in the Paroxetine, EPAS, and P + EPAS groups all showed markedly increased IELT ([0.92 ± 0.11] vs [4.07 ± 0.11] min, P < 0.01; [0.92 ± 0.12] VS [2.78 ± 0.17] min P < 0.05; [0.91 ± 0.09] vs [5.31 ± 0.13], P < 0.01) and decreased CIPE-5 scores (12.5 ± 3.0 vs 22.0 ± 2.1, P < 0.01; 12.8 ± 2.9 vs 19.5 ± 1.9, P > 0.05; 13.1 ± 2.8 vs 25.2 ± 2.1, P 0.01), with statistically significant differences between the P + EPAS group and the other two (P < 0.05). The total effectiveness rate was 95.7% in the P + EPAS group, remarkably higher than in the Paroxetine (72.7%, P < 0.05) and the EPAS group (47.8, P < 0.01).
CONCLUSIONOral Paroxetine combined with mid-frequency EPAS has a higher safety and efficacy than either Paroxetine or EPAS alone in the treatment of PE.

The present paper, in a new perspective, summarized the determination of the species and valence state of heavy metals and deleterious elements in recent years, discussed the progress of the pharmacological effect and toxicology, and prospected for future study which might provide reference for mineral medicine.

7.Analysis for myeloperoxidase genetic polymorphism in gastric cancer.

OBJECTIVETo investigate the association of myeloperoxidase(MPO) genetic polymorphism and gastric cancer.
METHODSA case-control study was performed including 62 patients with gastric cancer and 61 healthy controls. Peripheral blood was collected for genetic analysis of MPO-463.
RESULTSThere were no significant differences in gender, age, and smoking between the two groups(P>0.05). However, the two groups differed in drinking, family history of gastric cancer, and Helicobacter pylori(HP) infection(P<0.05). The frequencies of MPO-463GG, GA and AA were 87.1%, 11.3% and 1.6%in the study group, and were 72.1%, 23.0%, and 4.9% in the control group, respectively. Carriers of MPO-463 GA or AA had a significantly higher risk of gastric cancer than those of MPO-463 GG(χ(2)=4.253, P<0.05, OR=0.383, 95% CI: 0.151-0.972). Carriers of G allele had a significantly lower risk of gastric cancer compared to carriers of A allele(χ(2)=4.935, P<0.05, OR=0.399, 95% CI: 0.174-0.916).
CONCLUSIONMPO-463 G/A polymorphism is associated with gastric cancer with A being a protective gene.

8.Comparison among three reconstruction techniques after distal radical gastrectomy.

OBJECTIVETo compare surgical efficacy after three different reconstruction techniques after radical resection of distal gastric cancer.
METHODSClinical data of 169 cases of distal gastric cancer operated in our hospital from 2007 to 2010 were retrospectively analyzed. The reconstruction techniques included Billroth I (anastomosis (n=60), Billroth II (anastomosis (n=41), and Roux-en-Y anastomosis (n=68). Efficacy among 3 groups was compared. Specific symptoms scale was used to evaluate the quality of life in three methods after three months.
RESULTSCompared to Billroth I(anastomosis and Billroth II (anastomosis, Roux-en-Y anastomosis had longer operative time [(266.3±70.4) min vs. (196.2±54.3) min, and (228.5±67.7) min], more blood loss [(220.9±67.6) ml vs. (170.5±61.5) ml and (188.5±76.7) ml], and shorter time to gastric tube removal [(2.6±1.5) d vs. (3.1±1.3) d and (3.6±1.2) d], milder postoperative reflux and heartburn sensation(specific symptoms scale, 1.8±0.4 vs. 1.9±0.6 and 2.6±0.4, P<0.05).
CONCLUSIONSAlthough Roux-en-Y anastomosis is not consistent with physiological route and the procedure is more complex to perform, it can effectively prevent reflux complications. Roux-en-Y anastomosis is a better reconstruction technique after radical resection of distal gastric cancer.

9.Role of zonula occludens-1 in early brain injury after subarachnoid hemorrhage and regulatory mechanism of c-Jun N-terminal kinase

Objective To clarify the role ofzonula occludens-1 (ZO-1) in early brain injury (EBI) after subarachnoid hemorrhage (SAH) and regulatory mechanism of c-Jun N-terminal kinase (JNK)to discuss the molecular mechanism of SAH-induced blood-brain barrier (BBB) dysfunction. Methods Seventy-two male SD rats were randomly assigned to sham-controlled, SAH, SAH+SP600125 (the inhibitor of JNK) and SAH+ dimethyl sulfoxide (DMSO) groups. SAH models were induced by injection of autologous blood into the cisterna magna of rats, and they were subsequently sacrificed 24 h later. The brain water content was measured, and BBB disruption was evaluated by Evans blue dye;immunohistochemistry was employed to compare the expressions of ZO-1 and JNK. Results As compared with the sham-controlled group, the SAH group 24 h after the success of model making was detected a significant increase of water content, obviously increased Evans blue dye extravasation,especially in the brain stem and cerebellum (P<0.05) and both structural damage and neuronal damage were noted; the mean optical density of immunohistochemistry showed a strong increase of JNK expression while a obvious decrease of ZO-1 expression was noted in the SAH group 24 h afar the success of model making (P<0.05). Treatment with SP600125 could significantly reduce the water content, decrease the Evans blue dye extravasation, inhibit the JNK expression and increase the ZO-1expression as compared with those in the SAH and SAH+DMSO groups (P<0.05), while no significant difference was noted as compared with those in the sham-controlled group (P>0.05). Conclusion SAH activates the phosphorylation of JNK, and represses the transcription and translation of ZO-1,subsequently, opens the tight junction and causes brain edema by redistribution of transmembrane proteins and rearrangement ofcytoskeletal proteins, therefore, EBI appears. However, treatment with SP600125 can reverse the above-mentioned processes and improve the outcome of EBI.
10.Application of lavation and drainage with continuous negative pressure in the debridement of diabetic foot complicated by plantar abscess.

OBJECTIVETo investigate the feasibility of debridement at early stage by combining lavation and drainage under continuous negative pressure in the limb salvage surgery of patients suffering from diabetic foot complicated by plantar abscess.
METHODSTen patients suffering from diabetic foot (10 feet affected) combined with plantar abscess were involved. After being debrided, wounds of patients were lavaged and drained for 7 - 14 ds under continuous negative pressure through a detained stomach tube. Suction under negative pressure continued for 3 - 5 ds after lavation, and then drainage tube was removed. Stitches on wound were removed about 3 weeks after surgery. The condition of the wounds was observed.
RESULTSAfter above-mentioned treatment, local inflammatory response of patients was obviously ameliorated, and the wounds healed with satisfactory preservation of function and shape of the feet.
CONCLUSIONSDebridement combined with local lavation and drainage under continuous negative pressure is satisfactory for the treatment of diabetic foot complicated by plantar abscess, and it can preserve the length of the affected limb.
