1.Effect of ginkgolide B on plasma levels of interleukin-12 and PAF in severe acute pancreatitis in rats.

AIMTo observe the levels of interleukin-12 (IL-12) and platelet activating factor (PAF) in severe acute pancreatitis (SAP) in rats and the efficacy of Ginkgolide B (BN52021) in treating SAP.
METHODSWistar rats were randomly divided into 3 groups: model group (SAP), treatment group (BN) and negative control group (NC). SAP was induced by retrograde infusion of 5% sodium taurocholate into the pancreatic duct in Wistar rats. NC rats only receive abdominal incision. In groups of SAP and NC rats received the femoral vein injection of isotonic Na chloride 15 minutes after induction of SAP; in BN group,rats received BN52021 instead. After operation rats were sacrificed at 1, 6 and 12 hour for plasma IL-12 and PAF determined with ELISA.
RESULTSAn increase of IL-12 in group BN was observed VS group SAP or group NC at 1 h stage (p = 0.011, P < 0.01). At 6 h or 12 h stage,an increase of IL-12 in group SAP was observed VS group NC (P < 0.01, P < 0.05). The plasma level of PAF in group SAP or group BN was increased significantly at 1 h time stage VS group NC (P < 0.001). At 6 h or 12 h stage, a decrease of PAF in group BN or group NC was observed VS group SAP (P < 0.05, P < 0.01).
CONCLUSIONIt confirmed that the plasma level of cytokine IL-12 in SAP group was decreased significantly in early stage and it witnessed a remarkable increase of cytokine PAF. The plasma level of IL-12 was increased in early stage but PAF was decreased in rats treatment by BN52021 which inhibited the development of SAP.

2.Survey on iodine nutrient in vulnerable population in Linxia Hui Autonomous Prefecture of Gansu in 2006

Objective To assess iodine nutrient levels of vulnerable population in Linxia Hui Autonomous Prefecture of Gansu. Methods Local pregnant, lactation, child-bearing age women and 0 - 3 years old infants were selected. The urinary iodine (UI) were determined by arsenic-Ce catalytic spectrophotometry (WS/T 107-1999) and salt iodine content was determined using direct titration method (GB/T 13025.7-1999). The development quotient (DQ) among 0 - 3 years old infants were surveyed by the scales of neuropsychologists development among 0 - 6 years old, and their body height and weight were then investigated. Results The consumption and coverage rates of iodized salt at household level were all below than 90% in 5 counties among 8 counties. The median urine iodine (MUI) among pregnant, lactation, child-beating age women and 0 - 3 years old infants were 90.17, 89.28, 84.85 and 107.28 μg/L, respectively. The UI proportion less than 50 μg/L were 29.6%(278/938), 30.7%(239/778), 32.2% (265/824), and 23.9%(252/1056), respectively. The UI proportion equal or more than 50 μg/L and less than 100 μg/L were 25.1%(235/938), 24.0%(187/778), 23.7%(195/824), 23.7%(250/1056). MUI in pregnant were all less than 150 μg/L in 8 counties, and MUI in lactation women were less than 100 μg/L in 6 counties. The DQ median of infants 0 - 3 years old was 93.0, and 59.9%(395/659), 34.4%(227/659) and 5.6%(37/659)had a DQ below middle, at middle and above middle, respectively. Forty point seven percent (266/654), 50.2%(328/654) and 9.1% (60/654) had a body height below middle,at middle and above middle, respectively. Thirty-five percent (232/663), 51.3%(340/663) and 13.7%(91/663) had a body weight below middle, at middle and above middle, respectively. Conclusions The local qualified iodized salt consumption rate and iodized salt coverage rate are still low. The urine iodine level is below than the reference value recommended. The iodine nutrient is not enough among iodine deficiency disorder vulnerable population, especially in pregnant and lactation women. Infants growth development is retarded.
3.99Tcm-MIBI SPECT in estimation of the treatment response to three-dimensional conformal radiotherapy in patients with non-small cell lung cancer

Objective To explore whether the degree of 99Tcm-methoxyisobutylisonitrile (MIBI) uptake in non-small cell lung cancer (NSCLC) could be correlated with the treatment response to three-dimensional conformal radiotherapy.Methods A total of 102 patients with NSCLC were studied with 99Tcm-MIBI SPECT before radiotherapy.The patients were classified by a follow-up CT as responders (complete or partial remission) and non-responders (stable or progressive disease).After intravenous administration of 740 MBq 99Tcm-MIBI, SPECT imaging at 10-30 min (early) and 2-3 h (delayed) were performed.Region of interest (ROI) was placed over the tumors and contralateral normal lung tissue.The uptake ratio of tumor to contralateral normal lung (T/N) was obtained from both early (ER) and delayed (DR) SPECT images.The retention index (RI) was measured as:RI = (DR - ER)/ER×100%.Statistical analysis was performed by two independent-sample t-test and Mann-Whitney U test using software SPSS 13.0.Results 99Tcm-MIBI uptake was significantly higher in responders than in non-responders:2.36 ±0.17 vs 1.82 ±0.14 (ER) and 2.48 ± 0.20 vs 1.94 ± 0.16 (DR), respectively (t = - 13.1,- 12.7, both P< 0.05).The median RI in the responders group was also significantly higher than that in the non-responders group (6.60% vs 5.13%, z = - 6.83, P < 0.05).Conclusion ER, DR and RI of 99Tcm-MIBI SPECT might be useful to estimate the treatment response to three-dimensional conformal radiotherapy in patients with NSCLC.
4.Rabeprazole 10 mg versus 20 mg in preventing relapse of gastroesophageal reflux disease: a meta-analysis.

BACKGROUNDSeveral randomized controlled trials (RCTs) have compared endoscopic and symptomatic relapses in patients with erosive gastroesophageal reflux disease (GERD). We have summarized current evidence for rabeprazole 10 or 20 mg once daily for GERD maintenance treatment over 1 or 5 years.
METHODSMEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials were searched, through August 2012, for eligible RCTs of adults with erosive GERD. The efficacies of rabeprazole 10 and 20 mg/d were compared.
RESULTSThe search identified 288 citations, and five RCTs containing 1480 patients were considered eligible. Heartburn relapse rates did not differ significantly between patients treated with rabeprazole 10 and 20 mg/d for 1 year (relative risk (RR) = 1.29; 95% confidence interval (CI): 0.97-1.72), but differed in patients treated for 5 years (RR = 1.274; 95% CI: 1.005-1.615). Endoscopic relapse rates differed significantly between rabeprazole 10 and 20 mg/d for 1 year (RR = 1.92; 95% CI: 1.21-3.06), for 5 years (RR = 1.667; 95% CI: 1.073-2.589), and in combined 1- and 5-year maintenance trials (RR = 1.785; 95% CI: 1.298-2.456).
CONCLUSIONRabeprazole 20 mg/d was superior to rabeprazole 10 mg/d in preventing endoscopic relapse of erosive GERD, but that the two dosages were equivalent in symptomatic relief over 1 year.

5.Detection of Y chromosome microdeletions in AZF region by liquid chip technology.

OBJECTIVETo establish a liquid chip technology to detect Y chromosome microdeletions in Chinese infertile males with azoospermia or oligozoospermia.
METHODSMultiplex PCR and liquid chip technology were used to detect the Y chromosome microdeletions in AZF region in 178 infertile patients with azoospermia and 134 infertile patients with oligozoospermia as well as 40 fertile control men.
RESULTSForty out of 312 patients (12.8%) were found to have deletions in AZF region. The microdeletion frequency was 14%(25/178) in the azoospermic group, 9.6%(11/114) in the oligospermic and 20%(4/20) in the severe oligospermic group.
CONCLUSIONThe authors developed a high-throughput, fast and simple assay to screen the AZF region microdeletions of Y chromosome.

6.Reconstruction of complex proximal tibial defects using the long-stem tibial component combined with metallic wedge.

OBJECTIVETo investigate results of total knee arthroplasty using the long-stem tibial component combined with metallic wedge of knee prosthesis for the treatment of proximal defects.
METHODSFrom January 2011 to May 2013, 10 patients (11 knees) were treated with total knee arthroplasties using the long-stem tibial component with metallic tibial wedge of knee prosthesis. All the patients were female and the average age was 67 years old (ranged, 60 to 77 years old). All the patients were osteoarthritis. All the patients were classified as T2A style. The patients were evaluated according to knee score system (KSS).
RESULTSAll the patients were followed up for 12 months on average (ranged 3 to 29 months). The clinical outcome was assessed using KSS score, including knee pain score, knee stability score, knee range of motion score and knee walking score, knee stairs score. There were significantly differences at 6 weeks, 3 months, 6 months and 12 months between pre-and postoperative KSS score.
CONCLUSIONThe mechanical stability of tibial fixation in primary TKA is significantly increased by using the long-stem tibial component with metallic wedge of knee prosthesis, even in the presence of poor proximal bone.

7.Ad-TFPI gene transfer attenuates intimal proliferation in rabbit carotid arteries after balloon injury.

OBJECTIVETo investigate the in vivo gene expression of adenovirus-mediated human tissue factor pathway inhibitor (hTFPI) and its inhibition effects on intimal proliferation in rabbit carotid arteries after balloon injury.
METHODSRabbits underwent carotid artery balloon injuries were treated with Ad-TFPI (n = 25), Ad-LacZ (n = 25) or PBS (n = 10), respectively. Sham operated rabbits (n = 10) serve as normal controls. The expressions of human TFPI at mRNA and protein levels were detected by RT-PCR and ELISA respectively on the 3rd, 7th, 10th, 14th, 28th day after operation. Intimal proliferation was detected by angiograms and morphometric analysis.
RESULTSTFPI mRNA and protein expressions were detected at 3 days and peaked at the 10th and 14th day after TFPI gene transfer. The expressions were still detectable on the 28th day. There was no TFPI expression in Ad-LacZ group. The carotid angiogram results indicated that the minimal lumen diameter in TFPI group was significantly larger and the lumina stenosis percentage was significantly lower in TFPI group compared those in Ad-LacZ and PBS groups (all P < 0.05). The morphometric analysis showed that the intimal area, the ratio of the intimal/media area, the lumina stenosis percentage in TFPI group were all significantly reduced compared with those in Ad-LacZ and PBS groups (all P < 0.01).
CONCLUSIONSThe TFPI gene could be effectively transferred by adenovirus vector to injured carotid arteries and transferred Ad-TFPI could significantly attenuate intimal proliferation in balloon injured carotid arteries in rabbits.

8.Effect of gingerol on substance P and NK1 receptor expression in a vomiting model of mink.

BACKGROUNDGingerol is the generic term for pungent constituents in ginger, which has been reported to be effective for inhibiting vomiting. We attempted to investigate the antiemetic effect of gingerol and its effective mechanism on substance P and NK(1) receptors in minks.
METHODSThe antiemetic effect of gingerol was investigated during a 6-hour observation on a vomiting model in minks induced by cisplatin, (7.5 mg/kg, intraperitoneal). The distribution of substance P and NK(1) receptors in the area postrema and ileum were measured by immunohistochemistry, and the expression of NK(1) receptor in the area postrema and ileum were measured by Western blotting.
RESULTSThe frequency of cisplatin induced retching and vomiting was significantly reduced by pretreatment with gingerol in a dose-dependent manner (P < 0.05). Substance P-immunoreactive was mainly situated in the mucosa and submucosa of the ileum as well as in the neurons of the area postrema. The immunoreactive production of NK(1) receptor was mainly situated in the muscular and submucosa of ileum and the neurons of area postrema, gingerol markedly suppressed the increased immunoreactivity of substance P and NK(1)1 receptor induced by cisplatin in a dose-dependent manner (P < 0.05), and exhibited effective inhibition on the increased expression levels of NK(1) receptor in both the ileum and area postrema dose-dependently (P < 0.05).
CONCLUSIONSGingerol has good activity against cisplatin-induced emesis in minks possibly by inhibiting central or peripheral increase of substance P and NK(1) receptors.

9.Treatment of displaced radial head fractures by internal fixation with absorbable pins.

OBJECTIVETo study the effect of internal fixation with absorbable pins on treatment of displaced radial head fractures.
METHODSFrom May 1999 to May 2004, 16 patients with displaced radial head fractures (Mason types II and III) were treated with internal fixation by absorbable pins. The duration of follow-up averaged 22.6 months (12-58 months). The outcome was assessed on the basis of elbow motion, radiographic findings and the functional rating score delineated by Broberg and Morrey.
RESULTSAll fractures healed within 10 months without avascular necrosis of radial head. The mean elbow flexion loss was 15 degrees (0 degrees-35 degrees), and pronation and supination decreased by 10 degrees (0 degrees-30 degrees) on average compared with those of the contralateral elbow. Five patients had an excellent result, 6 a good result, and 3 a fair result according to the criteria of Borberg and Morrey.
CONCLUSIONSInternal fixation with absorbable pins is an effective method in treating displaced radial head fractures. It can maintain the biomechanical stability of forearm, improve the elbow function and avoid second operation.

10.Feasibility and safety of CT-guided percutaneous needle biopsy and subsequent iodine-125 seed interstitial implantation for pancreatic cancer.

OBJECTIVETo discuss the feasibility and safety of different approaches for CT-guided percutaneous needle biopsy and subsequent iodine-125 seed interstitial implantation for pancreatic cancer.
METHODSA retrospective study was carried out on the complete data of 35 patients with pancreatic cancer who have received CT-guided percutaneous needle biopsy with or without subsequent iodine-125 seed interstitial implantation. There were 9 lesions located in the head of pancreas, 20 located in the body, and 6 in the tail. The maximum diameter of the lesions varied from 12 mm to 60 mm (mean 37.1 mm). The patients were treated with a needle in diameter of 16-21G. Operations were undertaken via anterior, posterior and lateral approaches.
RESULTSThirty-five patients underwent 43 times of CT-guided percutaneous needle biopsies. Thirty-one cases were pathologically diagnosed as cancer, 2 cases inflammatory lesions, and 2 were suspected tumors (one of which was finally diagnosed as cancer, while another was pancreatic pseudocyst). The ratio of correct diagnosis was 94.3%. Fourteen patients were treated subsequently with CT-guided iodine-125 seed interstitial implantation therapy, with a total of 65 times of needle puncture. The operations were performed via direct approach to the tumor in 18 cases, transhepatic approach in 2 cases, transgastric approach in 4 cases, transintestinal approach in 10 cases, and through mesenteric vessels in one case. Incidence of complications in the biopsy group was 2.32% (1/43), and in the implantation group was 6.15% (4/65), with a statistically non-significant difference (P = 0.600) between the two groups. Incidence of complications in the group using 16-18G needle was 4.65% (4/86), while in the group using 20-21G needle was 4.55% (1/22), also with a non-significant difference (P = 0.064). The accuracy rate of needle biopsy in this study was 94.28% (33/35).
CONCLUSIONCT-guided percutaneous needle biopsy and subsequent iodine-125 seed interstitial implantation are both feasible and safe for pancreatic cancer.
