1.Ginkgo biloba extract inhibits the expressions of cell adhesion molecules and nuclear factor-kappa B in arterial endothelial cells

BACKGROUND: Previous studies have indicated that hyperhomocysteinemia can induce atherosclerosis by enhancing oxidative stress, whereas Ginkgo biloba extract (GbE) can scavenge oxygen-derived free radicals.OBJECTIVE: To observe the role of reactive oxygen species (ROS) and nuclear factor-kappa B (NF-κB) in the expression of cell adhesion molecules (CAMs) induced by homocysteine (Hcy), and investigate the effect of GbE.DESIGN: A randomized controlled animal trial.SETTING: Department of Neurology, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology.MATERrALS: Twenty-four healthy male rabbits of 6 months old. Dl-methionine (Sigma Chemical, Co.,Ltd.); GbE (Guizhou Yibai Pharmaceutical Company; powder).METHODS: The experiment was accomplished in the Laboratory of Department of Neurology, Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from March 2003 to April 2004. ① After adaptive feeding for 2 weeks, the rabbits were randomly divided into three groups: Model group (n =12): The rabbits were treated with subcutaneous injection of dl-methionine (80 mg/kg per day); GbE group (n =8): The rabbits were administrated with GbE (mixed with feed, 50 m/kg per day) at 1 hour before the subcutaneous injection of dl-methionine; Control group (n=4): The rabbits were injected with equivalent sodium chloride. They were administrated for 7 weeks continuously. ②Histological changes were observed under light and electron microscopes: ROS level was determined with colorimetries (721 visible spectrophotometer); The expressions of CAMs and NF-κB in endothelial cells were detected with immunohistochemical methods; The concentration of plasma Hcy was measured with high-performance liquid chromatography.MAIN OUTCOME MEASURES: Histological changes, ROS level and expressions of CAMs and NF-κB in endothelial cells.RESULTS: All the 24 rabbits were involved in the analysis of results. ① ROS level: After administration, the ROS level in the model group was obviously increased (2.92±0.20,2.48±0.26, P < 0.05), whereas those in the GbE group and control group (2.41±0.23, 2.43±0.20) had no obvious differences as compared with those before administration (2.31±0.27,2.47±0.32, P > 0.05). ② Histological changes: Aortas of rabbits in the model group presented initial changes of atherosclerosis, including shedding or necrosis of endothelial cells and nuclear pyknosis or standing in a clutter of smooth muscle cells. There were scarcely any changes in the GbE group and control group. ③ Expressions of CAMs and NF-κB in endothelial cells: After treatments, the expressions of vascular cell adhesion molecule-1 (VCAM-1), intercellular adhesion molecule-1 (ICAM-1) and NF-κB in the model group were obviously higher than those in the control group (P <0.05), and there were no significant differences between the GbE group and control group (P > 0.05). ④ Concentration of plasma Hcy: After 7 weeks, the concentration of plasma Hcy was higher in the model group and GbE group than in the control group [(25.01±6.80), (26.71±2.36), (16.85± 1.64) μmol/L, P < 0.05].CONCLUSION: Hcy-induced oxidative stress plays an important role in NF-κB activation. GbE might suppress the activation of NF-κB and expression of CAMs by reducing ROS.
2.Change of heat shock protein 70 expression and ultrastructure of brain in sleep deprivation

Objective:To study the change of the expression of heat shock protein 70 and ultrastructure of brain in sleep deprivation(SD).Methods:44 male Sprague-Dawlay rats were randomly divided into 11 groups.The HSP70 positive cells were detected by using immunohistochemical methods.The ultrastructure of hippocampal neuron was observed through electromicroscope.Results:HSP70 were detected in the cerebral cortex and hippocampus after SD.The HSP70 could be found after 12 h SD,reached to a peak during 2-3 d SD,and declined after 7 d SD.The number of HSP70 positive cells of SDd12h group was markedly more than that of SDn12h group(P
3.THE PROTECTIVE EFFECTS OF VITAMIN B_(12) ON SPATIAL COGNITIVE ABILITY IMPAIRED BY HOMOCYSTEINE IN RATS

Objective: To observe the relationship between homocysteine(Hcy)and spatial cognitive ability and to evaluate whether vitamin B12 supplementation could protect spatial cognitive ability in rats. Methods: The rats were randomized into three groups. The experimental group was given methionine subcutaneously. The intervention group was given methionine and vitamin B12 subcutaneously. The control group received isometric normal saline. After being raised for 8 weeks, all rats were examined for plasma Hcy and serum vitamin B12 and scores of Y-maze test. The brain tissues of hippocampus were checked immunohistochemically for acetylcholinesterase (AChE). Results: The experimental group developed hyperhomocysteinemia and descreased spatial cognitive ability significantly (P0.05). Conclusion: Homocysteine can impair the spatial cognitive ability in rats and vitamin B12 is effective in lowering Hcy level and protect the spatial cognitive ability of rats.
4.Cloning,High Level Expression and Purification of Human Survivin

Survivin is a protein that inhibits apoptosis and regulates cell division.The cDNA sequence of survivin was amplified by RT-PCR and sub-cloned into the prokaryotic expression vector pET-21b(+),followed by transformation into E.coli strain BL21(DE3) and induction with IPTG.The recombinant survivin protein fusing with 6?His tag was expressed in E.coli in the form of inclusion body at the expression level over 60% of the total cell protein.Results of Western blotting showed that recombinant survivin reacted specifically with anti-human survivin antibody.After gel filtration,the recombinant protein reached the purity over 95%,which facilitate the study of diagnosing and inhibitor agents targeting survivin.
5.Progress on treatment and research of quadrilateral plate fractures of acetabular.

Acetabular is an important human joint for weight bearing. Quadrilateral plate is a crucial structure of medial acetabulum with special morphology and important function. Quadrilateral plate fractures are common fracture in acetabulum. Quadrilateral plate fracture is hard to expose and reduction because it is in the medial of acetabulum. At the same time,the bone in the quadrilateral plate is not easy to fixed for thinning bones and adjacent to the articular cavity. The operator should know well about the anatomy and choose the suitable internal fixation. After quadrilateral plate fractures, the femur head maybe displace medially even break into pelvis. That make reduction and treatment always be a challenge. With different kinds of fractures,the efficacy of treatment is not the same. This paper intend to review the relation of anatomic features,approaches, internal fixations, key point of treatment and efficacy.

7.Dynesys dynamic stabilization system versus posterior lumbar interbody fusion in treatment of lumbar degenerative disease

BACKGROUND:Posterior lumbar interbody fusion is a typical therapeutic method of lumbar degenerative disease. Present studies suggested that adjacent segment degeneration occurs after fusion. Recently, more and more scholars paid attention to the development of non-fusion of the spine. <br> OBJECTIVE:To compare clinical effects of Dynesys dynamic stabilization system fixation and posterior lumbar interbody fusion in treatment of lumbar degenerative disease. <br> METHODS:From July 2009 to July 2011, clinical data of 56 patients with lumbar degenerative disease were retrospectively analyzed. There were 28 cases of Dynesys dynamic stabilization system fixation, and 28 cases of posterior lumbar interbody fusion. Operation time, bleeding volume, and postoperative hospitalization time were compared in both groups. Visual analog scale was used to assess pain. Oswestry disability index was utilized to evaluate clinical effects. <br> RESULTS AND CONCLUSION:A total of 56 patients were fol owed up for 18-24 months. Visual analog scale and Oswestry disability index scores were significantly improved at 12 months after treatment in both groups (P<0.01). Significant differences in operation time, bleeding volume, and postoperative hospitalization time were detected between both groups (P<0.01). Dynesys dynamic stabilization system group was better than posterior lumbar interbody fusion group. Range of motion was better in the Dynesys dynamic stabilization system group than in the posterior lumbar interbody fusion group (P<0.01). During fol ow-up, nail rope system and polyester sleeve loose were not detectable in the Dynesys dynamic stabilization system group. These results verified that compared with posterior lumbar interbody fusion, Dynesys dynamic stabilization system for lumbar degenerative diseases has a high safety and smal trauma, and can keep advantages of a fixed segment, and exert a certain effect on degeneration of intervertebral disc in the adjacent segment.
8.Effect of omental flap wrapping of pancreaticoduodenectomy on postoperative pancreatic fistula and intra-abdominal bleeding: a meta-analysis

Objective To determine the effect of omental flap wrapping of pancreaticoduodenectomy on postoperative pancreatic fistula and intra-abdominal bleeding in patients undergoing pancreaticiduodenectomy through meta-analysis of published studies.Methods Articles about whether omental flap wrapping could prevent postoperative pancreatic fistula and intra-abdominal bleeding were searched in databases of MEDLINE,PUBMED and EMBASE.14 English article were found and according to including and excluding criteria,4 eligible articles with a total of 2971 patients were selected.Random effects model was used to analyze odds ratios and 95% confidence intervals (CIs).Results 1129 patients were in omental wrapping group,and 1842 patients in nonomental wrapping group.Omental roll-up during pancreaticoduodenectomy could not prevent postoperative pancreatic fistula (OR =0.81,95 % CI 0.40~1.63,P>0.05) or postoperative abdominal bleeding (OR=0.67,95% CI 0.28~1.59,P>0.05).The sensitivity analysis showed the pancreatic fistula rate was lower in the nonomental roll-up group than that in the omental roll-up group (OR=1.24,95% CI 1.03~1.50,P<0.05).Conclusions Omental wrapping can not decrease the risk of pancreatic fistula and abdominal bleeding after pancreaticoduodenectomy.Further randomized controlled trials are needed to identify the effect of omental wrapping technique for pancreaticoduodenectomy.
9.Exploration of Operation Pattern at the First Time of Spontaneous Perforation of Congenital Choledochal Cyst in Children

ObjectiveTo explore the diagnostic methods and surgery pattern at the first time of spontaneous perforation of congenital choledochal cyst.MethodsEleven cases(4 male,7 female) with spontaneous perforation of congenital choledochal cyst were 6 months to 5 years old,and their average course of disease were 4 days.Gustily abdominal distension,abdominal pain,crying and fever were present in all cases.Jaundice(7 cases) and emesis(5 cases) appeared.All cases were detected with physical sign of peritonitis by physical examination.Choledochal cysts were confirmed by CT or B ultrasound in 8 cases.All cases accepted abdominal paracentesis and biliary ascites was drawn.Three different operative procedures were performed:choledochocyst excision & Roux-Y choledocho-jejunostomy(2 cases),choledochotomy with T-tube drainage(3 cases),and cholecystostomy(6 cases).Nine children receiving external drainage operation accepted a second operation to rebuild biliary tract(choledochocyst excision & Roux-Y choledochoje-junostomy) after 3 to 6 months.ResultsAll cases had got satisfactory therapeutic efficacy without any grave complication such as fistula of anastomotic stoma,infection of biliary tract or obstruction of biliary tract.During operation,perforations were located in the juncture of choledochus and cystic duct in 5 children and were not found in the other 6 children.In the second operation,the cases receiving cholecystostomy had less peritoneal adhesion,anatomic structure changes,haemorrhage[(30-50) mL vs(100-200) mL] and operation time[(2.5-3.0) h vs(3.5-5.0) h] than those receiving choledochotomy with T-tube drainage,and did not appear inadequate drainage for cystic duct obstruction.ConclusionsFor children with more organ inflammatory edema and adherence and in a bad overall condition,the first-time operation of cholecystostomy is more reasonable.
10.Diagnosis and Operative Treatment of Concealed Penis in Children

Objective To improve the knowledge,diagnostic level on concealed penis in children,and explore the operative treatment methods of concealed penis.Methods Ten children with concealed penis were reviewed retrospectively in the First Hospital of Jilin University from Jun.2005 to Jul.2007.Patients were 6-12 years old.Penis length was 1.2-2.6 cm.Patients growthing and micturition were normal,the results of androgne and estrogen examination were normal.The etiology was simple obesity in one case,fiber streak of penis sarcolemma in 9 cases.The clinical symptoms and signs,diagnosis and operative method were analyzed.Results The diagnosis of 10 children were correct,and all patients were treated by Devine's operation and 10 cases patients had healed in one stage.There was no vessel,nerve and urethra injury during the operation.The length of penis was 3.0-5.5 cm,average in 3.6 cm after operation.Followed up 3 months to 2 years,10 children's phimosis were removed,the micturition were normal;the appearance of penis was also satisfactory after operation.One patient with under-developed penis had a amelioration after treatment with HCG postoperatively.The penis contour was dissatisfactory in 1 obesity patient,but it was ameliorated gradually while growing up.Conclusions The correct diagnosis and differential diagnosis are very important for operative treatment depend on the different etiology and pathologic changes of concealed penis.Suitable operation can extend the length;moreover,ameliorate the symptom of micturition and the appearance of the penis.
