1.Study on anti-cancer components of Fig residues with supper critical fluid CO2 extracting technique.

OBJECTIVETo optimize Supper Critical CO2 extracting technical (SFE-CO2) methods for extraction of anti-cancer active components of Fig Residues and to investigate the anti-cancer effect of the extract in vitro and in vivo.
METHODThe anti-cancer activity of extracted compound was measured on U937,95D and AGS cancer cells in vitro by MTT method. The anti-cancer effect of the extraction of Fig Residues was studied on mice transplant liver cancer in vivo.
RESULTThe SFE-CO2 condition for extraction of the anti-cancer components of Fig Residues was optimized as follows: granularity was 100, the extraction pressure was 30 MPa, the extraction temperature was 45 degrees C, the extraction time was 6 h and the CO2 flux was 12 L x h(-1); The IC50 of anti-cancer active components of Fig Residues on U937, 95D and AGS cells were 70.125 microg x mL(-1), 127.957 microg x mL(-1), 116.000 microg x mL(-1); The anti-cancer active components of Fig Residues inhibited 49.3% of the transplanted liver cancer in the mice.
CONCLUSIONThe method for extracting the anticancer active components of Fig Residues is stable and reasonable, and the extract from Fig Residues is of the anticancer effect.

2.Relationship between serum visfatin level and children and adolescent obesity and non-alcoholic fatty liver disease

Objective To examine the relationships between the level of serum visfatin and obesity,nonalcoholic fatty liver disease (NAFLD)in children and adolescents. Methods Serum visfatin levels of 106 obese children/adolescents (including 49 non-NAFLD children and adolescents and 57 NAFLD children/adolescents in which consisting of 42 mild ones,15 moderate-to-severe ones)with another 69 lean children/adolescents as their controls,were examined by indirect sandwich ELISA.Results When comparing the serum visfatin levels,there was no significant difference noticed between the obese group ( 1.71 ± 0.37) ng/ml and the lean group ( 1.75 ± 0.37) ng/ml (P=0.455).With the severity of NAFLD,the serum visfatin level showed an elevation in obese children and adolescents [ obese without NAFLD ( 1.59 ± 0.36) ng/ml,obese with mild NAFLD ( 1.74 ± 0.36) ng/ml,obese with moderate-to-severe NAFLD (1.97 ± 0.36) ng/ml,P<0.05].In the obese group,serum visfatin levels seemed to be related to age (^β=-0.326,P=0.000),alanine aminotransferase (ALT) (^β =0.286,P=0.004) and the degree of NAFLD (^β =0.246,P=0.014).Conclusion The level of serum visfatin was related to non-alcoholic fatty liver in obese children and adolescent.
3.Posterolateral autograft bone graft fusion transpedicular screw system internal fixation and anterior debridement in the treatment of dorsal and lumber spinal tuberculosis

ObjectiveTo evaluate the clinical effectiveness of posterolateral autograft bone graft fusion transpedicular screw system internal fixation and anterior debridement in the treatment of dorsal and lumber spinal tuberculosis. MethodsFrom March 1996 to July 2000,posterolateral autograft bone graft fusion transpedicular screw system internal fixation and anterior debridement procedures were used in 62 patients suffering from dorsal and lumber spinal tuberculosis in our department,48 of them were involved in a longitudinal study follow-up for a mean of 3.6(1.5-5.5)years postoperatively. ResultsAll patients showed successful posterolateral bone graft fusion. Among 38 cases of Pott's paraplegia, 30 were completely recovered,5 were partly recovered,the rate of recovery was 92.1%. The average immediate postoperative correction of kyphosis angle was 29.1°,the average loss of correction was only 3.2°at final follow-up.ConclusionsPosterolateral autograft bone graft fusion transpedicular screw system internal fixation and anterior debridement procedure were found helpful in strengthening the stability of the spine in dorsal and lumber spinal tuberculosis, providing successful interbody fusion and recovery of Pott's paraplegia, correcting the kyphosis, and preventing progression of kyphosis.
4.Effect of iptkalim on myocardial enzymes and free radicals metabolism with hypoxic pulmonary hypertension.

OBJECTIVETo explore the effects of iptkalim on myocardial enzymes and free radicals metabolism with hypoxic pulmonary hypertension (HPH), in order to provide evidence for the mechanism of iptkalim on clinical treat.
METHODS110 young men stayed at high altitude above 5 000 m were divided into iptkalim group (n = 74) and placebo group (n = 36), aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyltransferase (gamma-GT), creatine kinase (CK), lactate dehydrogenase (LDH), superoxide dismutase (SOD), malonaldehyde (MDA), nitric oxide(NO) and nitric oxide synthase(NOS) were detected before and after took medicines for 6 mouths.
RESULTSAfter took medication for 6 mouths, ALT, AST, gamma-GT, CK and LDH were reduced, SOD, NO, and NOS were increased, MDA were reduced, there were very significant difference (P < 0.05).
CONCLUSIONOxygen free radicals have taken part in the process of HPH, iptkalim have the effect of anti-peroxidation of lipid and protect myocardial cells stress injured by hypoxia which related with mitochondrial membrane and cell membrane's K(ATP) channel activation.

5.Dynamic changes of the epididymal size after vasectomy: An ultrasonographic observation.

OBJECTIVETo investigate the dynamic changes of the epididymal size 1 year after vasectomy.
METHODSFifty male volunteers received vasoligation. Before and at 1, 2, 3, 6, and 12 months after operation, we measured the size and detected the internal echoes of the epididymis using color Doppler ultrasonography.
RESULTSThe bilateral epididymides were both thickened post-operatively in all the 50 cases, with statistically significant differences between the baseline and the 1st month, the 1st and the 2nd month, the 2nd and the 3rd month, or the 3rd and the 6th month after surgery (all P < 0.01), but not between the 6th and the 12th month (P > 0.05).
CONCLUSIONWithin 6 months after vasectomy, the bilateral epididymides manifested a progressive thickening, but basically restored their balance of secretion-absorption after 6 months.

6.Variety of ESR and C-reactive protein levels during perioperative period in spinal tuberculosis.

OBJECTIVETo investigate the clinical significance of erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) in the treatment of spinal tuberculosis.
METHODSSixty-seven patients (41 males and 26 females, ranging in age from 23 to 61 years) with active spinal tuberculosis in our hospital (from Mar. 2004 to Mar. 2007) were included in this study. The tuberculosis focus were located either in cervical spine, thoracic spine or in lumbar spine. After 4 to 6 weeks anti-tuberculosis chemotherapy, all the patients underwent one-stage operation (focus debridment) and auto-bone graft combined with internal fixation. Blood test for ESR and CRP were carried out at different times before and after operation.
RESULTSThe average ESR was (79.4 +/- 35.6) mm/h, and the average CRP was (44.3 +/- 17.5) mg/L before chemotherapy, indicating active tuberculosis focus. After 4 to 6 weeks chemotherapy, the average ESR was (45.3 +/- 21.0) mm/h,and the average CRP was (26.7 +/- 11.8) mg/L, the differences were statistically (P < 0.05), and the clinical symptoms of spinal tuberculosis relieved in all patients. Four weeks after operation, the average ESR dropped to (42.8 +/- 16.5)mm/h, the average CRP dropped to (23.8 +/- 10.0) mg/L statistically (P < 0.05). Eight weeks after operation, the average value of ESR and CRP were at normal level in 47 cases, indicating inactive tuberculosis focus. Focus healing was achieved in 65 patients after short-term chemotherapy.
CONCLUSIONThe level of ESR and CRP are high in active spinal tuberculosis and low when focus controlled. ESR and CRP are reliable parameters in evaluation the treatment and prognosis of spinal tuberculosis.

7.Clinical outcome of single-bundle versus anatomic double-bundle reconstruction of the anterior cruciate ligament: a meta-analysis.

OBJECTIVETo evaluate clinical outcome after anterior cruciate ligament (ACL) reconstruction with double-bundle or single-bundle by meta-analysis.
METHODSRandomized controlled trials (RCTs) on differences of clinical outcomes of ACL reconstruction were retrieved in Ovid Medline, PubMed, Embase, Cochrane Library, CBM and VIP database. Relevant journals or conference proceedings were also searched manually. Then extracted the date of KT-1000 arthrometer, pivot-shift testing, Lysholm score and IKDC final score in these researches. RevMan 5.0.23 software was used for data analyses.
RESULTSEight prospective RCTs met the inclusion criteria. The combined results of meta-analysis indicated that there was statistical difference between two operative procedures on postoperative KT-1000 arthrometer side-to-side [WMD = -0.35, 95%CI (-0.61, -0.08), P = 0.01], Lysholm score [WMD = -1.91, 95%CI (-3.45, -0.37), P = 0.01]. But the difference of KT-1000 arthrometer side-to-side is demonstrated to be clinically insignificant. Others indicated that there was no statistical differences with respect to IKDC final score [OR = 1.80, 95%CI (0.98, 3.31), P = 0.06], having a normal or nearly normal pivot-shift testing [OR = 1.64, 95%CI (0.85, 3.16), P = 0.14].
CONCLUSIONSDouble-bundle reconstruction does not result in clinically significant advantage when compared with single-bundle. The results do not support the theory that double-bundle reconstruction controls knee rotation better.

8.Surgical treatment of drug-resistant spinal tuberculosis.

OBJECTIVETo study the curative effect of surgical treatment of drug-resistant spinal tuberculosis.
METHODSFrom March 2005 and April 2009, the clinical data of 60 patients with drug-resistant spinal tuberculosis were retrospectively analyzed. Including 36 males and 24 females; aged from 5 to 79 years with an average of 47.3 years. Thirty-four patients had neurological deficits, among them, 2 cases were grade A, 5 cases were grade B, 13 cases were grade C, 14 cases were grade D according to ASIA standard. According to the severity and location of the infection, the patients underwent anterior, posterolateral costotransversectomy or posterior debridement and bone grafting and internal fixation. The antituberculous chemotherapy for a total of 12 to 18 months was guided by conventional and genotypic drug susceptibility testing. Tubercular relapse, neurological function, spinal fusion were observed by ASIA grade, X-ray and CT scan.
RESULTSAll cases were followed up from 1 to 5 years with an average of 3.1 years. Recurrence was found in 2 cases who were cured after second operation. 34 cases with neurological deficits recovered totally or partially. X-ray or CT films showed spinal fusion in 57 patients.
CONCLUSIONThe therapeutic effect of individuall operative options is good in treating drug-resistant spinal tuberculosis after antituberculous chemotherapy based on conventional and genotypic drug susceptibility testing.

9.Surgical treatment of lower lumbar tuberculosis with different operative procedures.

OBJECTIVETo evaluate the surgical indications and clinical effectiveness of different operative procedures in the treatment of lower lumbar tuberculosis.
METHODSFrom June 2001 to Oct 2008, 72 patients with lower lumbar tuberculosis were treated by different operative procedures. Including 38 males and 34 females, with an average age of 38.5 years old ranging from 16 to 70 years. The average duration of symptom was 6.8 months (ranging from 4 months to 2 years). A single vertebrae was involved in 10 patients,two contiguous vertebrae in 50 cases and three vertebrae in 12 cases. The average preoperative lordotic angle was 13.1 degree (ranging from -5.0 degrees to 34.0 degrees). Three different operative procedures included: (1) posterior debridement and posterolateral fusion and posterior instrumentation in 28 patients; (2) anterior radical debridement and anterior fusion and anterior instrumentation in 32 patients; (3) image-guided percutaneous drainage (PCD) of tuberculous abscesses in 12 patients. The selection of the procedure was made according to the degree of the lesions. The resolution of inflammatory process, bony fusion, correction of sagittal angles and JOA scores were used for evaluating the result of the surgery and the complications were analyzed.
RESULTSAll patients were followed up from 1.5 to 8.0 years (means 3.6 years). PCD was an effective treatment in 11 out of the 12 patients, one required surgical debridement and fusion. Among them, 57 (95%, 57/60) patients were treated by open operation showed successful bony fusion. The complications maily included common iliac vein injury in 3 patients, dural tear in 2 patients, they were all cured by intro-or postoperative treatment. The average immediate post-operative lordotic angle was 27.3 degree (35.0 degrees to 16.0 degrees), the average lordotic angle was 25.6 degree (33.0 degrees to 15.0 degrees) at final follow-up. Preoperatively and at final follow-up, JOA scores were respectively (15.2 +/- 3.4), (25.6 +/- 2.4) (P<0.01).
CONCLUSIONDifferent operative procedures should be selected to treat lower lumbar tuberculosis according to the degree of lesions. Aggressive surgical treatment was found helpful in the resolution of inflammatory process and correcting the loss of lordosis, preventing progression of kyphosis.

10.Association between sleep duration and puberty blood pressure among students aged 9 to 17 years in China.

OBJECTIVETo explore the association between sleep duration and the level of puberty blood pressure of students aged 9 to 17 years in China.
METHODSThe data were from physical fitness and health research of chinese school students in 2010. A total of 123 919 boys aged 11 to 17 years and girls aged 9 to 17 years were included in our study, whose record of height, weight, blood pressure, sleep duration and first spermatorrhea/menarche were performed. Students were grouped as relative enough sleep duration group and short sleep duration group by their sleep duration reported. The level of blood pressure, as well as the prevalence of high blood pressure (HBP), was compared between different sleep duration groups, and multilevel model was used and analyzed the relationship between sleep duration and blood pressure.
RESULTS123 919 students were surveyed in this study, including 52 549 boys and 71 370 girls. The mean blood pressure of boys was 109.41/67.05 mm Hg (1 mm Hg = 0.133 kPa), 30 830 boys (58.7%) experienced first spermatorrhea, and 18 692 boys (35.6%) were defined as relative enough sleep duration. The mean level blood pressure of girls surveyed was 103.65/65.12 mm Hg, 46 031 girls (64.5%) experienced menarche, and 23 143 girls (32.4%) were defined as relative enough sleep duration. Among the boys without first spermatorrhea, 7481 boys (34.4%) were defined as relative enough sleep duration, whose mean blood pressure was 105.25/65.01 mm Hg and prevalence of HBP was 4.3% (325/7481), and 14 238 boys (65.6%) were defined as short sleep duration, whose mean blood pressure was 105.9/65.27 mm Hg and prevalence of HBP was 5.6% (802/14 238), boys with relative enough sleep duration showed a significant lower blood pressure level and the prevalence of HBP compared with boys with short sleep duration (F = 15.38, P < 0.01;χ(2) = 16.55, P < 0.01). Among girls without menarche, 9311 girls (36.7%) were defined as relative enough sleep duration, whose mean blood pressure was 100.47/63.25 mm Hg and prevalence of HBP was 2.9% (271/9311), 16 028 girls (63.3%) were defined as short sleep duration, whose mean blood pressure was 101.06/63.57 mm Hg and prevalence of HBP was 3.3% (536/16 028), girls with relative enough sleep duration showed a lower level of blood pressure compared with girls with short sleep duration (F = 17.44 and 7.11, respectively, all P values < 0.01). Among girls experienced menarche, 13 832 girls (30.0%) were defined as relative enough sleep duration, whose mean blood pressure was 105.27/66.19 mm Hg and prevalence of HBP was 6.25% (864/13 832), and 32 199(70.0%) girls were defined as short sleep duration, whose mean blood pressure was 105.12/66.19 mm Hg and prevalence of HBP was 6.0% (1924/32 199), girls with relative enough sleep duration showed significant higher level of blood pressure than girls with short sleep duration (F = 5.98, P = 0.015). After adjustment of region, BMI, height, age and urban/rural area by multilevel model, the level of systolic pressure decreased 0.18 mm Hg with 1 h increase of sleep duration among boys without first spermatorrhea (Wald χ(2) = 7.27, P = 0.007), the level of diastolic pressure decreased 0.13 mm Hg with 1 h increase of sleep duration among girls without menstruation (Wald χ(2) = 5.52, P = 0.019), and the systolic pressure and diastolic pressure increased 0.19 mm Hg and 0.14 mm Hg with 1 h increase of sleep duration among girls with menstruation, respectively (Wald χ(2) = 13.47 and 9.94, respectively, all P values < 0.01).
CONCLUSIONShort sleep duration may be one of the influential factors of HBP during puberty.
