4.Effects of Relaxing Music on Stress Response of Patients with Acute Myocardial Infarction.

PURPOSE: The purpose of this study was to determine whether a relaxing music was effective in reducing stress response of patients admitted to a coronary care unit with the diagnosis of acute myocardial infarction. METHOD: The research design was the chosen convenience sample of a randomized control group pre-posttest quasi- experimental design. Forty patients, 20 for an experimental group and another 20 for control group. The study was to provide the patients three different kinds of relaxing music. The experimental group listened to relaxing music for a 20-minute one time a day for 3days. The control group was just provided with a 20-minute period of rest. Test for hypothesis was done by repeated measured ANOVA. RESULT: The experimental group which received relaxing music showed a significantly lower level of electrodermal response, myocardial oxygen consumption and respiration rate, and a higher level of peripheral skin temperature than the control group. The experimental group which received relaxing music showed a significant reduction in state anxiety than the control group. CONCLUSION: The results showed that relaxing music is an effective nursing intervention for reducing physiologic and psychologic stress response of the patients with acute myocardial infarction in a critical intensive care unit.

7.A Case of Encephalopathy Presented with Motor Aphasia and Quadriplegia Following Intrathecal Methotrexate .

Intrathecal administration of methotrexate(IT-MTX) has constituted the standard approach to prophylaxis and treatment of central nevous system(CNS) leukemia. We experienced a quadriplegia and motor aphasia in a 14-year-old boy following repeated IT-MTX for the prophylaxis of meningeal leukemia. He was diagnosed as ALL without CNS involvement and treated by CCG- 1882 protocol. IT-MTX was administered for CNS prophylaxis. The patient began complaining of urinary incontinence, motor aphasia and weakness in his right leg from 12 days after the 5th dose of the IT-MTX therapy. Even though the IT-MTX was discontinued, loss of muscle power progressed upward resulting in quadriplegia. The patient showed slow and partial recovery on right extremities over 3 months. We report this case with brief review of literature.

8.Immunocytochemical Study on the Monoaminergic Innervation of Diencephalic Visual Structures in the Squirrel Monkey.

This study was for investigating relations between distributions of monoamines-norepinephrine, serotonin, and dopamine-on the visual system and their functions. Distributions of these monoamines in the lateral geniculate body, pulvinar, lateral posterior nucleus, and suprachiasmatic nucleus were investigated. Brain of a squirrel monkey was removed and frozen sectioned. Immunocytochemical study was performed for the tissue of the brain. Results showed that the anterior part of the lateral geniculate body contained more monoamines than the posterior part. More serotonins were distrbuted at the magnocellular part, and more dopamines were found at the parvocellular part. In pulvinar, more norepinephrines were distributed at the medial part, while serotonins were evenly distributed at all parts. In lateral posterior nucleus and suprachiasmatic nucleus, three kinds of monoamines were distributed with high density. Among the three, density of the serotonin showed the highest value. The lateral geniculate body relates with visual perception such as visual acuity, form and color perception, and stereopsis, while the pulvinar relates with visual functions, such as visual attention, sensory integration, and differentiation. Since norepinephrine and serotonine are distributed with high density in the pulvinar than in the lateral geniculate body those two monoamines are expected to playa major role for visual functions. Inferior part of the pulvinar relates with visual imagination, and the lateral posterior nucleus relates with integration of visual sensory. Relatively high distribution of dopamine in these two parts means that dopamine may playa major role for visual imagination and integration. As suprachiasmatic nucleus relates with controlling biorhythm, dense distribution of monoamines in suprachiasmatic nucleus implies that the monoamines may work for controlling biorhythm.

9.The Comparison of Sleep Characteristics According to the Sleep Positions in Healthy Newborns.

The purpose of this study was to compare the sleep characteristics between the prone and the supine position in healthy newborns. The 48 newborns were observed in the prone position and the supine position respectively on the 2nd day after birth. The data were collected from January to May, 1999. The state of a newborn was classified and categorized to 6 states (deep sleep, light sleep, drowsy, quiet alert, active alert, crying) by Barnard. The movements of eyes, face and extremities, pulse and arterial oxygen were observed and recorded continuously from the start of sleep after feeding until the time of being woken for the next feeding by a trained nurse. The data was analyzed by using paired t-test. The results of this study were as follows; 1. There was no significant difference in the length of sleeping time between the prone and the supine position. 2. There was no significant difference in the length and frequencies of each states(deep sleep, light sleep, drowsy, quiet alert, active alert, crying) between the prone and the supine position. But the frequency of light sleep in the supine position was significantly higher than that of the prone position. 3. There was no significant difference in the numbers of eyes movements between the prone and the supine sleep position. But the amount of facial and extremity movement in the supine position was significantly higher than those in the prone position 4. There was no significant difference in the arterial oxygen content between the prone and the supine sleep position. 5. There was no significant difference in the heart rates between the prone and the supine sleep position. The above results indicated that the newborns in the prone position moved less and slept deeper than those in the supine position, though there was no difference in the length of sleep or arterial oxygen content between the prone and the supine sleep position. But, Nurses and mothers should consider the relationship between the sleep position and SIDS suggested by previous researches. The infant's 'awakening' during sleep is a normal process and rather valuable because it can provide an opportunity to promote a stronger relationship between mother and baby. So, It is suggested that the supine sleep position is better than the prone sleep position for infants.

10.An Assessment of Asthmatic Knowledge of School Teachers.

PURPOSE: School-age children spend more than half of the day at school and teachers or school nurses are responsible for managing students with asthma during school hours. Thus, the teachers' knowledge about asthma is very important for the care of asthmatic school children. METHODS: Total 159 teachers were asked to fill out questionnaire on their knowledge about asthma and their personal profiles. The survey was administered by mail. The questionnaire includes 17 items about pathophysiology, symptoms, precipitating factors, treatment, care and prognosis, which were to be answered by either 'true' or 'false'. The correct or incorrect answers were examined data was statistically analyzed by SPSS. RESULTS: Most teachers were in their thirties (54.7%) and years of teaching experience were from six to fifteen years (50.4%). The teachers who experienced asthma students were 23.6%, and 12.6% were either asthma patients or had asthma patients in their family. They had knowledge about precipitating factors of asthma, but, the knowledge was not correct in detail. Only 34% teachers knew what to do and where to contact in case of asthma attack among the students. Teachers' demographic and professional factors did not affect the knowledge about asthma, except their working experience as school nurses and/or with school nursing asthmatic students. CONCLUSION: The survey results demonstrated that their teachers' knowledge about asthma was not sufficient enough to take good care of the asthmatic students at school including emergency management. Asthma education for the school teachers is required and efficient educational programs for the teachers should be developed.
