Paragonimiasis is prevalent in Far East and a kind of endemic Korean disases. The primary site of humanparagonimiasis is the lung, but hte ectopic infection of lung fluke has been reported by many authors. Weexperienced one case of abdominal paragonimiasis in a 44 year old male with a complaint of left lowerquadrantpain. Physical examination, barium enema, and other data suggested the possibility of tumor originating from thewall of descending colon. (intramural tumor). Postoperative specimen taken from the solid tumorous lesion showedparasitic granuloma, characteristic of Paragonimus Westermani. Here, we report a case of very peculiar ectopicparagonimaisis involved descending colon wall simulating neoplastic tumor, and also review the literaturesbriefly.

Situs inversus totalis is a mirror image of the normal anatomy in the thoracic and the abdominal cavity. The cardiac apex, the stomach and the aortic arch are all on the right side. It is a rare condition with a genetic predisposition that is autosomal recessive and is associated with a 5-10% incidence of congenital heart disease. In a situs inversus totalis with cholelithiasis and choledocholithiasis, laparoscopic cholecystectomy was performed three days after endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy. In the laparoscopic cholecystectomy, pneumoperitoneum was made by a closed method, and the cholecystectomy was performed using the three trocar technique. Although we should pay much attention to the details of left-right reversal, these techniques can be safely and effectively applied to situs inversus totalis patients.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

PURPOSE: This survey was done to describe parenting self-efficacy and beliefs in corporal punishment as they are related to child abuse. Also demographic variables that influence child abuse were investigated. METHOD: A cross-sectional study was conducted during the second week of April 2002. The 160 participants were parents of students in five elementary schools in the J area of Korea. They replied to a structured questionnaire, and 129 replies were included in the final analysis. The WIN SPSS program was used for the analysis. RESULT: Parenting self-efficacy, beliefs in corporal punishment and child abuse showed no significant differences according to gender of the children. Child abuse by parents has significantly negative correlation with parenting self-efficacy (r=-.369, p=.000), socioeconomic states of family(r=-.290, p=.001), educational level of mother(r=-.211, p=.027), educational level of father(r=-.342, p=.000), parent's age(r=-.200, p=.028). Stepwise multiple regression showed that parenting self-efficacy and beliefs in corporal punishment significantly influence child abuse in Korean parents. CONCLUSION: As parenting self-efficacy explained 49.7% of child abuse, it is the most important variable for preventing child abuse. Belief in corporal punishment was the second most important variable in preventing child abuse. These two variables explained 53.3% of variance in child abuse by parents.

T cell lymphoblastic lymphoma is characterized by immature lymphoid cells that are indistinguishable from the lymphoblasts and prolymphocytes of acute lymphoblastic leukemia. Several characteristic clinical features of lymphoblastic lymphoma, with include a high male-to-female ratio, a relatively high incidence in older children and young adults, the frequent presence of mediastinal involvement at the time of diagnosis. Also, this disease is rapidly progressive, and early dissemination to the bone marrow, blood, and central nervous system leads to the evolution of a picture resembling a acute lymphoblastic leukemia. We have experienced a rare case of congenital T-cell lymphoblastic lymphoma in which 1/365 year old female newborn had generalized multiple irregular protruding mass on her body. On bone marrow biopsy, CSF analysis, ultrasonogram and whole body MRI studies. We found metastasized tumor mass to, orbit, abdominal cavity, bulva, skin, and lower extremities. Histopathologically, specimen from mass on the right thigh showed diffuse infiltration of poorly differentiated and immature lymphoid cells in the skelectal muscle and subcutaneous soft tissue. By immunophenotyping studies using anti T cell and B cell monoclonal antibody, these cells were reactive with UCHL-1, MB2, but unreactive with MB1, L26. Based on these findings, the lesion was diagnosed as lymphoblastic lymphoma of the T-cell type, which occurred congenitally. Most T cell lymphoblastic lymphoma are noted, but congenital case was not reported. So we report it with a brief review of literature.

PURPOSE: This study was done to identify menstrual discomfort and dietary habits, and factors related to the menstrual discomforts. METHOD: Participants were 320 female middle school students in G city. The Menstrual Discomfort Questionaire(MDQ) and dietary habit lists were used as tools. RESULTS: Factors related to MDQ were found to be the VAS scores (r=.361, p=.002), a mount of menstrual bleeding (r=.131, p=.019), height (r=.134, p=.016), adequacy of meal time (t=7.19, p=.008), consumption of milk & milk products (F=3.20, p=.042) and, hot, salty & irritant foods (F=8.01, p=.000), eating more than 3 kinds of side-dishes with each meal (F=8.32, p=.000), and various protein foods (F=5.15, p=.006). In stepwise regression, 4 variables (VAS scores, height, hot, salty & irritant foods, more than 3 kinds of side-dishes with each meal) explained 20.3% of the variance in the total MDQ scores. CONCLUSION: Reduction of hot, salty & irritant foods and having more than 3 side-dishes with each meal would be effective in menstrual discomfort management. As well, good and, balanced dietary habits should be encouraged for early adolescent girls. To keep up with the ideal dietary habits, nutritional education & counseling should be continued. In a further study, a replication study with larger samples and more specified nutritional assessment are recommended.

This study was carried out to compare the difference between assist(A) and controlled(C) ventilation group's blood gases during the general anesthesia. All samples were taken from the radial artery. The results were as follows, 1) Pa CO2was increased to 43. 9 5torr+/-7. 85 in group A and decreased to 28. 65 torr+/-6. 05 in group C. (p(0. 005) 2) Acidemia(7.335+/-0.51) in group A and alkalemia(7. 481+/-0.07) in group C were noticed. (p(0. 005) 3) B.E. and bicarbonate levels were within normal range in both groups. It's necessary to choose the patterns of ventilation before anesthesia, because of the anesthesiologist were apt to give alveolar hyperventilation during the controlled ventilation manually, but mild respiratory acidosis were developed during the assist ventilation.

PURPOSE: The aim of this study was to identify the present situation of hospital infection and route of infection by clarifying the transmission aspect of methicillin-resistant Staphylococcus aureus(MRSA) in a Neurosurgical Intensive Care Unit by analysing genotype. METHODS: MRSA was cultured from twenty five patients with a tracheostomy, twenty five health care workers, and environments in the Neurosurgical Intensive Care Unit of one hospital in D city. Data was collected from December 21, 2004 to November 5, 2005. MRSA isolates representing each genotype were analyzed by spaA typing and a multiplex PCR method capable of identifying the structural type of the staphylococcal cassette chromosome mec(SCCmec) carried by the bacteria. RESULTS: As the same genotype and gene sequence were found among health care workers, patients, and environments, it was assumed that there was cross transmission among them. CONCLUSION: This study suggests that first, as the hospital infection by MRSA between health care workers and patients in the Neurosurgical Intensive Care Unit was due to result of cross transmission and the relevance of transmission between them was verified, it is necessary to take preventive measures and conduct education. Secondly, development of nursing interventions and study of infection are needed. Thirdly, consistent investment in prevention against hospital infections and environmental renovation is needed.

The pretent study was performed to study the epiderrnal changes of ultraviolet irradiation and PUVA treatment in the black mice. The following results were obtained: 1) The histologic changes of the epidermis, number of DOPA positive melanocytes and ultrastructural changes of keratinocytes due to UVB and PUVA were rnore prominent than those of UVA irradiation. 2) The histological changes after 8-MOP photosensitization closely mirnic those of UVR irradiation, and constant, finding was hyperkeratosis. 3) Early histological change after UVL irradiation & IUVA treatment was appearance of dytkeratotic cells. 4) Pro]onged exposure to UVB & PUVA resulted in transient epidermal hyperplasia, and epidermal atrophy occurred 3 weeks after PUVA treatment. 5) Early electron microscopic changes was perinuclear edema, juxtanuclear vacuoles and clumping of tonofilament.
