80 patients, 40 patients without preexisting EKG abnormality (group 1) and 40 patients with preexisting abnormalities of EKG (group 2), receiving general anesthesia in the operating room were monitored continously with electrocardioscope, Servomed SMK 155-1 and were recorded. 1) 7 cases(17.5%) of group 1 developed a variety of arrhythmia, and 13 cases (32.5%) of group (32.5%) of group 2 developed a variety of arrhythmia. 2) The most common arrhythmia was premature ventricular contraction including bigeminies (13 cases, 60% of the total arrhythemia) and the most serious arrhythmia was seen 1 case of rapid ventricular tachycardia without artrial activity. 3) Continous cardiac monitoring is valuable, easy and practical in virtually all instance during anesthesia and surgery.

Pulmonary comp1ications following surgery continue to be a major cruse of postoperative mobidity. The purpose of this study was to measure the effects of various surgical procedures upon arterial blood gas tension and ventilatory function in the early postoperative period. 30 patients with no history of cardiopulmonary disease before surgery were selectively chosen and divided into 4 categories depending on the location of the operative incision and the type of anesthesia. Group l was composed of upper abdominal incision cases, Group ll lower abdominal incision cases, and Groups lll and lV had nonabdominal surgery. Groups, l, ll and lll received general anesthesia, while lV was liven spinal anesthesia. Lung volumes were measured and blood samples were taken preoperatively and 70 minutes after surgery. Patients were maintained in semisitting position before and after surgery. Vital capacities were significantly reduced in the postoperative period in the general ane- sthesia groups, and tidal volumes were significantly reduced in the upper and lower abdominal incision groups. The mean PaO2 value after anesthesia decreased significantly in the upper and lower abdominal incision groups, but there were no significant differencea in the PaCO2 value. It seems that supplemental oxygen and encouragement of deep inspirations are essential and beneficial in the early postoperative period, especially for the patients with abdominal surgery.

TO determine the normal neonatal adrenal gland size, ultrasonographic examinations were performed in 145 newborn infants. They were divided into 3 groups according to the days of age. The group I is 1~3 days, group II is 6~9 days and group III is 21~50 days of age. 1) The adrenal gland size was as follows. In group I, the length was 29.05mm and the width was 4.02mm. In group II, the length was 24.04mm and the width was 2.79mm. In group III, the length was 19.54mm and the width was 2.21mm. 2) With increasing age, the size of adrenal gland became smaller. 3) The size of adrenal gland correlated well with the birth weight and height. We conclude that the ultrasonographic examination is useful in infant adrenal gland disease.

Hydroa vacciniforme is a rare, chronic photosensitivity disorder manifested in childhood by recurrent vesicles that hea1 with srarring. We report a case of hydroa vacciniforme in which vesicles were induced with multiple exposures ta UVA. The clinical features and ap-propriate laboratory evaluation of hydroa vacciniforme are reviewed.

PURPOSE: To evaluate the clinical interpretation and application of Teller Acuity Card(TAC) test compared with optotype acuity chart in the diagnosis of amblyopia. METHODS: The visual acuities of 112 children(54 normal, 58 amblyopes), whose mean age was 6.5 years(3~11 years) were measured with Han's letter chart and TAC. The sensitivity, specificity, false-positive and false-negative rates of TAC Test were analyzed according to the visual acuity, age and type of amblyopia. RESULTS: TAC acuity was higher than optotype acuity in both strabismic and anisometropic amblyopia group, however, the anisometropic amblyopia group showed higher correlation between two tests(P<0.01,r=0.0755). The sensitivity and specificity of TAC test increased in lower visual acuity group, which means that TAC test is useful in detecting amblyopia. While the false-positive rate increased in higher visual acuity group, false-negative rate increased in lower visual acuity group and in the group less than 6 years of age. And the bilateral visual acuity were underestimated than unilateral visual acuity in about 30%. CONCLUSIONS: Though TAC test is a useful method in detecting amblyopia of infancy and childhood, an appropriate clinical diagnosis should be accompanied for the accurate diagnosis of amblyopia.
