1.Effects of Obesity on Pulmonary Functions According to Fat Distribution.

PURPOSE: Obesity is one of the most frequent markers of health risk and the incidence of obesity has increased in Korea recently. Obesity leads to higher risks of hypertension, hyperlipidemia and insulin resistance. It also leads to risks of respiratory complications. This study was done to view the different effects of obesity on pulmonary functions according to body fat distribution before and after exercise. METHODS: 257 obese children enrolled in this study. All subjects were divided 2 groups, upper body obesity and lower body obesity group according to waist to hip ratio, trunk obesity and extremity obesity group according to centrality index. Pre-and post-exercise loading FEV1, FVC and PEFR were checked in all subjects and the changes of values in each groups were observed. RESULTS: 1) There were 68, 189, 136 and 121 children in upper body obesity group, lower body obesity group, trunk obesity group and extremity group respectively. 2) The % predicted value of FEV1, FVC and PEFR in upper body obesity group were lower than those in lower body obesity group before exercise loading. And the reduction of % predicted values of each parameters was more prominent in upper body obesity group than in lower body obesity group after exercise loading. 3) There was no difference of pulmonary function between trunk and extremity obesity group before exercise, but the reduction of % predicted values of each parameters was more prominent in trunk obesity group than in extremity obesity group after exercise loading. CONCLUSION: The exercise induced bronchospasm is more prominent in upper body and trunk obesity group than in lower body and extremity obesity group. Therefore the fat distribution of obese children must be considered in diagnosis and control of obesity.

2.The influence of Self-efficacy on Activities of Daily Living in Patients with Rheumatoid Arthritis.

The aim of this study was to investigate the differences in the level of disease condition and ADL by self-efficacy in patients with rheumatoid arthritis. The subjects of the study consisted of eighty-four outpatients with rheumatoid arthritis at a university hospital in Taegu between July 20, 1999 and August 25, 1999. The instruments used in this study were the self-efficacy developed by Lorig et al. (1989), the pain scale developed by Lee and Song(1987), the erythrocyte sedimentation rate(ESR), the pain joint count to measure disease condition scale, and the ADL scale developed by Katz et al. (1970) and Barthel(1973). Analysis of data was done by use of descriptive statistics: Pearson Correlation, Chi-square test, t-test, ANOVA and Scheffe, with the SPSS program. The major findings can be summarized as follows: 1. The first hypothesis, "There will be differences in the level of disease condition by self-efficacy in patients with rheumatoid arthritis" was partially supported. 1-1. "There will be differences in the level of pain by self-efficacy in patients with rheumatoid arthritis" was supported(F=3.422, p=.037). 1-2. "There will be differences in the level of ESR by self-efficacy in patients with rheumatoid arthritis" was the Disease Condition and supported (F=3.314, p=.041). 1-3. "There will be differences in the level of pain joints count by self-efficacy in patients with rheumatoid arthritis" was rejected(F=2.602, p=.080). 2. The second hypothesis, "There will be differences in the level of ADL by self-efficacy in patients with rheumatoid arthritis" was supported(F=7.067, p=.001). With the above results, it can be concluded that the higher level of self-efficacy contributed to the better level of disease condition and ADL in patients with rheumatoid arthritis. Therefore, by providing nursing intervention to promote the level of self-efficacy of rheumatoid arthritis patients with low self-efficacy, disease condition and ADL would be much better.

3.A Case of Encephalopathy Presented with Motor Aphasia and Quadriplegia Following Intrathecal Methotrexate .

Intrathecal administration of methotrexate(IT-MTX) has constituted the standard approach to prophylaxis and treatment of central nevous system(CNS) leukemia. We experienced a quadriplegia and motor aphasia in a 14-year-old boy following repeated IT-MTX for the prophylaxis of meningeal leukemia. He was diagnosed as ALL without CNS involvement and treated by CCG- 1882 protocol. IT-MTX was administered for CNS prophylaxis. The patient began complaining of urinary incontinence, motor aphasia and weakness in his right leg from 12 days after the 5th dose of the IT-MTX therapy. Even though the IT-MTX was discontinued, loss of muscle power progressed upward resulting in quadriplegia. The patient showed slow and partial recovery on right extremities over 3 months. We report this case with brief review of literature.

7.A Comparative Study on Spiritual Well-Being and Perceived Health Status between Above Average and Low Income for Elderly People.

PURPOSE: This study was to provide basic data for comprehensive nursing care for elderly people and to compare the spiritual well-being and perceived health status between elderly people who have above average income and those who have low income. METHODS: The data were collected from 80 elders with above average income and 81 with low income through face-to-face interviews. An elder was defined as a person over 60 years of age. The data were analyzed using t-test, ANOVA, ANCOVA, Pearson correlation coefficients and Scheffe test. RESULTS: The mean score for spiritual well being for elders with above average income was 2.90 of a total possible score of 4 and for elders with low income, 2.49 and the difference was significant. The mean score for perceived health status for elders with above average income was 8.93 of a total possible score of 14 and for elders with low income, 7.47 and the difference was also significant. There was a statistically positive correlation between existential well-being and perceived health status for the total sample of elderly people and for the elders with low income. CONCLUSION: Spiritual nursing care should be included in comprehensive health care programs for elderly people in Korea. Especially, it is important to develop nursing interventions for elders with low income that will increase their spiritual well-being and help them to develop positive thinking towards perceived health status.

8.The Hope, Burden, and Family Function in Mothers of Children with Cancer.

The purpose of the descriptive correlation study was to examine relations among the hope, the burden and the family function in mothers caring for children with cancer. 145 mothers completed the three questionaires of the study divided into tree sections: a) The Hope Scale, b) The Burden Scale, c) FACES-lll. The collected data was analysed with t-test, ANOVA, Scheffe test and Pearson correlation coefficients. The results were as follows: 1) The average age of mothers of children with cancer was 35.6 years old and the ages between 30 to 39 were the most abundant. 57.3% of the mothers had an education level of below high school education and 66.0% had religion. The average age of the children was 8.6 years old. Ages between 1 to 7 were the most and 60.8% were diagnosed as leukemia. 2) Regarding the section of degree of burden, mothers of children with cancer marked 2.6 out of 5, and the degree of hope 3.2 out of 4. The result for family function came out to be 3.5 out of 5, an average of family cohesion of 3.9 and family adaptation of 3.1. 3) There were significantly less burden to the mothers who were living together with a spouse compared to the mothers who were not. Also mothers who replied that they preserved good health came out to be exposed to less burden compared to the mothers who did not. In analysing hope according to the general characteristics of mothers of children with cancer, mothers who were employed marked high in the degree of hope compared to unemployed mothers. Furthermore, the degree of family cohesion marked higher with mothers who had higher education of college graduate, mothers with religion and mothers with a monthly family income of over W3,000,000, compared to the group of mothers with lower education of high school graduate, non-religious or with a monthly family income of less than W1,000,000. 4) Excluding the fact that the group sorted with children diagnosed as leukemia marked a perceivably high score regarding family cohesion, compared to groups with other cancers, the degree of burden, hope and family cohesion did not show any noticeable difference according to characteristics of children with different cancers. 5) In the correlation of the hope, the burden and the family function regarding the mothers of children with cancer, the burden did not have any manifest relationship with hope or family function. However, the degree of hope and family function cohesion had a direct proportional relationship, as family cohesion marked higher when the degree of hope were high.
