1.Clinical observation on acupoint pressure plus long-snake moxibustion for upper-limb spastic hemiplegia after cerebral infarction

Objective: To observe the clinical efficacy of acupoint pressure plus long-snake moxibustion for upper-limb spastic hemiplegia after cerebral infarction. Methods: A total of 100 patients were randomized into a control group and an observation group, with 50 cases in each group. Both groups were treated with the same conventional internal medicine and rehabilitation training. The control group was treated with additional acupoint pressure therapy, and the observation group was treated with long-snake moxibustion on the basis of the treatment given to the control group. The Ashworth grade, Fugl-Meyer assessment upper limb scale (FMA-UL) and Barthel index (BI) were evaluated, and the root mean square (RMS) values of biceps brachii and flexor carpi radialis on the affected side were measured before and after treatment. The efficacy was evaluated after treatment. Results: After treatment, the total effective rate of the observation group was significantly higher than that of the control group (P<0.05). After treatment, the Ashworth grade of the observation group was superior to that of the control group (P<0.05). The scores of FMA-UL and BI in both groups increased compared with those before treatment (all P<0.05), and the scores of FMA-UL and BI in the observation group were higher than those in the control group (both P<0.05). The RMS values of biceps brachii and flexor carpi radialis in both groups decreased compared with those before treatment (all P<0.05), and the RMS values of biceps brachii and flexor carpi radialis in the observation group were lower than those in the control group (both P<0.05). Conclusion: Based on conventional internal medicine and rehabilitation training, acupoint pressure plus long-snake moxibustion has great therapeutic efficacy for upper-limb spastic hemiplegia after cerebral infarction. It can improve the degree of spasticity of the affected upper limb, reduce the muscle tone of biceps brachii and flexor carpi radialis on the affected side, and enhance the mobility of the affected limb and the activities of daily living.
2.Progress on treatment and research of quadrilateral plate fractures of acetabular.

Acetabular is an important human joint for weight bearing. Quadrilateral plate is a crucial structure of medial acetabulum with special morphology and important function. Quadrilateral plate fractures are common fracture in acetabulum. Quadrilateral plate fracture is hard to expose and reduction because it is in the medial of acetabulum. At the same time,the bone in the quadrilateral plate is not easy to fixed for thinning bones and adjacent to the articular cavity. The operator should know well about the anatomy and choose the suitable internal fixation. After quadrilateral plate fractures, the femur head maybe displace medially even break into pelvis. That make reduction and treatment always be a challenge. With different kinds of fractures,the efficacy of treatment is not the same. This paper intend to review the relation of anatomic features,approaches, internal fixations, key point of treatment and efficacy.

4.Effect of midazolam pretreatment on propofol sedation by closed-loop TCI during epidural anesthesia

Objective To compare the effect of midazolam pretreatment on propofol sedation using closed-loop target-controlled infusion (TCI) between two age groups - the adult and the aged. Methods Forty-eight ASA Ⅰ-Ⅱ patients of both sexes weighing 45-81 kg undergoing elective lower abdominal or lower extremity operation under epidural anesthesia were divided into two age groups : (A) the adult group (18-39 yrs) and (B) the aged group (66-79 yrs). The two groups were further divided randomly into 2 subgroups : midazolam subgroup ( n=12) received midazolam 0.04 mg?kg-1 10 min before propofol TCI and placebo subgroup ( n = 12) received normal saline instead of midazolam 10 min before propofol TCI. The patients were unpremedicated. An intravenous line was established before operation, which was connected to a TCI system comprising a Graseby 3500 infusion pump and a closed-loop TCI automatic control system. BP, HR, SpO2 and BIS were continuously monitored during operation. During epidural anesthesia the patients were sedated with propofol administered by TCI. The initial target blood concentration of propofol was set at 1.5?g?ml-1 . The level of sedation was assessed by OAA/S scale (5 = alert,0 = no response to prodding). The target blood propofol concentration was then increased or decreased in 0.5?g?ml-1 increment to maintain OAA/S score at 3. The BIS value at this level of sedation (OAA/S=3) was used as feedback in controlling TCI of propofol. The induction dose and the total dose of propofol, induction time and emergence time (OAA/S=5) were recorded. Results Midazolam premeditation significantly reduced the induction dose and total dose of propofol, shorten the induction time and prolonged the emergence time compared with placebo in both groups, especially in the aged group (P
5.Reform on Classroom Teaching for Training Student to Have Innovative and Applied Ability

At the present time, training student to have innovative and applied ability has become the object of higher education in China. In this paper, it was proposed that teacher was obligated to create certain atmospheres in classroom to achieve this goal, including friendliness, harmoniousness, encouragement, happiness, discussion, exploration, etc. At the same time, student-centered study should be encouraged. Through these measures, the spirit of innovation will be inspired, the applied ability will be trained, the capability of self-study will be enhanced.
6.Clinical efficacy observation of heat-sensitive moxibustion with different doses for irritable bowel syndrome.

OBJECTIVETo compare differences of clinical therapeutic effect of heat-sensitive moxibustion with different doses for irritable bowel syndrome (IBS).
METHODSSixty cases of IBS were randomly divided into a saturated-dose group (30 cases) and a traditional-dose group (30 cases). Heat-sensitive moxibustion was applied in both groups. The acupoints that had the strongest heat-sensitive feeling were selected and treated by warm and suspended moxibustion with moxa stick. Disappearance of heat-sensitive feeling was taken as the sign of treatment time in the saturated-dose group, while the traditional-dose group was treated for 15 min each time. The treatment in both groups was given twice a day for first 5 days, and from the sixth day it was given once a day for continuous 25 times, totally 30 days. Clinical symptom scores and therapeutic effect before and after treatment in two groups were observed.
RESULTSAfter the treatment, the cured and markedly effective rate was 75.0% (21/28) in the saturated-dose group, which was inferior to 44.4% (12/27) in the traditional-dose group (P < 0.05). The clinical symptom scores, including diarrhea, abdominal distension and pain, were obviously reduced in two groups compared with those before the treatment (all P < 0.05). Compared with the traditional-dose group, the symptom scores of diarrhea and abdominal distension in the saturated-dose group were obviously decreased (0.87 +/- 0.13 vs 1.27 +/- 0.21, P < 0.01; 1.12 +/- 0.41 vs 1.32 +/- 0.26, P < 0.05).
CONCLUSIONThe moxibustion featured with disappearance of heat-sensitive feeling and saturated dose has better therapeutic effect than that with traditional-dose for treatment of IBS. As individual dynamic amount of moxibustion, disappearance of heat-sensitive and quantitation varies from person to person, which is one of the key factors to obtain the best curative effect.

7.Free superficial iliac circumflex artery skin flap: the clinical application and management of donor site defects.

OBJECTIVETo investigate the clinical application of free superficial iliac circumflex artery skin flaps, as well as the management of donor site defects.
METHODS17 free superficial iliac circumflex artery skin flaps were applied for the traumatic defects or deformities on face, neck, foot, hand, ankle and lower leg, respectively. The donor site defects were closed directly or covered by paraumbilical island flaps.
RESULTSThe 17 flap size ranged from 5 cm x 3 cm to 19 cm x 14 cm. 16 flaps survived completely except 1 flap with partial necrosis, which was closed by free skin graft. The donor site defects were closed directly in 10 cases, and covered by paraumbilical island flaps in 7 flaps without no flap necrosis. The abdomen had a good appearance.
CONCLUSIONSGood appearance can be achieved with free superficial iliac circumflex artery skin flaps for the defects on face, neck, foot, hand, ankle and lower leg. Paraumbilical island flap can be used for the donor site defects.

9.Reconstitution kinetics of dendritic cells following allogeneic hematopoietic stem cell transplantation.

As the most important antigen present cells in the processes of post-transplant complications in vivo, dendritic cells (DCs) play an important role in graft versus host disease (GVHD), infection, and relapse. 10.Construction and identification of ha1 recombinant lentiviral vector.

This study was aimed to construct a lentiviral vector of ha1 gene and obtain a stable packaging cell lines, which will be beneficial to guiding further study on the foundation of ha1 on the dissociation of graft versus host disease (GVHD) and graft versus leukemia (GVL) effect after allogenic hematopoietic stem cell transplantation (allo-HSCT). The targeting ha1 gene was obtained from plasmid T-ha1 by digesting. Then the ha1 gene was subcloned into the lentiviral vector pRRLSIN, cPPT, PGK/GFP, WPRE to construct a lentiviral vector carrying ha1 gene named pLenti-ha1 plasmid. And the pLenti-ha1 plasmid was confirmed by PCR, digesting and sequencing. 293T cells were co-transfected with lentiviral vector pLenti-ha1 and packaging system. The titer of virus was tested by real-time PCR. The results indicated that the construction of ha1 recombinant lentiviral vector was confirmed to be exact by PCR assay, digesting and sequencing; the assayed titer of virus was 2.0 × 10(8) TU/ml. In conclusion, the lentiviral vector of ha1 gene is constructed successfully.
