The pathogenesis of allergic rhinitis(AR)is extremely complex.In recent years,a variety of allergens and other complexes have been developed to induce a series of signal transduction mechanisms by activating mast cells.Intracellular media release(mast cells,MCs)play an important role in the pathogenesis of AR.In this paper,we reviewed the progress of mast cells in the pathogenesis of allergic rhinitis in recent years in order to further understand its role in the pathogenesis of allergic rhinitis and provide new ideas on the therapeutic target for allergic rhinitis.

3.Proteomics analysis of hepatitis B virus and aflatoxin B1 related hepatocellular carcinoma in Guangxi,China

Objective To explore the effects and the molecular mechanism of synergistic hepatocarcinogenesis of HBV and AFB1 in the development of HCC by studying the difference in protein expression profiles in hepatocellular carcinoma exposed to hepatitis B virus and Aflatoxin B1.Method 32 HCC specimens were labeled under four categories based on their biomarkers of HBV and AFB1 exposure:group A:HBV(+)/AFB1 (+),10 cases,group B:HBV(+)/AFB1 (-),10 cases,group C:HBV(-)/AFB1(+),6 cases,groupD:HBV(-)/AFB1(-),6 cases.Normal hepatic tissues from 10 cases of hepatic hemangioma,liver resection and liver transplant donor were chosen as the normal control group.Isobaric Tagging Reagent Quantitative (iTRAQ) Proteomics together with 2DLC MS/MS were applied to analyze the differentially expressed proteins among the 4 groups.Result (1) A total of 117 unique differentially expressed proteins including 53 up-regulated proteins and 64 down-regulated proteins were identified in the four groups.The number of unique differentially expressed proteins,including up-regulated and down-regulated proteins,in group A,group B,group C and group D were 106,97,104 and 74 respectively.(2) Among the 117 differentially expressed proteins,9 proteins were heat shock proteins or chaperones,and they were up regulated in group A,B and C.Besides,15 proteins were detoxification and drug metabolism pathway related proteins,12 of them were down-regulated in group A,and more than half of them were also down-regulated in group B and C.(3) The Reverse Transcriptase PCR result showed the mean expression level of AKR1B10 mRNA in group A was significantly higher than group B,C and D (all P<0.05,respectively).Group C also showed significantly a higher expression level of AKR1B10 mRNA than group D (P<0.05).The Western-blot results showed the mean expression level of AKR1B10 protein in group A was significantly higher than group B and D (all P<0.05,respectively).Conclusions The up-regulated expression of heat shock protein and the down-regulated expression of most protein enzymes related to detoxification and drug metabolism were the common molecular biological events of HCC associated with exposure to HBV and AFB1.This suggested the synergistic hepatocarcinogenesis effects of HBV and AFB1 may be related to dysregulation of protein enzymes related to detoxification and drug metabolism.The overexpression of AKR1B10 may be involved in the AFB1-related hepatocarcinogenesis process.
4.Tiamcinolone acetonide and indocyanine green-assisted vitrectomy combined with inner limiting membrane peeling for idiopathic macular hole

BackgroundWhether the peeling of the inner limiting membrane (ILMP) increase the closure rate of idiopathic macular hole is still in controversy.Some ophthalmologist recommend vitrectomy combined with inner limiting membrane peeling for the treatment of idiopathic macular hole.However,the removal of ILMP is difficult because of its similar appearance to adjacent tissues.Objective This study was to investigate the efficacy of triamcinolone acetonide(TA) and indocyanine green(ICG) double staining-assisted vitrectomy combined with ILM peeling during the surgery.Methods A consecutive case- observational study was designed.The standardized vitrectomy was performed in 25 eye of 23 cases with IMH.During the vitrectomy,TA and ICG were injected into posterior pole vitreous to visualize and assist the ILM peeling.The dying effectiveness was observed,and the closure rate of macular hole,visual acuity,intraocular pressure and complications were evaluated after surgery.Written informed consent was obtained from each patient prior to operation.Results Posterior vitreous cortex and ILM were visible and the residual vitreous and cortex were removed clearly after dying of TA and ICG in all the 25 eyes.During the following-up duration of 3-8 months,the completely anatomical reattachment of the macular area was in 22 eyes ( 88.0% ) and partially reattachment in 3 eyes( 12.0% ).The best corrected vision was 0.07-0.60 in all of the operated eyes 2 months after surgery.Conclusions TA and ICG- assisted vitrectomy combined with ILM peeling appears to be a safe and effective method for IMH repair.
5.Effect of diazepam and modafinil on acute hepatic failure in mice.

OBJECTIVE:
To investigate the effect of diazepam and modafinil on acute hepatic failure in mice.
METHODS:
Acute liver failure was induced in male Kunming strain mice by enterocoelia injecting the mice with D-GalN and LPS . The mice in the treatment groups were given corresponding drug 2 h before the administration of D-GalN and LPS, and the mice in the control group were given the same dose of distilled water. The 24-hour survival rate, serum alanine aminotransferase (ALT), and aspartate aminotransferase (AST) levels were compared. Serum levels of TNF-alpha and IL-1 and the levels of SOD, MDA, GR, GSH, NO and NOS in the liver were determined.
RESULTS:
Treatment with diazepam increased the survival rate and improved liver histological feature. Diazepam inhibited the serum levels of ALT, AST, TNF-alpha and IL-1, and reduced levels of MDA, NO and NOS and increased levels of GR and SOD in the liver. Modafinil decreased liver histological feature, increased the serum levels of ALT, AST, TNF-alpha and IL-1, increased level of MDA, and inhabited levels of SOD and GR in the liver.
CONCLUSION
Treatment with diazepam may suppress the D-GalN/LPS-induced acute hepatic failure and modafinil may facilitate the acute hepatic failure.

6.Analysis of the characteristics of articular cartilage lesions under knee arthroscopy in different age groups.

OBJECTIVETo provide reliable data on articular cartilage lesions in different age group of patients with symptomatic knees requiring arthroscopy, and to analyze the characteristics of articular cartilage lesions.
METHODSFrom November 2004 to September 2005, 568 patients were under arthroscopy in orthopaedics of Chinese PLA General Hospital. There were 242 males and 326 females, with a mean age of (43.0 +/- 11.2) years (ranged, 12 to 80 years). Surgery information including causes of injury, onset of symptoms, meniscus lesions, cruciate ligament pathology, articular cartilage lesions were collected by a special Data Management software. All the patients were divided into two groups : youth group and middle-aged group. The data of articular cartilage lesions of patients in the two groups were collected.
RESULTSThere were 923 articular cartilage lesions in 389 patients, and 13 lesions were iatrogenic. In youth group, 212 lesions were found in 121 patients, and the main cause of lesions was menisci menisci injury (57/121 cases). In middle-aged group, 711 lesions were found in 268 patients,and the main cause of lesions was osteoarthritis (218/268 cases). The articular cartilage lesions located majorly in the lateral femoral condyle, medial femoral condyle and patellae in patients of the two groups. The major depth of the lesion was grade IV (330 lesions) in middle-aged group and grade III (71 lesions) in youth group,respectively.
CONCLUSIONArticular cartilage lesions are common in patients under arthroscopy, and articular cartilage lesions of different age group of patients are different, so the best treatment method should be chosen regarding to age factors of patients.

Objective Previous studies indicated that activation of Toll-like receptor4 (TLR4) was involved in the progression and instability of atherosclerotic plaque.Anti-inflammatory effects were shown in statins. However,the mechanisms underlying these effects have not been well explored.We test the hypothesis that a por- tion of these anti-inflammatory effects are mediated by regulation of TLR4 expression.Methods One hundred twenty-one subjects (22 normal persons,17 patients with stable angina and 82 patients with ACS) were recruited. 41 patients with ACS were randomized to atorvastatin 10 mg/d or atorvastatin 40 mg/d on top of routine anti-anginal treatment.Serum level of hsCRP,blood lipids,TLR4 expression on CD_(14)~+ monocytes were measuered before and after one month treatment.TLR4 expression on CD_(14)~+ monocytes were quantified via flow-cytometry.Results hsCRP and TLR4 expression on CD_(14)~+ monocytes in patients with ACS were higher than patients with stable angina and normal persons(hsCRP,ACS:11.1?14.3 vs stable angina:2.5?2.7 mg/L vs normal:2.3?4.2 mg/L,P
8.Analysis of the application of SPIDER limbs positioning device in arthroscopic surgery on shoulder joint of beach chair position

Objective To investigate the clinical application effect of SPIDER limbs positioning device in arthroscopic surgery of beach chair position.Methods From August 2009 to August 2010 in our hospital 18 cases of patients with rotator cuff injury in shoulder were collected and treated with arthroscopic exploration and suturing operation,including 9 patients in the standard beach chair position under surgical operation,9 cases of beach chairs position combine with SPIDER Limb positioning device.Follow up on the preoperative position placement time,operating time,doctors' satisfaction of surgery posture and postoperative complications.Results The preoperative placement position time of standard beach chair with a mean (27 ± 6.7 ) min,beach chair position combined with SPIDER limb positioning device with a mean (31 ±7.9)min,the two groups were no significant differences ( t =- 1.158,P > 0.05 ) ; the operation time of two groups were ( 78.2 ± 12.7 ) min and (66.2 ± 15.4) min,beach chair position combined with SPIDER limb positioning device group compared with the former was short,there was a between the two groups significant difference; doctors' surgery position satisfaction were 77.8% and 88.9% respectively,and there was no complications occurred related with posture.Conclusions SPIDER limb positioning device in the beach chair position in shoulder arthroscopy allows surgeon change positions of limbs during the operation in the sterile area.It has high satisfaction with the surgical posture,practicability,advanced and worthy of clinical application.
9.Study of Jianpi-Bushen Fang on Treatment of Ulcerative Colitis by Promoting BMSCs Proliferation

OBJECTIVE To explore the mechanism of ulcerative colitis(UC)treatment by Jianpi-Bushen Fang through pro-moting the differentiation of BMSCs in model rats.METHODS The rats were divided into 5 groups:normal group,model group,BMSCs group,BMSCs intervene group and combine group.BMSCs intervene group and combine group were injected with decoction intervened BMSCs through tail veins in vitro (1×106/mL),the combine group were also given the decoction in oral for 10 d.BMSCs group were injected with BMSCs through tail veins(1×10 6/mL).The normal and model groups were in-jected with 1 mL of normal saline through tail veins.5 rats were killed on 10th day after the intervention,respectively,for specimen.The expression of Lgr5,Ephrin-B3 and E-cadherin were detected with Western blot,and the levels of IL-6,IL-17 and TGF-βwith ELISA assay.RESULTS The expression of Lgr5 and Ephrin-B3 in treatment groups increased(P <0.05), compared with the model group.E-cadherin in treatment groups increased compared with the model group,and the combine group and BMSCs intervene group increased significantly(P <0.05).The levels of IL-6 and IL-17 in combine group and BM-SCs group significantly decreased(P <0.05),while TGF-βincreased(P <0.05).The difference between the combine group and BMSCs group was significant(P <0.05) in Lgr5,Ephrin-B3,E-cadherin,IL-6 and TGF-βexpression.CONCLUSION Jianpi-Bushen Fang can promote the venously transplanted BMSCs proliferation and differentiation into intestinal stem cell, regulate immune function and repair intestinal mucosa,which may be one possible mechanism for UC treatment.
10.The effect of extracellular signal-regulated kinase 1/2 signaling pathway on the expression of transforming growth factor-beta1 in lung fibroblast activated by silicon dioxide.

OBJECTIVETo investigate the effect and regulation of extracellular signal-regulated kinase (ERK1/2) signaling pathway on the expression of transforming growth factor-beta1 (TGF-beta1) in human embryonic lung fibroblasts induced by SiO2.
METHODSHuman alveolar macrophages were collected from a silicotic patient by bronchoalveolar lavage and in the presence or absence of SiO2 (50 ug/ml) exposition for 18h, and then the conditioned supernatants were used to incubate HELF. The expressions of TGF-beta1, of the HELF acted with the conditioned AM supernatant fluid were detected with the immunocytochemistry method after treatment with PD98059 of inhibitor of ERK.
RESULTSThe expression of TGF-beta1 in HELF of the SiO2 treatment group (OD value is 0.322 7 +/- 0.023 8) exceed blank group (OD value is 0.163 7 +/- 0.019 6) and AM control group (OD value is 0.240 6 +/- 0.022 5) by the immunocytochemistry method. But the expression of TGF-beta1 had reduction in some extent in the PD98059 intervention group (OD value is 0.271 1 +/- 0.022 9). The values were statistically different (P < 0.05).
CONCLUSIONERK inhibitor PD98059 have inhibition effect on the expression of transforming growth factor-beta1 and expression of cytokine of human embryonic lung fibroblasts stimulated by SiO2. The study indicate that the proliferation and collagen production of HELF activated by SiO2 are mediated by ERK/MAPK signal pathway in some extent. PD98059 may antagonizes silica-induced lung fibrosis by inhibiting the expression of transforming growth factor-beta1.
