1.Prognostic Value and Histologic Correlation of Sonography in Childhood Nephrotic Syndrome.

The uterine cervix is an uncommon site of primary non-Hodgkin's lymphoma (NHL). Although the cytologic findings of NHLs are well known, most cervicovaginal smear of uterine NHLs give lower diagnostic yield than common epithelial malignancy because abnormal cells do not appear in the sample in the absence of surface ulceration. Herein, we describe cytologic findings of a case of uterine cervical NHL which was initially diagnosed by cervicovaginal smear. The tumor cells were relatively uniform, isolated, large-sized with scanty cytoplasm and round or indented nuclei. The nuclei had stippled chromatin and small nucleoli. Histologically and immunohistochemically the tumor was proven to be large cell lymphoma of T-cell lineage.

2.An Ethnographic Study on Eating Styles of Adult Diabetics in Korea.

PURPOSE: The purpose of this study was to explore adult diabetics' eating styles and factors which influence them. METHOD: The study adopted an ethnographic method based on a perspective which views the eating style as a cultural phenomenon. Data was collected through a personal interview, participant observation, and documented materials from Oct.2001 to Sept. 2002. In this study, fifteen adult diabetics, with an average age of 57, participated. Data analysis was done by the Spradley's taxonomic analysis technique. RESULT: The patients' eating styles were rooted in their viewpoint on illness as well as the meaning of food. Eating styles were classified into 4 types: Pathology-centered, symptom-centered, need-centered, and role-centered. CONCLUSION: A conventional approach to the treatment and management of diabetes did not consider the patient's inner world which may play an important role in the successful management of the disease. We found that it was critical for health care personnel to understand patients' values, beliefs and their way of life in order to facilitate the most successful self-care diet.

3.A Case of Epstein's Syndrome.

Epstein's syndrome is a rare disease whish is characterized by the association of thrombocytopenia, macrothrombocytopathia, nephritis and deafness. We experienced a case of Epstein's syndrome in a 12 years old male patient who was presented with a life long history of bleeding, usually as epistaxis, bilateral sensorineural deafness and hematuria with proteinuria starting in late childhood. Hematologic studies showed thrombocytopenia with giant platelets and anemia. A bone marrow aspirate revealed the megakaryocytes to be adequate in number and many giant size platelets. Platelet do not respond to addition of A and epinephrine; collagen and ristocetin induced agglutination response is decreased. It is difficult to be certain the association of thrombocytopenia with giant platelets, nephritis and deafness constitutes a new hereditary disease with a distinct pathogenesis or if it is an expansion of the well recognized Alport's syndrome of hereditary nephritis deafness. We report a case of Epstein's syndrome syndrome with brief review of related literatures.

5.The Suppressive Effect of Evening Primrose Oil on Murine Contact Sensitivity.

BACKGROUND: Evening primrose oil(EPO) is a rich source of cis-linoleic acid and gammalinolenic acid(GLA) and has been used as a therapeutic agent in various skin diseases such as atopic dermatitis. OBJECTIVE: The purpose of this study was to evaluate the suppressive effect of EPO on murine contact sensitivity. METHODS: BALB/c mice were divided into 3 groups, positive control, experimental and negative control groups: the positive control group represents a group of mice which were sensitized and challenged with DNFB, the experimental group represents EPO-pretreated positive control group and the negative control group represents a group of mice which were challenged only. The changes of ear thickness were measured, and H & E staining and immunohistochemical staining for ICAM-1 expression of ear skin were performed to evaluate the histological changes of each group. RESULTS: The Pretreatment of mice with EPO resulted in suppression of contact sensitivity by more than 82%. On H & E staining, only a mild inflammatory reaction was observed in the dermis. Also ICAM-1 expression of keratinocytes, the intensity of the staining was significantly decreased in the experimental group compared with positive group. CONCLUSIONS: Our study indicates that EPO was able to suppress the induction of contact sensitivity.

6.The Effect of Extracorporeal Magnetic Innervation Therapy on Pain, Urination and Quality of Life in Chronic Pelvic Pain Syndrome.

PURPOSE: Chronic pelvic pain syndrome(CPPS) is the most common category of the chronic prostatitis. CPPS is a poorly defined, multifactorial condition characterized by pelvic pain and voiding symptoms. The effectiveness of extracorporeal magnetic innervation(ExMI) therapy was evaluated in patients with CPPS. MATERIALS AND METHODS: From July 2003 to December 2004, 28 men who were diagnosed with CPPS who were refractory to medication underwent ExMI therapy for 2 visits weekly for 8 weeks. The patients completed the National Institute of Health Chronic Prostatitis Symptom Index(NIH-CPSI) and a maximal flow rate index. RESULTS: Significant decreases occured in NIH-CPSI pain(9.7 to 8.3, p <0.05), NIH-CPSI urination(4.4 to 3.9, p <0.05), NIH-CPSI quality of life 8.7 to 7.7, p <0.05), and total NIH-CPSI(22.8 to 19.9, p <0.05) scores after treatment. There was an insignificant decrease in the maximal flow rate(21.5 to 21.4, p> 0.05) score after treatment. CONCLUSIONS: ExMI therapy effectively improves the symptoms of CPPS. ExMI therapy appears to be a safe and effective treatment in improving both the symptoms and the quality of life of men with CPPS refractory to conventional treatment. A larger controlled study is required to confirm these results.

7.Meaning of Well-being for the Middle Aged Adults.

PURPOSE: This research was done to explore the meaning of 'well-being' as experienced by middle aged people. METHOD: The data were collected by individual in-depth interviews with 107 middle aged adults and it was analysed using qualitative content analysis. RESULTS: 4 components of the meaning of well-being as perceived by the participants were identified as follows: the life of free without physical discomfort and psychosocial distress; the life of comfort with plenty of time, space, material, and mind; the life of purity with natural material and honest mind; the life of harmony with extended consciousness. CONCLUSION: Therefore the identified meanings of the well-being in this study should be reflected to the nursing education and the nursing practice.

9.A Case of Congenital T Cell Lymphoblastic Lymphoma.

T cell lymphoblastic lymphoma is characterized by immature lymphoid cells that are indistinguishable from the lymphoblasts and prolymphocytes of acute lymphoblastic leukemia. Several characteristic clinical features of lymphoblastic lymphoma, with include a high male-to-female ratio, a relatively high incidence in older children and young adults, the frequent presence of mediastinal involvement at the time of diagnosis. Also, this disease is rapidly progressive, and early dissemination to the bone marrow, blood, and central nervous system leads to the evolution of a picture resembling a acute lymphoblastic leukemia. We have experienced a rare case of congenital T-cell lymphoblastic lymphoma in which 1/365 year old female newborn had generalized multiple irregular protruding mass on her body. On bone marrow biopsy, CSF analysis, ultrasonogram and whole body MRI studies. We found metastasized tumor mass to, orbit, abdominal cavity, bulva, skin, and lower extremities. Histopathologically, specimen from mass on the right thigh showed diffuse infiltration of poorly differentiated and immature lymphoid cells in the skelectal muscle and subcutaneous soft tissue. By immunophenotyping studies using anti T cell and B cell monoclonal antibody, these cells were reactive with UCHL-1, MB2, but unreactive with MB1, L26. Based on these findings, the lesion was diagnosed as lymphoblastic lymphoma of the T-cell type, which occurred congenitally. Most T cell lymphoblastic lymphoma are noted, but congenital case was not reported. So we report it with a brief review of literature.

10.Influence of Parenting Self-efficacy and Belief in Corporal Punishment on Physical Abuse of Children in Korea.

PURPOSE: This survey was done to describe parenting self-efficacy and beliefs in corporal punishment as they are related to child abuse. Also demographic variables that influence child abuse were investigated. METHOD: A cross-sectional study was conducted during the second week of April 2002. The 160 participants were parents of students in five elementary schools in the J area of Korea. They replied to a structured questionnaire, and 129 replies were included in the final analysis. The WIN SPSS program was used for the analysis. RESULT: Parenting self-efficacy, beliefs in corporal punishment and child abuse showed no significant differences according to gender of the children. Child abuse by parents has significantly negative correlation with parenting self-efficacy (r=-.369, p=.000), socioeconomic states of family(r=-.290, p=.001), educational level of mother(r=-.211, p=.027), educational level of father(r=-.342, p=.000), parent's age(r=-.200, p=.028). Stepwise multiple regression showed that parenting self-efficacy and beliefs in corporal punishment significantly influence child abuse in Korean parents. CONCLUSION: As parenting self-efficacy explained 49.7% of child abuse, it is the most important variable for preventing child abuse. Belief in corporal punishment was the second most important variable in preventing child abuse. These two variables explained 53.3% of variance in child abuse by parents.
