1.Clinical Observation on Therapy of Clearing Heat and Draining Dampness,Cooling Blood and Normalizing Gallbladder Function for Severe Icterohepatitis

【Objective】To observe the jaundice-relieving effect of therapy of clearing heat and draining dampness combined with cooling blood and normalizing gallbladder function for severe icterohepatitis with damp-heat in liver and gallbladder.【Methods】Seventy-one severe icterohepatitis patients with damp-heat in liver and gallbladder were randomized into groups A(n=38) and B(n=33).The two groups received routine basic treatment and symptomatic treatment,group A was treated with heat-clearing,dampness-draining,blood-cooling and bile-draining herbs,and group B with intravenous drip of small-dose 654-2 additionally.Serum levels of total bilirubin(TBil),direct bilirubin(DBil),glutamic pyruvic transaminase(ALT),prothrombin activation(PTA) and jaundice duration were observed for the evaluation of jaundice-relieving effect.Meanwhile,the changes of gallbladder ultrasonograms were observed.【Results】After treatment,serum levels of TBil and ALT were decreased in both groups(P0.05)but the markedly effective rate in group A was higher than that in group B(P
2.Clinical observation of nicorandil in treatment of 85 patients with angina pectoris

ObjectiveTo observe the therapeutic effect of nicorandil in treatment of angina pectoris. Methods155 cases of angina pectoris patients were randomly divided into the therapeutic group(85 cases)and control group(70 cases),both of which took normal doses of aspirin vitamin C effervescent tablets 0.1g/day,clopidogrel 50mg/day,elantan 50mg/day and simvastatin 40mg/day,where hypertensions or diabetics were given blood pressure and sugar control and heart failure patients were given proper diuretic therapy and other methods to release heart Ioad.Patients in the therapeutic group were additionally given 5 mg nicorandil,3 times/day.The two groups were both given one period of treatment about one month. ResultsIn therapeutic group,32 cases had significant therapeutic effectiveness,34 cases had effectiveness and 14 cases had no effectiveness with a total effective rate of 82.5%.In the control group,23 cases had significant therapeutic effectiveness,28 cases had effectiveness and 24 cases had no effectiveness with a total effective rate of 67.0%.The comparative difference of the total effectiveness of the twogroups had statistical significance(x2 =4.40,P<0.05).No other significant adverse reaction was found except several cases of light dizzy in the two groups. ConclusionNicorandil has good therapeutic effective in treatment of angina pectoris and no significant adverse reaction.
3.Determination of tauroursodeoxycholic acid in compound bile capsule by HPLC.

OBJECTIVETo discriminate and determine of the artificial bear bile of the compound bile capsule.
METHODTaking the pharmacopoeia as reference, the artificial bear bile was discriminated and determined by HPLC.
RESULTThe compound bile capsule and the control sample had chromatographic peak at the same time from HPLC. The content of the artificial bear bile was above 10 mg per tablets.
CONCLUSIONThe artificial bear bile of compound bile capsules can be discriminated effectively and determined accurately by HPLC method.

4.Research status of conbercept treating age-related macular degeneration

Age-related macular degeneration( AMD) is one of the major reasons of blindness among the elderly in the developed countries. As AMD patients are increasing year by year, AMD has become one of the important topics of ophthalmic research to prevent blindness. Its pathogenesis is not fully understood, but many studies have shown that vascular endothelial growth factor ( VEGF ) plays an important role in the pathogenesis. With the development and application of anti - VEGF drugs, there are a variety of drugs applied to the disease. This article introduces conbercept for the treatment of AMD.
5.Treating non-alcoholic fatty liver disease accompanied by hyperlipidemia with Xuezhikang Capsule

AIM: To study the curative effect of Xuezhikang Capsule on non-alcoholic fatty liver disease accompanied by hyperlipidemia. METHODS: Small dosage of Xuezhikang Capsules had been administrated to the patients with non-alcoholic fatty liver disease accompanied by hyperlipidemia for 6 months. 32 healthy people had been chosen as control group at the same time. At the time points of before and after curing, body weight index. Type B ultrasonic, liver function, serum lipids, serum glucose and insulin levels had been tested. The McAuley index and the insulin resistance index of the homeostasis model assessment had been taken to evaluate insulin resistance. Liver biopsy had been conducted in some of the patients to compare the tissue changes before and after curing. RESULTS: After being treated with Xuezhikang Capsule, levels of insulin resistance serum lipids and ALT/AST in the patients with non-alcoliolic fatty liver disease had gone downward markedly. The histological observation in some of the patients had improved. No toxicity had been found. CONCLUSION: It is safe and potent to take the small dosage of Xuezhikang Capsule to cure the patients with non-alcoholic fatty liver disease accompanied by hyperlipidemia.
7.Clinical efficacy of central pancreatectomy and distal pancreatectomy:a Meta-analysis of the reserved endocrine and exocrine functions of the pancreas

BACKGROUND:Central pancreatectomy is a surgical treatment for tumors at the neck or the middle part of the pancreas, which can reserve more normal pancreas, not cut adjacent organs, and reduce the incidence of postoperative internal and external pancreatic secretion deficiency with respect to the expanded proximal and distal pancreatectomy. OBJECTIVE: To systematicaly evaluate the clinical efficacy of the central pancreatectomy and distal pancreatectomy. METHODS: A computer-based search of Chinese and English databases was performed, and then 15 controled clinical trials were included and systematicaly evaluated using RevMa5.2 software. RESULTS AND CONCLUSION: Totaly 1 079 cases were included in this study, which consisted of 436 central pancreatectomy cases and 643 distal pancreatectomy cases. Meta-analysis showed that compared with the distal pancreatectomy group, the incidences of postoperative pancreatic fistula and complications were significantly higher, the risk of postoperative endocrine and exocrine insufficiency were significantly lower, while the surgical time (SMD: 59.23, 95%CI: 22.41-96.05, P < 0.01) and hospital stays (SMD: 7.01, 95%CI: 1.94-12.09,P< 0.01) were longer in the central pancreatectomy group. These findings indicate that although the central pancreatectomy has a high postoperative complication incidence, it can be accepted clinicaly, which may be a reasonable operation method to preserve pancreatic exocrine and endocrine function.
8.A meta-analysis of early enteral nutrition versus total parenteral nutrition in patients with severe acute pancreatitis

Objective To study the effect of early enteral nutrition(EN) and total parenteral nutrition(TPN) in patients with severe acute pancreatitis by a mata analysis.Methods Randomized controlled trials of TPN and early EN in patients with severe acute pancreatitis were searched in Medline,Pubmed and Embase from Jan 1970 to June 2009,and Cochrane Library(Issue 2,2009).Seven studies were enrolled into the analysis.The detail about the trial design,characters of the subjects and results of the studies were reviewed by two independent authors and analysed by using Revman 5.0.18 software.Results Compared with TPN,early EN was associated with a significantly lower incidence of pancreatic infections(OR =0.38,95% CI :0.18~0.82,P =0.01),organ failure(OR =0.43,95% CI :0.23~0.79,P =0.007),fewer surgical interventions(OR =0.34,95% CI :0.18~0.63,P =0.0006) and mortaily(OR =0.41,95% CI :0.19~0.88,P =0.02).However,there was no significant difference in total infections [ OR =0.43,95% CI(0.17,1.10),P =0.08]between patients with TPN and early EN.Conclusions Early EN could be the preferred nutrition feeding method for patients with severe acute pancreatitis.
9.Posterior indirect reduction and internal fixation and laminectomy in the treatment of thoracolumbar vertebrae burst fracture complicated with spinal cord injury

Objective To explore the clinical effect of posterior indirect reduction and internal fixation and laminectomy in the treatment of thoracolumbar vertebrae burst fracture complicated with spinal cord injury.Methods Eighty patients with thoracolumbar vertebrae burst fracture and spinal cord injury treated in our hospital from March 2014 to March 2015 were selected as the objects,and they were divided into reset group and laminectomy group with forty cases in each group according to surgical method.All the patients were followed up for 1 year,the lumbar function of two groups at 1 week and 1 year after operation were observed respectively,and the pain degree was observed in 1month,3 months and 6 months after operation.The amount of bleeding,operation time,hospitalization time and fracture healing time were observed.Neurological function was assessed by classification criteria of the American Spinal Cord Injury Association(ASIA),and incidence of complications was figured in the two groups.Results The anterior heights of the injured vertebra were higher than those before the operation,and the Cobb's angles were lower than those before the operation,the differences were significant(P < 0.05);while there was no significant differences in the anterior heights of the injured vertebra between the two groups at 1 week and 1 year after operation(P > 0.05).VAS scores of the two groups after 1 month,3 months and 6 months decreased significantly when compared with the preoperative scores(P < 0.05),and VAS scores of each time in the reset group were significantly lower than those in the laminectomy group(P < 0.05).The amount of bleeding,operation time,hospitalization time and fracture healing time in the reset group were less than those in the laminectomy group (P < 0.05).The neurological function recovery of the two groups were significantly improved when compared with that before the operation(P <0.05).There was no significant difference in recovery of neurological function between the two groups(P > 0.05).The complication rate was 7.50% in the reset group,lower than 12.50% of the laminectomy group,the difference was significant (P < 0.05).Conclusion Posterior indirect reduction and internal fixation of lamina both have a certain effect in the treatment of thoracolumbar vertebrae burst fracture complicated with spinal cord injury.But posterior indirect reduction has less complications and less amount of bleeding,which is beneficial to postoperative recovery.
10.Comparison of injecting tissue glue and lauromacrogol in modiifed sandwich method with synchronization or sequential ligation for esophageal and gastric variceal

Objecive To evaluate and compare the efficacy of injecting tissue glue and lauromacrogol in modified sandwich method combined with synchronous ligation for patients with esophageal and gastric variceal.Methods 42 cases of patients with esophageal and gastric variceal were randomly divided into synchronous treatment group (22 cases) and sequential treatment group (20 cases). Both group received modiifed sandwich method for gastric varices (lauromacrogol-tissue adhesive-lauromacrogol). Synchronous treatment group meanwhile received synchronization ligation for esophageal varices, but sequential treatment group received sequential ligation after one week. The hemostasis rate, effective rate of varices, rebleeding and complications rate were analyzed.Results There were no signiifcant difference in hemostasis rate, effective rate and rebleeding rate between two groups. The incidence of complications, such as chest and abdominal pain, fever, transient bacteremia were equal between these two groups. No patients had ectopic embolism. The number of ligation device in synchronous treatment group was more than sequential treatment group (8.19 ± 2.01 vs 7.81 ± 1.78,P > 0.05), and the rate of esophageal varices disappearance was inferior to sequential treatment group (59.09% vs 70.00%), however, there were no statistical difference (P > 0.05).Conclusion Endoscopic injection of tissue glue and lauromacrogol synchronization ligation is safe and effective for esophageal and gastric variceal with low complication rates. This method could reduce gastroscopy times and medical expenses, and is worthy of further investigating.
