PURPOSE: We studied the change of bronchial hyperresponsiveness(BHR) from childhood to early adulthood and to know the risk factors which influence on the persistence of BHR. METHODS: Seventy two atopic asthma children with nonspecific BHR to methacholine were observed for about 10 years, longitudinally. They were divided into 3 groups depend on the degree of BHR; no BHR group(A group), asymptomatic BHR group(B group), symptomatic BHR group(C group) and compared clinical and atopic findings, inflammation degree of airway, and environmental factors between the groups. RESULTS: In the group A, B, C, the number of subjects were, respectively, 15(20.8%), 19 (26.4%), 38(52.8%). The frequency of group C was higher in the subjects with more severe asthma symptoms and higher BHR at diagnosis. The positive rates of skin prick test to Dp, Df, and pets, serum total IgE, sputum eosinophils, sputum ECP at the time of follow-up were significantly higher in group C. The frequency of subjects with carpets/sofa/bed, pets at home and smokers at the time of follow-up were higher in group B, C. CONCLUSION: Significant number of childhood asthma showed persistent nonspecific BHR in early adulthood. The persistence of nonspecific BHR in early adulthood was related with severity of asthma at diagnosis, laboratory findings and environmental factors at follow up.

Sixty-three patients with permanent sigmoid colostomy were surveyed to evaluate their satisfaction and complications with the "irrigation technique" and the "atural evacuation" of the colostomy management (irrigation technique; 32 patients, natural evacuation; 31 patients). All patients had colostomy for at least 12 months without disease recurrence. Each patient was interviewed in addition to standard questionnaire. The irrigation was not associated with any major complication including colonic perforation. The irrigation was used younger age group than the natural evacuation (53+/-10 vs. 62+/-12, P=0.01). The frequency of bowel movement was lower in the irrigation than in the natural evacuation (5.1+/-2.5/wk vs. 10.8+/-9.0/wk, P=0.04). Five patients (16%) of the irrigation experienced spontaneous bowel action but only one patient suffered from it. The time spent for managing irrigation was 59+/-13 minutes. Twenty-three patients (74%) of the natural evacuation suffered from one or moreproblems such as expensive apparatus, leakage, skin irritation or odor. Sixteen patients (52%) of the natural evacuation and 2 patients (6%) of the irrigation were restricted in social activity (P=0.0001). Thirteen patients (42%) of the natural evacuation and 1 patient (3%) of the irrigation were dissatisfied with colostomy management (P=0.002). Thus, the irrigation technique seems to be more effective and satisfactory method for managing colostomy without compromising patient's social activity when it is performed in appropriately selected patients.

PURPOSE: This study investigates the effect of pregnant women's growth mindset on the stress of pregnancy through ego-resilience. METHODS: The subjects included 551 pregnant women in Seoul and Gyeonggi area. Data were collected using a structured questionnaire; statistical, frequency, correlation, and structural equation analyses were performed using SPSS21 and M-plus 7.0. RESULTS: First, pregnant women's growth mindset had a direct effect on pregnancy stress. Second, the mediating pathway that influenced pregnancy stress through the ego-resilience of growth mindset was significant. CONCLUSION: These results suggest that it is important to devise and practice a method to improve the growth mindset of pregnant women, enhance ego-resilience, and reduce the negative effects of stress by reducing pregnancy stress.

PURPOSE: The purpose of this study was to analyze the prevalence of allergic disease among elementary school students in rural and urban areas. METHODS: In this study, 1,513 elementary students (1,163 in urban areas, 350 in rural areas) were surveyed. Data were analyzed using frequency, percentage of allergic symptoms and chi2 test was used to identify differences in the prevalence of allergic symptoms between urban and rural area students. The SAS program was used in the data analysis. RESULTS: There were significant differences in the prevalence of allergic disease according to whether there was a family member with a prior history of allergy symptoms. 48.7% of surveyed students (49.4% in urban, 46.3% in rural) had allergic symptoms. Allergic rhinitis was the most frequent allergic symptom in both urban and rural students. CONCLUSION: These results suggest that there is a need to prevent and manage allergies among elementary students. The family history should be considered an important factor when a program for allergy prevention and management is developed. Interventions are needed in both areas, especially for students with allergic rhinitis.

PURPOSE: The purpose of this study was to identify whether hearing a recording of mother's voice affected physiological reactions, behavioral state related to sleep and weight of very low birth weight (VLBW) infants. METHODS: This was a nonequivalent control group pre-post test quasi-experimental design. The participants were 11 VLBW infants in the experimental group and 11 in the control group. In the experimental group, a recording of mother's voice was played to her infant by voice recorder four times each day for five minutes over 10 consecutive days in the incubator. RESULTS: For physiological response (heart rate, respiration rate, oxygen saturation), there were statistically significant differences between the experimental and control groups. Behavioral state during sleep was more very quiet and quiet in the experimental group. In the daily weight change, there was no significant difference between the experimental and control groups. CONCLUSION: The findings suggest that, for VLBW infants, the intervention of hearing a recording of mother's voice had some significance as a nursing intervention having a positive impact. Such interventions can help pediatric nurses to stabilize the physiological response and maintenance more very quiet sleep state and improve VLBW infants' growth.

Fat embolism is a significant complication following total joint arthroplasty. Fat embolism syndrome has both pulmonary and neurologic manifestations that can be life threatening. But, with appropriate fluid management, adequate ventilation, and the prevention of hypoxemia, outcome is usually excellent. We experienced fat embolism following bilateral total knee replacement. A 65-year-old woman had a surgery under general anesthesia. There was no specific anesthetic problems during the operation. After bilateral tourniquet release, O2 saturation decreased to 85% and neurologic symptom was developed. With adequate supportive treatment including mechanical ventilation, she was cured without complication.

We report an annular type of subacute cutaneous lupus erythematosus(SCLE) which is rare in Korea, A 23-year-old female had erythematous annular rashes with a mild itching sensation on the face, trunk and both extremities. She also had painless oral mucosal ulceration, diffuse hair loss and mild arthralgia. Serologic abnormalities of the patient were positive anti-Ro Ab, ANA and Anti-ds-DNA Ab. The histopathologic findings showed vacuolar degenerations in the basal cell layer, mononuclear cell infiltrate in the upper dermis, and dermal edema. Direct IF revealed granular deposition of IgG and IgA at the dermo-epidennal junction.

PURPOSE: The purpose of this study was to analyze the effects of a health education program to improve knowledge about environmental health and allergy symptoms among elementary school students. METHODS: This study has nonequivalent control group pre-post test design. Participants of this study were 60 elementary students (30 experimental and 30 control) who agreed to participate in the study. The education program consisted of information on environmental health and allergies, the atmosphere, indoor environments, food and allergies, and activities with parents. The program was carried out one time (50 minutes) per week during 6 weeks. Knowledge and symptoms of allergies were recorded before and after the program among both groups. The collected data were analyzed by ANCOVA and t-test using SAS program. RESULTS: There was a significant increase in knowledge about allergies in the experimental group compared to the control group, but allergic symptoms were similar in the two groups. CONCLUSION: These findings suggest that environmental health education programs are effective in the area of school health nursing for increasing knowledge about environmental health and allergies. Further research is needed to develop programs for reducing allergic symptoms as an environment health problem among children.

Background: This study categorized older Korean adults’ social networks and analyzed their characteristics and digital literacy differences based on type. Methods: We analyzed data from 9,377 Korean older adult participants of the 2020 National Survey of Older Koreans, and performed latent class analysis (LCA) chi-square and Welch’s F analyses to understand the characteristics of each social network type. The Games–Howell post-hoc test was applied to determine the significance of differences between groups. Results: The three social network types derived using LCA were “child-centered,” “child-friend,” and “friend-centered.” The digital literacy levels differed significantly according to social network type. Conclusion The results of this study can be used to propose intervention programs and services associated with older adults’ social networks by examining their social network types and the corresponding differences in digital literacy.
