BACKGROUND:Metal vertebral fusion cage can induce some disadvantages and pseudoarticulation formation such as stress dodge, fusion cage subsidence, bone resorption, prolapse and bone collapse.Biodegradable absorbing material-made vertebral fusion cage can avoid disadvantages mentioned above. There are no studies on correlation of material function to design of absorbing fusion cage of lumbar vertebrae.OBJECTIVE:To design and fabricate the poly DL-lactic acid biodegradable fusion cage of lumbar vertebrae, and to analyze the material characteristics.DESIGN,TIME AND SETTING:The material in vivo experiment was performed at the Chengdu Institute of Organic Chemistry of Chinese Academy of Sciences from January 2003 to December 2006.MATERIALS:Twenty dogs aged 12 months were obtained from Animal Laboratory of West China,Sichuan University.Poly DL-lactic acid biodegradable fusion cage of lumbar vertebrae was produced by the Chengdu Institute of Organic Chemistry of Chinese Academy of Sciences.METHODS:According to the anatomical characters and measured data of lumbar intervertebral disc,we designed the poly DL-lactic acid biodegradable fusion cage of lumbar vertebrae. The rectangle cage consists of two styles:23 mm and 25 mm,13 mm in width, and 9,10,11 and 12 in depth(anterior wall). The depth was small half-moon shape from anterior to posterior, and the posterior wall was thinner than anterior wall about 1.5mm to accord with the shape of intervertebral discs. The depth of four walls of the fusion cage was 2.5 mm,with several 1-mm diameter holes on bilateral walls. The posterior wall had 3.5-mm diameter spiral hole, which was used for connecting with the implanted tool.Using precise shaping processing craft, the produce process was DL-lactic acid→raw material storage→lactide preparation→lactide purification→polymerization→drying package→product. After general anesthesia, bilateral thighs were incised in 20 dogs, and then a fusion cage was implanted into each dog to detect compressive strength and viscosity in the process of biodegradation.MAIN OUTCOME MEASURES:Preparation of fusion cage;compressive strength and viscosity in the process of biodegradation in vivo.RESULTS:The size of the products is 23 mm?13 mm?10 mm and the appearance of the products were saw tooth shape, which could prevent the slide, enhance the solid strength between fusion cage and the surface of vertebra.Small holes on the bilateral wall were beneficial for vascular growth and nutrient fluid exchange, resulting in bony fusion.Compressive strength and viscosity decreased at 3,6 and 9 months after implantation(r=0.961,P=0.009),but compressive strength and viscosity were good at 12 months(163 MPa and 0.66 dl/g).CONCLUSION:Poly DL-lactic acid biodegradable fusion cage has a big loading implanted bone mass and high stability. The integrity and supportive effect of the fusion cage can meet the requirement of bony fusion.

BackgroundResearches found that the posterior capsular opacification (PCO) after lensextraction is associated with the elevation of the transforming growth factor-β(TGF-β).To seek the drug for inhibitingproliferation of lens epithelial cells (LECs) is crucial in the treatment and prevention of PCO.ObjectiveThisstudy was to investigate the preventing effects of decorin on the proliferation of LECs.MethodsRabbit LECs wascultured and passaged. The LECs in growth phase were incubated in 96 well plate at the density of 8×106/L.Decorinwith the concentrations 0.1,1.0,10.0 mg/L was added into the medium for 24,48 and 72 hours respectively.0.1%DMSO was used at the same way as positive control group, and the regular cultured cells worked as blank controlgroup. The inhibitory rates of different concentrations of decorin on the growth of LECs were detected by MTT at 24,48and 72 hours after addition of decorin. The percentage of LECs in different cell cycles in various groups was assayedusing flow cytometry.TGF-β level in medium suspension was detected using ELISA.The expression of TGF-β mRNA in LECs was checked by RT-PCR,and α-SMA expression in LECs was determined using immunochemistry. Results ELISA assay showed a statistical difference in the TGF-β levels of different groups (F=39.24,P=0.03 ).The TGF-β levels in 1.0,10.0 mg/L decorin groups were significantly decreased in comparison with blank control group (P<0.01) and 0.1 mg/L decorin group (P<0.05 ).The inhibitory rates of decorin in the concentrations of ≥ 1.0 mg/L on the growth of LECs were higher than the blank control group, and those in various concentrations of decorin groups were considerably lower in 24 hours compared with 48 and 72 hours ( P<0.05 ) and so was the 48 hours compared with 72 hours (P<0.05 ).The percentages of LECs in G0/G1 phase were ascent in 0.1,1.0 and 10.0 mg/L decorin groups in comparison with G2/M and S phase (P<0.05).Immunochemistry revealed the weak expression of α-SMA in various decorin groups in comparison with control group. Conclusions Decorin can effectively inhibit LECs growth and induce LECs apoptosis in concentration- and time-dependent manner. It is suggested that decorin can be used in the prevention and treatment of after cataract.

OBJECTIVETo observe the effect of Shen warming Pi strengthening method on expressions of serum T cell subsets (C045+%, C03+%, and C04 +/COB+) in diarrhea-predominant irritable bowel syndrome (IBS-0) rats. Methods An IBS-0 rat model was established referring to AL-Chaer's modeling method combined with tail clamp and intragastric administration of sanna leaf. After modeling 30 SO rats were randomly divided into 6 groups according to random digit table, i.e., the model group, the high, middle, low dose Wenshen Jianpi Recipe (WJR) groups, and the Sishen Pill control group, 6 in each group. A normal control group consisting of 6 SO rats were also set up. Rats in high, middle, low dose WJ R groups were administered by gastrogavage with boil-free WJ R at the daily dose of 3. 100, 1. 550, 0. 775 g/kg, respectively. Rats in the Sis hen Pill control group were administered by gastrogavage with boil-free Sis hen Pill at the daily dose of 0. 736 g/kg. Equal volume of normal saline was given by gastrogavage to rats in the model group and the normal control group. All medication lasted for 2 successive weeks. Rats' general state, expressions of T cell subsets (CD45+%, CD3+%, and CD4+ /CDB+) changes were observed.
RESULTSCompared with the normal control group, expressions of CD45+% and CD3+% increased, but CD4+ /CDB+ decreased with statistical difference (P < 0. 05). Compared with the model group, expressions of CD45+% and CD3+% decreased, but CD4+ ICDB+ increased with statistical difference in high, middle, low dose WJR groups, and the Sis hen Pill control group (P <0. 05). Compared with the Sis hen Pill control group, there was statistical difference in all indices except CD45+ value in the low dose SWPSM group (P <0. 05). Compared with the low dose WJ R group, the expression of CD3+% decreased in high and middle dose WJR groups, and the Sis hen Pill control group; CD4+ /CD8+ increased in the Sishen Pill control group and the high dose SWPSM group (all P < 0. 05).
CONCLUSIONSWJR showed better treatment effect. The mechanism of Shen warming Pi strengthening method might be achieved by regulating expressions of CD45+% and CD3+%, and CD4+ /CD8+ ratios.

BACKGROUND: Minimally invasive total hip arthroplasty is less invasive and has less tissue injury than traditional approach. However, perioperative anemia is still common, affecting hip function recovery and increasing patient burden. OBJECTIVE: To assess the curative effects of recombinant human erythropoietin on hemorrhagic anemia after minimally invasive total hip arthroplasty. METHODS: Clinical data of 35 patients undergoing SuperPATH minimally invasive total hip arthroplasty in the Department of Orthopedic Surgery, the Affiliated Hospital/Clinical Medical College of Chengdu University were collected. The patients were divided into the control group (n=17) and the erythropoietin group (n=18) according to the presence and absence of erythropoietin. The patients in the erythropoietin group were subcutaneously injected with recombinant human erythropoietin 3 days before replacement, once a day, lasting to 8 days after operation, for 12 consecutive days. During erythropoietin administration, iron sucrose was added once a day. Tranexamic acid 1 g was intravenously infused 0.5 hour before replacement, and 1 g for washing medullary cavity during replacement. The patients in the control group only took iron sucrose, once a day, adding tranexamic acid through intravenous infusion 0.5 hour before replacement. Levels of hemoglobin and hematocrit were monitored on the day of admission, the preoperative 3 days, the operation day, the postoperative 4 days and the postoperative 8 days. The operation time, blood loss, Visual Analogue Scale pain score 4 days before and after replacement and Harris hip function score before replacement, 4 and 8 days after replacement were recorded. RESULTS AND CONCLUSION: (1) Baseline data were not significantly different between the two groups (P > 0.05). (2) Visual Analogue Scale pain score significantly decreased 4 days after replacement in both groups (P < 0.05), and was not significantly different between both groups at various time points (P > 0.05). Harris hip score was not significantly different before and 1 day after replacement between the two groups (P > 0.05). At 4 days after replacement, Harris hip score was significantly higher in the erythropoietin group than in the control group (P< 0.05). Mean blood loss and operation time were not significantly different between the two groups (P > 0.05). (3) Levels of hemoglobin and hematocrit were not significantly different on the day of admission and 1 day before replacement (P > 0.05). Levels of hemoglobin and hematocrit were higher in the erythropoietin group than in the control group during emergency examination, 4 and 8 days after replacement (P< 0.05). The changes in levels of hemoglobin and hematocrit were consistent in different groups. The decreased trend was smooth, but the increased trend was relatively steep in the erythropoietin group. (4) Results confirmed that recombinant human erythropoietin combined with iron sucrose combined adding tranexamic acid, can effectively improve the anemia in perioperative period of minimally invasive total hip arthroplasty. Moreover, hemoglobin levels can increase to the stable and normal levels.
