2.CT Findings of Palpable Neck Masses in Children.

PURPOSE: We performed this study to assess the value of CT in the differential diagnosis of palpable neck masses in children. MATERIALS AND METHODS: We retrospectively reviewed the CT scans of the palpable neck masses in 30 children. The masses were proved histopathologically and classified into cystic, solid, and inflammatory mass and their CT findings were analyzed. RESULTS: Twelve cases were cystic masses, 4 were solid masses, and 14 were inflammatory lesions. Cystic masses included cystic lymphangiomas (n=6), branchial cleft cysts (n=3), thyroglossal duct cysts (n=2), and ranula (n=l). Cystic lymphangiomas showed insinuating appearances into adjacent structures and 4 cases occurred in the posterior cervical space. All branchial cleft cysts were round cystic masses with smooth wall and displaced the submandibular gland anteriorly and the sternocleidomastoid muscle posteriorly. Two thyroglossal duct cysts occurred centrally adjacent to the hyoid bone and 1 ranula in the submental area. Solid masses were juvenile hemangioma, pleomorphic adenoma in submandibular gland, neurilemmoma, and fibromatosis colli. Juvenile hemangioma showed well-enhancing mass with indistinct margin and the other solid masses had well-defined margin with their characteristic location. Inflammatory lesions were abscess (n=4), deep neck infections with lymphadenopathy (n=4), submandibular gland inflammation (n=3), and tuberculous lymphadenitis (n=3) and they showed strand-like enhancement in adjacent subcutaneous tissues. Tuberculous lymphadenitis had multiple lymph node enlargement with internal low attenuation areas and showed less surrounding strand-like enhancement than suppurative lymphadenopathies. CONCLUSION: Most neck masses in infants and children were of congenital or inflammatory origin. CT is useful for the evaluation of the child presenting with a neck mass, because it can differentiate various forms of neck masses and is able to reveal the relationship of the masses to the adjacent structures with their characteristic location.

3.A Case of Tuberculosis-related Retinal Vasculitis.

PURPOSE: Tuberculosis is a chronic infection caused by mycobacteria species. M. tuberculosis causes disease predominantly in the lung, although every organ in the body may be involved. Retinal tuberculosis is very rare and usually unilateral. Possible hypotheses on the pathogenesis of tuberculosis-related retinal vasculitis are an infectious process and a hypersensitivity mechanism. In Korea, no case has been reported. METHOD: We experienced a case of bilateral retinal vasculitis who was suffering from tuberculous meningitis. RESULT: His vasculitis was successfully treated with anti-tuberculosis medications. We report a case of tuberculosis-related retinal vasculitis with the review of literature.

4.Prognostic Factors in Vitrectomy for Proliferative Diabetic Retinopathy.

Medical record of 66 eyes which had undergone pars plana vitrectomy for the treatment of proliferative diabetic retinopathy were reviewed to study their clinical features, incidences and types of complications, visual outcome, and visual prognostic factors. The final visual outcome showed the improvement in 50[76%] eyes, no change in 13[20%]eyes, and the worsening in 3[4%] eyes. Preoperative prognostic factors favoring final visual acuity included the followings:1]insulin dependent diabetes mellitus which developed before 30 years of age, 2]absence of iris neovascularization, 3]absence of tractional macular detachment. Postoperative complications included vitreous hemorrhage in 34[52%]eyes, cataract in 12[19%]eyes, increased intraocular pressure in 6[10%]eyes, rhegmatogenous retinal detachment in 4[6%]eyes, choroidal effusion in 3[5%]eyes, corneal epithelial defect in 2[3%]eyes, neovascular glaucoma in 1[2%] eye. Postoperative complications associated with poor visual outcome were rhegmatogenous retinal detachment and neovascular glaucoma.Intraoperative complication of iatrogenic retinal tear did not seem to deteriorate the final visual acuity.

6.Nasal Bone Fractures : Evaluation with Thin-section CP.

PURPOSE: To determine the value of thin-section CT in the diagnosis of nasal bone fractures. MATERIALS AND METHODS: We evaluated the thin-section CT scans of 40 patients with nasal bone fracture. CT scans were obtained with both axial and coronal planes, 1.5mm collimation with 2mm interval, and 9.6cm field-of-view. The axial scan plane was kept parallel to the orbitomeatal line from the nasion to the lower limit of the nose and the coronal plane was kept perpendicular to the axial plane. The data were reconstructed with bone algorithm. Nasal bone fracture was classified into 1 of 3 types on thin section CT:(I) simple fracture;(ll) simple fracture with displacement;(III) comminuted fracture. Associated facial bone injuries were also evaluated Simple radiographs of nasal bone were reviewed for comparison. RESULTS: Six patients had simple fracture, 10 patients had simple fracture with displacement, and 24 patients had comminuted fracture. Twenty-six patients had associated facial bone injuries which included fracture of nasal septum (n=15), fracture of frontal process of maxilla (n=9), fracture of ethmoid (n=6), widening of nasofrontal suture (n=5), and fracture of nasolacrimal duct (n=2). In 15 of 40 patients, CT could identify nasal bone fractures not detected on simple radiographs. CONCLUSION: Thin-section CT is a valuable aid in the evaluation of nasal bone fracture for accurate identification, nature, and combined facial injury.

7.Effects of Obesity on Pulmonary Functions According to Fat Distribution.

PURPOSE: Obesity is one of the most frequent markers of health risk and the incidence of obesity has increased in Korea recently. Obesity leads to higher risks of hypertension, hyperlipidemia and insulin resistance. It also leads to risks of respiratory complications. This study was done to view the different effects of obesity on pulmonary functions according to body fat distribution before and after exercise. METHODS: 257 obese children enrolled in this study. All subjects were divided 2 groups, upper body obesity and lower body obesity group according to waist to hip ratio, trunk obesity and extremity obesity group according to centrality index. Pre-and post-exercise loading FEV1, FVC and PEFR were checked in all subjects and the changes of values in each groups were observed. RESULTS: 1) There were 68, 189, 136 and 121 children in upper body obesity group, lower body obesity group, trunk obesity group and extremity group respectively. 2) The % predicted value of FEV1, FVC and PEFR in upper body obesity group were lower than those in lower body obesity group before exercise loading. And the reduction of % predicted values of each parameters was more prominent in upper body obesity group than in lower body obesity group after exercise loading. 3) There was no difference of pulmonary function between trunk and extremity obesity group before exercise, but the reduction of % predicted values of each parameters was more prominent in trunk obesity group than in extremity obesity group after exercise loading. CONCLUSION: The exercise induced bronchospasm is more prominent in upper body and trunk obesity group than in lower body and extremity obesity group. Therefore the fat distribution of obese children must be considered in diagnosis and control of obesity.

8.The Current Status and Requirements for Non-pharmacological Treatment of Depression in Korea.

OBJECTIVES: As a part of plan to develop evidence-based treatment guidelines for depression that is more suitable for Korean situation, we investigate the present status and client's requirements for non-pharmacological treatment of depression in Korean clinical situation. METHODS: Subjects were patients with depression in 12 university hospitals which are located in metropolises in Korea. We analyzed the records from questionnaires about current clinical status and requirements for the non-pharmacological treatment of depression in Korea. RESULT: 50.8% of the subjects have experienced non-pharmacological treatments for their depression. The preference of non-pharmacological treatment method of depression is exercise/interesting activity, counseling by psychiatrists and psychotherapy, and the best effective treatment method is psychotherapy (Es=4.36). Actually, the mean consultation time by psychiatrist is 11.31+/-7.16 min, and the appropriate consultation time for client's situation is 18.39+/-8.95 min. During consultation, patients' satisfaction measurement for psychiatrist's explanation about pharmacological treatment is 64.17+/-27.11, and satisfaction measurement for psychiatrist's counseling for their depression about personal problems, resent stress, interpersonal relationship is 61.66+/-26.63. CONCLUSION: In Korea, many psychiatrists offered biologically oriented treatment to their patients with depression, and patients' satisfaction measurement about consultation by psychiatrists is low. Many patients wanted to combined pharmacological and non-pharmacological treatment for their depression, and aspired to information about complementary and self-help treatment methods. It is necessary to develop non-pharmacological treatment guideline for depression which reflect the clinical situation in Korea and meet Korean patients' need.

9.The Effects of Moderate Treadmill Running on Free Fatty Acids, Glucose, Growth Hormone, Insulin and Appetite.

BACKGROUND: The level of physical adaptation through active lifestyle can result in changes in appetite; and caused by physical activity and exercise, physical adaptation may change carbohydrate and fat metabolism during exercise. This study investigated the differences in carbohydrate and fat metabolism after moderate treadmill running and the difference in appetite response before and after exercise in active, regularly exercising individuals and those inactive. METHODS: We analyzed the effects of moderate physical activity (70% of oxygen uptake reserve, 30 minutes on the treadmill) on free fatty acids, glucose, growth hormone, insulin and an appetite visual analogue scale (VAS). Our subjects included 28 healthy males who were divided into two groups, the activity group (n=14) and the inactivity group (n=14) according to the International Physical Activity Questionnaire (IPAQ). The serum samples and VAS were collected at pre, post and 30 min post-exercise. RESULTS: The results of the serum analysis showed no significant effects on free fatty acids, glucose, growth hormone and insulin between the groups, but showed significant changes in free fatty acids and growth hormone between points of measurement. Significant increases in VAS were seen with moderate exercise in both groups, with the inactivity group expressing greater hunger than the activity group. CONCLUSIONS: This study found that inactive persons were hungrier than active persons after moderate exercise. This information might be useful to staff and participants of weight loss programs.

10.Radiological Findings of Angiosarcoma in Breast: A Case Report .

Angiosarcoma is a rare primary malignancy occurring anywhere in the body, but most often in the skin, liver or breast. The radiological findings of angiosarcoma of the breast have rarely been reported. We encountered a case in which the condition involved a 32-year-old woman in whom mammography revealed a obscuved-marginated and lobular-shaped high-density lesion, and ultrasonography demonstrated an ovalshaped, microlobular-marginated, inhomogenous hypoechoic mass which metastasized to the whole body after surgery. We report the radiological findings of this case of angiosarcoma of the breast, and review the literature.
