1. Intervention and prognostic study in patients with severe stroke complicated with plasma hyperosmolality

Objective: To explore the clinical value in the intervention of patients with severe stroke complicated with plasma hyperosmolality. Methods: Th irty-four patients with severe stroke (Glasgow coma score [GCS] ≤12 on admission), who complicated with plasma hyperosmolality were selected. The changes of plasma osmotic pressure before and after the intervention and its related factors, as well as the effect of interventional therapy on prognosis were observed dynamically by controlling fluid replacement, diuretic, and blood sugar. Results: The most common interventional measures for plasma hyperosmolality were to control the fluid replacement (85.3%), the secondly to control the dosage of osmotic diuretic (41.2%) and blood glucose level (41.2%. Before the intervention, plasma osmotic pressure, natrium intaking and blood glucose level were (321 ± 10), (146 ± 5), and (9.5 ± 2.8) mmol/L, respectively; after the intervention, they were reduced to (308 ± 18), (142 ± 7), and (7.5 ± 3.1) mmol/L, respectively. The in-hospital mortality in patients who were intervened successfully (plasma osmotic pressure reduced to normal levels) in hyperosmolality group was 15.8%, and it was significantly lower (P < 0.01) than that in patients (100%) who were intervened unsuccessfully (plasma osmotic pressure did not reduced to normal levels). Multifactor logistic regression analysis showed that the major risk factors that influenced the in-hospital mortality were the increase of plasma osmotic pressure, and the increase of acute physiology and chronic health evaluation II (APACHE II) scores. Conclusion: Reducing the plasma osmot ic pressure may improve the prognosis in patients with severe stroke complicated with plasma hyperosmolality, and reduce the in-hospital mortality.
2.Establishing the System of Academic Assessment and Evaluation on Introduced Talented Personnel

Objective To evaluate the system of academic assessment and evaluation on introduced talented personnel so as to establish a consummate talented personnel introduction mechanism and to further standardize the introduction work. Methods On the basis of referring to document papers and soliciting management as well as human resource experts iteratively,the research method of expert consultation was adopted.(Results)The index system of academic assessment and evaluation on introduced talented personnel in school of medicine was preliminary established,and the objective index and subjective evaluation were scientifically combined to form the method of academic assessment and evaluation on introduced talented personnel. Conclusion Through simulative assessment and evaluation as well as probational demonstration,there exits definite scientificality and applicability on this academic assessment and evaluation index system,and it can provide a properly objective evaluation and essential reference foundation in the aspect of introducing more advanced talented personnel in the near future.

5.Effect of Xiaoyao Powder Combined with Prozac on Post-stroke Depression

ObjectiveTo observe the effect of Xiaoyao powder combined with prozac and psychological therapy on post-stroke depression (PSD).Methods85 PSD patients were randomly divided into the treatment group (n=43) and control group (n=42). All patients were treated with routine therapy, including prozac and psychological therapy. Patients in the treatment group were also given Xiaoyao powder. Scores of Hamilton Depression Scale (HAMD), modified Barthel index (MBI) and Scandinavian Stroke Scale (SSS) of all patients were evaluated before and after therapy.ResultsAfter treatment, the effective rate and MBI scores were significantly higher and scores of HAMD and SSS were significantly lower for patients of the treatment group compared with those of the control group (P<0.01).ConclusionXiaoyao powder combined with prozac and psychological therapy can improve depression and neural function of PSD patients significantly.
6.Evaluation of dose-related effects of 2', 3', 5'-tri-O-acetyl-N6-(3-hydroxylaniline)adenosine using NMR-based metabolomics.

2', 3', 5'-Tri-O-acetyl-N6-(3-hydroxylaniline)adenosine (WS070117) is a derivative compound of natural product cordycepin. It has significant lipids regulating activity and low toxicity which has been proved by in vitro and in vivo experiments. In this study, 1H NMR-based metabolomics was used to investigate the dose-related effects of WS070117 on hyperlipidemia of high-fat-fed hamsters. The hyperlipidemic hamsters were administrated with six different doses of WS070117, including 3, 12, 50, 100, 200 and 400 mg x kg(-1) x d(-1). 1H NMR spectra of hamster serum were visually and statistically analyzed using two multivariate analyses: principal component analysis (PCA) and orthogonal partial least squares-discriminant analysis (OPLS-DA). As a result, WS070117-treated groups showed dose-related regulation of metabolites associated with lipid metabolism, choline metabolism and glucose metabolism. The dose of 3 mg x kg(-1) x d(-1) of WS070117 only exhibited a little lipids regulating activity. However, the doses of 12 and 50 mg x kg(-1) x d(-1) of WS070117 both regulated the contents of metabolites to reverse significantly toward normal levels. When the dose of WS070117 reached 100 mg x kg(-1) x d(-1), it was more effective than positive control drugs. The work suggested that NMR-based metabolomics might be a valuable approach to evaluate dose-related effects of lipids regulating compounds.

7.Effect of two different body positions on the scalp acupuncture among infant

Objective to explore the best bady position for the scalp acupuncture among infant.Methods A total of 160 cases in out patients received scalp acupuncture were recruited and randomly divided into the experiment group ( n =80 ) and control group ( n =80 ).Experiment group adopted the type of baby cartier and control group adopted the type of parents' arms encircle when they received the scalp acupuncture.Then,the success rate of one-time puncture,the repeated puncture times among infant that fluid volume was 50 ml and the satisfaction of patients' family on the nursing skill in two groups were observed and compared.Results The success rate of one-time puncture of experiment group was 90.00% and 87.50% of the control group,and the difference was statistically significant( P >0.05).The rate of repeated puncture of experiment group was 11.25% which was significantly lower than 27.50% of the control group( x2 =0.652,P < 0.01 ),and the satisfaction of patients' family on the nursing skill of experiment group was 95.00% which was significantly higher than 86.25% of the control group( x2 = 8.721,P < 0.01 ).Conclusions The position of baby cartier is better than the position of parent' s arms encircle when infant receive the scalp acupuncture,which can be popularized in clinic.
8.BmK I, an alpha-like scorpion neurotoxin, specifically modulates isolated rat cardiac mechanical and electrical activity.

In this study, cardiotonic and cardiotoxic effects of Buthus martensi Karsch (BmK) I, a modulator of voltage-gated sodium channels, were investigated on the isolated rat hearts. The results showed that BmK I evoked complex effects characterized by a change in both cardiac mechanical and electrical activity. Langendorff perfusion showed that: (1) maximal left ventricular developed pressure (LVDP(max)) and dp/dt(max) were markedly increased by BmK I (0.5-10 micromol/L) in a dose-dependent manner (n=6, P<0.05), positive chronotropic effects were also induced by BmK I (n=6, P<0.05); (2) negative inotropic action and bradycardia could be elicited at a larger dose of BmK I (20 micromol/L); (3) the coronary flow varied inversely with the positive inotropic effects, coronary flow reduced during positive inotropic effects from 14.5 to 8.6 ml/min after administration of 500 nmol/L BmK I (n=6, P<0.05). In addition, tachycardia and complex cardiac arrhythmias were induced by BmK I (0.5-10 micromol/L). The modulating of BmK I on the heart mechanical, electrical activity could be partially recovered after washing. As propranolol was applied to block the release of catecholamines before administration of BmK I, suggesting that the changes in cardiac mechanical and electrical activity induced by BmK I might not due to catecholamine release from the nerve terminal and subsequent stimulation of the beta-adrenoceptor but attributable to the modulation of BmK I on cardiac voltage-gated sodium channels.
