1.A Case of Early Age Onset Hailey-Hailey Disease Treated with Surgical Operation.

Hailey-Hailey disease is a rare hereditary dermatosis that begins in the 2nd or 3rd decade of life. The skin lesion is characterized by a localized, recurrent eruption of small vesicles on an erythematous base. It courses remissions and exacerbations. It seldom begins in early childhood, and main treatment modalities are conservative ones. We report a case of Hailey-Hailey disease that began on a 7-month old infant and improved by surgical treatment. In according to review of the previous reports, it is probably the earliest onset age and it is may be the first case which was treated with surgery in Korea.

2.Pedunculated Lipofibroma.

A 45-year-old woman had a bean-sized, asymptomatic, soft nodule on the right buttock for 4 years. Histopafhologic evaluation revealed acanthosis and a central core of adipose tissue with thin papillary and reticular dermis containing eccrine glands. Staining with alcian blue showed a slight increase of mucopolysaccharides in the reticular dermis. We made the diagnosis of pedunculated lipofibroma as a solitary form of nevus lipomatosus superficialis.

3.Traumatic Hyphema.

Among 135 ocular injuries with Hyphema that had been treated at the St. Benedict(Sung-Bun-Do) Hospital, Pusan, Korea between the period of July 1979 to June 1982, the medical records of 47 cases of pure contusion Hyphema were collected; there were 41 male, 6 femla and 10 were ambulatory, 37 were admitted, of whom, 10 underwent surgical evacuation. The out-patients were managed with uni-ocular patches or pin-hole spectacle; and all hospitalized cases were cared by complete bedrest with binocular eye-pads until blood clot absorbed, and then discharged by wearing pin-hole-spectacle too. All cases were followed-up by every other day till the day of disappearance of chamber cellularity. The clinical results of this treatment and some points of controversy were reviewed and disc ussed.

4.A Case of Multiple Trichilemmal Cysts.

A case of multiple trichilemmal cysts (TC) is presented. TC is known to be a kind of keratinous cyst with lining cells showing trichilemmal keratiniration. A 63-year-old female patient presented with a 30 year duration of increasing in size and number of twenty five nodular lesions on the scalp. All twenty five TC on the scalp were totally excised and examined microscopically. However, the evidence of proliferation or malignant change like the previous case reports was not found in our case.

6.The Suppressive Effect of Evening Primrose Oil on Murine Contact Sensitivity.

BACKGROUND: Evening primrose oil(EPO) is a rich source of cis-linoleic acid and gammalinolenic acid(GLA) and has been used as a therapeutic agent in various skin diseases such as atopic dermatitis. OBJECTIVE: The purpose of this study was to evaluate the suppressive effect of EPO on murine contact sensitivity. METHODS: BALB/c mice were divided into 3 groups, positive control, experimental and negative control groups: the positive control group represents a group of mice which were sensitized and challenged with DNFB, the experimental group represents EPO-pretreated positive control group and the negative control group represents a group of mice which were challenged only. The changes of ear thickness were measured, and H & E staining and immunohistochemical staining for ICAM-1 expression of ear skin were performed to evaluate the histological changes of each group. RESULTS: The Pretreatment of mice with EPO resulted in suppression of contact sensitivity by more than 82%. On H & E staining, only a mild inflammatory reaction was observed in the dermis. Also ICAM-1 expression of keratinocytes, the intensity of the staining was significantly decreased in the experimental group compared with positive group. CONCLUSIONS: Our study indicates that EPO was able to suppress the induction of contact sensitivity.

7.A Study on Burden and Quality of Life in Mothers of Child with Nephrotic Syndrome.

The purpose of this study was to identity the level of burden and quality of life of the subjects. The subjects of this study were 68 mothers of nephrotic syndrome patients who children hospitalized in 2 Pediatric wards of University Hospital in Seoul. The data were collected through a questionnaires and the period of the data collection was from August 1st to September 30th ,1998. The instruments for this study were Burden Measurement Instrument developed by Montgomery et. al (1985) and Quality of life scale designed by Ro, Yoo JA (1988). The statistical analysis was used by SPSS, t-test, ANOVA and Pearson correlation coefficient. The results of were as follows : 1. The level of burden showed a mean score 54.47 and the level quality of life, a mean score 140.20. 2. The level of burden differed according to s religion, pt's purpose of admission and perceived patient's condition by mothers. 3. The level of quality of life differed according to perceived pt's condition by mothers. 4. There was a negative correlation between burden and quality of life(r=-3.97, p<.001).

8.Effect of Case Management Program on Self-Efficacy, Self-Care Behaviors and Glycometabolic Control in Patient with Diabetes.

OBJECTIVES: We examined the effects of diabetes mellitus (DM) case management program developed by National Health Insurance Corporation in Korea. METHODS: One trained nurse performed the case management for 30 DM patients recruited through the reimbursement information of National Health Insurance in a rural community. The case manager had four home visits and two telephone calls for 12 weeks. We evaluated the effectiveness of the program in terms of self-efficacy, self-care behaviors and glyco-metabolic control. RESULTS: The self-efficacy score was significantly increased from 70.0 of pre-intervention to 80.5 of post-intervention (p<0.001). The self-care score was also significantly increased from 2.86 to 3.60 (p<0.001). The scores of its components such as nutritional care, self-test of sugar level, foot care, oral care, and aerobic exercise were significantly improved (p<0.001). However, the improvement of score in medication management was statistically marginal (p=0.071). The fasting blood sugar level and HbA1c level was significantly decreased from 128.6 mg/dl to 114.5 mg/dl (p<0.001) and from 6.78 % to 6.54 % (p<0.001) respectively. CONCLUSIONS: The case management program was effective to improve the self-efficacy and self-care behaviors, and to control blood sugar level in DM patients.

9.A Case of Desmoid Tumor

A case report of desmoid tumor(musculo-aponeurotic fibromatosis) arising from sternum and upper part of anterior sheath of rectus abdominis muscle in a one and half years old male child is reported. The patient's parent noticed a protruding hard mass on the lower end of sternal portion two weeks previous to surgery. The mass had dense, grayish white and glistering character invading the upper part of rectus abdominis muscle and cartilagenous portion of sternum including xyphoid process. Wide excision of mass including myo-fascial tissue of the rectus abdominal apponeurosis was performed. The excised mass was 4×4×2cm in size and the microscopic findings were typical desmoid tumor. The operative wound had partial skin necrosis due to poor circulation of skin because of the mass extended up to subcutaneous tissue and biopsy scar but eventually healed. There is no any signs of recurrence of tumor on 7 months postoperative follow up but may need more observation to define it.

10.Relationship of Body Fat Distribution and Serum Lipids in Men.

This study was conducted to investigate the relationship of body fatness indicators and serum lipids (total cholesterol, high density lipoprotein (HDL) cholesterol, low density lipoprotein(LDL) cholesterol, triglycerides, myocardial infarction(MI) index) to find anthropometric indicators of body fat distribution that are strongly associated with serum lipids. 347 adult male workers of an industry in Kyungpook province were selected and data on the general characteristics were obtained using questionnaire from June 26 to June 29, 1991. Serum lipids were tested with 5 ml fasting blood and height, weight, circumferences of waist, hip and mid-thigh, five subcutaneous fat thickeness(biceps, triceps, supscapular, suprailiac, thigh) were mea sured using Ultrasound type A TATT. Waist circumference, waist/hip circumference ratio and subcutaneous fat thickness of suprailiac area are significantly increased with increasing age, but thigh circumference is significantly decreased (P< 0.05). These results suggest that central obesity is closely related to age and waist circumference may be the best indicator of body fatness or body fat distribution in men. In younger age group(below 40 year old), the result of canonical correlations analysis suggests that circumferences(0.501) was slightly closely related to serum lipids than subcutaneous fat thickness(0.493), however, in older age group (above 40 year old), subcutaneous fat thickness(0.528) were more strongly associated with serum lipids than circumferences(0.419). Weighted canonical analyses suggest that biceps and suprailiac areas in younger age group and sub scapular area in older age group are stronger indicators of serum lipids among .subcutaneous fatness measurements. Of the circumferences, waist is the most strongest indicator of serum lipids in both age groups. Therefore, it is recommended that circumference of waist and subcutaneous fat thickness of subscapular area should be measured to estimate the degree of central obesity for preventing cardiovascular disease in men.
