1.Diagnosis and treatment of simple macromastia and complex macromastia

Objective To study the clinica features, diagnosis and surgical treatment of simple and complex macromastia. Methods The clinical and pathological data of 21 patients with macromastia in our hospital from 1990 to 2000 were analysed retrospectively. Simple macromastia in 16 patients with 29 breasts, complex macromastia in 5 patients with 8 breasts. Results The average age of patients was 23 years old. All the 21 patients were treated surgically. Among the 5 patients with complex macromastia, subcutaneous mastectomy and reconstruction were performed on 2 patients with breast fibroadenomatosis; of the 2 patients with giant fibroadenoma, one underwent tumor excision, the others were subjected to tumor excision with reduction mammaplasty; the last one with breast fibrous disease, selective partial breast excision and reduction mammaplasty was performed. All the patients had satisfactory mammary appearance and hadn't recurrrence of the breast disease in follow up period. Conclusions (1) Macromastia often happens in the adolescent and young women. (2) It is necessary to do biopsy during operation for distinguishing the simple from the complex macromastia. (3) The treatment for simple macromatia is reduction mammaplasty; for complex macromatia is dissection of the disease area completely and redution mammaplasty simultaneously. For macromastia with fibroadenomatosis, subcutaneous mastectomy with reconstruction can prevent tumor recurrence and get better mammary appearance.
2.Changes of vital signs in dog progressive cirulatory failures induced by soman

Objective To investigate the changes of vital signs and the damage of important organs in dog progressive circulatory failures induced by soman.Method Seven male dogs,weighing(12~15)kg,were injected intramuscularly 1/3 LD sornan(1 LD=10μg/kg)per ten minutes.The moan blood pressure decreased to (40~45)mmHg was defined as circulatory failure.The changes of heart rate,blood pressure.and hemodynamic parameters were evaluated by an eight-channel direct-witing oscillograph,blood gas,pH value,electrolyte,and the damage of important organs were observed before and after sornan injection.Statistical analysis of the data was performed using the self control t test with the SAS 6.12 Software Program.Results In anesthetized dogs intoxicated with sornan,the circulatory failure was characterized by the significant decreases in blood pressure,heart rate and hemodymrnic parameters(P<0.05).Partial pressure of oxygen was less than 60 mmHg,saturation of oxygen Was less than 90% and partial pressure of carbon dioxide was greater than 50 mmHg in arterial blood of the dog model.These results showed mix respiratory failure occurred during intermittent positive pressure.Significant metabolic acidosis was induced by soman[pH(7.345±0.064)vs.(6.956±0.022),P<0.01].The concentralion of sodium ion and chloride ion in blood were changed gently.The concentrations of GTP,GOT,Cr,BUN,CK-MB and LDH were increased significantly(P<0.05),which showed multiple important organs including liver,kidney and heart were damaged by sornan.Conclusions The severe progressive circulatory failure induced by cholinesterase inhibitor sornan leads to the darnage of vital signs and important organs significantly.
3.Determination of benzyl glucosinolate in Lepidium meyenii from different regions by HPLC.

The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The content of benzyl isothiocyanate (BITC) which as the enzymatic hydrolysis product of benzyl glucosinolate through thioglucosidase was determined by HPLC. The chromatography condition was as follows: Kaseisorb LC ODS 2000 (4.6 mm x 150 mm, 5 min) column with the mobile phase of acetonitrile(A)-water( B) under gradient elution (0-5 min, 3%-8% A; 5-9 min, 8%-48% A; 9-23 min, 48%-62% A; 23-28 min, 62%-99% A); the flow rate was 1.0 mL x min(-1) with 10 microL injection volume; detection wavelength was 246 nm and temperature of column was 40 degrees C. The content of benzyl glucosinolate was in the range of 10.76-17.91 g x L(-1). The method is simple, accurate and good reproducibility which can be used for the determination of benzyl glucosinolate in Lepidium meyenii, effectively.

5.Expression and significance of HIF-1? and CXCR4 protein in tissues of breast cancer

8.Effect of mechanical stimulation on the expression of E-cadherin/cateninin complex in salivary adenoid cystic carcinoma.

OBJECTIVETo investigate the effects of cyclic biaxial mechanical stimulation on adhesion of salivary adenoid cystic carcinoma(SACC) extracellular matrix (ECM) and expression of E-cadherin/cateninin complex in the SACC high metastasis cell lines ACC-M, SACC low metastasis cells line ACC-2, we observed the functions of mechanical stimulation in the adhesion of SACC cell-ECM and investigate the mechanism in the adhesion of ACC.
METHODSMechanical stimulation were applied to the cells for periods of 1, 3 and 6 hours every day, lasting for 2 days. The amplitude of mechanical stimulation applied to the cells were 1000, 4000 micro strain, at a frequency of 3 Hz. Unstrained cells were used as control. The expression of E-cadherin/cateninin complex on the cell of ACC-M, ACC-2 were studied with laser scanning confocal microscope and image analysis. SACC cell-ECM adhesion was assayed by MTT technique.
RESULTSThe results showed that expressions of alpha, beta, gamma-cateninin on the cell of ACC-2 were obviously higher than that ACC-M and E-cadherin on the cell of ACC-M were obviously higher than that ACC-2 without mechanical stimulation. Mechanical stimulation can change the expression of E-cadherin/cateninin complex on the cell of ACC-2 and ACC-M with time. The results also showed that cell-ECM adhesion on the cell of ACC-2 were obviously higher than that of ACC-M without mechanical stimulation. Mechanical stimulation can change the cell-ECM adhesion of ACC-2 and ACC-M with time.
CONCLUSIONMechanical stimulation can change the adhesion of the SACC cell-ECM and expression of E-cadherin/cateninin complex of the SACC cell. We think it played an important role in metastasis of the cancer.

9.Treatment of clavicular fractures using intramedullary nailing or K-wire versus plating fixation: a meta-analysis.

OBJECTIVETFo compare the efficacy and complications rate of intramedullary (IM) nailing or K-wire versus plating fixation for clavicular fractures.
METHODSPubmed, Embase, Cochrane Library databases, CNKI, VIP and Wangfang databases were searched to find all randomized or quasi-randomized controlled trials of clavicle fractures using plating versus IM nailing or K-wire. The methodologic quality of the studies was assessed. After independent study selection by 2 authors ,data were collected and extracted independently. Outcomes of postoperative shoulder functional measurement, the efficacy and information of the operation and complications rate were meta-analyzed using RevMan 5 software.
RESULTSNine hundreds and seventy-six patients in 10 randomized controlled trials (RCTs) and 3 quasi-RCTs were involved in the meta-analysis,of which 5 studies compared the K-wire and the plating fixations and 8 studies compared the IM nailing and the plating fixations. The overall odds ratio(OR) (with 95% CI) of the operation efficacy for K-wire versus the plating was 3.79 (1.93, 7.46). The overall weighted mean difference (with 95% CI) of Constant Shoulder score for plating versus IM fixation was -1.39 (-3.43, 0.65) in 6 studies. The overall OR of the plating versus IM nailing was 9.34(2.70, 32.32) for the overall major complications in 5 studies and 5.04 (1.52,16.77) for the revision rate in 5 studies.
CONCLUSIONThe current limited evidences suggested that the IM fixation could reduce the incidences of the overall major complications and the revision surgery, while the post-operative efficacy of the plating was superior to the K-wire. More high quality RCTs are still needed in the future.

10.Randomized controlled trial on effect of Tuina for treatment of sub-health people of somatic pain.

OBJECTIVETo evaluate the clinical effect and safety of Tuina for treatment of somatic pain of sub-health.
METHODSA randomized, double-blind and blank parallel controlled trial was done. The experiment group was treated with Tuina and the control group lied down for rest, 45 minutes each time, twice each week for three weeks.
RESULTSTuina treatment could improve more on sensory, affective, evaluation, pain rating index and extant pain intensity of the pain index, and score of subjective sensation of life quality and health status together with physiology and psychology field of life quality.
CONCLUSIONMassage is an effective therapy for treatment of somatic pain of sub-health without adverse reactions and it should be generalized to application.
