1.Detection of antifungal agent activity against the filamentous fungi by Etest

Objective To investigate the susceptibility of amphotericin B,itroconazol and voriconazole against filamentous fungi.Methods Etest was used to determine the MIC of amphotericin B, itroconazol and voriconazole against filamentous fungi including Aspergillus,Penicillium,Alternaria alternate,Mucor and Rhizopus species.Results The average MIC of voriconazol,amphotericin B and itroconazol against Aspergillus fumiagtus is O.29 ?g/ml,1.16 ?g/ml and 5.88 ?g/ml;the average MIC of amphotericin B and voriconazol to Aspergillus flavus is 6.39 ?g/ml and 0.22 ?g/ml;the average MIC of voriconazol,amphotericin B and itroconazol against Aspergillus niger is 0.69,2.31,and 19.75 ?g/ ml.Most of Penicillium are susceptable to amphotericin B,but 3 strains showed very high MIC to voriconazol and itroconazol.Both of the testing strains of Mucor and Rhizopus were resistant to all of the three antifungal agents.Conclusion Amphotericin B,itroconazol and voriconazole possessed different susceptibility on different types of filamentous fungi.It is important for clinical laboratories to identify the filamentous fungi to the level of genus and species to help physicians choose antifungal agents correctly.
2.Observation of Testis Tissue in Experimental Cryptorchidism after Orchiopexy in Various-Day-Rats

ObjectiveTo observe testis tissue changes of experimental cryptorchidism after orchiopexy in various-day-rats.MethodsSeventy-two Sprague-Dawley(SD) male rats were divided into 3 groups randomly and made artificial cryptorchidism:unilateral cryptorchidism group(n=24),bilateral cryptorchidism group(n=24)and sham operation group(n=24)at age 21-day-old.Intra-abdominal testicle was resetting in 2 weeks,at age 40 days and 60 days,the rats were sacrificed for detection tubular fertility index(TFI) and mean tubular diametar(MTD) with hematoxylin-eosine staining and germ cell apoptosis by terminal deoxynucleotidyl transferase mediated d-UTP nick end labeling(TUNEL) assay.ResultsThere were significant differences of MTD,TFI and apoptosis index(AI) between cryptorchidism and scrotal testes(P0.05).The AI of cryptorchidism testes of unilateral cryptorchidism group were significantly lower than that of bilateral cryptorchidism group in 40 days(P0.05).ConclusionsAI of artificial resetting testis is increased and contralateral descended testes in unila-teral cryptorchidism have various damage.It is to lighten that pathological damage of testes of cryptorchidism with prolongation of reset time.

4.Gene Gly82Ser polymorphism of receptor of advanced glycaton end products in patients with transient ischemia attack

Objective To explore the relationship between receptor of advanced glycaton end products(RAGE)gene Gly82Ser polymorphism and patients with transient ischemia attack(TIA).Methods The Gly82Ser gene at the position of RAGE gene exon 3 was identified by a polymerase chain reaction- restriction fragment length polymorphism(PCR-RFLP)method in 70 cases of TIA & Diabetes(DM), 60 of simply TIA and 66 healthy control subjects.Results The genotypes of RAGE gene Gly82Ser identified were GG, GS and SS.The frequencies of RAGE gene Gly82Ser GS heterozygous genotype of TIA & DM and control were respectively 62.9% and 43.9%, significantly higher in TIA & DM patients than in control subjects(OR 2.036, 95% CI 1.021--4.062, P=0.042), however no significant difference was found between simply TIA and control(53.3% vs 43.9%, OR 1.299,95% CI O.644--2.618, P=0.465). Significant difference of the frequency of S allele was found neither between TIA & control and control(being 34.3% and 26.5%, respectively, OR 1.446,95% CI 0.859--2.434, P=0.164), nor between simple TIA and control(28.3% vs 26.5%, OR 1.096,95%CI 0.630--1.907, P=0.746).Conclusions RAGE gene Gly82Ser GS heterozygous genotype may be associated with TIA & DM patients.RAGE gene Gly82Ser polymorphism is a risky factor for TIA & DM patients, but not for TIA patients.
5.Epidemiological and etiologic characteristics of ocular fungal infection in Haikou.

OBJECTIVETo analyze the epidemiological and etiologic characteristics of ocular fungal infection in Haikou.
METHODSThe samples of corneal, conjunctivae, anterior chambers, lacrimal sacs were taken from the outpatients and inpatients in the Department of Ophthalmology for fungal culture. The positivity rates of fungal culture were statistically analyzed to describe the seasonal and genus distribution of the fungal infections.
RESULTSA total of 683 patients were involved in this study, and the total positivity rate of fungal culture was 27.96%. The positivity rate was 33.42% in male patients and 21.52% in female patients (P<0.01). The positivity rates were 31.42%, 11.32%, 10.81%, 21.28% and 29.17% in the cornea, conjunctivae, anterior chambers, lacrimal sacs and other locations (P<0.01), and were 34.88%, 22.58%, 11.76%, 4.11%, 21.43% and 13.33% in farmers, workers, teachers, government officers, students and personnel of other occupations (Chi2=39.550, P=0.001), respectively. In terms of age, the rate was 32.30% in 21-40 years group, 31.72% in 41-60 years group, 17.12% in over 60 years group and 26.61% in below 20 years group, showing significant differences (P<0.01). The rates were higher in August, September, October, December, November, all above 30%, but lower in March. Nine genera were identified, including Candida (49.21%), Aspergillus (19.37%), Fusarium (9.42%), mucor (5.76%), Actinomyces (5.24%), Penicillium (3.66%), Saccharomyces (2.62%), non-spore group (1.57%), Alternaria (1.57%), and other genera (1.57%). Among the 94 strains of Candida, 63 (67.02%) belonged to Candida albicans and 14 to Candida tropicalis (14.89%).
CONCLUSIONOcular fungal infection occurs more often in male patients and in the cornea. The common pathogen of ocular fungal infection is Candida, which is different from the etiological characteristics in other areas possibly in association with the tropical climate.

6.Calcium-regulated growth and differentiation of the mouse epidermal keratinocytes.

The effect of the calcium ion (Ca2+) on the growth and differentiation of the mouse epidermal keratinocytes cultured in serum-free medium was investigated. It was found that the optimal level of calcium ion in the medium was about 0.2 mmol/L. Under such a culture condition the colony forming efficiency, attachment percentage, percentage of the cells with cornified envelops, and percentage of the senesced cells were measured to be about 10.8%, 30.8%, 5.1%, and 26.8%, respectively. However, the Ca2+ concentrations in the medium above 0.6 mmol/L resulted in significant differentiation and senescence of the keratinocytes, which was found to be harmful for keratinocyte growth and expansion in vitro.

7.A new method for quick analysis of tanshinone II(A) and salvianolic acid in Fufang Danshen tablets by AOTF-near infrared spectroscopy.

OBJECTIVETo establish a new method using AOTF-Near infrared spectroscopy (NIRS) for quick determination of tanshinone II(A) and salvianolic acid B in Fufang Danshen tablets.
METHODHPLC was used as a reference method to determine the contents of tanshinone II(A) and salvianolic acid B in Fu Fang Dan Shen tablets. Multivariate calibration models based on PLS1 algorithm were developed to correlate the spectra and the corresponding values determined by the reference methods.
RESULTRMSECV (root-mean-square error of cross-validation) of the models for tanshinone II(A) and salvianolic acid B were 0.0103 and 0.1868 respectively. The correlation coefficients of the calibration models were 0.9873 and 0.9832 respectively. External validation with external validation samples proved that the relative coefficient of the predicted value and the truth value were R2 = 0.9743 and R2 = 0.9886 respectively, with measured recycle rates of 103.0% and 99.0%.
CONCLUSIONNIRS can be used in the determination of tanshinone II(A) and salvianolic acid B, which sets up the foundation of product-line control of Fuang Danshen tablets.

8.Effects of electroacupuncture on circadian rhythm of temperature and melatonin in depression rats model induced by chronic stress.

OBJECTIVETo observe the effects of electroacupuncture (EA) on circadian rhythm of temperature and melatonin (MT) in depression rats model induced by chronic stress, so as to explore the biological mechanism of EA for depression.
METHODSTwenty-four SD rats were randomly divided into a control group, a model group and an EA group, 8 cases in each one. Rats in the control group were treated with normal diet for 21 days without any treatment. In the model and EA group, rat model was established by chronic unpredictable stress combined with solitarily feeding method, and rats in the EA group was treated with EA at "Baihui" (GV 20), "Yintang" (GV 29) 1 h before stress stimulation everyday, 2 Hz in frequency and intensity was favorable with the head of rat slightly shivering. The needles were retained for 20 min, once a day for totally 21 days. After EA treatment, open-field experiment was adopted to observe the behavioral improvement; the rats temperatures were monitored at six time points (2:00, 6:00, 10:00, 14:00, 18:00, 22:00) and orbital blood sampling was collected. The level of serum MT was tested by enzyme linked immunosorbent assay. The circadian rhythm changes of temperature and serum MT in each group were compared.
RESULTSThe numbers of horizontal movement and vertical movement in the model group were obviously lower than those in the control group (both P < 0.05), while those in the EA group were significantly improved compared with those in the model group (both P < 0.01). The circadian rhythm of temperature and MT disappeared in the model group, which was improved into normal level after EA treatment.
CONCLUSIONThe electroacupuncture has regulation effects on circadian rhythm of temperature and melatonin in depression rat model induced by chronic stress.

9.Infrastructure and contents of clinical data management plan.

Establishment of quality management system (QMS) plays a critical role in the clinical data management (CDM). The objectives of CDM are to ensure the quality and integrity of the trial data. Thus, every stage or element that may impact the quality outcomes of clinical studies should be in the controlled manner, which is referred to the full life cycle of CDM associated with the data collection, handling and statistical analysis of trial data. Based on the QMS, this paper provides consensus on how to develop a compliant clinical data management plan (CDMP). According to the essential requirements of the CDM, the CDMP should encompass each process of data collection, data capture and cleaning, medical coding, data verification and reconciliation, database monitoring and management, external data transmission and integration, data documentation and data quality assurance and so on. Creating and following up data management plan in each designed data management steps, dynamically record systems used, actions taken, parties involved will build and confirm regulated data management processes, standard operational procedures and effective quality metrics in all data management activities. CDMP is one of most important data management documents that is the solid foundation for clinical data quality.

10.Anatomical study of unilateral endoscopic endonasal transsphenoidal approach for sellar operation

Objective To study the identification of the endoscopic anatomical landmarks in the unilateral endoscopic endonasal transsphenoidal approach, and discuss the verification of the anterior wall of the sphenoid sinus and expansion of the bone exposure of the sellar region according to different growth patterns of the selhr tumors. Methods Ten formalin-fixed adult cadaveric heads and 5 fresh cadaveric heads wefe perfused with red silicone through the internal carotid artery.and blue glass cement was perfused through the internal jugular vein.The endoscope Was introduced through the unilateral nasal cavity, and the contralateral nasal cavity was fully exposed by removing the superficial nasofacial structures for observation and measurement. Results The distance from the phatyngopalatine arch to the inferior edge of the sphenoidal ostium Was 15.13±1.99 mm,and that from the inferior edge of the posterior middle turbinate to the center of the sellar floor Was 10.20±0.15 mm.The content of the sphenoid sinus Was 8.73±2.90 mL after removal ofthe compartment of the sphenoid sinus,with the sellar floor thickness of 3.68±1.96 mm.The angles between the nasal septurn and the anterior wall of the sphenoid sinus varied significantly on different planes(P<0.05).Conclusion When difficult to locate the sphenoidal ostium,the opening site can be determined by observing the peculiar oval protuberance on the anterior wall of the sphenoid sinus.the relationship between the anterior wall of the sphenoid sinus and the middle turbinate,and the average distance from the superior limit of the pharyngopalatine arch to the sphenoidal ostiam.The voger on the midline position can be used as the landmark of the midline of the sellar floor.When the tumor has an extrasellar portion,as in the planum sphenoidale,the modified enlarged approach can be the choice.
