2.Current Status and the Future Prospect of Rehabilitation Nursing in Korea.

The history of rehabilitation of disabilities in Korea began with the foreigners and missionaries who were interested in it after Korean War. In 1981. Disabled Persons Welfare Act was enacted and the 88 Paralympics brought the nations attention to the welfare and rehabilitation of persons with disabilities. Since then, the facilities and the services for the disabled persons have expanded rapidly and the rehabilitation treatment and nursing intervention are drawing more attention. Against this background, the survey on the current status of disabilities, welfare service, facilities, and rehabilitation nursing was conducted. The results of this survey are as follows 1. According to the 2000 census of disabilities. the number of persons with disabilities in Korea is estimated at 1.449.500. or 3.09% of the entire Korean population, 0.74% up from 2.35% in 1995. 2. Disability Types in 2000 The 2000 census showed that the persons with disabilities numbered 1.449, 496 out of the total population and 1.024,371 persons are registered for disability. making up 70.7% of the estimated disabled population. Among them, physically disabled persons accounted for the largest 41. 7% (605.127) and mentally retarded persons stood at the smallest 9% (13.481). 3. Percentage of Disability Presence The survey showed that more than 90% of disability were acquired. However, 44.8% of mental disability and 61.4% of hearing /speaking disability were not acquired after birth. This means that these disabilities happened by congenital cause or birth accident. 4. Yearly Figure of Registered Disabled Persons In 1989. 218,601 persons registered for disability and, in 2000, the number increased by 4.7 times to 1.024.371. These figures are different from the actual number of disabled persons. According to the 1995 census. 1.053.486 were disabled persons but only 378,323 registered for disability. And. in the 2000 census. 1.024,371 out of the 1.449.496 of disabled persons registered for disability. 5. Welfare Service for Persons with Disability 62.6% of the total disabled people are registered and physically disabled persons accounted for the highest percentage of 96.7%. 26.5% of non-registered disabled people said that they didn't know the registration procedure. The rest of them replied that they didn't think they were disabled or that registration didn't seem to give any benefits 6. Welfare Policies for Disabled Persons The welfare benefits given to the disabled are as follows: Issuance of disabled sign for car drivers. Permission to use LPG fuel. Reduction of public facility fees. Household allowance. Tax reduction or exemption, Medical allowance and education subsidy for children, and Housing. 7. Current Condition of Welfare Facilities by Disability Type. The welfare institutions for disabilities numbered 188i in total and they can accommodate 16,823 persons. Categories of these institutions are physical disability(37), visual disability(10). hearing/speaking disability(l4). mental retardation(59), and sanatoriums(68). 6. Human Resource of Rehabilitation of Disabilities Advanced education programs include rehabilitation nursing in its curriculum and this was selected as the program of Korean Academic Society of Nursing in 1990. In November 1997, Korean Academic Society of Rehabilitation Nursing was launched and many academic meeting and seminars were held. This organization is also making efforts to develop the education program for qualified rehabilitation nursing professionals and to develop the standards of rehabilitation nursing practice. In the professionals of the rehabilitation there are rehabilitation specialist, physical therapist, speech therapist, occupational therapist. It is needed to come up with the measures to supply stable human resources following the demand of disabled persons and to recognize the private certificates for rehabilitation professionals as official ones after reviewing the education and training programs of private institutions. 9. Rehabilitation Nursing 1) Rehabilitation nursing was taught as an independent subject in 11 undergraduate programs and 9 graduate programs. 2) Research on rehabilitation nursing in Korea were 24 experimental research and 11 non-experimental research. The intervention of experimental research were mostly education and exercise rehabilitation programs. 3) In the three rehabilitation hospitals, nursmg IS divided into two categories, direct nursing and education & counseling. Direct nursing includes tracheostomy or nasogastric tube care, urination and defication, skin care, pain control, complication, prevention and care, prevention of injury from a fall. etc.

4.The Role of Apoptosis in the Carcinogenesis of the Cervical Cancer.

A considerable body of evidence has been accumulated suggesting that invasive squamous cell carcinoma develops from cervical intraepithelial neoplasia(CIN). Most women with invasive cancer of the cervix are from lower socioeconomic groups, have begun heterosexual activity early in life, marry early, are multiparous, and have many sexual partners. Although the epidemiology of the cervical cancer is known well, the pathogenesis of the cervical cancer from CIN is subtle yet. Apoptosis, including the programmed cell death, is important event in normal cell turnover and maintenance of adult tissues. Apoptosis exerts a homeostatic function in relation to tissues dynamics, as the steady state of continuously renewing tissues achieved by a balance between cell replication and cell death. The specific labelling of nick ends of fragmented DNA was used to see the apoptotic cells from normal epithelium of the cervix to invasive cervical cancer. The apoptotic cells were found normally in the parabasal layer of the epithelium. As the grade of CIN increase, the apoptotic cell were found in superficial of the cervix and number of the apoptotic cell were increased. In the cervical cancer, the apoptotic cell were found in the cancerous tissues more than in the normal epithelium. This results suggest that the cell proliferation is more important than the inhibition of the apoptosis in the carcinogenesis of the cervical cancer.

6.Predictors of Cardiovascular Risk Factors among Type 2 Diabetic Patients.

PURPOSE: The purpose of this study was to explore the predictors of cardiovascular risk factors among type 2 diabetic patients. METHOD: Data were collected from November, 2003 to June, 2004 using a physiological index and questionnaires. Patients(N=159) aged 40 and above were conveniently recruited from health care centers in B city. Data were analyzed with descriptive statistics, Pearson correlation and stepwise multiple regression using SPSS WIN 10.0 program. RESULTS: The cardiovascular risk factors were negatively related with female gender, household monthly income, educational experience about diabetes, physical activity, self-care, self-efficacy and problem oriented coping, while positively related with the duration of diabetes, diabetic family history and depression. Self-care, diabetic family history, female gender, monthly household income, self-efficacy, affective-oriented coping and physical activity predicted 41.5% of the variance in cardiovascular risk factors of diabetic patients. CONCLUSION: According to the findings of this study, we concluded that cardiovascular risk factors of type 2 diabetic patients are related to the modifiable and non-modifiable variables. Self-care, self-efficacy, affective- oriented coping, and physical activity were identified as modifiable variables. Intervention programs to increase those variables are warranted to reduce cardiovascular risk factors among type 2 diabetic patients.

7.The Pretreatment Effects of Morphine, Propofol, Atropine, and Midazolam on Fentanyl Cough Response.

BACKGROUND: The afferent and efferent pathways of fentanyl cough response (FCR) and central organization are poorly understood at present. The aim of this study was to investigate the pretreatment effects of morphine, propofol, atropine, and midazolam on FCR. METHOD: The 120 healthy patients were randomly assigned to six equal pretreatment groups. They received 2ug/kg fentanyl rapidly through a peripheral venous catheter. The patients in each group were pretreated before the time necessary for peak plasma levels with different drugs as follows: group 1, no premedication; group 2, morphine 0.05 mg/kg iv; group 3, morphine 0.05 mg/kg iv naloxone 0.01mg/kg iv; group 4, propofol 0.5 mg/kg iv; group 5, atropine 0.01 mg/kg iv; group 6, midazolam 0.05 mg/kg iv. The patients were observed for any coughing or side effects, including oxygen desaturation, bronchoconstriction, chest wall rigidity and seizure. RESULT: 40% of patients in group 1 (control) had a cough response to fentanyl. Group 2 (morphine) and group 3 (morphine naloxone) showed a reduced FCR of 10%. The incidence of coughing was 60% of the patients in group 4 (propofol), 30% in group 5 (atropine), and 40% in group 6 (midazolam). These were not statistically significant. CONCLUSION: FCR is not altered by pretreatment with propofol, atropine, or midazolam, but morphine inhibits cough response and this antitussive effect was not antagonized by naloxone.

8.Three Cases of Secondary Membranous Lipodystrophy.

Membranous lipodystrophy is a peculiar type of fat necrosis, present in patients with various types of skin diseases. It is charaeterized by the presence of microcysts and macrocysts lined by amorphous eosinophilic material with a crenelated arabesque appearance and microgranules in the subcutis with massive fat necrosis. The eosinophilic lining and microgranules stain positively with periodic acid-Schiff, are resistant to diastase, and also stain with Sudan black B. We report three cases of subcutanous membranous lipodystrophy in patients with erythema induratum, posttraumatic panniculitis and morphea with typical clinical and histopathologic findings.

9.The Epidemiological Trend of Rotavirus Gastroenteritis in Children in a Single Center from 2004 to 2012: A Retrospective Study.

PURPOSE: This study was performed to investigate the epidemiological trend of rotavirus acute gastroenteritis (RV-AGE) in children. METHODS: A retrospective review was performed in patients (1 month to 18 years of age) with acute gastroenteritis at KEPCO Medical Center from September 2004 to August 2013. Comparative analyses were performed based on periods: pre-vaccine (2004-2006) and post-vaccine (2008-2012) in all patients; 2004-2006 (period A), 2007-2009 (period B) and 2010-2012 (period C) in patients under 5 years of age. RESULTS: Proportion of RV-AGE decreased from 25.0% (337/1,346) in pre-vaccine period to 20.8% (459/2,210) in post-vaccine period (rate ratio (RR), 0.83 [95% CI, 0.73-0.93]; P=0.0029). The median age of patients with RV-AGE in post-vaccine period (2.6 years) was significantly (P<0.0001) higher than that in pre-vaccine period (1.6 years). In patients hospitalized with AGE, proportion of RV-AGE was significantly reduced in patients 6 to 23 months old (RR, 0.62 [95% CI, 0.51-0.75]; P<0.0001). Significant decline in proportion of RV-AGE was observed in patients under 5 years of age: period A, 26.9% (308/1,144); period B, 22.7% (295/1,299); period C, 20.6% (186/902) (P=0.0007). After the introduction of rotavirus vaccine, a significant decreasing trend of RV-AGE proportion was observed in patients 6 to 11 months old (P=0.0018) and 12 to 23 months old (P=0.0152). CONCLUSION: Decrease in RV-AGE proportion and increase in age of patients with RV-AGE were observed after the introduction of rotavirus vaccine in this single center study. Continued and systematic surveillance is needed to assess the impact of rotavirus vaccine.

10.The Effect of Slow Stroke Back Massage on Anxiety and Immune Response in the Patients undergoing Open Heart Surgery.

The purpose of this study was to examine the effect of slow stroke back massage(SSBM) on anxiety and immune response in patients undergoing open heart surgery and to compare the effect of 5-minutes with 10-minutes SSBM. Among the sixty-four patient subjects, twenty-one were one experimental group receiving massage for 10-minutes, twenty for the other experimental group receiving massage for 5-minutes and twenty-three for the control group. Subjects were admitted at Hospitals in Inchon and Puchon for open heart surgery. This study was carried out from October 10, 1997 to May 10, 1998. The levels of anxiety were measured by the Visual Analogue Scale(VAS), Trait anxiety scale, systolic and diastolic blood pressure, pulse rate, blood cortisol and the levels of immune response were measured by the blood T-lymphocyte and Natural killer cell. Study measurements were taken before and after SSBM on the 1st pst operative day. Data were analyzed using x2test, oneway ANOVA, paired t-test, t-test and Pearson product moment correlation. The results were summarized as follows: 1. After SSBM, VAS anxiety level, systolic and diastolic blood pressure and pulse rate of the experimental group were decreased significantly than those measurements before the SSBM. After SSBM, significant difference in the VAS anxiety level, systolic blood pressure and pulse rate between the experimental and control groups were found. 2. After SSBM, the blood cortisol of the experimental and control groups were increased significantly compared with before SSBM. But the significant difference in blood cortisol between the two groups was not found. 3. After SSBM, the blood T-lymphocyte percentages of the experimental and control groups were decease significantly and blood Natural killer cell percentages of the two groups were increased compared with before SSBM. But significant difference in blood T-lymphocyte and Natural killer cell percentages between the two groups was not found. 4. Significant difference in VAS anxiety level, systolic and diastolic blood pressure, pulse rate, blood cortisol, blood T-lymphocyte and Natural killer cell percentages between SSBM for 5 minutes and SSBM for 10 minutes were not found. Based upon the results, this study concludes that the slow stroke back massage for 5 minutes is a useful intervention that decreases anxiety and produces relaxation in patients undergoing open heart surgery.
