2.EEG Abnormalities in Henoch-Schonlein Purpura.

Schonlein-Henoch purpura is a generalized small vessel vasculitis characterized by nonthrombocytopenic purpura, arthritis, abdominal pain and nephritis. In 1914 Osler described an allergic purpura associated with hemiplegia. After then Lewis et al. reported the cases of Schonlein-Henoch purpura associated with convulsion, coma, confusion, intracranial hemorrhage, and chorea, CNS complication has been reported in 1-8% of children and subsided spontaneously in most cases. Headache is a remarkable sympton and appears nonspecific nature. In 1991 Ostergaard and Storm reported that headache occured during the first week following skin rash and frequently showed abnormal EEG findings. We investigated prospectively the presence of a possible cerebral and renal involvement in the case of Schonlein-Henoch purpura. EEG abnormality demonstrated in 52.6% of all cases, and headache or irritability in 47.4% of all cases. A significant association was found between abnormal EEG finding and presence of headache, but was not found between EEG findings and presence of renal involvement and hypertension. Patients with abnormal EEG had no Past or famity history of febrile convulsion or ididopathic epilepsy.

Lung transplantation has been successfully employed for variety of obstructive lung disease. Single lung transplantation has become a therapeutic option for end-stage obstructive lung disease. The patient, a 57 year old man with emphysema, suffered from severe dyspnea, which progressively aggravated him for the last three years. A single lung transplantation was performed from a young brain-dead donor on April 7th, 1999 in the department of thoracic surgery, Respiratory Center, Yongdong Severance hospital, yonsei University. The immunosuppressive regimen was based on cyclosporine A and azathioprine from beginning, adding steroid. Single lung transplantation was feasible and beneficial in patients with end-stage emphysema.

Non-Hodgkin's lymphoma of the primary central nervous system is a rare neoplasm. With the increased incidence of this tumor in both the immunocompetent and immunocompromised population, unusual locations and imaging manifestations are likely to be encountered. We report the radiologic findings of a case of primary lymphoma involving the third ventricle, confirmed by surgery and the pathologic findings.

It is very rare that the bone scan after 72 hours from the trauma doesn't exhibit the increased radio-nuclide uptake in the patient with fracture. The purpose of this study is to report the case that indicate the false negative finding in the bone scan performed after 78 hours from the trauma in the 56-year-old man with L2 compression fracture, including a review of the relevant literatures.

6.A Case of Pulmonary Actinomycosis.

Pulmonary actinomycosis is a chronic suppurative disease usually caused by Actinomyces israelii, characterized by suppuration, sinus tract formation, and purulent discharge containing yellowish "sulfur granules". It is usually caused by aspiration of contaminated material from mouth or oropharynx. The diagnosis of pulmonary actinomycosis is difficult and its initial clinical manifestations mimic tuberculosis or neoplasm. We experienced a case of thoracic actinomycosis in a 47-year old male patient who had complained of cough, chest pain and fever for 2 months. The diagnosis was confirmed pathologically by ultrasonography guided gun-biopsy of pleural mass and thoracoscopic wedge biopsy of parenchymal lung lesion.

7.Clinical Analysis of Recurrent Gastrointestinal Stromal Tumor (GIST) Patients .

PURPOSE: Gastrointestinal stromal tumors (GISTs) are of a mesenchymal origin, and they arise predominantly from the gastrointestinal tract. This study aims to aid the post operative management of GIST patients by analyzing the clinical and immunopathological features of recurrent GISTs. METHODS: We enrolled 98 GIST patients who had been operated on for their primary tumor from 1987 to 2001 at the Catholic Medical Center. Among them, 28 patients had clinical and radiological features that were proved to be recurrence, and the patients' survival rates were compared according to the treatment modalities. RESULTS: When the maximum length of tumor was shorter and the mitotic index lower, then these patients had higher disease free survival rates and lower recurrence rates. The Ki-67 negative group had lower recurrence rates than their Ki-67 positive counterparts. Recurrence was mostly observed as liver metastasis. The mean length of time to recur was 22.96 months. Fifteen patients received additional treatments such as surgery, chemotherapy and radiation therapy, but there was no significant difference in survival rates when they were compared to the 13 patients who did not receive further treatments. CONCLUSION: Of the patients diagnosed with GIST after surgery, those with worse prognostic factors, i.e. a bigger tumor size and higher mitotic index, require more meticulous surveillance for tumor recurrence, and especially for liver recurrence, which was the most common recurrence site, during the follow up exams. In addition, although there have not been any remarkably effective treatments for the recurrent GIST patients, further researches for new therapy such as STI-571 is mandated.

8.Atretic Cephalocele: Report of Two Cases.

In cases of meningoencephalocele, brain tissue, which is enveloped by the meninges and cerebrospinal fluid, herniates through the skull defect. Atretic cephalocele, on the other hand, is a congenital malformation in which small meningeal and vestigial glial tissue herniates through this defect; a benign nodular lesion is found near the midline of the scalp. Atretic cephalocele can occur in the parietal or occipital area; in cases involving the latter, the prognosis is poor. We report the radiologic findings of two cases of atretic cephalocele confirmed by surgery and pathologic findings.

9.Comparison of Isokinetic Strength between Stage 1,2 Impingement Syndrome and Rotator Cuff Tear.

PURPOSE: The goal of this study was to evaluate differences in strength deficits between (i) patients with stage 1 or 2 impingement syndrome and (ii) patients with rotator cuff tears. MATERIALS AND METHODS: We enrolled 43 patients with stage 1 or 2 impingement syndrome (group 1) and 21 patients with rotator cuff tears (group 2). The isokinetic strength of both groups was evaluated at 60degrees/sec for external rotation, internal rotation, adduction and abduction. We measured the peak torque, total work, average power of bilateral sides, peak torque relationship to body weight, and the ratio between unilateral agonist and antagonist. RESULTS: The isokinetic strength deficits assessed in ratio of peak torque to body weight for group 1 and group 2 respectively were; 28.48+/-23.76% and 29.12+/-32.81% for abduction (p=0.929), 7.20+/-13.98% and 18.94+/-19.52%; for adduction (p=0.021), 16.88+/-13.76% and 25.80+/-24.07%; for external rotation (p=0.221), and 14.1+/-25.67% and 29.02+/-35.06% for internal rotation (p=0.059). For average power and total work, group 2 showed a significantly greater deficit for adduction and internal rotation than group 1. CONCLUSION: Those with rotator cuff tears have more isokinetic muscle strength deficits than those with stage 1 or 2 impingement syndrome. Progression from stage 1 and 2 impingement syndrome to stage 3 may result in greatest changes in strength deficits for internal rotation and adduction.

10.Apoptosis of the GABAergic interneuron in the dorsal horn of the chronic post-ischemic pain model.

BACKGROUND: It is well known that the GABAergic inhibitory interneuronal system plays an important role in modulation of the noxious stimulation transmitted from the primary afferent input. Some studies have revealed the role that the GABA inhibitory interneuronal system plays in the modulation of pain transmission and the changes in the GABAergic interneurons that occur during the neuropathic pain. This study was conducted to evaluate the apoptosis of the GABAergic interneuron, which is assumed to contribute to neuropathic pain. METHODS: Male Sprague-Dawley rats weighing 290-310 g were used to create a CPIP (chronic post-ischemic pain) model, which was made by placing a tourniquet on the left hindpaw of the rats. The tourniquet was maintained for 3 hours, after which it was released to allow reperfusion. Thirty minutes prior to reperfusion, N-acetyl-L-cysteine (NAC group) or normal saline (control group) was injected. After reperfusion, mechanical allodynia and cold allodynia were measured. In addition, the release of cytochrome c into the cytosol was evaluated through western blot or immunohistochemistry of the spinal cord. RESULTS: Mechanical and cold allodynia developed and the number of GABA interneurons was reduced in the control group. Additionally, The cytochrome c from the GABA interneuron was released into the cytosol in the control group, but the amount released was reduced in response to treatment with NAC. CONCLUSIONS: The results of this study showed that the GABA interneuron in the Rexed laminae I, II released cytochrome c into the cytosol in CPIP neuropathic pain model, which is known to lead to apoptosis. However, treatment with N-acetyl-L-cysteine prevented this process.
