3.Mid-term follow-ups of hydroxyapatite extensively-coated femoral prosthesis for elderly patients

Objective To evaluate the effects of hydroxyapatite (HA) extensively-coated prosthesis on elderly patients undergoing hip arthroplasty.Methods From February 1997 to May 1999,96 patients Were treated with hydroxyapatite (HA) coated prosthesis and followed up for their hip arthroplasty.Their results were studied clinically and roentgengraphically to evaluate chiefly their Harris scores,complications and thigh pain.The survival of prosthesis was evaluated according to Kaplan-Meier methods.Results The 60 cases were followed up for eight years on average(range,sevon to nine years).Their mean Harris scores were improved from 42 preoperatively to 90 postoperatively.Fifty-eihgt cases felt no pain at their hip,but two did at their thigh.No radiolucent lines were observed.All the prostheses achieved bony stabilization and remolding.Conclusions HA coated prosthesis can achieve fine fixation,because it helps bone conduction,resists axial force and torsion,and reduces operative risks.
4.Clinical Pathological Features of Mesenteric and Omental Inflammatory Myofibroblastic Tumor in 5 Children

Objective To study the clinical pathological features of mesenteric and omental inflammatory myofibroblastic tumor (IMT) in children.Method Clinical features,laboratory result,pathological evidence for diagnosis and treatment of 5 cases with mesenteric and omental IMT were analyzed and evaluated.Results Intra-abdominal mass was most frequently found in childhood mesenteric and omental IMT.Gastrointestinal obstruction was showed in 2 cases.Anemia,leucocytosis,thrombocytosis,polyclonal-hyperglobulinemia appeared in 5 cases.The histological pattern showed:4 cases were myxoid pattern IMT,1 case was compact spindle cells pattern IMT.Immunohistochemistry showed:the spindle cells were expressed vimentin,smooth muscle action(SMA) and desmin.Partial spindle cells were anaplastic lymphoma kinase(ALK) and cytokeratin(CK) positive,while S-100 protein,CD34 were negative.Complete surgical excision was performed on 5 cases.Follow-up studies ranged from 5 months to 3 years,and no patient developed recurrence.Conclusions Childhood mesenteric and omental IMT is a rare interstitial tumor.The diagnosis should be based on pathological and immunohistochemistry examinations.Most cases are benign,and complete surgical excision can avoid local recurrence.
5.Cloning,High Level Expression and Purification of Human Survivin

Survivin is a protein that inhibits apoptosis and regulates cell division.The cDNA sequence of survivin was amplified by RT-PCR and sub-cloned into the prokaryotic expression vector pET-21b(+),followed by transformation into E.coli strain BL21(DE3) and induction with IPTG.The recombinant survivin protein fusing with 6?His tag was expressed in E.coli in the form of inclusion body at the expression level over 60% of the total cell protein.Results of Western blotting showed that recombinant survivin reacted specifically with anti-human survivin antibody.After gel filtration,the recombinant protein reached the purity over 95%,which facilitate the study of diagnosing and inhibitor agents targeting survivin.
6.Association of HLA-Cw and -DRB1 alleles with psoriasis vulgaris in Mongolian population

ObjectiveTo investigate the association of HLA-Cw and -DRB1 alleles with psoriasis vulgaris,and to provide a clue to the study into the etiology of psoriasis.MethodsVenous blood samples were obtained from 81 patients with psoriasis vulgaris collected during 2006-2011 at the Department of Dermatology,First Affiliated Hospital of Inner Mongolia Medical College,as well as 100 age- and gender-matched healthy controls.Both the patients and controls are unrelated Mongolia in Inner Mongolia.PCR with sequence-specific primers(PCR-SSP) technique was used to genotype the HLA-Cw and DRB1 loci.ResultsThe patients with psoriasis vulgaris showed a significantly higher frequency of HLA-Cw*06(0.438 vs.0.175,Pc < 0.01) and DRB1*07(0.241 vs.0.110,Pc < 0.012),but a lower frequency of HLA-Cw*04(0.031 vs.0.150,Pc < 0.01 ) and DRB1*04 (0.093 vs.0.235,Pc < 0.01 ) than the healthy controls did.Increased frequencies of HLA-Cw*06 and DRB1*07 alleles were observed in patients with an onset before 40 years of age and those without a family history,together with a decreased frequency of HLA-Cw*04 and DRB1*04 alleles,compared with the healthy controls(Pc < 0.05).The frequency of HLA-Cw*06 allele was significantly higher in patients with a positive family history and patients with an onset of no younger than 40 years of age than in the healthy controls (both Pc < 0.05).ConclusionsHLA-Cw*06 and -DRB1*07 alleles may be susceptibility determinants to psoriasis vulgaris,while HLA-Cw*04 and -DRB1*04 alleles may be protective factors against psoriasis vulgaris,in Mongolia from Inner Mongolia.HLA-DRB1*07 allele may be a susceptibility gene for psoriasis,while HLA-Cw*04 and -DRB1*04 alleles may be protective factors against psoriasis,in patients with an onset before 40 years of age.
8.Changes of anterior fontanel size in children aged 0 - 2 years.

OBJECTIVETo study the development of anterior fontanel(AF) in children less than 2 years of age.
METHODThe size of AF of the children under 2 years of age was measured. The criteria were: (1) All the children were singletons and term (37 weeks ≤ gestational age ≤ 40 weeks) at birth, birth weight > 2500 g. (3) Those with intracranial diseases (included trauma and asphyxia) and scalp hematoma were ruled out. (3) Healthy children (without intracranial disease, growth retardation, congenital syndrome or bone metabolic diseases such as rickets).
RESULT(1) The mean value of AF in neonates was 1.5 (0.3 - 2.5) cm, and the average of the AF at 1 month after birth was 2.2 cm, which was the largest one. The size of AF was 1.0 (0.3 - 2.0) cm at age 12 months, and 0.5 (0.3 - 0.7) cm at 24 months. (2) The percentage for the closure of the AF was 3% at 6 months, 26.5% at 12 months, and 93.0% at 24 months. (3) There were no gender differences in the size of the AF (P > 0.05). And the size of AF was not correlated with the development levels of weight, length, and head circumference (P > 0.05).
CONCLUSION(1) The size of AF at 1 month was maximum (2.2 cm), and then decreased by years. The AF was almost closed (93%) at 24 months. (2) There were no gender differences in anterior fontanel (P > 0.05). The size of AF was not correlated with the growth of weight, length, and head circumferences (P > 0.05). (3) The fontanel dimensions should be represented by oblique diameters of the fontanel in clinical pediatrics. (4) The AF closure time needs to be further evaluated in normal children.

10.Accuracy of analgesia/nociception index in assessing severity of postoperative pain

Objective To investigate the accuracy of analgesia/nociception index (ANI) in assessing the severity of postoperative pain in patients.Methods Eighty patients,aged 21-77 yr,of American Society of Anesthesiologists physical status Ⅰ-Ⅲ,scheduled for elective operation,were admitted to the postanesthesia care unit after the patients were extubated at the end of operation,and ANI was monitored.Immediately after admission to the postanesthesia care unit (T0),numerical rating scale (NRS) score was recorded.If NRS score was 0-3,the patients received no treatment,and NRS score was recorded again 10 min later (T1).If NRS score was 4-10,fentanyl 50 μg was injected intravenously,and NRS score was recorded again 5 min later (T2).ANI value was recorded at T0,T1 and T2.NRS score was considered as the criterion for assessment of the severity of pain.The receiver operating characteristic curve was used to analyze the sensitivity and specificity of ANI in determining NRS scores,the cut-off value was determined,and the area under the curve and 95% confidence interval were calculated.Pearson correlation was used to analyze the correlation between ANI and NRS score.Results The area under the curve (95% confidence interval) of ANI in determining NRS scores was 0.873 (0.816-0.929),and the cut-off value was 45 with a sensitivity of 74.8% and a specificity of 87.5%.ANI was negatively correlated with NRS score,and the correlation coefficient was-0.705 (P<0.05).Conclusion ANI provides high accuracy in assessing the severity of postoperative pain with a cut-off value of 45 in the patients.
