1.Application value of DOSE Index Scoring System in Chronic Obstructive Pulmonary Disease

Objective To explore the application value of DOSE index score in the peitients with chronic obstructive pulmonary disease.Methods 122 cases of plateau COPD patients were followed up for 12 months,and we recorded and analyzed the patient's health and life.We also recorde FEV 1 and DOSE scores of the patients with COPD,and record the COPD risk events,including the number of respiratory failure and death,and the times of hospitalization,total such confinement,outpatient expenses,hospitalization expenses,mMRC,and scored in the number of exacerbations,etc.Results The DOSE index score was negatively correlated with FEV1% pred (r=0.73,P < 0.05) for 122 COPD patients,and were positively correlated with mMRC (r=085,P < 0.01),the annual number of exacerbations (r=0.71,P < 0.01),respiratory failure (r=0.65,P < 0.01),heart failure (r=0.50,P < 0.01),number of outpatient service (r=0.12,P<0.01),hospitalization time (r=0.70,P<0.01),the totalsuchconfinement (r=0.66,P<0.01),outpatient expenses (r=0.13,P> 0.13),hospitalization expenses (r=0.65,P<0.01).ROC curve was used to analyzed the cut-off point and curve area of COPD DOSE index.Conclusion The DOSE index is a simple COPD assessment tools,and is closely related to the prognosis of patients and health.
2.Stages of medical rescue after disaster based on clinical features and their significance

Objective To define the stages of medical rescue after earthquake or tsunami disaster and their significance.Method Three stages of medical rescue were defined according to the clinical features of diseases in 3-37 days after earthquake or tsunami in Pakistan and Indonesia.The classification of diseases was based on the criteria of disaster medical rescue set by World Health Organization.Five diseases including injuries/wounds,acute upper respiratory tract infection,acute hemorrhagic enterocolitis,acute enterecolitis and other diseases of internal medicine except epidemic or suspected epidemic diseases were employed to define the stages of disaster medical rescue.Results Injuries/wounds mainly occurred in 6 days after disaster.The incidences were 61.45% to 79.52% and 61.48% to 72.35% in Pakistan and Indonesia,respectively.The incidence of acute upper respiratory tract infection reached to a considerably high rate one week after disaster.Other diseases usually appeared one week later with incidence of 33.93% to 71.11% and 31.50% to 52.11% in Pakistan and Indonesia,respectively.The rate of acute hemorrhagic enterocolitis was 2%o in Indonesia whereas 35‰of acute enterocolitis was found in Pakistan.Conclusion Three stages of disaster medical rescue could be well defined.(1)Early or critical stage(in 6 days after disaster):Treatment of injuries/wound and resuscitation was the primary step to deal with.(2)Intermediate of subaeute stage(7-30days following disaster):The incidences of various kinds of diseases increased and more medical resources were needed in order to treat the upper respiratory tract infection and to lower the disable rate,and to prevent the prevalence of epidemic diseases.(3)Later or recovery stage(1-3months after disaster):The general condition of victims became even better than that of those affected by commonly occurred local diseases,and during this period,there were shortages of medical facilities and medical personnel.Therefore,supplement of medical resources was required.The well defined stages of medical rescue have an important significance for medical rescue teams to treat victims and prevent epidemic diseases more efficiently and timely,and also provide a guidance of scientific basis for the logistics officers of local government as well as our government to make a proper and reasonable distribution of medical resources,and dispatch and arrangement of medical rescue teams.This guidance also helps to make victims re- reliant.
3.AC OCT of the anterior chamber iris-fixated phakic intraocular lens in high myopia eyes

· AIM: To study the in situ relative intraocular position of the iris-claw phakic intraocular lens (PIOL)for high myopia using an anterior chamber optical coherence tomography (AC OCT)prototype.· METHODS: Six PIOLs (11.50 to 22.00D lens powers) were implanted in phakic myopic eyes. Using AC OCT, tomography was taken in the anterior chamber to measure the preoperative anterior chamber depth, postoperative distance between the PIOL and the corneal endothelium (endothelial-optic distance), and the postoperative distance between the PIOL and the crystalline lens.· RESULTS: Preoperative anterior chamber depth ranged from 3.27 to 3.91 mm and the postoperative endothelial-optic distance measured 2,07 to 2,24 mm. The distance between the crystalline lens and the posterior surface of the IOL ranged from 0.82 to 1.32 mm. Several tomography revealed the position of the PIOL on the iris, The pigment layer of the iris did not seem to be disturbed by the presence of the PIOL.· CONCLUSION, The original anterior chamber depths were reduced by 36,1% to 44.6% after implantation. This study of 6 eyes revealed that tomography taken by AC OCT are useful in verifying the intraocular position of the PIOL within the anterior chamber. Adequate space was maintained between the iris-fixated phakic intraocular lens and the corneal endothelium, angle, and crystalline lens.
4.Research on the construction of the 5 +3 integrated clinical medical personnel training objective system

Objective To construct the objective system of 5+3 integrated clinical medicine personnel training mode.Methods For the attitude,knowledge and ability clinical medical students should have,who received 5+3 integrated training,we adopted literature research,expert interviews intending to make a preliminary questionnaire items,and invited experts and graduates to modify the expressions of the items a number of times,and eventually formed the final questionnaire that met the study objective.We asked 500 teachers in basic medicine,clinical medicine,and medical humanities and so on,to evaluate the importance of the questionnaire.406 valid questionnaires were recovered and the effective recovery rate was 81.2%.Epidata 3.1 and SPSS 19.0 were used for survey data summary statistics,exploratory factor analysis and factor weighting method were used to get the goal of talent training base,and on these basis target system was constructed.Results In 5+3 integrated medical education model,clinical medical personnel training target system included knowledge,attitude and ability of the part,a total of 9 target groups.Each group contained elements in the training target system for different weights,among which attitude included a common factor,proportion of 27.51%;knowledge consisted of three factors,the proportion of 29.34%;and the ability contained five common factors,accounting for highest proportion of 43.15%.Conclusion 5+3 integrated clinical medical talents training target system established in this study highlights the ability requirements,emphasizes the comprehensive quality training,which accords with the principles of talent training goal and the requirements of 5+3 medical education model,has certain guiding significance for the curriculum reform,and can be used for reference in the development of talents training program for medical colleges and universities.
5.Study on drug properties of components in Euodiae Fructus based on rat stress gastric ulcer model with cold syndrome.

OBJECTIVETo clarify drug properties of components in Euodiae Fructus.
METHODThe rat cold syndrome model was induced by cold water stress method. The content of neurotransmitters sand hormones such as DA, 5-HT, NE, AChE and 17-OHCS in serum of model rats were taken as the indexes to evaluate drug properties of components in Euodiae Fructus.
RESULTEuodiae Fructus and its components could correct or relief the content of energy metabolism and substance metabolism-related neurotransmitters sand hormones in serum of model rats with water-stressed cold syndrome.
CONCLUSIONEuodiae Fructus and its components are proved to show hot property.

6.A new feruloyl tyramine glycoside from the roots of Achyranthes bidentata.

AIM:
To study the chemical constituents of the roots of Achyranthes bidentata Bl.
METHODS:
The chemical constituents were isolated and purified by macroporous adsorptive resin D101, silica gel, and ODS column chromatographies and preparetive HPLC. Their structures were elucidated on the basis of 1D and 2D NMR analyses.
RESULTS:
Two feruloyl tyramine glycosides and seven triterpenoid saponins were obtained and identified as N-trans-feruloyl-3-methoxytyramine-4'-O-β-D-glucopyranoside (1), N-trans-feruloyl-3-methoxytyra mine-4-O-β-D-glucopyranoside (2), PJS-1 (3), chikusetsusaponin IVa (4), oleanolic acid 3-O-[β-D-glucuronopy ranoside-6-O-methyl ester]-28-O-β-D-glucopyranoside (5), oleanolic acid 3-O-[β-D-glucuronopyran-oside-6-O-ethylester]-28-O-β-D-glucopyranoside (6), oleanolic acid 3-O-[β-D-glucuronopyranoside-6-O-butyl ester]-28-O-β-D-glucopyranoside (7), ginsenoside R(0) (8) and hederagenin-28-O-β-D-glucopyranosyl ester (9).
CONCLUSION
Compound 1 is a new feruloyl tyramine glycoside, while compounds 2 and 9 are reported from A. bidentata for the first time.

8.Analysis on the depression related factors in elderly patients after stroke and the effect of Chinese medicine on them.

OBJECTIVETo study the depression related factors in elderly patients after stroke and to explore the effect of Chinese medicine anti-depression treatment for improving neurological function in patients with after-stroke depression.
METHODSThree hundred and seventy-seven after-stroke elderly patients were sorted, according to their Hamilton Depression Scale-17 (HAMD17) scores, to the non-depression group (NDG, 116 patients) and the depression group (DG, 261 patients). The depression related factors in them were analysed. Moreover, patients in DG were randomly subassigned to two groups, the 135 patients in the treated group were treated with Chinese medicine and general stroke-treatment, and the 126 patients in the control group were treated only with general stroke-treatment for 4 weeks. HAMD17 scores of neurological deficit (NDS), scores of sleep dysfunction rating scale (SDRS), Hamilton anxiety rating scale (HAMA), and treatment emergent symptom scale (TESS) in them were observed before and after treatment.
RESULTSPrevalence rate of depression in patients with cerebral hemorrhage was 79.78% (71/89), significantly higher than that in patients with cerebral infarction, 65.97% (190/288, P < 0.05). Patients of stroke with lesion occurred at cerebellum, brain stem, or cerebral hemisphere (frontal lobe) and those with moderate/severe neurological deficit were more liable to suffer from depression. The HAMD17, HAMA, SDRS, and NDS scores in the treated group after treatment were all improved more significantly than in the control group (P < 0.05). NDS score reduced in the treated group significantly from 19.1 +/- 16.3 before treatment to 31.3 +/- 14.8 after treatment, showing the evident recovery of nerve function.
CONCLUSIONThe occurrence of depression after stroke is closely related with the nature and position of the lesion, as well as the degree of neurological deficit in patients. Chinese medicine could improve the depressive manner effectively and thus to make for recovery of neurological function.

9.The effect of CD137-CD137 ligand interaction on the expression of nuclear factor of activated T cells c1 in apolipoprotein E-deficient mice atherosclerotic plaque model.

OBJECTIVETo investigate the effects of CD137-CD137L interaction on the nuclear factor of activated T cells c1 (NFATc1) in apolipoprotein E knockout (ApoE(-/-)) mice.
METHODSAtherosclerotic plaque model was produced by rapid perivascular carotid collar placement in ApoE(-/-) mice. In vivo, the expression of NFATc1 in mice plaque and lymphocytes was detected by immunohistochemical and flow cytometry, respectively. In vitro, the NFATc1 mRNA and protein expressions in cultured lymphocytes of ApoE(-/-) mice were measured by RT-PCR and flow cytometry, respectively.
RESULTSIn vivo, after stimulating CD137-CD137L signal pathway, the expression of NFATc1 was significantly upregulated in the atherosclerotic plaques and lymphocytes. In vitro, the mRNA and protein expressions of NFATc1 in cultured leukocytes of ApoE(-/-) mice were also significantly increased, the maximal effect appeared post 20 µg/ml anti-CD137 mAb-stimulation and reached maximum at 24 h at any concentrations. Anti-CD137L mAb significantly downregulated the mRNA and protein expressions of NFATc1 in lymphocytes of ApoE(-/-) mice, maximal effect appeared at 20 µg/ml anti-CD137L mAb and reached minimum at 24 h.
CONCLUSIONCD137-CD137L interactions can modulate the expression of NFATc1 in this ApoE(-/-) mice atherosclerotic plaque model.

Cultivation of innovative dental talents is highly requested by modernization of China. Textbooks are crucial in cultivating high quality students of stomatology. A completely new epoch proposition appeared on how to construct an innovative teaching textbook system. The countermeasures, which are aiming at such aspects as the textbook evaluation system, author groups and textbook systems, are discussed in this paper.
