BACKGROUND: The afferent and efferent pathways of fentanyl cough response (FCR) and central organization are poorly understood at present. The aim of this study was to investigate the pretreatment effects of morphine, propofol, atropine, and midazolam on FCR. METHOD: The 120 healthy patients were randomly assigned to six equal pretreatment groups. They received 2ug/kg fentanyl rapidly through a peripheral venous catheter. The patients in each group were pretreated before the time necessary for peak plasma levels with different drugs as follows: group 1, no premedication; group 2, morphine 0.05 mg/kg iv; group 3, morphine 0.05 mg/kg iv naloxone 0.01mg/kg iv; group 4, propofol 0.5 mg/kg iv; group 5, atropine 0.01 mg/kg iv; group 6, midazolam 0.05 mg/kg iv. The patients were observed for any coughing or side effects, including oxygen desaturation, bronchoconstriction, chest wall rigidity and seizure. RESULT: 40% of patients in group 1 (control) had a cough response to fentanyl. Group 2 (morphine) and group 3 (morphine naloxone) showed a reduced FCR of 10%. The incidence of coughing was 60% of the patients in group 4 (propofol), 30% in group 5 (atropine), and 40% in group 6 (midazolam). These were not statistically significant. CONCLUSION: FCR is not altered by pretreatment with propofol, atropine, or midazolam, but morphine inhibits cough response and this antitussive effect was not antagonized by naloxone.

BACKGROUND: Obesity is linked to a wide range of respiratory diseases. Several studies have shown that body weight at baseline and weight change were related to pulmonary function. The purpose of this study was to investigate the relationship between change in body mass index (BMI) and pulmonary function in adults. METHODS: Of those aged 40-64 years at baseline who had initially visited the health promotion center at one university-level hospital from January 2000 to December 2002, 499 (men: 309, women: 190) patients revisited the center over a 5-year period up to December 2009 and were enrolled in the study. Subjects were classified into 4 groups- group 1: normal-normal, group 2: normal-obese, group 3: obese-normal, and group 4: obese-obese, based on their BMI at baseline and follow-up. Forced expiratory volume in one second (FEV1), forced vital capacity (FVC) and FEV1/FVC were measured by spirometry. RESULTS: Change in FEV1/FVC was significantly associated with change in BMI for men in all 4 groups. Change in FEV1/FVC was significantly different between group 1 and 3 and between group 1 and 4. Changes in FEV1, FVC and FEV1/FVC were significantly associated with change in BMI for women in all 4 groups. Change in FEV1 was significantly different between group 3 and 4, and change in FVC was significantly different between group 1 and 3. CONCLUSIONS: These results suggest that a change in BMI is negatively associated with change in pulmonary function. Obesity itself can be a risk factor for pulmonary dysfunction, and a decrease in BMI through weight reduction could reduce pulmonary dysfunction or improve pulmonary function in adults.

BACKGROUND: The aim of this study was to compare the cardiovascular changes followed by laryngoscopy with the McCoy laryngoscope blade with those followed by laryngoscopy with the Macintosh laryngoscope blade. METHODS: Forty eight patients were randomly divided into two groups. Following induction with fentanyl 2 mcg/kg and thiopental 5 mg/kg, and muscle relaxation with vecuronium 0.1 mg/kg, the vocal cords were visualized with either the McCoy or the Macintosh laryngoscope blade, then tracheal intubation was performed. Heart rate and arterial blood pressure were measured just before and after laryngoscopy, and 1, 3 and 5 min later. RESULTS: There was a significant increase in both heart rate and arterial blood pressure after tracheal intubation using the Macintosh laryngoscope. Also, use of the McCoy blade resulted in a significant increase in both heart rate and arterial blood pressure. CONCLUSIONS: There was no significant difference on arterial pressure and heart rate to laryngoscopy and tracheal intubation with either the McCoy blade or the Macintosh.

BACKGROUND: Accessory axillary breast tissue is present in approximately 2% to 6% of women, but receives little attention in the surgical literature. Furthermore, a carcinoma, as well as other pathologic changes, can arise within accessory axillary breast tissue. METHODS: Between January 1990 and December 1997 at our clinic, 83 patients underwent surgical removal of accessory axillary breast tissue. All the patients were female, and the ages ranged from 14 to 57 years, with a median of 33.8. The clinical characteristics, indications, and techniques of surgical removal, pathologic findings, and postoperative results were analyzed. RESULTS: The major clinical manifestations were axillary mass (95.2%), pain and/or tenderness (41%), and cyclic change of size (21.7%). In most patients, these symptoms appeared first during puberty (25.3%) and pregnancy (43.4%). On examination, the mass was in the right axilla in 35 patients (42.2%), the left axilla in 19 patients (22.9%) and both axillae in 29 patients (34.9%), and 3 patients had rudimentary accessory nipples. Surgical removal was performed for cosmetic problems (61.5%), clinical symptoms (21.7%), and suspicion of pathologic lesions (9.6%). The excised breast tissue varied in size from 2.5 cm to 14 cm with a median of 6.2 cm. The pathological findings demonstrated ductal dilatation in 21 patients (25.3%), chronic inflammation in 4 patients (4.8%), fibrocystic changes in 3 patients (3.6%) and lactating adenosis in 2 patients (2.4%). Postoperative complications developed in 8 patients (8.3%), and seroma was the most common. In our study, none of the patients experienced recurrence of disease, and 96.4% of the patients enjoyed cosmetically satisfying outcomes. CONCLUSIONS: In conclusion, from our experience, the treatment of choice for accessory axillary breast tissue is surgical removal, which eliminates the clinical symptoms and the aesthetical deformity and provides tissue to rule out any pathologic changes including malignancy. For cosmetically satisfying outcomes, a scar hidden from sight and balanced shapes of both axillae should be kept in mind during the operation.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

Sternal metastasis of poorly differentiated thyroid carcinoma (PDTC) is rare, and only a few cases have been reported in the literature. Here, we report a case of sternal metastasis of PDTC in an 83-year-old woman, 2 years after right hemithyroidectomy, treated with sternal resection and reconstruction.
