OBJECTIVETo explore effective approaches of treating elderly patients with distal tibiofibular fractures.
METHODSFrom August 2008 to October 2012,175 elderly patients with distal tibiofibular fractures were treated with locking compression plate (LCP) through anterior tibial. There were 112 males and 63 females with an average of 71.3 (ranged 60 to 83) years old. Of them,89 cases were treated by anterior tibial tension reduced incision with LCP,including 62 males and 27 females with a mean age of (71.8 +/- 6.4) years old. Eighty-six patients were treated by distal tibial incision with LCP,including 58 males and 28 females with a mean age of (70.3 +/- 6.7) years old. Swelling time, operation time, intraoperative blood loss, hospital stay, healing time, complications and AOFAS scores were compared between two groups after operation.
RESULTSSwelling time in anterior tension reduced incision with LCP and distal tibial incision with LCP was (5.6 +/- 1.3) and (9.7 +/- 2.1) days, healing time was (4.2 +/- 1.4) and (5.4 +/- 1.9) months, and complications were found 3 in tension reduced incision and 10 in distak tibial incision respectively;and all data shown statistically significant differences between two groups (P < 0.05). At 12 months after operation,AOFAS score was 89.0 +/- 9.7, 87.9 +/- 9.4; and there was no statistically significant difference between two groups (P > 0.05).
CONCLUSIONTension reduced incision through anterior tibial combined with locking compression plate fixation in treating elderly patients with distal tibiofibular fractures can provide good clinical effects with quick fracture healing and low complications.

AIM: To evaluate the efficacy and safety of two different surgical techniques on congenital cataract on children.METHODS: Twenty-two children (1-3 years old) with congenital cataract were randomly divided into two groups (group A and group B). With group A (10 patients, 20 eyes), we applied 23-gauge (23G) trans corneal limbus vitrectomy system to complete lens cortex gettering, posterior capsulotomy and anterior vitrectomy; With group B (12 patients, 24 eyes), we used the phacoemulsification I/A to complete lens cortex gettering, and performed anterior vitrectomy with anterior vitreous cutting instrument. After that, the differences in intraoperative and postoperative complications between two groups were compared. RESULTS:In group A, the width of corneal limbal incision was 0.6mm, the incision was self-sealing, and the anterior chamber was stable and iris did not prolapse during the surgery. In group B, the width of corneal limbal incision was 3mm, anterior chamber was unstable and intraoperative iris prolapse occurred in 14 eyes (58%). And the incision need to be stitched up after surgery. In the postoperative follow-up of 6-24 months (an average of 14 months), we found that corneal neovascularization did not occur in group A, while in group B, corneal neovascularization occurred in four eyes (17%); Other complications, such as posterior capsular opacification, retinal detachment, glaucoma, hypotony or endophthalmitis did not occur in either group.CONCLUSION: The 23G trans corneal limbus vitrectomy system used in pediatric cataract surgery is safer and more effective than phacoemulsification I/A. It is promising in treatment of congenital cataract on children.

Objective: To investigate the changes of gene expression in CD34+ hematopoietic stem and progenitor cells (HSPCs) under different growth environments. Methods: Umbilical cord blood mononuclear cells (UCB MNCs) were cultured in static and stirred systems. After 7 days of culture, CD34+ cells were isolated and total RNA was extracted. Gene expression patterns of CD34+ cells from fresh, static and stirred cultures were compared using differential display (DD). Results: 30 gene fragments displayed differential expression levels based on the conditions of DD. One of differentially expressed genes was identified as RAN, which is a member of oncogene RAS family. This gene may be associated with proliferation of hematopoietic cells. Conclusion: Different growth environments induced differential gene expression patterns of CD34+ HSPCs. These differentially expressed genes would give new insights into optimizing in vitro environments for expanding hematopoietic cells.

The association rate constant and dissociation rate constant are important parameters of the drug-cyclodextrin supermolecule systems, which determine the dissociation of drugs from the complex and the further in vivo absorption of drugs. However, the current studies of drug-cyclodextrin interactions mostly focus on the thermodynamic parameter of equilibrium constants (K). In this paper, a method based on quantitative high performance affinity chromatography coupled with mass spectrometry was developed to determine the apparent dissociation rate constant (k(off, app)) of drug-cyclodextrin supermolecule systems. This method was employed to measure the k(off, app) of meloxicam and acetaminophen. Firstly, chromatographic peaks of drugs and non-retained solute (uracil) on β-cyclodextrin column at different flow rates were acquired, and the retention time and variance values were obtained via the fitting the peaks. Then, the plate heights of drugs (H(R)) and uracil (H(M,C)) were calculated. The plate height of theoretical non-retained solute (H(M,T)) was calculated based on the differences of diffusion coefficient and the stagnant mobile phase mass transfer between drugs and uracil. Finally, the k(off, app) was calculated from the slope of the regression equation between (H(R)-H(M,T)) and uk/(1+k)2, (0.13 ± 0.00) s(-1) and (4.83 ± 0.10) s(-1) for meloxicam and acetaminophen (control drug), respectively. In addition, the apparent association rate constant (k(on,app)) was also calculated through the product of K (12.53 L x mol(-1)) and k(off, app). In summary, it has been proved that the method established in our study was simple, efficiently fast and reproducible for investigation on the kinetics of drug-cyclodextrin interactions.

The contents of schisandrol A, schisandrol B, schisantherin A, schisandrin A , schisandrin B, schisandrin C in Schisandrae Chinensis Fructus (SCF) were determined simultaneously by HPLC. Collect 100-seed weight, color, pulp content, longitude and latitude of SCF of different batches were collected. SIMCA-P and SPSS were applied to make PLS-DA analysis of 24 batches of SCF and correlation analysis of relevant parameters. According to the 13 parameters, SCF from three different places of origin could be distinguished effectively. It was found that the content of chemical component of SCF increased with latitude and longitude first, and then decrease. The results provide some theoretical basis for study of SCF genuineness and traditional method of identifying just from experience.
