Steatocystoma multiplex, a cutaneous disorder involving the appearance cysts on the trunk and upper arms, is often inherited as an autosomal dominant trait. The authors present mammographic and US findings of steatocystoma multiplex in a 35-year-old woman with breast masses. On mammograms, multiple uniform thin walled round or oval shaped oil cysts were seen in the peripheral portion of the breasts and axilla, and on US, well-defined or smooth ill-defined round or oval shaped nodules were seen in the subcutaneous layer of the breasts and axilla ; compared with subcutaneous fat, there was iso- or slightly increased echogenicity.

BACKGROUND: It is well-known that dyspeptic symptoms in functional dyspepsia are related to psychological factors such as stress and mood disorder. But it is still controversial whether H. pylori infection is a important factor for functional dyspepsia. We conducted this study to investigate whether H. pylori infection or stress are related to dyspeptic symptom severity in functional dyspepsia. METHODS: From Mar. 1 1999 to Apr. 30 1999, attendees to a Health Screening Center and a Family Practice Center were eligible study subjects. The subjects were responded to questionnaire including dyspeptic symptom severity items and BEPSI-K(stress scale). And among eligible subjects, one hundred and thirty two diagnosed with functional dyspepsia by gastrofibroscopy were final study subjects. H. pylori infection was confirmed by antral biopsies & smear (Giemsa stain). RESULTS: In functional dyspepsia, high BEPSI-K score group(upper tertile) complained more severe dyspeptic symptom severity than low BEPSI-K score group(lower tertile) after adjusted for other variables(adjusted OR=8.389, 95% C.I.=2.232-31.231). But the dyspeptic symptom severity was not associated with H. pylori infection status (adjusted OR=1.514, 95% C.I.=0.567-4.046). CONCLUSIONS: The dyspeptic symptom severity in functional dyspepsia was associated with stress but not with H. pylori infection status.

OBJECTIVES: Organophosphates make their clinical effects after absorbed through gastrointestinal tract, lungs and skin. We commonly see the gastrointestinal tract and lung as routes of organophosphates (OP) intoxication, but there have been few reports that said the skin as a route OP intoxication. We have experienced many patients that showed OP intoxication symptoms during or after the control of pine gall midge. So we analyzed the clinical characteristics of the patients and evaluated the route of OP intoxication. METHODS: We analyzed retrospectively 26 cases who were diagnosed as 'OP intoxication after control of pine gall midge' from June 1 1995 to July 31 1996. RESULTS: 1) The mean age of the cases, mean duration of work and mean initial cholinesterase level were 52 years, 11.9 days, 318.2U/L respectively. And the over all ratio of male to female was 11:15. 2) All cases were engaged in drug injection and 7 cases (32%) weared mask. Face and upper body were excluded from protective clothings. 3) The cardinal symptoms were diarrhea and dizziness followed by nausea, vomiting, headache, anorexia, paralysis in order of frequency. 4 Directly contributing factors to symptom onset were rain, excessive sweating due to hot weather and direct contact. 5) Most of cases (92%) were recovered completely. 2 cases died during hospitalization due to acute respiratory failure and sepsis. Mechanical ventilation were applied in 4 cases for mean 12 days. In 2 cases, there were neurobehavioral changes as delayed neurologic sequelae. CONCLUSION: We have concluded that the route of organophosporus intoxication after the control of pine gall midge was skin (transdermal absorption). Sufficient education and protective measures should be done for preventing organophosporus intoxication in the control of pine gall midge.

For the past two years, nephropexy using PCN was underwent in 9 cases of nephroptosis (8 patients) with variable degrees of flank pain. All were female and 6 on right, 1 on left and 1 on bilateral. All patients had a very mobile kidney and exact distance of movement measured on the film was greater than 6cm (6 to 15 cm). After this operation, 14-20 Fr. nephrostomy catheter was indwelled for about two weeks. All patients were followed at least 3 months(3 to 32, median 17 months). We followed the patients with symptom relief and IVP after 3 months. Nephropexy was regarded as success if she remained asymptomatic for more than 3 months. Five cases were successful and four cases felt recurrent flank pain within 1 month after the surgery. Among four cases of recurrence, one had repeated PCN and got successful result, another had open nephropexy and the others have been followed so far So overall success rate was 67% (6/9). In conclusion, nephropexy using PCN is less invasive, needs shorter period of admission and leaves ignorable scars postoperatively. Furthermore it can be repeated even in the case of failure or recurrence, which would make this new technique available as the first step for the surgical treatment of nephroptosis. And the success rate could be increased if the nephrostomy tract is dilated upto 30 Fr. and the PCN catheter is placed about 4 weeks.

OBJECTIVES: The purpose of this study is investigate the clinical characteristics and the change of depression and anxiety in pathological gamblers who completed 10-week admission treatment programs. METHODS: To investigate the clinical characteristics, we included 40 pathological gamblers who completed admission treatment programs from Jan 2006 to Jun 2010. To measure the change through treatment, we included 16 patients who completed pre-treatment and post-treatment assessment scales, including the Korean-National Opinion Research Center DSM-IV Screen for Gambling Problems (K-NODS), Beck's Depression Inventory (BDI), and the State-trait Anxiety Inventory (STAI), from May 2008 to Aug 2008. RESULTS: Mean age of the patients was 42.9+/-9.0 years and mean intelligent quotient was 109.7+/-10.4. Thirty (75%) patients showed comorbid disorders, including depression (14 patients, 35%), anxiety disorder (five patients, 12.5%), alcohol use disorder (five patients, 12.5%), and personality disorder (four patients, 10%). After treatment, K-NODS, BDI, STAI-I, and STAI-II scores showed improvement, from 9.1+/-1.5 to 7.6+/-2.7, from 23.5+/-5.8 to 11.1+/-3.2, from 52.6+/-10.3 to 41.9+/-7.1, and from 53.7+/-10.6 to 46.0+/-4.4, respectively. CONCLUSION: In treatment of pathological gambling, comorbid psychiatric risk factors, such as depression and anxiety should be considered.

Entrapment of the extensor pollicis longus tendon is reported rarely on Smith's fractures in children. In our case, a 15 year old boy with Smith's fracture received treatment of closed reduction at another hospital. When he visited our hospital, a wide gap at the fracture site was detected on radiograph and the thumb movement was limited. We have doubt the entrapment of the soft tissue, especially the tendon. We decided on open reduction. In the operation field, entrapment of the extensor pollicis longus tendon at the gap of the fracture site was found through dorsal approach. In addition, fracture treatment with K-wire fixation after reduction of extensonr pollicis longus tendon reduction was done. Therefore, we report this case with a review of the literatures.
