2.Clinical Significance of Amniotie Fluid Cell Culture Failure.

The reports of all amniocentesis samples received in our cytogenetic laboratory from 1986 to 1998 were reviewed to identify cases in which culture failure of amniocytes occurred. Medical records were then reviewed for the prenatal ultrasonographic findings, karyotype when available, and clinical outcome. We investigated the clinical aspects associated with second trimester amniotic fluid cell culture failure. During the study period, 5,325 second trimester amniotic fluid samples were processed, of which 42(0.8%) failed to yield a result. Ninety-seven percent of the samples were obtained before 24 weeks' gestation, mainly because of advanced maternal age. Three percent of the samples were obtained after 24 weeks' gestation. Culture failure was more common in samples obtained after or at 24 weeks' gestation(11.8%) than in those obtained before 24 weeks(0.44%)(p<0.05, chi-square test). This difference was also observed when the results were analysed according to the cases with known normal or abnormal karyotypes, The frequency of culture failure did not differ significantly between the groups with normal and abnormal karyotypes. We conduded that amniotic fluid cell culture failure is more common in advanced pregnancy and is not associated with a higher incidence of chromosomal abnormalities.

3.Arterial Blood Gas Analysis Following Ventilatory Patterns .

This study was carried out to compare the difference between assist(A) and controlled(C) ventilation group's blood gases during the general anesthesia. All samples were taken from the radial artery. The results were as follows, 1) Pa CO2was increased to 43. 9 5torr+/-7. 85 in group A and decreased to 28. 65 torr+/-6. 05 in group C. (p(0. 005) 2) Acidemia(7.335+/-0.51) in group A and alkalemia(7. 481+/-0.07) in group C were noticed. (p(0. 005) 3) B.E. and bicarbonate levels were within normal range in both groups. It's necessary to choose the patterns of ventilation before anesthesia, because of the anesthesiologist were apt to give alveolar hyperventilation during the controlled ventilation manually, but mild respiratory acidosis were developed during the assist ventilation.

7.Clinical Analysis of Adrenal Tumors.

BACKGROUND: Adrenal tumor is relatively rare disease. But according to the development of diagnostic tools, their incidence is rising. Therefore, new concept of surgical treatment for adrenal tumor should be established. METHODS: We analysed medical records for 41 cases of adrenal tumor patients who was performed adrenalectomy at Samsung Medical Center from Sep. 1994 to Apr. 1997. RESULTS: 39 cases were adenoma and 2 were adenocarcinoma. 24 cases were functioning tumors. Of these, 4 were Cushing's syndrome, 12 were pheochromocytoma and 8 were primary aldosteronism. Mean age for the patients was 45.6 years old. Among various diagnostic tools, abdominal CT scan was particularly helpful for diagnosis and localization. 2 cases of pheochromocytoma were MEN II. So, when we treat pheochromocytoma, a possibility of MEN II should be considered. CONCLUSION: 2 cases of 12 pheochromocytoma were proven to MEN II. So, when we treat pheochromocytoma, a possibility of MEN II should be considered. Considering improvement of surgical technique, such as laparoscopic adrenalectomy, we could consider more aggressive treatment for adrenal tumors.

9.Evaluation of Home-Delivered Meals Service Program for Home-Bound Elderly with Low Income Class.

The purpose of this study were to : a) evaluate the nutrition status of one hundred and forty five recipients and one hundred and forty nine non-recipients of home-delivered meals service program, b) examine the teeth status of recipients of the program, c)analyze the nutritional values of meals, and identify the attitudes of recipients and non-recipients were found. Four meals offered by Nambu seniors center were met the recommended dietary allowances in calorie, protein, and vitamin C. The mean score of meal satisfaction was 3.7 out of five-point scale.

10.Clinical observation of aseptic meningitis associated with mucocutaneous lymph node syndrome.

Clinical observation was carried out for 147 patients with mucocutaneous lymph node syndrome (MCLS) who were admitted to the Department of Pediatrics, Wonju Christian Hospital during the period from June, 1983 to June, 1992. A spinal tap was performed on 114 of these patients and the following results were obtained. 1) The incidence of aseptic meningitis in patients with MCLS was 52.6%. (60 cases of the 114 cases who had spinal taps (52.6%).) 2) The predominent age group for aseptic meningitis was 6 months to 1 year of age (35%). 3) For the majority, the value of glucose and protein in the CSF were within normal limit or only mildly elevated. 4) The neurologic manifestations associated with aseptic meningitis in patients with MCLS were irritability (78.3%), vomiting (25.0%), nuchal rigidity (11.7%), convulsion (5.0%) and facial nerve palsy (1.7%) in that order.
