1.Temporary Cardiac Pacemaker Therapy on 9 Children with Adam-Stoke Syndrome

Objective To explore the value of temporary cardiac pacemaker therapy on children with Adam-Stoke syndrome(ASS).Methods Nine children with ASS was implanted the emporary cardiac pacemaker under X-ray or blinding inserted beside bed.Results Seven children was successfully implanted the temporary cardiac pacemaker within 1-2 d of ASS attack,and two children was successfully implanted at 10 d and 20 d after ASS attack, respectively. Five patients was cured, two children was died, and two children was implanted the permanent cardiac pacemaker. During pacing, two patients had the electrode shifted,and one patient had the cardiac murmur,and one patient had the local skin infected.Conclusion Temporary cardiac pacemaker can successfully treat children with ASS attack;the earlier the implantation,the better the prognosis.

3.Application of recombinant human erythropoietin combining with tranexamic acid in perioperative period of minimally invasive total hip arthroplasty

BACKGROUND: Minimally invasive total hip arthroplasty is less invasive and has less tissue injury than traditional approach. However, perioperative anemia is still common, affecting hip function recovery and increasing patient burden. OBJECTIVE: To assess the curative effects of recombinant human erythropoietin on hemorrhagic anemia after minimally invasive total hip arthroplasty. METHODS: Clinical data of 35 patients undergoing SuperPATH minimally invasive total hip arthroplasty in the Department of Orthopedic Surgery, the Affiliated Hospital/Clinical Medical College of Chengdu University were collected. The patients were divided into the control group (n=17) and the erythropoietin group (n=18) according to the presence and absence of erythropoietin. The patients in the erythropoietin group were subcutaneously injected with recombinant human erythropoietin 3 days before replacement, once a day, lasting to 8 days after operation, for 12 consecutive days. During erythropoietin administration, iron sucrose was added once a day. Tranexamic acid 1 g was intravenously infused 0.5 hour before replacement, and 1 g for washing medullary cavity during replacement. The patients in the control group only took iron sucrose, once a day, adding tranexamic acid through intravenous infusion 0.5 hour before replacement. Levels of hemoglobin and hematocrit were monitored on the day of admission, the preoperative 3 days, the operation day, the postoperative 4 days and the postoperative 8 days. The operation time, blood loss, Visual Analogue Scale pain score 4 days before and after replacement and Harris hip function score before replacement, 4 and 8 days after replacement were recorded. RESULTS AND CONCLUSION: (1) Baseline data were not significantly different between the two groups (P > 0.05). (2) Visual Analogue Scale pain score significantly decreased 4 days after replacement in both groups (P < 0.05), and was not significantly different between both groups at various time points (P > 0.05). Harris hip score was not significantly different before and 1 day after replacement between the two groups (P > 0.05). At 4 days after replacement, Harris hip score was significantly higher in the erythropoietin group than in the control group (P< 0.05). Mean blood loss and operation time were not significantly different between the two groups (P > 0.05). (3) Levels of hemoglobin and hematocrit were not significantly different on the day of admission and 1 day before replacement (P > 0.05). Levels of hemoglobin and hematocrit were higher in the erythropoietin group than in the control group during emergency examination, 4 and 8 days after replacement (P< 0.05). The changes in levels of hemoglobin and hematocrit were consistent in different groups. The decreased trend was smooth, but the increased trend was relatively steep in the erythropoietin group. (4) Results confirmed that recombinant human erythropoietin combined with iron sucrose combined adding tranexamic acid, can effectively improve the anemia in perioperative period of minimally invasive total hip arthroplasty. Moreover, hemoglobin levels can increase to the stable and normal levels.

5.Effects of scalp acupuncture combined with auricular point sticking on cognitive behavior ability in patients with vascular dementia.

OBJECTIVETo compare the therapeutic differences among scalp acupuncture combined with auricular point sticking, body acupuncture and western medication for treatment of vascular dementia (VD).
METHODSNinety cases were randomly divided into a combined therapy group (31 cases), a body acupuncture group (29 cases) and a western medication group (30 cases). The combined therapy group was treated with scalp acupuncture at forehead middle line, parieral middle line, temporal front line and temporal rear line as well as auricular point sticking at naogan (AT3,41), shen (CO10), shenmen (TF4), zhen (AT3), once a day; the body acupuncture group was treated with acupuncture at Baihui (GV 20), Fengchi (GB 20), Zusanli (ST 36) and so on, once a day; the western medication group was treated with oral administration of aniracetam tablets, 0.2 g per time, twice a day. Fourteen days were considered as a treatment course, and totally 3 courses were required. The mini-mental state examination (MMSE) and activities of daily living (ADL) were applied to assess the changes of cognitive behavior ability before and after treatment among three groups. Also the efficacy among three groups were compared.
RESULTSOne case dropped out in the body acupuncture group and western medication group, respectively. The total effective rate was 90.334 (28/31) in the combined therapy group, which was superior to 85.734 (24/28) in the body acupuncture group and 79.3% (23/29) in the western medication group (both P < 0.05). After the treatment, the scores of MMSE and ADL were all improved among three groups, which was the most significant in the combined therapy group (MMSE: 23.32 +/- 4.45 vs 21.23 +/- 4.13, P < 0.05; 23.32 +/- 4.45 vs 20.41 +/- 4. 01, P < 0.01; ADL: 53.18 +/- 21.55 vs 51.92 +/- 20.42, P < 0.05; 53.18 +/- 21.55 vs 49.42 +/- 19.43, P < 0.01).
CONCLUSIONThe scalp acupuncture combined with auricular point sticking could improve the clinical symptoms and cognitive behavior ability in patients with vascular dementia, which has superior total efficacy to body acupuncture and western medication aniracetam tablets.

6.Effects of culture medium of amniotic cells on NO and NOS in retina tissues in vitro

Objective To observe the effects of culture medium of a mn iotic cells on NO and NOS in retinal tissues of rabbits in vitro in order to pro vide a protective method for antioxidation in retina transplantation. M ethods Thirty adult healthy rabbits (30 right eyes) were divided into 3 groups. Group Ⅰ: fresh retinal tissue; group Ⅱ: routine culture medium; group Ⅲ: culture medium of amniotic cells. The retinal tissues in group Ⅱ and Ⅲ we re cultured in the corresponding culture medium for 1 week. The content of NO an d NOS in retinal tissues in the 3 groups were determined. Results Compared with group I, the content of NO and NOS of group Ⅱ increased obvio usly (t=3.821, 3.854; P0.05) . Conclusion Culture medium of amniotic cells may remove free r adicals and enhance the ability of antioxidation.
7.A study on relationship between the contour of articular eminence and traces of the condylar kinematic center and its application in TMD patients.

OBJECTIVETo investigate the relationship between the contour of articular eminence and traces of the condylar kinematic center during jaw opening movement in healthy subjects, and compare trace characteristics of condylar kinematic center and MRI findings in TMD patients.
METHODSIn 10 healthy subjects, jaw-opening motion was recorded. The kinematic center and terminal hinge axis point of the condyle were used as trace reference points. The contour of articular eminence was examined by MRI. Seven patients with TMD signs and/or symptoms (disk displacement) were selected for this study. The condylar trace was recorded during jaw protrusion and opening-closing. The internal derangement in temporomandibular joints was detected by MRI and defined as: (1). normal disk position, (2). disk displacement with reduction, (3). disk displacement without reduction.
RESULTSIn healthy subjects, most of the opening traces of the kinematic center coincided with the contour of articular eminence (8/10 joints in left, 9/10 joints in right). For terminal hinge axis point, no trace coincided with the contour of articular eminence (0/20 joints in left and right). In TMD patients, according to MRI findings, the condylar traces of kinematic center in 3 normal disk position joints showed normal shape. However, in 6 disk displacements with reduction joints and 5 disk displacement without reduction joints, the condylar traces of kinematic center showed irregular patterns except 1 disk displacement with reduction joint.
CONCLUSIONSIn comparison with the terminal hinge axis point, the opening traces of the kinematic center can be interpreted as the translatory movement of the condyle/disc along the articular eminence. The study suggests the use of kinematic center in condylar movement studies.

8.Changes in T cell subsets in children with sepsis, and their clinical significance.

OBJECTIVETo observe changes in T cell subsets in children with sepsis and their prognosis, and to investigate the clinical significance of these changes in the occurrence and development of sepsis.
METHODSFifty children with severe sepsis and 150 children with general sepsis were enrolled as subjects, and 50 age-matched healthy children were included as controls. The percentages of CD3(+), CD4(+) and CD8(+) T cells in peripheral blood and CD4(+)/CD8(+) ratio were measured by flow cytometry. The pediatric critical illness score (PCIS) was calculated within 24 hours of admission.
RESULTSThe children with severe sepsis showed significantly lower percentages of CD3(+), CD4(+) and CD8(+) T cells CD4(+)/CD8(+) ratio and PCIS than the controls and children with general sepsis (P<0.01). Among the 200 cases of sepsis, the percentages of CD3(+), CD4(+) and CD8(+) T cells, CD4(+)/CD8(+) ratio and PCIS were significantly lower in the cured group than in the deceased group.
CONCLUSIONSChildren with sepsis have different degrees of cellular immunosuppression, and the degree of cellular immunosuppression is significantly correlated with the severity of the disease. Detection of T cell subsets in peripheral blood is of great significance for evaluating immune function and judging disease severity in children with sepsis.

9.Study on the expression of cytokeratin 19 mRNA in oral squamous cell carcinoma.

OBJECTIVETo elucidate the possible mechanism of oral carcinogenesis and to explore the value of clinical application of the detection of cytokeratin (CK) 19 for oral squamous cell carcinoma (OSCC) patients.
METHODSThe cancerous tissues, para-cancerous tissues and excised lymph nodes were collected from 20 operated patients with OSCC. The patients didn't receive radiotherapy and chemotherapy before hospitalization. The relative expression of CK19 mRNA in those tissues was detected by fluorescent quantitative polymerase chain reaction (FQ-PCR).
RESULTSThe expression of CK19 mRNA in the cancerous tissues was 1.85 and 1.66 times higher than that in normal oral mucosa and in para-cancerous tissues, respectively. The expression of CK19 mRNA in lymph nodes from 9 patients with OSCC was positive and the positive rate was 45% (9/20). The positive rate of CK19 mRNA in all lymph nodes from 9 patients with OSCC was 81.8% (18/22), and the positive rate of CK19 mRNA in all lymph nodes from 20 patients with OSCC was 41.9%(18/43). CK19 mRNA level in the cancerous tissues relative to para-cancerous tissues and normal oral mucosa of the patients whose CK19 mRNA expression was positive was lower than that of the patients whose CK19 mRNA expression was negative in lymph nodes, respectively.
CONCLUSIONThe possible reason that the expression of CK19 mRNA in the cancerous tissues was higher than that in para-cancerous tissues and normal oral mucosa was that the CK19 synthesis in cancerous tissues increased obviously. The detection of CK19 mRNA in lymph nodes was regarded probably as one of the markers for detecting OSCC micrometastasis in lymph nodes. The detection of CK19 mRNA in lymph nodes by FQ-PCR was more sensitive than hematoxylin-eosin staining in diagnosing OSCC micrometastasis.

10.Application of neoadjuvant chemotherapy for operable breast cancer.

OBJECTIVETo investigate the clinical value of neoadjuvant chemotherapy in the treatment of operable breast cancer.
METHODSA total of 120 patients with pathologically proven operable breast cancer were randomized into two groups to receive 2-4 cycles of neoadjuvant chemotherapy with docetaxel combined with pirarubicin (TThp) or docetaxel combined with epirubicin (TE). Operations were performed two weeks after the neoadjuvant chemotherapy. The short-term therapeutic effect, toxic reaction and the impact of neoadjuvant chemotherapy on the choice of surgical approaches were evaluated.
RESULTSThe total effective rate for the primary sites was 87.2% in these patients, and the rate of breast conservation significantly increased from 12.7% to 41.8% (P<0.05), with a tumor resection rate of 97.2%. The major adverse effects of the therapy included leukopenia, nausea, vomiting and alopecia.
CONCLUSIONNeoadjuvant chemotherapy can enhance the breast conservation rate, lower the clinical staging of the tumors and minimize the surgery area to improve the postoperative quality of life of the patients.
