6.Histopathologic changes of the craniomandibular joint according to the amount of distraction after 6 weeks of distraction osteogenesis in rabbits.

The purpose of this study is to observe histopathologic changes in the bilateral craniomandibular joints after allowing 6 weeks of consolidation by varying the amount of distraction in rabbit mandible. Eight rabbits weighing about 2 to 3kg were used. After corticotomy was performed on the left mandibular body between the first premolar and the second premolar region, a unilateral fixation device was placed. Then, a 7-day period was allowed without distraction of the device. The mandible was lengthened 0.5mm/day. Corticotomy and lengthening of mandible were not performed in control group. After the completion of the lengthening process, a 6-week-consolidation period was allowed. Then, the rabbits were sacrificed, and histologic examination of the craniomandibular joints was performed. Proliferative changes were observed in the craniomandibular joints in all groups. With the increasing amount of distraction, hypertrophy of the cartilage layer became more severe, bone formed was dense and enchondral ossification was clearly shown in subchondral bone. Hypertrophy of the cartilage layer was also seen in the non-distracted side as the distracted side in the experimental group. These results indicate that when physical force is applied constantly to joints, the proliferation of articular cartilage and bone formation are present. When more than 6 weeks of consolidation period is allowed at the time of performing distraction for more than 5mm, articular changes, especially, in the contralateral side should also be noted.

7.Diagnostic significance of computed tomography in gastric cancer

Gastric cancer is the most common gastrointestinal malignancy in Korea. Identification and evaluation ofgastric mass lesions and regional-distant metastases by abdominal CT scan are important for the treatment planningand prognostic implications of gastric cancer patients. Author reviewed CT scan are important for the treatemntplaning and prognostic implications of gastric cancer patients. Author reviewed CT scan of 61 cases of pathologyproven gastric cancer, retrospectively, for recent 20 months from July 1983 to Feb. 1985 at department ofradiology, Korea University, Hae Wha Hospital. The results were as follows: 1. There were 50 cases of advanced adenocarcinoma, 8 cases of early gastric cancer, 2 cases of leiomyosarcoma, and 1 case of lymphoma in total 61cases. 2. The sex ratio of male to female was 2:1. Age distribution was from 24 to 75 year old and peak incidencewas in 6th decade. 3. The most frequent site of involvement with gastric cancer was gastric antrum in 51%. 4. 48of the 50 patients with advanced gastric adenocarcinoma(96%) had a wall thickness greater than 1 cm, and all of 8cases of early gastric cancer had a wall thickness less than 1 cm. Regional lymph node tumor infiltration wasfound in 100% of gastric wall thickness greater than 2.0cm, in 64% of cases of 1.5 to 2.0cm, in 50% of cases of1.9 to 1.5cm, and 12.5% of cases of less than 1.0cm. 5. In a comparison of enlargement of reginal lymph node by CTscan to tumor infiltration of regional lymph node by histology, senitivity was 52%, specificity was 87%, and reliability was 66%. 6. The structures involved by distant metastases of these cases were the retroperitoneallymph node in 15, liver in 8, and pancrease in 3. 7. The diagnostic accuracy of CT staging was considered about68% by correlation of the surgical and histological findings. 8. The CT scan is one of the accurate and simpletool for evaluation of size, shape, extent, as well as distant metastases in the cases of gastric malignancies.

10.Industrial Ocular Injary in ST. Mary's Industrial Accident Hospital.

The authors analysed 215 ocular injured patients sent to ST. Mary's Industrial Accident Hospital for the last 5 years form January 1982 to December 1986. 1. Majority of patients(72.1%) were young adult and almost all were male(96.7%). 2. The occupational accident rate showed the highest among blacksmiths and tool makers(33.5%), followed by woodworkers(13.6%), and electrical set-up workers (9.4%). 3. The occurrence rate of ocular injury was the most frequent by the flying objects(75.8%), followed by struck-by(11.2%), and stabbed (7.8%). The source of the injuries were mostly metallic materials(68.1%), followed by non-metallic ma terials(11.9%), and pressure boilers(5.0%). 4. The incidence rate was about the same in non-penetrating injuries as penetrating injuries. But the visual prognosis was worse in penetrating cases than nonpenetrating cases(P<0.05). 5. The most frequent complication was traumatic cataract(32.0%), followed by corneal of scleral laceration(16.0%) and intraocular foreign body(11.9%). 6. In visual prognosis, the final visual acuity of 0.15 or higher was achieved in only 25.3% among those with initial visual acuity of lower than 0.02 but 66.2% among those with initial visual acuity of 0.02 or higher.
