2.Different Circles of Neoadjuvant Chemotherapy Combined with Colorectal Cancer Operation in Multi-Disciplinary Team

0.05),which were (5.64?2.00)d in single-circle group,(5.80?3.74)d in double-circle group,(6.22?2.76)d in triple-circle group.According to the treatment effects,CEA value decreased during pre-and post-neoadjuvant chemotherapy in each groups(P0.05).Through our survey,used different neoadjuvant chemotherapy circle,patients in single-circle group and double-circle group were completely accepted within full confidence;but receptance of strategy in triple-circle group was 66.7 %(12/18).All operations were suc- cessful.The difference of postoperative aerofluxus time between single-circle group and double-circle group had sta- tistical significance(P0.05).Conclusion Analyzing neoadjuvant chemo- therapy circles,time between neoadjuvant chemotherapy and operation,treatment effect and operation results,it is a feasible and secure colorectal cancer multi-discipinary strategy for patients in West China that choose the treatment of neoadjuvant chemotherapy with double-circle and short preparation time.
3.Effects of Nd: YAG laser on micro-tensile bond strength of single bond adhesive system for noncarious sclerotic dentin

Objective To investigate the effect of Nd: YAG laser on microtensile bond strength of single bond adhesive system for non-carious sclerotic dentin. Methods Ten human molars with occlusal wearing were cut into equal halves ( nearly 12 mm2 ), and randomly divided into experimental group and control group. The teeth in experimental group were processed with Nd: YAG laser( 1 W,10 Hz) ,and then applied with Scotchbond and filled with Z350 resin. In control group, the teeth were processed with single bond and filled with Z350 resin. The specimens were sectioned, and the microtensile bond strengths of each sample was tested by a universal testing machine. Results The bond strength of the experimental group [ (26. 11 ±1.62) MPa] was significantly higher than that of the control group[ (22. 27 ±2. 16) MPa],P<0. 05. Stero-microscope examination indicated that most of the fractures occurred in dentin-resin interface. Conclusions Nd: YAG laser can increase the rnicrotensile bond strength of single bond adhesive system in non-carious sclerotic dentin.
5.Premature ovarian failure treated with acupoint catgut implantation and artificial periodic therapy: a randomized controlled trial.

OBJECTIVETo compare the difference in the clinical efficacy on premature ovarian failure (POF) between the acupoint catgut implantation combined with artifical periodic therapy and the simple artificial periodic therapy and explore its effect mechanism.
METHODSSixty-five patients of POF were randomized into the two groups. In a western medication group, 32 cases were treated with the artificial periodic therapy with the oral administration of medroxyprogesterone acetate tablets. In a catgut implantation + western medication group, 33 cases were treated with the acupoint catgut implantation combined with artificial periodic therapy. The acupoints of Neiguan (PC 6), Zusanli (ST 36), Guanyuan (CV 4) and Sanyinjiao (SP 6) were selected. The treatment was lasted for half a year and the follow-up visit was for another half a year in the two groups. Kupperman index was used to assess the improvements in the clinical symptoms. The levels of serum sexual hormones such as follicle stimulating hormone (FSH) and estradiol (E2) were evaluated of the patients in the two groups before and after treatment. The efficacy was compared between the two groups.
RESULTSThe scores of the clinical symptoms were all significantly improved after treatment and in the follow-up in the two groups (P < 0.01, P < 0.05). In the 6-month follow-up visit after treatment, the result in the catgut implantation + western medication group was better than that in the western medication group (8.17 +/- 1.19 vs 13.68 +/- 1.08, P < 0.01). FSH was reduced after treatment in the two groups (all P < 0.01) and E2 was increased (all P < 0.05). The curative and remarkably effective rates were 75.8% (25/33) and 81.8% (27/22) after treatment and in the follow-up visit in the catgut implantation + western medication group, which were better than 67.9% (19/28) and 53.6% (15/28) in the western medication group separately (P < 0.05, P < 0.01).
CONCLUSIONThe acupoint catgut implantation combined with artificial periodic therapy achieve the remarkable improvements in the clinical symptoms of POF in the patients and the better results as compared with the simple western medication therapy. The combined therapys efficacy is stable and the long-term efficacy is apparently superior. The effect mechanism is related to the improvements in the serum sexual hormone levels.

8.Status Analysis and Countermeasures for Service of TCM Non-communicable Chronic

Objective To provide new ideas for establishment of prevention and control strategy for non-communicable chronic disease (NCD) that are suitable for Chinese people. Methods Sampling survey of patients with chronic disease from 14 provinces combined with literature study and researches on national statistical data were conducted to investigate the application situation and problems of traditional Chinese medicine in the prevention and control of NCD. Results Status of utilization and satisfaction of traditional Chinese medicine in the prevention and control of NCD has developed well. The main problems in the promotion and application of TCM lie in insufficient investment, lagging behind of construction of prevention and control system, insufficient professionals and ineffective heritance of practical technique. Conclusion In order to tackle these problems, relevant laws and regulations should be implemented;information management system with TCM features should be improved;construction of diversified technological innovation system should be reinforced, practical and strong technology should be promoted;construction of TCM prevention and control system for chronic diseases should be perfected.
9.Pedicle fixation combined with intervertebral fusion for lumbar spondylolithesis:whether restoring disc height affects the biomechanics and spinal function

BACKGROUND: According to the current most prevailed posterior pedicle screw reduction and Cage fusion for spondylolisthesis, the previous focus is to restore the vertebral sagittal displacement, and whether restoring disc height is needed and the degree of recovery remain unclear. Moreover, there is still a lack of better quantitative indicators to restore the disc height.OBEJCTIVE: By comparing the surgery of lifting reduction, to study the clinical effect of distraction and lifting reduction combined with posterior intervertebral fusion in the treatment of lumbar spondylolisthesis. METHODS: Sixty patients with L5 spondylolisthesis located in L5/S1 were selected, and were divided into two groups based on the surgery methods (n=30 per group): group A (single lifting reduction group) and group B (lifting and distracting reduction group). The patients in the group B were given distracting reduction amid insertion of 12 mm-height Cage; while the patients in the group A were subjected to lifting reduction, and intervertebral fusion with 8 mm-height Cage. The general information and the Visual Analogue Scale, Japanese Orthopaedic Association and Oswestry Disability Index scores at baseline, postoperative 1 month and last follow-up, as well as the postoperative correction and bone fusion were recorded. Additionally, the clinical effectiveness was assessed by Macnab score. RESULTS AND CONCLUSION: (1) At postoperative 1 month and last follow-up, the disc height, disc height/vertebral height, neuroforamen height and clinical effectiveness in the group B were superior to those in the group A (P < 0.05). (2) To conclude, the pedicle screw system combined with posterior intervertebral cage fusion for spondylolisthesis, can rapidly restore the disc height, contribute to restructure better biomechanics of spine. Furthermore, it significantly improves clinical systems, is easy to operate, and achieves better efficacy; therefore, it is recommended firstly.
10.Relationship between the morphological and functional alterations of pancreatic islets and peripheral insulin resistance in rats with long-term high-fat diet

Objective To investigate the effect of long-term high-fat diet on insulin resistance and the morphology and function of islets in rats and the relationship between them.Methods Thirty normal male Wistar rats (8 weeks old) were divided into two groups and fed either with normal chow (NC,n=15),or high-caloric and high-fat diet (HF,n=15).Insulin resistance was assessed by euglycemic hyperinsulinemic clamp technique. The insulin secretory function of islets was evaluated by intravenous insulin releasing test.Morphological and quantitative analysis of pancreatic tissues was performed by double-label insulin and glucagon immunohistochemistry.Proinsulin mRNA was detected by RT-PCR.Results The glucose infusion rate (GIR) in HF rats was significandy lower than that in NC rats [(5.83?0.79)mg?kg~(-1)?min~(-1) vs (7.60?1.29)mg?kg~(-1)?min~(-1),P<0.05].Immunohistochemistry showed that HF rats had larger islet size [(15168?1327)?m~2 vs (6264?1840)?m~2,P<0.01] and significantly reduced insulin relative concentration of?cells[(-5.15?0.03) vs (-4.81?0.17),P<0.01],as compared with NC rats.The islet relative?cell volume was decreased signifieandy (P<0.01),whereas the relative?cell volume was increased (P<0.01).So the ratio of?to?were lower in HF [(4.68?1.01) vs (11.84?3.82),P<0.05].The peak of insulin secretion in intravenous insulin releasing test in HF was at 10 min,whereas that in NC rats was at 5 min.AUC (area under curve) 10-60 rain of insulin in HF was higher than that in NC rats [(152.51?34.53)mIU?L~(-1)?min~(-1) vs (86.40?21.21) mIU?L~(-1)?min~(-1),P<0.01].There was no difference in proinsulin mRNA levels between two groups. Conclusion Long-term high-caloric and high-fat diet results in early impairment of islet morphology and function, as well as significant insulin resistance,suggesting that the compensation ability of islets has already been impaired in the early course of type 2 diabetes mellitus.
