1.A Case of Pediatric Idiopathic Intracranial Hypertension Presenting with Divergence Insufficiency.

An 11-year-old female presenting diplopia only at distance was found to have comitant esotropia of 20 prism diopters (PD) at distance and normal alignment at nearer proximity. Other ocular movement, including abduction, was normal and a thorough neurologic examination was also normal. The deviation angle of esotropia was increased to 35 PD in 6 months, and a brain magnetic resonance imaging with venogram at that time demonstrated no intracranial lesion. A lumbar puncture showed increased opening pressure but the cerebrospinal fluid composition was normal. The patient was diagnosed as having idiopathic intracranial hypertension and treated with oral acetazolamide. Three months after treatment, the deviation angle decreased to 10 PD. This is a case report of divergence insufficiency in pediatric idiopathic intracranial hypertension, with an increasing deviation angle of esotropia. Although sixth cranial nerve palsy is a common neurologic manifestation in intracranial hypertension, clinicians should be aware of the possibility of divergence insufficiency. Also, ophthalmoparesis may not be apparent and typical at first presentation, as seen in this case, and therefore ophthalmologists should be aware of this fact, while conducting careful and proper evaluation, follow-up, and intervention.

2.Late-Onset Citrobacter koseri Endophthalmitis with Suture Exposure after Secondary Intraocular Lens Implantation.

A 54-year-old male patient was seen in clinic for ocular pain and decreased vision in the right eye with duration of two days. He underwent a cataract operation for his right eye 12 years ago, then a sclera-fixated secondary intraocular implantation and pars plana vitrectomy three years ago due to intraocular lens dislocation. At the initial visit, his visual acuity was restricted to the perception of hand motion. An edematous cornea, cells, flare with hypopyon, and exposed suture material at were observed at the six o'clock direction by slit lamp. Vitreous opacity was noted from B-scan ultrasonography. The patient was diagnosed with late-onset endophthalmitis and an intravitreal cocktail injection was done. On the next day, the hypopyon was aggravated, and therefore a pars plana vitrectomy was performed. A vitreous culture tested positive for Citrobacter koseri. After 12 weeks, the best corrected visual acuity of the right eye improved to 0.7 and a fundus examination revealed a relatively normal optic disc and retinal vasculature. We herein report the first case of endophthalmitis caused by Citrobacter koseri in Korea. Exposed suture material was suspected as the source of infection in this case and prompt surgical intervention resulted in a relatively good visual outcome.

3.Clinical Manifestations of Idiopathic Optic Perineuritis in Korea.

PURPOSE: To describe the clinical manifestations and magnetic resonance imaging (MRI) characteristics of idiopathic optic perineuritis patients in Korea. METHODS: A retrospective chart review of 5 patients (6 eyes) with idiopathic optic perineuritis and 7 patients (7 eyes) with optic neuritis was performed. RESULTS: The mean age of the 5 patients with idiopathic optic perineuritis was 59 years, and the mean age of the 7 patients with optic neuritis was 31.1 years. A relative afferent papillary defect (RAPD) was present in 4 eyes with idiopathic optic perineuritis, and in all 7 eyes with optic neuritis. All six eyes with idiopathic optic perineuritis showed relatively severe optic disc swelling, and 6 eyes with optic neuritis showed mild optic disc swelling. MRI demonstrated optic nerve sheath enhancement in all patients with idiopathic optic perineuritis and optic nerve enhancement in six patients with optic neuritis. CONCLUSIONS: On MRI, compared with optic neuritis, idiopathic optic perineuritis had a higher onset age, relatively severe optic disc swelling, and optic nerve sheath enhancement. Because idiopathic optic perineuritis is known to have different treatment strategies and prognosis compared with optic neuritis, differential diagnosis is crucial. The clinical manifestations and MRI characteristics of idiopathic optic perineuritis in the present study will help in the diagnosis of Korean patients.

4.Isolated Oculomotor Nerve Palsy due to Direct Invasion of Recurrent Diffuse Large B-Cell Lymphoma.

PURPOSE: Compared with multiple cranial nerve palsies, isolated nerve palsy is very rare in Non-Hodgkin's lymphoma. We experienced a case of isolated oculomotor nerve palsy as the first sign of recurrent diffuse large B-cell lymphoma (DLBCL). CASE SUMMARY: A 45-year-old woman visited the ophthalmology clinic with a one-month history of left upper eyelid ptosis and diplopia. She had a history of DLBCL of the nasopharynx and has been in complete remission for three years after systemic chemotherapy. Ophthalmologic evaluation showed ptosis of the upper eyelid, anisocoria, but no definite limitations in ocular movement. After six weeks, aggravated ptosis, exodeviation of the left eye in the primary position, and ocular movement limitations in all directions except abduction were observed. Brain magnetic resonance angiography taken four weeks earlier demonstrated no intracranial vascular lesion, but an enhancing lesion in the cistern along the left oculomotor nerve was shown in brain magnetic resonance imaging. We diagnosed isolated oculomotor nerve palsy due to direct invasion of recurrent DLBCL. Cerebrospinal fluid analysis revealed tumor cells, and follow-up MRI showed progression of lymphoma into the cavernous sinus. CONCLUSIONS: Although not common, isolated oculomotor nerve palsy can be the first sign of malignant lymphoma, even after complete remission.

5.Changes of Plasma Homovanillic Acid in Neuroleptic Responsive and Non-responsive Schizophrenics.

OBJECTIVES: Changes in plasma homovanillic acid(HVA) were investigated in neuroleptic responsive and non-responsive schizophrenics in order to delineate parameters of dopamine regulation, which may underlie differences in neuroleptic responsivity. METHOD: Twenty newly admitted acute schizophrenic patients were treated with haloperidol for 6 weeks. HVA was sampled at baseline, 3 days after initial neuroleptic dose, and after 1, 2, 3, 4, 6 weeks of treatment. Nine patients were classified as responders in this prospective haloperidol treatment trial. They had a score of change in the BPRS total scores of 25% or greater. Eleven patients were classified as nonresponders, based on a score of changes in the BPRS total scores of less than 25%. RESULTS: 1) The age of onset in respnder was older than nonresponder. 2) There were no significant changes in plasma HVA levels in total patients during 6 weeks haloperidol treatment period, but the nonreponders had a robust decrease in HVA level from baseline to 3 days and one week after haloperidol treatment in successive comparison. 3) There were no significant correlations between plasma HVA level and total scores of BPRS. CONCLUSIONS:This study suggested that neuroleptic non-responsive schizophrenics had a different plasma HVA concentration during haloperidol treatment but could not provide support to the idea that change in plasma HVA in response to neuroleptics can predict eventual clinical response to treatment. Further study is required in order to better characterize the changes in dopamine turnover in subgroups of schizophrenics.

6.Postoperative Changes in Vessel Density according to Macular Hole and Macular Pseudohole Subtypes

Purpose:
To determine the postoperative changes in vessel density according to macular hole and macular pseudohole (MPH) subtypes and to investigate the differences in the mechanisms underlying their development. We also investigated whether changes in vessel density are correlated with changes in the multifocal electroretinogram (mfERG) and best corrected visual acuity (BCVA).
Methods:
We reviewed the medical records of patients with MPH or a macular hole who underwent pars plana vitrectomy. We included 15 eyes of 15 patients with a full thickness macular hole (FTMH), nine eyes of nine patients with a tractional lamellar macular hole (LMH), eight eyes of eight patients with a degenerative LMH, and nine eyes of eight patients with a MPH. The BCVA, foveal avascular zone (FAZ), foveal and parafoveal vessel density, and mfERG ring 1 and ring 2 P1 amplitudes were analyzed before and 1 and 6 months after surgery.
Results:
One month postoperatively, the foveal vessel density of patients with a MPH or tractional LMH increased (p = 0.011, p = 0.008). The parafoveal vessel density of patients with a MPH, tractional LMH, and FTMH increased (p = 0.007, p = 0.038, p = 0.031). There was no significant increase in foveal or parafoveal vessel density in patients with a degenerative LMH (p = 0.201, p = 0.171). There was a significant correlation between the change in parafoveal vessel density and that in BCVA 6 months postoperatively in patients with a FTMH (r = -0.543, p = 0.037).
Conclusions
By assessing changes in vessel density after vitrectomy, it is possible to estimate the effect of traction according to the type of macular hole. There was a significant correlation between parafoveal vessel density and BCVA in patients with a FTMH. Restoration of the retinal structure and vessel density might improve visual acuity.
7.Incidence Rate and Risk Factors of Intraocular Lens Dislocation in South Korea

Purpose:
To analyze the incidence rate and risk factors of intraocular lens dislocation in South Korea from 2002 to 2020 using Health Insurance Review and Assessment Service data.
Methods:
This study included 15% of patients aged > 40 years had pseudophakia or intraocular lens primary implantation codes during 2002-2020 (737,606 patients). The annual incidence rate and risk factors of intraocular lens dislocation during 2002-2020 were analyzed.
Results:
The mean annual incidence rate of intraocular lens dislocation during 2002-2020 was 0.20 ± 0.7%. The annual incidence rate increased significantly from 0.17% in 2002 to 0.34% in 2020 (p < 0.001). The annual mean incidence rate for women was 0.1 ± 0.05%, and there was no increasing trend (p > 0.05). The annual mean incidence rate for men was 0.35 ± 0.13% and an increasing trend was found (p < 0.001). High myopia, uveitis, retinitis pigmentosa, glaucoma, pseudoexfoliative glaucoma and previous vitreoretinal surgery significantly increased the risk of intraocular lens dislocation (p < 0.001). Hemodialysis and hypertension were not significant risk factors (p > 0.05).
Conclusions
The mean annual incidence rate of intraocular lens dislocation in South Korea was 0.2 ± 0.7%, and it showed an increasing trend, among men. High myopia, uveitis, retinitis pigmentosa, glaucoma, pseudoexfoliative glaucoma and previous vitreoretinal surgery significantly increased the risk of intraocular lens dislocation.
8.Two Cases of Gastritis Cystica Superficialis without Previous Gastric Surgery.

Gastritis cystica superficialis (GCS) is a rare lesion which is characterized by glandular hyperplasia with regeneration and degeneration in the mucosa and muscularis mucosa. Recently, GCS is revealed as precancerous lesion, but most report has been associated with those found at the site of a gastroenterostomy. So, we report two cases with GCS who had not previous gastric surgery. A 45-year-old woman visited for epigastric discomfort and another 4Q-year-old woman for epigastric pain. They had not undergone any gastric surgery. The gastroscopy discovered one polyp on anterior wall of greater curvature, upper body and another polyp in the center of the fold of greater curvature, lower body. We removed it by snare polypectomy and the histologic finding showed the character of GCS.

10.Two Cases of Tubular Apocrine Adenomas.

Tubular apocrine adenoma(TAA) is a rare tumor characterized by papillary projection and apocrine differentiation. We report two cases of tubular apocrine adenomas. One patient was a 13-year-old male who had had a finger-tip sized asymptomatic cystic mass on his scalp since birth. Microscopically, the mass was composed of various sized cystic spaces and tubular structures lined by two layers of epithelia with nonvillous intraluminal papillary projections, decapitation secretions and connection with the epidermis. The other patient was a 55-year-old female who had had a 1.3x0.3cm sized dark red hard nodule on the left forearm since birth. Histopathologic finding showed a well circumscribed dermal neoplasm that predominantly consisted of multiple tubular structures lined by double layers of epithelial cells with decapitation secretions and intraductal papillary projections, and accompanied by underlying apocrine nevus. In both cases, immunohistochemical and enzyme studies showed positive findings in EMA, CEA, pancytokeratin, lysozyme, alkaline phosphatase, GCDFP-15.
