Objective To examine mycological profile of eryptococcal meningitis in patients with non-acquired immune deficiency syndrome (AIDS) during treatment and follow-up so that to support clinical therapy. Methods Data of 28 cuhure-confirmed cryptoeoccal meningitis patients with non-AIDS were retrospectively analyzed. Fungat smear, count, culture and latex agglutination test of cerebrospinal fluid (CSF) were done during treatment and follow-up. Initial treatment included intravenous amphotericin B plus oral flucytosine or f;uconazole for at least 6 weeks, and consolidation treatment included oral fluconazole and (or) itraeonazole for at least 2 months. All 28 patients were cured. The data were analyzed by rank-sum test. Results The positive rate of CSF fungal smear was 92.9% before treatment and gradually decreased, and the fungal count was significantly reduced over time after treatment. While fungal smears of some patients were still positive after initial treatment. Fungal growth time in culture was gradually extended, and fungal culture turned to be negative in all patients after 2 weeks of treatment. The positive rate of latex agglutination test of CSF was 100%. Cryptococcal antigen titer decreased steadily after treatment, which was not correlated with the decrease of fungal count. Conclusion Mycological tests of patients with eryptococcal meningitis should be interpreted comprehensively during treatment, and result of each test should be specifically analyzed.

Objective To evaluate the role of hypoxia-inducible factor-1α (HIF-1α) in sevoflurane postconditioning-induced reduction of myocardial ischemia-reperfusion (I/R) injury in mice. Methods Forty pathogen-free healthy male C57 mice, aged 8 weeks, weighing 20-30 g,were assigned into 4 groups (n=10 each) using a random number table:sham operation group (group Sham),myocardial I/R group (group I/R),sevoflurane postconditioning group (group SPC) and HIF-1α inhibitor 2Me2 group (group 2Me2).Myocardial I/R was induced by occlusion of the left anterior descending branch of the coronary artery for 30 min followed by 120 min of reperfusion in pentobarbital sodium-anesthetized mice.In group SPC,3% sevofiurane was inhaled for 15 min starting from the onset of reperfusion. In group 2Me2,30% 2Me2 (30 mg/kg) was intraperitoneally injected at 30 min before ischemia. The mice were sacrificed at the end of reperfusion, and the hearts were removed for determination of the myocardial infarct size (using the Image J software) and expression of HIF-1α in thc nucleus of cardiomyocytes (by Western blot). Results Compared with group Sham, the myocardial infarct size was significantly increased in I/R,SPC and 2Me2 groups, the expression of HIF-1α was significantly up-regulated in I/R and SPC groups, and the expression of HIF-1α was significantly down-regulated in group 2Me2 (P< 0.05).Compared with group I/R,the myocardial infarct size was significantly decreased, and the expression of HIF-1α was up-regulated in group SPC,and the myocardial infarct size was significantly increased, and the expression of HIF-1α was down-regulated in group 2Me2 (P<0.05).Compared with group SPC,the myocardial infarct size was significantly increased, and the expression of HIF-1α was down-regulated in group 2Me2 (P<0.05).Conclusion The mechanism by which sevoflurane postconditioning reduces myocardial I/R injury is related to up-regulation of HIF-1α expression in mice.

Objective: To optimize a preparation technology for pyretic arthralgia cataplasma. Methods: The preparation technology were studied by a uniform design experiment in which NP-700, tartaric acid, PVP, dihydroxyaluminum aminoacetate, glycerol, water and medicinal powder were factors and viscosity, infiltration, gel mobility and gel strength were indices. Results: The best ratio of this cataplasma matrix was NP-700:tartaric acid:PVP:dihydroxyaluminum aminoacetate: glycerol:water:powder = 4.0:0.2:1.0:0.1:25.0:35.0:2.0. According to optimized formula, to prepare the poultice, then to spread the poultice uniformly onto non-woven fabrics, cover CPP membrane and pack after 1 week at room temperature. Conclusion: Pyretic arthralgia cataplasma was well moldable and its process technology was feasible.

To prepare ginseng saponin Compound K with ultrasound-assisted total zymolytic ginseng saponins. The conversion rate was taken as the index to detect the pre-treatment factors such as ultrasonic power and ultrasonic time, as well as the impact of enzymatic factors, such as pH value, temperature, concentration of substrate, dosage of enzyme and reaction time, on the conversion rate. The response surface method was used to optimize the preparation conditions. The enzymolytic products were identified with MS, 1H-NMR and 13C-NMR. The results showed that the optimum conditions of the ultrasound-assisted enzymolysis were 250 W for ultrasonic power, 15 min for ultrasonic time, 5.5 for enzymolytic pH, 50 degrees C for enzymolytic temperature, 36 h for enzymolytic time, 4:5 for enzymolytic dosage: substrate and 1.0 g x L(-1) for concentration of substrate. The relative molecular mass of reaction products was 622.4. Therefore, the nuclear magnetic map verified that the reaction product was rare ginseng saponin Compound K. Under the above conditions, based on the total zymolytic ginseng saponins, the conversion rate of rare ginseng saponin Compound K was 6.91% in proportion to the total of ginsenosides. The process features gentle reaction conditions, high conversion rate and simple and reliable process, which is suitable for industrial production.

With low molecular weight chitosan and poloxamer 188 as the joint carriers, ginsenoside Rg3 solid dispersions were prepared by using the solvent evaporation method for an in vitro dissolution test. Subsequently, differential scanning calorimetry (DSC), scanning electron microscopy (SEM) and X-ray diffraction (X-RD) were adopted for a phase analysis. The results showed that the 60 min in vitro cumulative dissolution rate of ginsenoside Rg3 solid dispersions prepared with low molecular weight chitosan and poloxamer 188 at the ratio of 2:1 exceeded 90%, and the drug was dispersed in carriers in an amorphous state. Therefore, ginsenoside Rg3 solid dispersions prepared with low molecular weight chitosan and poloxamer 188 could help significantly improve the drug dissolution, with a practical application value.

To establish the online two-dimensional liquid chromatography by using double gradient liquid chromatography system and UV detector, in order to simultaneously determine the content of paeoniflorin, paenol, amygdaloside and cinnamic acid. A pump of the two-dimensional liquid chromatography was adopted as the one-dimensional separation pump. C18 (3.0 mm x 150 mm, 3 microm) was used as the analytical column, with acetonitrile as the organic phase and 0.08% phosphoric acid + 0.08% triethylamine as the aqueous phase for gradient elution at the flow rate of 0.5 mL x min(-1). Another pump of the two-dimensional liquid chromatography was adopted as the two-dimensional separation pump. PAII C18 was used as the analytical column, with acetonitrile as the organic phase and 20 mmol, pH 3.0 monopotassium phosphate as the aqueous phase for gradient elution at the flow rate of 0.8 mL x min(-1). The detection wavelengths were set at 218, 230, 275 nm by using wavelength time-switching program. The linearity range of paeoniflorin, amygdaloside, paeonol and cinnamic acid were 5.55-222 (r = 0.999 7), 6.6-264 (r = 0.999 8), 3.3-132 (r = 0.999 5) and 0.315-12.6 mg x L(-1) (r = 0.999 7), respectively. The average recoveries of the four components were between 96.12% and 103.9%. The experiment proved that this method was so rapid and accurate in determination results that it could be used for evaluating drug quality.
