1.Histopathologic findings of necrotizing lymphadenitis.

Necrotizing lymphadenitis was first recognised as a self-limiting lymphadenitis by Japanese workers in 1972. It is a distinct clinicopathologic entity, but can be mistaken as malignant lymphoma. We have studied clinicopathologic features in 15 cases of necrotizing lymphadenitis. This disease occurs predominantly in young adult. Male-female ratio is 2 : 1. The commonest presentation is lateral cervical lymphadenopathy. Pain, tenderness, and fever can be seen. Biopsy of the lymph nodes from all patients demonstrates the characteristic histologic features : multifocal, relatively circumscribed nodules in the cortex and/or paracortex, consisting of a mixture of activated large lymphoid cells, histiocytes and Small lymphocytes. Numerous karyorrhetic debris are present. Neutrophils and plasma cells are strikingly absent.

2.Dentoalveolar compensation according to skeletal discrepancy in Normal occlusion.

The purpose of this study was to investigate the dentoalveolar compensation according to anteroposterior skeletal discrepancy in normal occlusion and to evaluate cephalometric parameters that quantitatively describe dental compensations. The study consisted of 90 subjects (50 males, 40 females) who were selected among specimens of normal occlusion at Seoul National University Dental Hospital, Dept. of Orthodontics. Lateral cephalograms in centric occlusion were traced and digitized for each subject. According to the anteroposterior skeletal pattern, the sample was divided into three groups. Cephalometric data were analyzed for the three groups using the SPSS program. Independent t-test, correlation analysis and regression analysis were carried out. The results were as follows: Dentoalveolar compensation was found in upper and lower incisor inclination and occlusal plane inclination. As the mandible located anterior to the maxilla, the maxillary incisors inclined more labially, the mandibular incisors more lingually, and the occlusal plane continued to flatten. The dental parameters most correlated with anteroposterior skeletal discrepancy were L1 to SN and L1 to FH. Among the compensatory dentoalveolar changes, lower incisor inclination was strongly related to the anteroposterior jaw relationship and played an important role in obtaining a normal incisor relationship. U1 to PtGn and L1 to APog were constant irrelevant to anteroposterior skeletal discrepancy.

5.A Prospective Study on the lncidence of Ventilator-associated Pneumonia in Patients with Circuit Changes every 3 days Versus Weekly Changes.

BACKGROUND: Ventilator associated pneumonia (VAP) is the most serious nosocomial infection of intensive care units. Several studies have investigated the relationship between the interval of ventilator circuit changes and the incidence of pneumonia in foreign countries, but there are no reports about it in Korea yet. So we performed this study to compare the clinical and cost impact between 3 days and 7 days interval in ventilator circuit changes. METHODS: Seoul National University Hospital is a 1,500-bed, university affiliated, tertiary and acute care hospital. All patients admitted to medical intensive care unit (MICU) and surgical intensive care unlt (SICU) between April 1, 1998 and October 31, 1998, requiring mechanical ventilation were included. Patients were divided into two groups of a-cay circuit changes and weekly changes. Daily surveillance was conducted using the criteria of VAP of the National Nosocomial Infection Surveillance System. Incidence of VAP and risk factors for VAP were evaluated. Standard microbiologic methods were used for the identification of clinical and environmental isolates. Statistical analysis was done by SAS Program (version 6.12), analysis of difference in variables was performed using chi-square test and t-test. Analysis of odds ratios was done with logistic regression analysis. RESULTS: VAP developed at a rate of 12.2 per 1,000 ventilator-days in the 3 days change group and 15.6 per 1,000 ventilator-days in the weekly change group (P=0.7240). The only statistically significant risk factor of VAP was duration of mechanical ventilation, The risk of VAP in patients with more than 7 days was 2.23 times higher than in patients with 7 days and below (OR; 2.2296). Estimated annual savings of nursing time by extending ventilator circuit change interval from 3 days to 7 days were 26,806 min 48 sec and estimated savings of cost by reduction of nursing times was calculated as 6,701,700 won. CONCLUSIONS: Weekly ventilator circuit changes in patients undergoing ventilation therapy in the ICU do not contribute to increased the rates of VAP and are cost-effective.

6.A Study on Major Health Components of National Health Examination Survey in Korea.

The purpose of this study was to identify the major health components and measurements to be conducted in National Health Examination Survey(KNHES). The prevalence and severity of disease, acceptability of population and the possibility, of standardization of measurement were considered as guideline for selecting the components. On the base of magnitude and severity of disease, chronic liver disease, hepatic cancer, gastric ulcer, stomach cancer, essential hypertension, cerebrovascular disease, ischemic heart disease, pulmonary tuberculosis, lung cancer, DM, breast cancer, cervical cancer, arthritis and intervertebral disc disorder were selected as the preliminary target diseases. Questionnaire survey for 648 persons in 'K' city and medical specialists in five clinical societies were conducted for evaluating the acceptability of general population for the measurements and the possibility of standardization for the procedures. In conclusion, the major target diseases were chronic liver disease, hypertension and DM and the total cholesterol, high density lipoprotein, triglyceride, total protein, albumin, hemoglobulin, hematocrit, platlet count, anti-HBs, HBsAg, height and weight were selected for basic physical components.

7.Computerization of Surgical Pathology Reporting by Personal Computer.

The authors have been developed a menu-driven FoxBASE system for surgical pathology reporting and automatic encoding in Systematized Nomenclature of Medicine. The system requires no prior knowledge of FoxBASE and is readily installed on any IBM or it's compatible personal computer. Working sheet generation is automatically accompanied by data from previous cases on the same patient. Important data which include patient name, age, sex, surgical number, hospital unit number and encoded diagnoses, are stored on the hard disk permanently; complete reports are saved on floppy diskettes. Cases can be retrieved by patient name, surgical number, hospital unit number and SNOMED codes within 0.1 second. Daily work lists and listings of incomplete cases are easily obtained. This FoxBASE system has been in use for 1 year and 6 months and resulted in increased efficiency of retrieval and gathering of basic information for specific study, cost effectiveness, markedly diminished workload of typist and very short wasting time during complete restoration of data file for hard disk failure.
8.Treatment of congenital hypogonadotropic hypogonadism in male patients

Congenital hypogonadotropic hypogonadism (CHH) is characterized by complete or partial failure of pubertal development because of inadequate secretion of gonadotropic hormones. CHH consists of hypogonadotropic hypogonadism with anosmia or hyposmia, Kallmann syndrome, and the normosmic variation normosmic idiopathic hypogonadotropic hypogonadism. CHH is one of the few treatable diseases of male infertility, although men with primary testicular dysfunction have irreversibly diminished spermatogenic capacity. The approach to CHH treatment is determined by goals such as developing virilization or inducing fertility. This review focuses on the current knowledge of therapeutic modalities for inducing puberty and fertility in men with CHH.
10.Waist-height ratio and body mass index as indicators of obesity and cardiometabolic risk in Korean children and adolescents

Purpose:
We assessed the clinical relevance of waist-height ratio (WHtR) as an indicator of cardiometabolic risk and body fat mass measured by dual-energy x-ray absorptiometry (DXA) among Korean children and adolescents.
Methods:
Data from 1,661 children and adolescents aged 10–18 years who participated in the Korea National Health and Nutrition Examination Survey were analyzed. Unadjusted Pearson correlation, age- and sex-adjusted Pearson correlation, and multiple linear regression analyses were performed to investigate the relationships between WHtR standard deviation score (SDS) and cardiometabolic risk factors, as well as DXA-assessed parameters.
Results:
WHtR SDS was correlated with cardiometabolic risk factors, including systolic blood pressure, glucose, total cholesterol, high-density lipoprotein cholesterol, triglyceride, and low-density lipoprotein cholesterol, as well as DXA-assessed parameters such as lean mass SDS, fat mass SDS, and fat mass percentage SDS in both whole body and trunk using an adjusted Pearson correlation analyses among all participants (p<0.001). WHtR SDS was strongly correlated with whole-body fat mass and trunk fat mass (r=0.792, p<0.001 and r=0.801, p<0.001, respectively) whereas WHtR SDS had a low correlation coefficient with whole-body lean mass and trunk lean mass SDS (r=0.512, p<0.001 and r=0.487, p<0.001, respectively). In multiple linear regression analyses, WHtR SDS was significantly associated with whole-body and trunk fat mass after adjustment for confounders.
Conclusion
Cardiometabolic risk factors and body fat mass assessed by DXA in Korean children and adolescents were highly correlated with WHtR. Additionally, WHtR has an advantage in distinguishing fat-free mass. WHtR can be a useful and convenient clinical indicator of cardiometabolic risk factors.
