Objective:To determine the valproate concentration in plasma of epilepsy patients by HPLC, and compare with the re-sults of chemiluminescence microparticle immunoassay ( CMIA) to evaluate the consistency of the two methods. Methods:HPLC and CMIA was respectively applied to determine the plasma concentration of valproate in 230 epileptic patients. The correlation of the two methods was studied by Passing-Bablok regression and Bland-Altman method. Results:The regression equation of the determination re-sults of HPLC (Y) and CMIA (X) was Y=1. 069 7X+2. 338 2 (R2 =0. 969, n=230), which showed promising correlation. Bland-Altman analysis showed that the consistency of the two methods was poor, and the values of HPLC were higher. Conclusion: HPLC and CMIA used for the determination of valproate plasma concentration show good correlation. However, the consistency is poor and there is system error. In the clinical treatment, adjustment and choice should be paid more attention.

Objective:
To understand the relationship between mental health status and vision-related quality of life( VRQL ) of students with moderate and high myopia, and to provide basis for the optimization of VRQL.
Methods:
Using convenient sampling method, the 8-18 years old students with moderate and high myopia were selected from Sichuan, Chongqing and Zhejiang. The mental health status and VRQL of students with moderate and high myopia were evaluated with Hospital Anxiety and Depression Scale and Quality of Life Scale for Ametropia. The multiple linear regression model was used to analyze the influencing factors for VRQL.
Results:
A total of 360 questionnaires were sent out and 354 were effectively recovered, with an effective rate of 98.33%. There were 116 ( 32.77% ) boys and 238 ( 67.23% ) girls; the median age was 16.65 years old. There were 211 ( 59.60% ) cases of moderate myopia and 143 (40.40%) cases of high myopia. There were 141 ( 39.83% ) found to be anxious and 176 ( 49.72% ) depressed. The median score of Quality of Life Scale for Ametropia was 64. The results of multiple linear regression analysis showed that sex ( β'= -0.179 ), diopter ( β'= 0.208 ), eyesight with glasses ( β'= -0.229 ) and anxiety ( β'= 0.439 ) were influencing factors for VRQL.
Conclusion
The mental health problems of the students with medium and high myopia are prominent; anxiety has a significant impact on the VRQL of the students.

Objective To evaluate the clinical efficacy and safety of enalapril or irbesartan combined with hydrochlorothiazide in the treatment of medium or severe hypertension. Methods Sixty -nine patients with moderate to severe essential hypertension were randomly divided into control group ( n=34) with enalapril 10 mg +hydrochlorothiazide 12.5 mg orally bid and treatment group ( n =35 ) with irbesartan 50 mg +hydrochlorothiazide 12.5 mg orally bid with 2 months. After treatment, the clinical efficacy, serum level of creatinine, acidum uricum and potassium and the ratio of adverse event were compared between the two groups. Results After 2 months treatment, the blood pressure was significant decreased in control and treatment groups. The clinical efficacy were 88.24% and 88.57% with not statistical difference ( P >0.05 ) .The serum level of creatinine, acidum uricum and potassium had no obvious difference before and after the treatment in the two groups (P>0.05).Four cases of cough were observed in the control group and the treatment group, respectively, without statistical difference (P>0.05).Conclusion There was no clinical efficacy difference for enalapril or irbesartan combined with hydrochlorothiazide in the treatment of medium or severe hypertension. But irbesartan combined with hydrochlorothiazide had less cough adverse event.
8.The different roles of the spinal protein nNOS and iNOS in morphine naloxone-precipitated withdrawal response.
Chinese Journal of Applied Physiology 2012;28(3):249-253
OBJECTIVETo explore the effects of intrathecal injection of neuronal nitric oxide synthase (nNOS) inhibitors 7-Nitroindazole (7-Ni) and inducible nitric oxide synthase(iNOS) inhibitors aminoguanidine (AG) on the behavioral changes of morphine-induced dependent and withdrawal rats; the expression of Fos, nNOS and iNOS in spinal cord.
METHODSTo set up morphine dependence model, rats were subcutaneously injected with morphine (twice a day, for 5 d). The dose of morphine was 10 mg/kg in the first day and was increased by 10 mg/ kg every day. On day 6, 4 h after the injection of morphine (50 mg/kg), morphine withdrawal syndrome was precipitated by an injection of naloxone (4 mg/kg ip). 7-Ni, an nNOS inhibitor or iNOS inhibitors AG were intrathecally injected 30 min before the administration of naloxone respectively. The scores of morphine withdrawal symptom and morphine withdrawal-induced allodynia were observed. One hour after naloxone-precipitated withdrawal, Fos protein expression was assessed by immunohistochemical analysis and Western blot was used to detect the expression of nNOS and iNOS in the rat spinal cord.
RESULTSIntrathecal administration of nNOS inhibitor 7-Ni and iNOS inhibitors AG decreased the scores of morphine withdrawal, attenuated morphine withdrawal-induced allodynia and also inhibited the increase of Fos protein expression in the spinal cord of morphine withdrawal rats. nNOS and iNOS positive neurons in dorsal horn in nNOS group and iNOS group were significantly lower than that in withdrawal group. Compared with withdrawal group, level of nNOS and iNOS protein in spinal cord in nNOS group and iNOS group were significantly lower.
CONCLUSIONIt is suggested that nNOS and iNOS in the spinal cord may contribute to naloxone-precipitated withdrawal in rats and may play different roles in the above-mentioned effect.

AlM:To compare the differences of tilt and decentration of two aspheric posterior chamber intraocular lens ( PC-lOL) implantation by ultrasonic biomicroscope ( UBM) .METHODS:Thirty-seven patients ( 45 eyes ) underwent cataract surgery were distributed to two groups randomly. Group A was implanted with Akreos AO ( Bausch &Lomb; four-haptic ) while group B implanted with ZCB00 ( Abbott Medical Optics, lnc. AMO; two-haptic) . All eyes underwent standard phacoemulsification with intraocular lens implantation. Diameter of capsulotomy was recorded. One month postoperatively, vision, best-corrected visual acuity ( BCVA) assessment, slit lamp examination, and anterior chamber depth ( ACD ) measured by UBM were performed. Tilt and decentration were measured horizontally and vertically, and total tilt and decentration were calculated by geometry method.
RESULTS:No statistical difference was found in BCVA and diameter of capsulotomy between two groups ( P>0. 05). The mean ACD of group A and group B were 3. 86mm ± 0. 31mm and 4. 14mm ± 0. 31mm respectively, which showed it had statistically significant difference ( P<0. 05). Horizontal decentration, vertical decentration and total decentration of group A were 0. 15mm ± 0. 09mm, 0. 15mm ± 0. 12mm and 0. 22mm ± 0. 12mm respectively, while it were 0. 22mm ± 0. 21mm, 0. 14mm ± 0. 15mm, 0. 29mm±0. 22mm for group B. Horizontal tilt, vertical tilt and total tilt of group A were 0. 63°±0. 62°, 0. 89°±0. 85°and 1.22°±0.76°, while it were 1.36°±1.48°, 1.46°±1.62° and 2. 21°±1. 97° for group B. No statistically significance was found in tilt and decentration between group A and group B, no matter horizontally or vertically or totally (P>0. 05).CONCLUSlON:Two-haptic lOL shows no difference in tilt and decentration with four-haptic lOL postoperatively.
