Objective:To introduce several international long-term care insurance systems so as to provide references for building China's long-term care insurance system.Methods:Literature of International long-term care insurance system were reviewed to extract and summarize the content so as to provide suggestion and references for China's system construction.Results:It concluded 4 modes of systems for long-term care insurance including the private long-term care social insurance like Netherlands,the public long-term care social insurance like German and Japan,long-term care commercial insurance like America and national long-term care insurance like England.The advantages of different modes could be learned to form Chinese system.Conclusion:The long-term care insurance systems of Netherlands,German,Japan,American could be learned for our system construction,but they also had disadvantages.Therefore,on the basis of the foster strengths and circumvent weaknesses,the reasonable long-term care insurance system was formed.

Objective To investigate the quality of life and metal stress of breast cancer patients'spouses in the perioperative period,and analyze the relationship between them.Methods Self-made General Information Questionnaire (WHOQOL-BREF) aud relative stress scale (RSS) were used to investigate 110 breast cancer patients' spouses during the perioperative period,in order to analyze the relationship betweeu quality of life and metal stress.Results A total of 110 copies of questionnaires were gave out,102 valid questionnaires,effective recovery was 92.73％.The scores of quality of life in physiology,psychology,social relationship and environment were (13.34 ± 1.88),(13.95 ± 2.63),(13.61 ± 2.45) and (12.22 ± 2.47)respectively and the score of RSS was (11.16 ±4.69).There was a negative correlation between the scores of WHOQOL-BREF and the RSS,the metal suffering as well as the confusion of life (P ＜ 0.05).Only the scores of psychology and social relationship were negatively related to the negative emotions because of the patients (P ＜ 0.05).Conclusions The quality of life of breast cancer patients' spouses during perioperative period is negatively affected by their metal stress.

BACKGROUND: It has been considered that Cestrum nocturnum L. (CNL) has the effects of antiarrhythmia, local anesthesia and central inhibition.OBJECTIVE: To investigate the analgesic effect of CNL extract on mice,so as to find new drugs for clinical treatment of pain.DESIGN: A randomized control observation.SETTING: Center of Modern Education and Department of Pharmacology,Gannan Medical College.MATERIALS: The experiments were carried out in the laboratory of scientific research center, Gannan Medical College between March and April in 2005. ① A total of 150 healthy adult Kunming mice were used in four independent experiments. ② Drugs: CNL extract was provided by the Department of Phytochemistry, Shenyang Pharmaceutical University (batch number: 2002080901), morphine hydrochloride injection by Shenyang No.1Pharmaceutical Factory (batch number: 000305), and naloxone hydrochloride injection by Yanqiao (Hunan) Pharmaceutical Co. Ltd., (batch number:20021109).METHODS: ① Effects of CNL extract on writhing times induced by acetic acid: Forty female mice were randomly divided into four groups with10 mice in each, and they were treated with intraperitoneal injections of 0.02 mL/g saline, 0.10 and 0.20 mg/g CNL extract and 0.10 mg/g aminophenazone respectively. The intraperineal injection of 6 g/L glacial acetic acid was given after 15 minutes. The writhing times of mice within 15 minutes were observed and recorded in each group. ② Effects of CNL extract on the pain induced by hot pla in mice: Forty female mice were randomly divided into four groups with 10 mice in each, and they were treated with intraperineal injections of 0.02 mL/g saline, 0. 10 and 0.20 mg/g CNL extract and 0.10 mg/g morphine respectively. The pain responses were detected at 15, 30 and 60 minutes after administration. ③ The antagonistic effect of naloxone on morphine and CNL extract to the pain induced by hot plate in mice: Thirty female mice were randomly divided into three groups ith 10 mice in each group, and they were given intraperitoneal injections of 0.02 mL/g saline, naloxone 0.004 mg/g +morphine 0.01 mg/g and naloxone 0.004 mg/g+CNL extract 0.01 mg/g respectively. The pain responses were detected at 15, 30 and 60 minutes after administration respectively. ④ Effects of CNL extract on electrostimulation induced pain in mice: Forty mice were randomly divided into four groups with 10 mice in ach group, and they were administrated with intraperineal injections of 0.02 mL/g saline, 0.10 and 0.20 mg/g CNL extract and 1 g/L morphine respectively. Repeated electrostimulations were given at 20, 35, 50 and 70minutes after administration, and the pain responses were detected by means of electrostimulation.MAIN OUTCOME MEASURES: ① Writhing times; ② Time for the pain response induced by hot plate; ③ Analgesic rate induced by electrostimulation.RESULTS: Totally 150 healthy adult Kunming mice were used in the four independent experiments, and all were involved in the analysis of results. ①Writhing times in the mice: 0.10 and 0.20 mg/g CNL extracts and 0.10 mg/g aminophenazone had very significant analgesic effects on writhing induced byacetic acid in mice, and the writhing times after administration were all fewer than those in the saline group (20.2±10.8, 14.5±7.6, 7.6±4.5,50.6±15.5, P ＜ 0.01), and the analgesic effects of CNL extract were dosedependently. ② Time for the pain response induced by hot plate: 0.10 and 0.20 mg/g CNL extracts had significant analgesic effects on the pain in duced by hot plate, and the time for pain sensation at 15, 30 and 60 minutes after administration were all longer than those in the saline group (P ＜ 0.05 or P ＜ 0.01), and the analgesic effect was dose-dependently. The times for pain sensation at each time point after administration in the naloxone 0.004 mg/g+CNL extract 0.01 mg/g group were all longer than those in the saline group, but those were close between the naloxone 0.004 mg/g+morphine 0.01 mg/g group and the saline group. ③ Analgesic rate induced by electrostimulation in the mice: The analgesic rates at20, 35, 50 and 70minutes after administration in the CNL extract 0.10 and 0.20 mg/g groups were all higher than those in the saline group (P ＜ 0.01).CONCLUSION: Our data suggested that CNL extract has obvious analgesic effect, and the analgesic intensity is dose-dependently. Naloxone, an opiate receptor antagonist, can antagonize the analgesic effect of morphine,but cannot antagonize that of CNL extract on mice with pain induced by hot plate, which indicates that CNL extract exert its analgesic role not through binding with opiate receptor.

2 cm) who were treated with endoscopic polypectomy in 2 different means,respectively.Methods The clinic data of 68 children with giant colonic polyps were review analyzed.Thirty-five cases were received endoscopic clips ligation and the other 33 cases were received endoscopic snare resection.Results All the 35 cases out of endoscopic clip ligtion group were sucessfully cured.There were only 3 cases showedl a little bleeding in this group.In the endoscopic snare resection group,there were 10 cases showed bleeding,8 cases showed polypectomy syndrome,1 case transferred into(operation).Conclusions The complication incidence in endoscopic clips ligation group is lower than that in endoscopic snare resection group.The endoscopic clipping brings about an effective and safe way to treat giant colonic polyps in children.

Objective To observe the cerebral edema occurrence ,evolution and prognosis of intra-arterial thrombolytic recanali-zation in patients with acute ischemic stroke .Methods From October 2010 to October 2012 ,36 patients who underwent the intra-arterial thrombolytic therapy were cerebral recanalization .They were randomly divided into two groups :1-3 h group and >3-6 h group .The alteration of brain edema was observed by cranial CT .Their mRS .NHISS and BI were scaled and recorded before and after intra-arterial thrombolytic therapy .Results After intra-arterial thrombolytic therapy ,the occurrence rate of cerebral edema was 94% ,appeared at the onset of more than 1 hours in all patients .Long-term follow-up showed ,cerebral edema location appeared obvious cerebromalacia .Cerebral edema and clinical outcome had not significant difference between 1-3 h group and 3-6 h group . Conclusion Cerebral edema and loss of cerebral tissue occurred almost inevitability in patients who received intra-artery thromboly-sis and recanalized their cerebral artery .It indicates that good local circulation and general condition may be favorable factors that can gradually reduce brain edema .

Colorectal cancer is a common malignant tumor in the gastrointestinal tract, with the characteristics of high morbidity and mortality. Studies have shown that the occurrence and development of colorectal cancer is closely related to the abnormal activation of Wnt signaling pathway. Abnormal expression of β-catenin in Wnt pathway is found both in the cytoplasm and nucleus of tumor cells. Different drugs can target the Wnt signaling pathway and its upstream and downstream related factors to inhibit or suppress the development of colorectal cancer. We review the components of Wnt signaling pathway, and the correlation between Wnt signaling pathway and colorectal cancer. Then, we summarize the current status of drug research targeting the Wnt signaling pathway in colorectal cancer. Finally, the challenges and prospects of these methods and drugs were briefly summarized.

BACKGROUND: As a plant in valerianaceae, patrina villosa juss, which characterizes by acrid and bitter in taste and cold in nature, has been proved that its extract has effect on central inhibition.OBJECTIVE: To observe the effect of patrina villosa juss extract on hypoxia tolerance of mice and acknowledge whether it has dosage-dependence or not.DESIGN: Randomized controlled animal study.SETTING: Pharmacological Department and Pathological Department of Gannan Medical College.MATERIALS: The experiment was completed at the Laboratory of Scientific Research Center of Gannan Medical College from March to April 2005. A total of 100 healthy adult Kunming mice were selected in three hypoxia experiments.METHODS: ① Hypoxia tolerance experiment under normal pressure:Forty mice were randomly divided into 4 groups. Mice were injected intravenously with 2 μL/g saline in saline group, with 0.02 mg/g propranolol solution (10 g/L) in propranolol group, with 0.02 mg/g patrina villosa juss extract in 0.02 mg/g patrina villosa juss group, and 0.04 mg/g patrina villosa juss extract in 0.04 mg/g patrina villosa juss group, respectively. Twenty-five minutes later, mice were put into wide mouthed bottle with the volume of 250 mL and the bottle was enclosed to observe the survival time. ② Rapid decapitation experiment: Thirty mice were randomly divided into 3 groups. Mice were injected intravenously with 2 μL/g saline in saline group, with 0.02 mg/g patrina villosa juss extract in 0.02 mg/g patrina villosa juss group, and 0.04 mg/g patrina villosa juss extract in 0.04 mg/g patrina villosa juss group, respectively. Twenty-five minutes later, heads of mice were cut rapidly to record the time from decapitation to the last gasp. ③ Experiment for ligating bilateral common carotid artery: Thirty mice were randomly divided into 3 groups. Mice were perfused with 2 μL/g saline in saline group, with 0.01 mg/g patrina villosa juss extract in 0.01 mg/g patrina villosa juss group, and 0.015 mg/g patrina villosa juss extract in 0.015 mg/g patrina villosa juss group, respectively, once a day for 7 days in total. Seven days later, bilateral common carotid artery was ligated to observe time of respiratory arrest.MAIN OUTCOME MEASURES: ① Survival time of hypoxia tolerance;② time from decapitation to the last gasp; ③ time from ligating bilateral common carotid artery to respiratory arrest.RESULTS: A total of 100 mice were involved in the final analysis. ① Survival time of hypoxia tolerance under normal pressure: Time in 0.02 mg/g and 0.04 mg/g patrina villosa juss groups was longer than that in saline group [(57.8±4.6), (76.2±4.9), (42.5±3.6) minutes, P ＜ 0.05, 0.01], but there was no significant difference from that in propranolol group (P ＞ 0.05).The higher the dosage was, the longer the survival time was. ② Gasping time of decapitation mice: Time in 0.02 mg/g and 0.04 mg/g patrina villosa juss groups was longer than that in saline group [(22.1 ±1.6),(25.3±2.2), (18.6±0.8) s, P ＜ 0.05, 0.01], and the higher the dosage was, the longer the survival time was. ③ Time of respiratory arrest: Time in 0.01 mg/g and 0.015 mg/g patrina villosa juss groups was longer than that in saline group [(123.4±25.1),(142.2±30.2), (86.0±12.8) s, P ＜ 0.05, 0.01], and the higher the dosage was, the longer the survival time was.CONCLUSION: Patrina villosa juss extract can improve symptom of myocardial hypoxia induced by cerebral hypoxia, whole-body hypoxia and increase of myocardial oxygen consumption; moreover, the higher the dosage is, the more remarkable the effect is. The mechanism is of possibility that patrina villosa juss extract can improve myocardial and cerebral oxygen consumption.

We predicted the anti-ulcerative colitis (UC) mechanism of Fructus Amomi based on network pharmacology. The anti-UC activity of Fructus Amomi were investigated by in vivo animal experiment, and the active components of Fructus Amomi were obtained through TCMSP, PubChem database and literature research. Animal welfare and experimental procedures follow the regulations of the Animal Ethics Committee of Institute of Materia Medica of Chinese Academy of Medical Sciences. The potential targets of the active components and UC were predicted by SwissTargetPrediction, GeneCards and TTD databases. The protein-protein interaction (PPI) network was constructed by String database and Cytoscape software was used to construct a visual network of active component-disease target and perform topological analysis. Gene Ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analyses were performed using Metascape platform. The molecular docking of key components and core targets was carried out by Sybyl X software. We screened out a total of 12 active components and 189 disease-component overlapping targets. Enrichment analyses obtained 227 related GO items and 168 signaling pathways. According to the results of molecular docking, most active components of Fructus Amomi showed good affinity with the JAKs receptor family. Furthermore, Western blot results verified that Fructus Amomi could effectively inhibit JAK/STAT signaling pathway, indicating that Fructus Amomi might exert the anti-UC activity by regulating JAK/STAT signaling pathway.

OBJECTIVETo evaluate the clinical effect of arthroscopic excision of the os subfibulare in anterior-lateral ankle pain.

METHODSFrom December 2005 to Augest 2014, 16 patients suffering from pain associated with an os subfibulare in the anterior-lateral side of their ankles were reviewed. Among the patients,11 patients were male and 5 were female, with a mean age of (33.5 ± 15.6) years old. The mean maximum diameter of os subfibulare was (0.70 ± 0.26) cm. All the patients underwent excision of the osseous fragments, and had anatomic reconstruction of the anterior talofibular ligament if the anterior-lateral ankle was instable. The average follow-up period was (18.0 ± 4.5) months. To analyze the surgical outcome, American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot pain and function scales,visual analogue scale (VAS) and Tegner activity scale were assessed preoperatively and postoperatively.

RESULTSAOFAS scales were preoperative 60.15 ± 14.52 and postoperative 92.35 ± 5.73. There was a significant difference between them (t = -8.251, P = 0.000). The mean VAS score were preoperative 7.35 ± 0.46 and postoperative 2.45 ± 0.98. Statistical significance was also notable (t = 18.105, P = 0.000). Tegner score was significantly increased from preoperative 2.87 ± 1.12 to postoperative 5.78 ± 1.06 (t= -7.548, P = 0.000).

CONCLUSIONIrrespective of the size of os subfibulare, in patients with pain or instability associated with the os subfibulare, arthroscopic excision combined with reconstruction of ther anterior talofibular ligament or not was effective in restoring ankle function and eliminating pain.
