1.Clinical efficacy and safety evaluation of enalapril or irbesartan combined with hydrochlorothiazide in the treatment of medium or severe hypertension

Objective To evaluate the clinical efficacy and safety of enalapril or irbesartan combined with hydrochlorothiazide in the treatment of medium or severe hypertension.Methods Sixty -nine patients with moderate to severe essential hypertension were randomly divided into control group ( n=34) with enalapril 10 mg +hydrochlorothiazide 12.5 mg orally bid and treatment group ( n =35 ) with irbesartan 50 mg +hydrochlorothiazide 12.5 mg orally bid with 2 months.After treatment, the clinical efficacy, serum level of creatinine, acidum uricum and potassium and the ratio of adverse event were compared between the two groups. Results After 2 months treatment, the blood pressure was significant decreased in control and treatment groups. The clinical efficacy were 88.24% and 88.57% with not statistical difference ( P >0.05 ) .The serum level of creatinine, acidum uricum and potassium had no obvious difference before and after the treatment in the two groups (P>0.05).Four cases of cough were observed in the control group and the treatment group, respectively, without statistical difference (P>0.05).Conclusion There was no clinical efficacy difference for enalapril or irbesartan combined with hydrochlorothiazide in the treatment of medium or severe hypertension.But irbesartan combined with hydrochlorothiazide had less cough adverse event.
2.Biomechanical comparison of Evans procedure and Chrisman-Snook technique for the treatment of II degree lateral collateral ligament of ankle joint.

OBJECTIVETo measure the stability of Evans procedure and Chrisman-Snook technique in the treatment of II degree lateral collateral ligament of ankle joint, and provide basis for treatment and prognosis.
METHODSFrom July 2008 to June 2009,18 frozen corpes were collected, including 10 males and 8 females, with an average age of fresh 39.3 +/- 11.2 years. The frozen corpes were randomly divided into three group, including normal controls(group A), Evans procedure (group B) and Chrisman-Snook technique ( group C), 6 specimens in each group. Anterior talofibular ligament and calcaneofibular ligament were cut off to cause II degree lateral collateral ligament in group B and C. Evans procedure or Chrisman-Snook technique were applied to restore lateral collateral ligament, and measure biomechnics. The displacement of tibiotalar joint and subtalar joint were observed.
RESULTS(1) The lateral stress results of tibiotalar joint showed the displacement by Evans procedure (group B) was greater than other groups (P < 0.0001). There were no significant differences between group A and C (P > 0.05). (2) The lateral stress results of subtalar joint showed the displacement by Evans procedure (group B) was greater than other groups (P< 0.0001). There were no significant differences between group A and C (P > 0.05).
CONCLUSIONAnkle instability is caused by ankle joint lateral collateral ligament injury. Chrisman-Snook technique is better than Evans procedure in stability on the early stage of ankle joint restoration, and conform to principle of biomechanics.

4.Clinical observation and nursing about treatment of Peptic Ulcer by Xiaokuiping decoction of Chinese Medicine

Objective To explore the effect and the nursing interventions of Chinese Medicine,namely Xiaokuiping decoction,on patients with gastric ulcer and duodenal ulcer.Methods 85 patients with Peptic Ulcer were divided into treatment group (53 patients) and the control group (32 patients) at random.The patients of these two groups both took Omeprazole,Amoxicillin and Clarithromycin.The treatment group patients add to Xiaokuiping decoction.The treatment lasted 4 weeks.After the treatment,the difference in therapeutic efficacy. Recurrence rate and the eradication rate were observed between groups.Results The therapeutic efficacy,the recurrence rate and the eradication rate in the above two groups showed statistical significance(P<0.05).Conclusions The therapeutic efficacy of Chinese medicine Xiaokuiping decoction together with Western medicine in healing gastric ulcer and duodenal ulcer is better than that of only using Western medicine.For the treatment group patients,the Hp eradication rate is higher and the recurrence rate is lower than that of the control group tients.Moreover,guiding the patients to take the decoction properly results in better effect.
5.The effect and mechanism of Yiqi Tongluo Jiedu capsule against cerebral ischemia reperfusion injury.

To observe the effect and mechanism of Yiqi Tongluo Jiedu capsule aganist cerebral ischemia reperfusion injury, the SD rats were randomly divided into following groups: sham-operated group, model group, the group of low, medium and high dose of Yiqi Tongluo Jiedu capsule, and nimodipine group. Using focal middle cerebral artery embolization (MCAO) model, following items were observed: symptoms of neurological deficit score; infarct volume; activity of SOD, content of MDA and NO, activity of NOS of ischemic brain tissue; Bcl-2 and Bax protein expression; content of IL-1beta, IL-6 and TNFalpha in serum; IL-1beta mRNA expression of ischemic brain tissue. Results showed that Yiqi Tongluo Jiedu capsule could significantly reduce the symptoms of neurological deficits, promote the recovery symptoms of neurological deficits; narrow infarct volume of brain tissue obviously, reduce the percentage of infarct volume; raise activity of SOD, reduce content of MDA and NO, reduce activity of NOS; increase Bcl-2 protein, reduce Bax expression; reduce content of IL-1beta, IL-6 and TNFa in serum; reduce IL-1beta mRNA expression of ischemic brain tissue. Yiqi Tongluo Jiedu capsule has significant protective effects against ischemic brain injury, it has significant anti-apoptotic, antioxidant and anti-inflammatory effects.

6.Dietary patterns and its influencing factors among freshmen students in college

Objective To investigate the dietary pattern in college freshmen students and to analyze the influencing factors on their dietary patterns. Methods A questionnaire survey on situation of dietary pattern and influencing factors was conducted among 1319 freshmen students.Results Four major dietary patterns were noticed and they were: Ⅰ , high consumption in hamburger,fried food, nuts, biscuit, chocolate, cola, coffee, sugars, Ⅱ, high consumption in pork, mutton, beef,poultry meat, animal liver, Ⅲ, high consumption in fresh fruits, eggs, fish and shrimps, kelp laver and sea fish, milk and dairy products, beans and bean products, Ⅳ, high consumption in rice and grain,fresh fruits, fresh vegetables, pork. Risk factors on dietary pattern were presented as follows: ( 1 )boys: having the food pattern Ⅰ and Ⅱ showed a strong positive association with the place where they live (OR= 1.67, 95%CI: 0.87-3.19; OR= 1.51,95%CI: 0.79-2.88), eating place (OR=1.63,95%CI: 1.O3-2.59; OR= 1.83, 95%CI: 1.04-3.23), level of mother' s education (OR=2.52,95%CI: 1.07-5.95; OR=3.38, 95%CI: 1.50-7.63), family income (OR=2.24, 95%CI: 1.30-3.88;OR=3.06, 95% CI: 1.77-5.29) and the status of passive smoking (OR= 1.80, 95%CI: 0.70-4.59;OR=1.83, 95%CI: 0.75-4.45). Inverse correlations was found on the level of mother's education (OR=0.56,95%CI: 0.17-1.79). The food pattern Ⅳ showed a strong positive association with place of eating(OR= 1.83,95%CI: 1.04-3.23) but having an inverse correlation with the level of mother's education (OR=0.56, 95%CI: 0.17-1.79). (2)girls when compared with boys, the food pattern Ⅰ showed minor association with the places of living and eating; while the food pattern Ⅱ and Ⅲ had minor association with the status of passive smoking. Conclusion Socio-demographic factors and lifestyle had influenced on the dietary patterns among college freshmen students who should be guided to have a reasonable, balanced diet in the college.
7.Automated ribotyping of Salmonella and Staphylococcus aureus in food poisoning of Guangdong province

Objective To understand the genetic polymorphism of Salmonella and Staphylococcus aureus in Guangdong province, as well as to explore methods for identifying and tracing the source of these two foodbome pathogens. Methods Using the automated ribotyping system, two foodbome pathogens were tested with either EcoR Ⅰ or Pvu Ⅱ restriction enzymes. BioNumerics software was then applied for image analysis, database establishment and other corresponding analysis. Results Digestion of 32 Salmonella isolates with Pvu Ⅱ yielded 19 different ribotypes,and digestion of 14 Salmonella isolates with EcoR Ⅰ yielded 2 different ribotypes. Staphyloccus aureus isolates showed greater genetic diversity, whereas EcoR Ⅰ digestion of 49 different isolates yielded 31 different ribotypes. Conclusion Unique Salmonella and Staphylococcus aureus isolates could be identified through ribotyping. Although Salmonella serotyping and ribotyping were not strongly correlated, the combination of both restriction enzymes could be used to more effectively identify the genetic relationship among different strains as well as the source of food poisoning. Thus, not only could the genetic relationships amongst the different strains be inferred through ribotyping skills, the source of food poisoning and mode of transmission could also be determined under the use of this method.
8.Combination of phenylbutyrate and 5-Aza-2'deoxycytidine inhibits human Kasumi-1 xenograft tumor growth in nude mice.

OBJECTIVETo investigate the tumor suppression efficacy of histone deacetylase inhibitor, phenylbutyrate (PB), in combination with DNA methylation inhibitor 5-Aza-2-deoxycytidine (5-Aza-CdR) in the treatment of Kasumi-1 xenograft tumor in nude mice and its mechanism.
METHODSThe nude mice model of Kasumi-1 xenograft tumor was established by subcutaneous inoculation. Latency of tumor formation, the ability of Kasumi-1 cells pre treated with PB to form the xenograft tumor, and the tumor suppression activity of PB and 5-Aza-CdR by intraperitoneal injection in xenografted mice model were detected. Cell differentiation and cell cycle parameters of the tumor cells were analyzed by flow cytometry analysis, apoptosis by TUNEL in situ hybridization, and tumor microvessel density (MVD) by immunohistochemistry study.
RESULTSThe latency of tumor formation in mice with or without previous lienectomy was 17 approximately 23 and 40 approximately 50 days, respectively. Tumor cells xenografted could not be found in other tissues than in inoculation area, and still harbored the specific t(8;21) and AML1-ETO fusion gene. When the xenografted mice models treated with PB, 5-Aza-CdR, or both, the tumor growth inhibition rates were 49.07%, 25.69% and 87.46% (P < 0.05), the apoptosis indexes (AI) of tumor cells were (2.25 +/- 0.85)%, (1.32 +/- 0.68)%, and (5.41 +/- 1.56)% (P < 0.05), and the microvessel densities (MVD) were 21.69 +/- 6.25, 28.34 +/- 4.24 and 9.48 +/- 3.21 (P < 0.01), respectively. All the data above were significantly different from that in control (P < 0.05). The expression of CD11b and CD13 antigen of the tumor cells was increased in xenografted mice model treated with PB when compared with the control \[(12.08 +/- 1.02)% and (54.91 +/- 2.72)%\], respectively (P < 0.01), and tumor cells showed a cell cycle arrest with increased G(0)/G(1)-phase cells and decreased S-phase cells.
CONCLUSIONPB inhibited the growth of Kasumi-1 xenograft tumor by inducing tumor cell apoptosis and differentiation, and suppressing its angiogenesis in vivo. 5-Aza-CdR could significantly enhance the antitumor activity of PB.

9.Progress on molecular biology of delta6-fatty acid desaturases.

Polyunsaturated fatty acids (PUFAs) including gamma-linolenic acid are valuable products because of their involvement in several aspects of human health care. GLA has been claimed to play a crucial role in development and prevention of some skin diseases, diabetes, reproductive disorder and others. At present, market demand for most gamma-linolenic acid is growing continually and current sources are inadequate for satisfying this demand due to the significant problems of low productivity, complex and expensive downstream process and unstable quality. Therefore, seeking for alternative sources are demanding. delta6-fatty acid desaturase is the rate-limiting enzyme for the biosynthesis of PUFAs, which catalyses the conversion of linoleic acid and alpha-linolenic acid to gamma-linolenic acid and stearidonic acid respectively. Unfortunately, the structure information on membrane desaturases is scarce because of the technical limitations in obtaining quantities of purified protein and the intrinsic difficulties in obtaining crystals from membrane proteins. With the isolation of the genes coding for delta6-fatty acid desaturase from various organisms, its characteristics will be elucidated gradually. Here we concisely reviewed the recent progress on studies of molecular biology including the cloning of delta6-fatty acid desaturase gene, structure and function, phylogeny and prospects of gene engineering application.

10.Design space approach to optimize first ethanol precipitation process of Dangshen.

Design space approach is applied in this study to enhance the robustness of first ethanol precipitation process of Codonopsis Radix (Dangshen) by optimizing parameters. Total flavonoid recovery, dry matter removal, and pigment removal were defined as the process critical quality attributes (CQAs). Plackett-Burman designed experiments were carried out to find the critical process parameters (CPPs). Dry matter content of concentrated extract (DMCE), mass ratio of ethanol to concentrated extract (E/C ratio) and concentration of ethanol (CEA) were identified as the CPPs. Box-Behnken designed experiments were performed to establish the quantitative models between CPPs and CQAs. Probability based design space was obtained and verified using Monte-Carlo simulation method. According to the verification results, the robustness of first ethanol precipitation process of Dangshen can be guaranteed by operating within the design space parameters. Recommended normal operation space are as follows: dry matter content of concentrated extract of 45.0% - 48.0%, E/C ratio of 2.48-2.80 g x g(-1), and the concentration of ethanol of 92.0% - 92.7%.
