M-mode echocardiography has been demonstrated to be a sensitive indicator of the sequential changes occurring in the heart in systemic arterial hypertension. The changes in function as well as anatomic features have been recorded. Previous analysis has focused on the anatomy and systolic function of the left ventricle, analysed in hypertensive heart disease. 13 normal subjects(Group I), 14 hypertensive patients without evidence of cardiac involvement(Group II) and 13 hypertensive patients with echocardiographic evidence of left ventricular hypertrophy(Group III) were compared using M-mode echocardiographic data and clinical. findings. The results are as follows : 1) Mean age are; Group I : 41 years(19-62), Group II : 49 years(25-67) and Group III : 53 years(30-83). There are no significant age difference between Group I and II. Sex ratio are 4:9, 8:6 and 7:6 respectively. 2) Mean arterial pressure are; Group I : 98+/-5mmHg, Group II : 126+/-7 mmHg and Group III : 131+/-101mmHg. 3) Left ventricular posterior wall thickness are; Group I : 0.87+/-0.12cm, Group II : 0.89+/-0.12cm and Group III : 1.32+/-0.18cm(p>0.05 in I vs II). 4) Left atrial dimension are; Group I : 2.77+/-0.68cm, Group II : 2.96+/-0.44cm and Group III: 3.12+/-0.60cm. 5) Left ventricular mass arel Group I : 194+/-51gm, Group II : 192+/-51gm and Group III : 318+/-77gm(p>0.05 in I vs II, p<0.001 in I vs II and II vs III). 6) Ejection fraction are; Group I : 0.57+/-0.09, Group II : 0.60+/-0.01 and Group III : 0.52+/-0.01)p>0.05). 7) Mitral valve EF solpe are; Group I : 113.70+/-23.26mm/sec, Group II : 81.14+/-17.22mm/sec and Group III : 69.92+/-22.70mm/sec(p<0.05 in I vs II, p<0.001 in I vs III and II vs III). 8) AEI are; Group I : 0.70+/-0.04, Group II : 0.56+/-0.07 and Group III: 0.48+/-0.07)p<0.001 in I vs II, I vs III and II vs III). From the above results, the atrial emptying index seems to be an early indicator of abnormalities of left ventricular diastolic compliance in hypertensive patients.

PURPOSE:This study was performed to elucidate that status epilepticus (SE) induces long- term neuronal damages in an immature brain and to evaluate that topiramate (TPM) has a protective effect. METHODS:We investigated the changes in a subtype expression of glutamate and gamma- amino butyric acid (GABA) receptors, and the structural integrity due to cell losses in the mouse pup hippocampus after SE using an immunoblot and confocal microscopy. RESULTS:SE induced significant cell losses with structural changes in the hippocampus 1 month later. SE up-regulated the glutamate receptor1 (GluR1) expression with an increased ratio of GluR1 to glutamate recptor2 (GluR2), leading to the formation of Ca2+ permeable alpha- amino-3-hydroxy-5-methyl-4-isoxazoleepropionic acid (AMPA) receptors for the enhanced neurotoxicity. TPM prevented the SE-induced GluR1 expression. The expression of GABAA receptors was highly increased 1 month after SE, whereas that of GABAB receptors was not changed. The TPM treatment attenuated SE-induced upregulation of GABAA receptors. SE induced significant cell losses and disruption of structural integrity in the hippocampus CA1 and CA3 regions, but the TPM treatment for 1 month in developing brains reduced the SE- induced hippocampal damage. CONCLUSION:TPM has a neuroprotective action, which might be mediated by the modulation of GluR1 and GABAA receptors.

Background: Hydroxyethyl starch (HES, 6% 130/0.4) has been used as a volume expander for the treatment of cerebral hypoperfusion in acute ischemic stroke. Although HES use was associated with renal failure in sepsis or critical illness, it still remains to be elucidated whether HES is linked to renal adverse events in patients with acute ischemic stroke. Methods: A total of 524 patients with acute ischemic stroke within 7 days of onset were included between January 2012 and May 2016. Renal function on admission and follow-up on day 7 ± 2 was assessed using serum creatinine (SCr) and estimated glomerular filtration rate (eGFR). Propensity score matching (PSM) was used to perform a 1:1 matched-pair analysis to minimize the group differences caused by covariates. The percentage of patients with newonset acute renal injury (AKI) using the Kidney Disease: Improving Global Outcomes or good functional outcome (modified Rankin Scale 0–2) at 90 days were compared between HES cohort and controls. Results: Among the included patients (mean age, 68.6 years; male, 56.5%), 81 patients (15.5%) were HES cohort (median cumulative dose, 1,450 mL). Baseline renal function was better in HES cohort compared to that in the controls (SCr, 0.87 ± 0.43 mg/dL vs. 1.15 ± 1.15 mg/dL, P < 0.001; eGFR, 86.91 ± 24.27 mL/min vs. 74.55 ± 29.58 mL/min, P < 0.001), which became not significant in PSM cohort (72 pairs). The percentage of new-onset AKI did not differ between the HES cohort and controls (1.4% vs. 1.4%, P = 1.000). In addition, newonset AKI was not related to HES (odds ratio, 1.422; 95% confidence interval, 0.072–28.068; P = 0.817) after adjusting for confounders. HES cohort tended to have higher percentage of good functional outcome at 90 days compared to controls, which failed to reach statistical significance (68.1% vs. 54.2%, P = 0.087). Conclusion A low cumulative dose of HES was not associated with renal adverse events in patients with acute ischemic stroke.

PURPOSE: The purpose of this study was to compare the geometry and position of the femoral tunnel between the anteromedial portal (AMP) and outside-in (OI) techniques after anatomic single-bundle anterior cruciate ligament (ACL) reconstruction. MATERIALS AND METHODS: We evaluated 82 patients undergoing single-bundle ACL reconstruction with hamstring autografts using either the AMP (n=40) or OI (n=42) technique. The locations of the tunnel apertures were assessed by postoperative 3-dimensional computed tomography imaging. The femoral graft bending angle, femoral tunnel aperture shape, femoral tunnel length, and posterior wall breakage were also measured. RESULTS: The mean femoral tunnel position parallel to the Blumensaat line was more caudally positioned in the AMP group than in the OI group (p=0.025) The mean femoral graft angle in the OI group (99.6°±7.1°) was significantly more acute than that of the AMP group (108.9°±10.2°, p<0.001). The mean height/width ratio of the AMP group (1.21±0.20) was significantly more ellipsoidal than that of the OI group (1.07±0.09, p<0.001). CONCLUSIONS: The mean femoral tunnel position was significantly shallower in the AMP technique than in the OI technique. The OI technique might be more disadvantageous than the AMP technique in terms of the more acute bending angle.

PURPOSE: Tension-free hernioplasty has become the most popular procedure for the repair of groin hernias in the United States and United Kingdom. The purpose of this study is to describe a 7-year personal experience with Lichtenstein's tension-free groin hernia repair under local anesthesia. METHODS: We retrospectively studied the clinical outcome of 321 cases of Lichtenstein repairs, performed consecutively by an experienced surgeon between Jan. 1994 and Dec. 2000. RESULTS: Of the 321 cases, 242 (75.4%) were indirect, 34 (10.6%) were direct, 8 (2.5%) were femoral, 7 (2.2%) were pantaloon, and 30 (9.3%) were recurred hernias. The mean age was 55 years; 91% were male. The mean number of injections of analgesics required in the postoperative period was 3.2. The mean hospital stay following repair was 2.7 days. Complications occurred in 23 cases (7.1%). Most of these were minor, consisting of five cases of bruising or hematomas (1.6%), four superficial infections (1.3%), three seromas (0.9%), two hydroceles (0.6%), six patients with persisting groin pain for more than a month (1.8%), one foreign body granuloma, one urinary retention, and one testicular atrophy. There were no recurrences or operative deaths. CONCLUSION: Lichtenstein's tension-free hernioplasty is an easy and simple technique with less pain, minor complications and only rare instances of recurrence. This procedure can be performed on a same-day basis under local anesthesia. Lichtenstein repair may be the most promising technique for the repair of groin hernias.

PURPOSE: Recent years have witness a dramatic increase in public awareness of the health risks associated with lead exposure-particularly concerning young children. The lead poisoning in children was considered a rarity, usually resulting from unique circumstances such as in appropriate use of lead-based body cosmetics or direct administration of lead-containing folk medicine or contaminated atmosphere. The increasing concern about children with lower levels of lead exposure has developed, but there are no data regarding the mean blood lead levels and the incidence of lead poisoning in Korean children. So, this study was conducted to measure the blood lead and ZPP(zinc protoporphyrin) levels of 125 children around Cheonan city. METHODS: 125 children around Cheonan city (from 1 month to 15 year-old, 68 males and 57 females) were studied. We analyzed the mean blood Zinc Protoporphyrin(ZPP) and lead values and excluded children with hematologic disorders in this study. RESULTS: 1) The mean blood lead level was 4.15 microgram/dl (95% CI: 3.82-4.488) and the mean ZPP level was 53.48 microgram/dl (95% CI: 48.44-58.52). Both were within normal range. 2) The mean blood lead level of 68 males was 4.23 microgram/dl and 4.06 microgram/dl in 57 females. The mean blood ZPP level was 49.88 microgram/dl in males and 57.77 microgram/dl in females. There was no significant difference in males and females. 3) There was no siginificant correlation between blood lead and ZPP concentration. CONCLUSIONS: We concluded that the mean blood lead level of 125 children around Cheonan city was 4.15 microgram/dl. That was within normal range and the same values with United States. It is important that there must be a national counter plan and that pediatricians continue to pay attention to lead poisoning in children.

PURPOSE: To describe the normal sonographic anatomy of the submandibular duct and to assess the value of sonography in the diagnosis of sialolithiasis of the submandibular gland. MATERIALS AND METHODS: Using continuous injection of saline via a sialography needle, we evaluated the normal sonographic anatomy of the submandibular duct in healthy volunteers (n=5). We also reviewed sonographic findings in 47 patients with (n=29) or without (n=18) stones. In each case, the presence or absence of stone was proved by sialography or surgery. RESULTS: In control subjects, sonography accurately depicted the ductal anatomy of the submandibular glands and its relationship with adjacent tissues. For the detection of stones, sonography had a sensitivity of 86%, a specificity of 100%, and an accuracy of 91%. For ductal dilatation, its sensitivity was 91%, its specificity, 100%, and its accuracy, 94%. In 68% of cases with stones, a circumferential low echoic rim surrounding the stone ("rim sign") was present on sonogram. CONCLUSION: Ultrasound is a useful procedure and could replace sialography in most patients with sialolithiasis of the submandibular gland.

Objective: The aim of the present study was to evaluate cortical hypometabolism of the F-18- fluorodeoxyglucose positron emission tomography (FDG-PET) based on a diagnostic cutoff point of the mini-mental state examination (MMSE) in de novo PD.Methods: We recruited 24 PD patients and 15 healthy controls to analyze FDG-PET. We divided the patients into two groups by the diagnostic cutoff point of MMSE for diagnosing dementia, with scores of>25 vs. < 25. FDG-PET was processed using statistical parametric mapping (SPM) 8 running on Matlab 11. Results: Patients with a MMSE < 25 presented lower score in time orientation, serial sevens, language and pentagon copying of MMSE compared to patients with a MMSE >25. Compared to healthy controls, patients with a MMSE> 25 and < 25 showed a fronto-temporo-parietal hypometabolism, which was more extended in patients with a MMSE < 25. Difference in cortical hypometabolism between patients with a MMSE >25 and < 25 was found in the right inferior parietal lobule. Conclusions: In the comparison by cutoff point of MMSE (25/24), hypometabolism in the right inferior parietal lobule suggests that the posterior cortical deficit is the main region ofde novo PD with cognitive impairment. Hypometabolism of right inferior parietal lobule is related to the damage of cerebral networkin de novo PD.

PURPOSE: The Rho family of GTPases are involved in actin cytoskeleton organization and are associated with carcinogenesis and progression of human cancers. The clinicopathological significance of RhoA is not yet well known in the case of colorectal cancer. To investigate the expression of RhoA protein in colorectal carcinoma and to evaluate the relationship between RhoA protein expression and invasion and metastasis of colorectal cancer, we examined the expression of RhoA protein by using Western blotting and immunohistochemistry. METHODS: The protein levels of RhoA in colorectal carcinomas of surgical specimens were analyzed in 71 consecutive patients with colorectal cancers by using immunohistochemistry and Western blotting. The relationships between the protein levels of RhoA in tumor tissues and the clinicopathological features of the patients were also assessed. RESULTS: RhoA was highly expressed in 48 colorectal carcinomas (67.6%). There was a significant association between RhoA expression and lymph nodal status. The expression of RhoA protein was related to lymph-node metastasis (P=0.032) and advanced TNM tumor staging (P=0.020). RhoA expression had a significant prognostic value for overall survival. Kaplan-Meier plots of survival in patients with high RhoA showed that high RhoA expression was associated with a shorter overall survival. However, no association was found between RhoA and other pathologic or clinical variables, including age, gender, degree of differentiation, and presence of perineural spread. CONCLUSIONS: The RhoA protein may be related to malignant transformation and development of colorectal caricinomas and may play an important role in the invasion and the metastasis of colorectal carcinomas.
