2.Value of three-dimensional color power angiography in the differential diagnosis of benign and malignant breast tumors

Objective To evaluate the value of three-dimensional color power angiography(3D-CPA) in the differential diagnosis of benign and malignant breast tumors.Methods 3D-CPA vascular parameters were collected and analyzed in 121 breast solid lesions (63 malignant,58 benign) in the 114 female patients.The diagnostic performances of each parameter were compared by receiver operating characteristic (ROC)cure.Results The parameters of intra-tumor flow index (FI1),flow index of shells with outside thickness of 3mm surrounding the breast tumors(FI2 ) and the ratio of FI2 and FI1 (G2) in malignant group were significantly higher than those in benign group( P <0.05,respectively).Analysis of the areas under ROC curve showed that the areas under FI2 curve was the largest.No significant difference( P >0.05) was found between the area under FI2 curve and the area under G2 curve.Comparing the area under FI2 curve to that under FI1,there was significant differences ( P < 0.05).Taking FI2 ≥ 27.385 as the critical value,the sensitivity and specificity for the diagnosis of malignant breast tumors were 88.9% and 81.0%,and the positive and negative predictive values were 83.6% and 87.0% respectively.Conclusions The sensitivity and specificity of 3D-CPA parameter FI2 in the diagnosis of malignant breast tumors were higher than others,FI2 with the optimal critical value of ≥27.385 could help to identify malignant from benign breast tumors.
3.Clinical Effect Observation of Jianpishugan and Huoxuehuatan Formula on Liver Depression and Spleen Deficiency Induced Non-Alcoholic Steatohepatitis

OBJECTIVE To study the clinical effect of Jianpishugan and Huoxuehuatan formula on liver depression and spleen deficiency induced non-alcoholic steatohepatitis.METHODS 140 patients with liver depression and spleen deficiency induced non-alcoholic steatohepatitis admitted in our hospital from Oct.2015 to Oct.2016 were selected and randomly divided into the observation group and the control group according to the random number.Both groups were given exercise and diet therapy,and the control group was also treated with polyene phosphatidylcholine capsules.Based on the control group,the observation group was also treated with modified Jianpishugan and Huoxuehuatan Formula.RESULTS Symptom scores and the total scores of the clinical symptoms in two groups decreased significantly (P<0.01) after the treatment,but that in the observation group decreased more significantly (P<0.01).Total effective rate of overall efficacy in the observation group was 94.28% after the treatment,which was significantly higher than that in the control group (77.14%,P<0.01).Total effective rate of TCM syndrome in the observation group was 95.71% after the treatment,which was significantly higher than that in the control group (77.14%,P<0.01).ALT,AST and GGT levels in two groups decreased significantly (P<0.01) after the treatment,but that in the observation group decreased more significantly (P<0.01).Abdominal B ultrasound results in two groups improved significantly after the treatment(P<0.01),but that in the observation group improved more significantly (P<0.01).During the treatment,patients in two groups did not complain of other discomforts,adverse reactions and toxic effects.CONCLUSION Jianpishugan and Huoxuehuatan Formula can noticeably improve the clinical symptoms and liver functions of patients with liver depression and spleen deficiency induced non-alcoholic steatohepatitis.
4.Effect of Jiuqiang Naoliqing on the behavior of Kunming mice

ObjectiveTo observe the effect of Jiuqiang Naoliqing (JNQ) on the behavior of Kunming mice.MethodsSpontaneous movement, Morris Water Maze, Rotarod, anti caffeine test, sleeping time of pentobarbital sodium, subthreshold dose of pentobarbital sodium, and anti pentylene tetrazol test were adopted to evaluate the behavioral changes. ResultsCompared with the control group, the low dose of JNQ can increase spontaneous movement of the mice, the middle and high dose of JNQ can increase time on the rotating rods. JNQ can also increase sleeping time of pentobarbital sodium test and percent of falling asleep in subthreshold dose of pentobarbital sodium test, as well as antagonize caffeine's effect on mice. ConclusionJNQ can also do sedative and hypnotic effect on Kunming mice as well as improve their ability of balance and coordination.
5.Effects of transforming growth factor beta1 on dendritic cells function.

OBJECTIVETo investigate the effects of transforming growth factor beta1 (TGF-beta1) on dendritic cells (DC).
METHODSMurine bone marrow cells were cultured with different cytokine combinations to develop immature DC (imDC, GM-CSF only) and TGFbeta-DC (GM-CSF + TGF-beta1), and their responses to lipopolysaccharide (LPS) stimulation were observed. The cell ultrastructure was observed by transmission electron microscopy and their phenotypes were assessed by flow cytometry (FCM). The allogeneic stimulating capacity of DC was assayed by mixed lymphocyte reaction (MLR) with BrdU incorporation. IL-12p70 protein was detected by ELISA and the expressions of Toll-like receptor 4 (TLR4) on DCs were analyzed with semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR).
RESULTSCompared to imDC, the TGFbeta-DC had no significant alterations in ultrastructure after LPS stimulation. The expressions of CD80, CD86 were lower on TGFbeta-DC than on imDC [(4.14 +/- 0.95)% vs (13.90 +/- 7.22)%; (8.60 +/- 0.75)% vs (20.63 +/- 5.03)%, P < 0.05, both]. The TGFbeta-DC kept their immature morphology after LPS stimulation, but the expressions of I-Ab and CD80 were slightly increased. After 96 h MLR, TGFbeta-DC had weaker stimulating capacity than imDC did, especially when DC/T cells ratios were 1:4 and 1:1 (P < 0.05, both). TGFbeta-DC showed impaired IL-12p70 production and down-regulation of TLR4 expression.
CONCLUSIONSTGF-beta1 can inhibit the expression of co-stimulatory molecules on DC. The TGFbeta-DC is resistant to maturation stimulus (LPS) and might be linked with TLR4 down-regulation.

6.Effect of chondroitin sulfate enzyme ABC on glial scar in brain injury models

Objective To explore the effect of chondroitin sulfate enzyme ABC (chABC) on glial scar in rat models of brain traumatic injury (TBI). Methods Thirty-eight Wistar rats were randomly divided into 5 groups, including normal control group (n=2), model group (rat models of TBI,n=9), 1.0 U/mL chABC treatment group (n=9), 2.5 U/ml chABC treatment group (n=9) and 5.0 U/ml chABC treatment group (n=9). After performing TBI by free falling in the later 4 groups, rats of the model group were given no treatment, while those of the other 3 groups were administrated with different concentrations of chABC by local injection respectively. One, 2 and 4 w after TBI, HE staining was performed on the brain tissues of these rat models;and immunohistochemical assay and Western blotting were employed to evaluate the secreting of chondroitin sulfate proteoglycans (CSPGs) and the therapeutic effect of chABC on glial scar. Data were statistically analyzed using t-test. Results Pathological test revealed the scars in the treatment groups were significantly fewer than those in the model group 2 w after TBI, with 5.0 U/mL chABC treatment group enjoying the fewest level (P<0.05). Immunohistochemical assay showed that the secreting of CSPGs in the treatment groups and model group was significantly increased than that in normal control group 2 w after TBI (P<0.05);the 5.0 U/ml chABC treatment group showed an obvious reduction of CSPGs secreting as compared with the model group (P<0.05). Western blotting indicated that the treatment groups showed an obvious reduction of CSPGs secreting as compared with the model group 1, 2 and 4 w after TBI (P<0.05);an obvious gradual reduction of CSPGs secreting in the model group, 2.5 and 5.0 U/ml chABC treatment groups was noted 1, 2 and 4 w after TBI (P<0.05). Conclusion ChABC could degrade the glial scar by degrading the CSPGs molecules and improve the microenvironment of local axonal regeneration after TBI;In this experiment, the highest concentration of chABC (5U/ml) shows the best effect on removing the glial scar.
7. Effect of Method of Regulating Qi to Dissipate Blood Stasis and Phlegm on Cardiac Rehabilitation in Patients with Qi Deficiency and Blood Stasis Type Chronic Heart Failure

Objective: To observe effect of method of regulating Qi to dissipate blood stasis and phlegm on degree of heart failure, ventricular remodeling, disease progression in patients with Qi deficiency and blood stasis type chronic heart failure (CHF).Method: One hundred and thirty-four patients with CHF were randomly divided into control group (62 cases) and observation group (62 cases) by random number table. The patients in control group got spironolactone tablets, 20 mg/time and qd. Benazepril, 20 mg/time and qd. Bisoprolol, 10 mg/time and qd. And digaoxin tablets if necessary. Based on the treatment in control group, patients in observation group additionally received Danshenyin and Xuefu Zhuyutang, 1 dose/day. The treatment course was 3 months in both groups. Before and after treatment, scores of Lee heart failure score were graded, cardiac function classification of the New York Heart Association (NYHA), 6 mins' walking test (6 MWT), scores of Qi deficiency and blood stasis and Minnesota living with heart failure questionnaire (MLHFQ) were evaluated. Echocardiography, left ventricular ejection fraction (LVEF), Left ventricular end-diastolic diameter (LVEDd), left ventricular end-stolic diameter (LVEDs), interventricular septum thickness at end-diastole (IVSd) and left ventricular myocardial mass index (LVMI) were recorded. Levels of matrix metalloprotein-9 (MMP-9), tissue inhibitor of matrix metalloprotease-1 (TIMP-1), transforming growth factor-β1 (TGF-β1), N-terminal pro-B-type na-triuretic peptide (NT-proBNP), galectin-3 and copeptin were detected.Result: Ridit analysis showed that after treatment, effect on cardiac function in observation group was better than that in control group (P<0.05). Classification of cardiac function of NYHA was lower than that in control group (P<0.05). The LVEDd, LVEDs and LVMI in observation group were all less than those in control group (P<0.05), while LVEF was higher than that in control group (P<0.05). There was no statistically significant difference in IVSd between two groups. Scores of Lee heart failure score, MLHFQ and Qi deficiency and blood stasis in observation group were lower than those in control group, and distance of 6 mins' walking was larger than that in control group (P<0.01). Levels of serum MMP-9, TGF-β1, NT-proBNP, galectin-3 and copeptin in observation group were lower than those in control group, while level of TIMP-1 was higher than that in control group (P<0.01).Conclusion: Based on the routine western medicine treatment, additional Danshenyin and Xuefu Zhuyutang can ameliorate symptoms of heart failure, relieve degree of heart failure, improve exercise tolerance and quality of life, inhibit ventricular remodeling, improve cardiac rehabilitation and delay progress of the disease for the CHF patients with Qi deficiency and blood stasis.
8.Screening for K-ras mutations in colorectal and lung cancers by using a novel real-time PCR with ADx-K-ras kit and Sanger DNA sequencing.

OBJECTIVEto map out the frequency and types of K-ras gene mutations present in colorectal and lung cancer patients; to evaluate the clinical applicability of a novel real-time double-loop probe PCR using the ADx-K-ras kit, and to compare its performance with the result by using traditional Sanger DNA sequencing in detection of somatic mutations of the tumor genes.
METHODSa total of 827 formalin-fixed paraffin-embedded (FFPE) blocks including 583 from the colorectal and 244 from the lung cancer patients were assayed. Genomic DNA of the sample tissues was extracted, purified and subjected to PCR amplification of K-ras gene codon 12 and 13 and DNA sequencing was carried on using both the traditional Sanger sequencing method and the ADx's K-ras mutation detection kit, respectively. The mutation rates for K-ras gene at codon 12 and 13, and the mutation frequencies detected by using both methods were analyzed.
RESULTS533 out of 583 (91.4%) colorectal cancer samples and 144 out of 244 lung cancer samples (59.0%) were detected using the traditional Sanger DNA sequencing technique, and 583 out of 583 (100.0%) colorectal plus 244 out of 244(100.0%) lung cancers were detected, respectively by using the ADx-K-ras kit. Of the 583 colorectal cancer samples, 192 (32.9%) showed mutations by using the ADx-K-ras kit in comparing with a result of 160 samples (27.4%) with K-ras gene mutation by using the traditional Sanger DNA sequencing technique. Of the 244 lung cancer samples, 26 (10.7%) showed K-ras gene mutations by using ADx-K-ras kit, while in 144 samples detected by using the traditional Sanger DNA sequencing technique, only 12 samples (8.3%) showed K-ras gene mutations. In colorectal cancer analyzed, GGT→GAT at codon 12 was the most common event with 35.1% (66/188) mutations, followed by GGC→GAC at codon 13 with 26.6% (50/188) and GGT→GTT at codon 12 with 18.6% (35/188), while GGT→GCT at codon12 was the most rare with only 1.6% (3/188) of the total mutation cases. In patients with lung cancer analyzed, GGT→GTT at codon 12 was the most common mutation, accounting for 40.9% (9/22), and GGT→GCT at codon 12 the most rare with only about 4.5% (1/22) of the total mutation cases.
CONCLUSIONSK-ras gene mutations were present in colorectal cases, and significantly more frequent than that in lung cancer. There were significant statistical differences between the two methods. ADx-K-ras real-time PCR showed much higher successful detection rates and mutation ratios compared to Sanger sequencing. As a result, the real-time PCR with ADx-K-ras kit proves to have a good clinical applicability and a strong advantage over the traditional Sanger DNA sequencing. It is a effective and reliable tool for clinical screening of somatic gene mutations in tumors.

9.Retrospective study of liver failure complicated with bacterium and fungous infection.

OBJECTIVETo study clinic character of liver failure complicated with bacterium and fungous infection.
METHODSThe patients with liver failure complicated with bacterial and fungous infection who were treated in our hospital from January 1986 to June 2006 were studied. All patients had clinical manifestation and positive of bacterium. The data were statistical analysis.
RESULTS507 patients diagnosed with fungous infection were found from January 1986 to June 2006 in which 132 patients were diagnosed with bacterial and fungous infection. There were 85 patients (64.39%) with chronic severe hepatitis and 40 patients (30.3%) with decompensation cirrhosis. Bacterial infection happened in 153 cases in which the rate of nosocomial infections was 54.90%. 204 bacterial strains were separated in which 143 strains (70.10%) were gram-negative bacterium and 61 (29.90%) strains were gram-positive bacterium. The main sites of bacterial infection were abdominal cavity (122 cases) and lung (30 cases). Fungous infection happened in 143 cases in which the rate of nosocomial infections was 86.71%. 155 fungous strains were separated in which 90 strains (58.06%) were Candida albicans, 17 strains (10.97%) were Aspergillus fumigatus and 25 (16.13%) strains were non-Candida albicans. The main sites of fouguns infection were lung (94 cases) and mouth (53 cases). 84 patients (63.64%) were ineffective and died after treatment.
CONCLUSIONThe patients with decompensation cirrhosis and chronic severe liver hepatitis were easy to be infected by bacterial and fungous. the rate of fungous nosocomial infections is higher than that of bacterium. The prognosis is bad in patients who had secondary fungous infection.

10.Screening for EGFR mutations in lung cancer by a novel real-time PCR with double-loop probe and Sanger DNA sequencing.

OBJECTIVETo map the frequency and types of EGFR gene mutations present in lung cancer tissues. To evaluate the clinical applicability of a novel real-time double-loop probe PCR of which the ADx-EGFR kit is based, and to compare its performance with traditional Sanger DNA sequencing in the detection of somatic mutations of tumor genes.
METHODSA total of 208 formalin-fixed paraffin-embedded (FFPE) tumor samples were tested. Genomic DNA of the tissue samples was extracted and purified, and subjected to both traditional PCR amplification, Sanger sequencing of EGFR gene in exon 18, 19, 20, 21, and ADx's EGFR mutation detection kit. The mutation rates for EGFR gene in exon 18, 19, 20, 21, as well as the frequency of each mutation detected by the two methods, were analyzed.
RESULTSThe traditional Sanger DNA sequencing technique was successfully performed in 196 out of 208 (94.2%) lung cancer samples, and 22 samples (11.2%) showed EGFR gene mutations. ADx-EGFR kit was successfully used in the lung cancers of all of the 208 cases (100.0%), and 40 samples (19.2%) showed mutations. In the lung cancer samples analyzed, mutations were mainly detected in the exon 19 and exon 21 L858R point mutation, i.e. 4.8% (10/208) and 11.6% (23/208) of total mutations, respectively, and the remaining mutations were rare.
CONCLUSIONSThe success rate of ADx-EGFR real-time PCR for formalin-fixed and paraffin-embedded tissues samples is significantly higher than that of Sanger sequencing (P < 0.01). There are significant differences between the two methods. ADx-EGFR real-time PCR shows a much higher successful detection rate and mutation rate of lung cancer tissues compared with that of Sanger sequencing. As a result, the real-time PCR with ADx-EGFR kit is proved to have a good clinical applicability and a strong advantage over the traditional Sanger DNA sequencing. It is an effective and reliable tool for clinical screening of somatic gene mutations in tumors.
