1.Treatment of Pierre-robin sequence Using Bilateral Mandibular Distractors.

The purpose of this study is to present our experience of distraction osteogenesis procedure used to improve the functional and consmetic problems of two patients associated with Pierre-Robin sequence, which is an uncommon congenital disease in combination with micrognathia, glossoptosis, and respiratory obstruction. A female child of 42 months and a male infant of 4 months suffering from pneumonia, difficult breathing, cyanosis and insomnia were admitted. We performed temporary tongue to lip dhesion by modified Routledge method and bilateral mandibular distraction. After the operation, symptoms of insomnia and impaired breathing had improved. We found an extension of the mandibular length. In addition, the posterior airway space and lower face area were expanded in the cephalometric tracing and profilogram. We report that bilateral distraction provedure could be an effective and safe method for improving of impaired breathing and cosmetic problems of patients with Pierre-Robin sequence.

2.Herpes Simplex Esophagitis: A report of two cases.

Herpes simplex esophagitis has been recognized with increased frequency as an opportunistic infection in immunosuppressed or debilitated patients. However, it is also documented as self-limited esophagitis in apparently healthy patients. We report two cases of herpetic esophagitis diagnosed by biopsies. One case was noted in an immunocompetent patient having gastric peptic ulcer who had clinical improvement with symptomatic treatment. And the other was noted in an immunosuppressed patient having chemotherapy for gastric carcinoma who had resolution of symptoms with acyclovir therapy. Histologically, esophageal mucosa exhibited erosion and inflammatory cellular exudate with intranuclear eosinophilic inclusions in the epithelial cells. These were confirmed by the PCR and immunohistochemical stain for herpes simplex virus using a biopsy material.

3.The Effect of the Horticultural Activities Program on Loneliness and Life Satisfaction of the Disabled Female.

PURPOSE: The research was done to investigate the effect of a horticultural activities program on loneliness and life satisfaction of women who have a disability. METHODS: Participants were 50 disabled women from a welfare center of G district of G city. Data were collected from August 18 to October 20, 2010. The program was provided for 8 weeks. The tool for life satisfaction was Choi's scale (1986) and loneliness was measured with the UCLA Loneliness Scale developed by Russell and translated by Kim & Kim (1989). Data were analyzed using frequency, t-test, and paired t-test with SPSS/WIN 16.0. RESULTS: As a result of the horticultural activities program, the first hypothesis was supported by significantly decreasing of loneliness in the experimental group than in the control one (t=-3.27, p =.002). The second hypothesis was verified by testing variable which was life satisfaction (t=3.39, p =.001). CONCLUSION: The results of the study indicate that this kind of program is effective in decreasing loneliness of women with a disability and in improving their life satisfaction. Horticultural activities programs can be unique interventions leading to a satisfactory life for disabled women.

4.Peritoneal Equilibration Test in Children with Continuous Ambulatory Peritoneal Dialysis.

The peritoneal equilibration test(PET) has been recommended in adults as a standardized means of estimating solute transport. However, it appears that norrns for pediatric PD patients may be different. We performed a PET on 16 children aged 3 to 18 years using a dwell volume for 30ml/kg of 2.596 dialysate. Our children transported glucose more rapidly than adults, however, creatinine transport was not significantly different. Age did not correlate with D/13% glucose or drainage volume. There was negative correlation between D/D% glucose and D/P Cr. There was no significant difference between initial and maintenance CAPD patients. There was slight difference in patients with previous peritonitis. In conclusion, there was difference in glucose transport between children and adults.

5.CT Features of Appendiceal Mucocele.

PURPOSE: To evaluate the usefulness of CT features of appendiceal mucocele in the diagnosis and evaluation of complications. MATERIALS AND METHODS: We retrospectively reviewed CT findings and compared with operative findings in 7 cases of pathologically proven appendiceal mucocele. CT findings such as location and extent of the lesion, Issue density, thickness or calcification of the wall, presence of adjacent inflammatory infiltration, and visualization of normal vermiform appendix were analyzed. RESULTS: Appendiceal mucocele was found as homogeneous low density cystic mass adjacent to the cecum, which has no surrounding inflammatory infiltration except in one case of perforation and one case of intussusception. Mean CT number measured in 4 cases was 21 Hounsfield unit. Thin curvilinear calcifications were noted along the cystic wall in 2 cases. Normal vermiform appendix couldn't be demonstrated in all cases. CONCLUSION: Appendiceal mucocele is characterized by homogeneously low density and thin walled cystic tumor adjacent to cecum without surrounding inflammatory infiltration, and absence of normal vermiform appendix on CT. Therefore, CT is valuable in preventing operative complications of appendiceal mucocele.

6.Unexpected Detachment of Solitaire Stents during Mechanical Thrombectomy.

OBJECTIVE: Unexpected Solitaire stent detachment can occur during mechanical Solitaire thrombectomy. The purpose of this study was to retrospectively evaluate the influencing factors causing unexpected Solitaire stent detachment and the clinical outcomes. METHODS: Between October 2011 to December 2013, 232 cases of mechanical Solitaire thrombectomy for acute ischemic stroke were performed in 3 stroke centers. During this period, we encountered unexpected Solitaire stent detachments during mechanical Solitaire thrombectomies in 9 cases. RESULTS: Solitaire stents unexpectedly detached in 9 cases (3.9%) during the retrieval of Solitaire stents. The median patient age was 76 years. The occlusion sites of the unexpected stent detachment were the proximal middle cerebral artery (MCA) in 7 cases and the internal carotid artery in 2 cases. The sizes of the stents that unexpectedly detached were 6x30 mm in 7 cases, 5x30 mm in 1 case, and 4x20 mm in 1 case. Four patients had unexpected detachment at the first retrieval, 1 patient at the second, 3 patients at the third, and 1 patient at the fifth. In all of the cases of unexpected detachment at the first retrieval, the stent deployment site was the proximal MCA. After detachment, a proximal marker of the Solitaire stent was observed in 3 patients. However, no marker was visible in the remaining 6 patients. CONCLUSION: Unexpected Solitaire stent detachment should be considered in the first instance of stent retrieval for a relatively large-diameter stent, especially in elderly patients with MCA occlusions.

8.Clinicopathological Study about Childhood Primary Nephrotic Syndrome Resistant to 4-week Daily Steroid Therapy.

PURPOSE: Steroid-resistant nephrotic syndrome in children is difficult to manage and tends to progress to chronic renal failure. We studied clinicopathological correlations in primary nephrotic syndrome in children resistant to 4-week daily steroid therapy. METHODS: Among children who had been admitted to Seoul National University Children's Hospital during the period between Oct. 1985 and Jul. 1995 and diagnosed as primary nephrotic syndrome, 87 patients were selected for this study. They showed poor response to 4-week daily steroid therapy either initially (initial nonresponder) or subsequently in the disease course (subsequent nonresponder). The medical records including renal pathologic findings were analyzed retrospectively. RESULTS: The mean age at the onset of nephrotic syndrome was 7.3+/-4.1 years and male to female ratio was 62:25. Pathologically, 28 (32%) had minimal change lesion (MCL), 47 (54%) had focal segmental glomerulosclerosis (FSGS) and 12 (14%) had others. There were 15 (54%) initial nonresponders and 13 (46%) subsequent nonresponders in the MCL group, and there were 26 (55%) and 21 (45%), respectively, in the FSGS group. The incidence of hematuria was less frequent in the MCL group. The frequencies of hypertension and azotemia were not significantly different between in the MCL and the FSGS group. Among 10 patients with MCL in whom the steroid therapy were extended to 6 weeks, 3 patients responded subsequently. And 1 of 3 patients among the FSGS group responded to 8-week daily steroid therapy. The 2nd line drug therapy such as oral cyclophosphamide, intravenous pulsed methylprednisolone, enalapril, dipyridamole, etc. was tried in 26 patients with MCL and all 47 patients with FSGS. In the MCL group, 7 of 13 initial nonresponders and 9 of 13 subsequent nonresponders responded to these 2nd line drug therapies. In the FSGS group, 10 of 26 initial nonresponders and 11 of 21 subsequent nonresponders responded to these therapies. While only 1 subsequent nonresponder in the MCL group progressed to chronic renal failure, 9 initial and 4 subsequent nonresponders progressed in the FSGS group. CONCLUSIONS: The FSGS group formed about a half and the MCL group formed about a third of steroid-resistant nephrotic syndrome in children. Although the response to 2nd line drug therapies was not different between 2 groups, the incidence of progression to chronic renal failure was significantly higher in the FSGS group.

9.An Epidemiological Investigation of an Outbreak of Rubella Occurred in a Male High School in Kyongju.

This epidemiological study was carried out to investigate the sources of infection and modes of transmission of an outbreak of rubella that occurred among male high school students in Kyongju in march, 1996. 770 male students(286 third grade, 262 second grade, 222 first grade students) were selected as the study subjects. A questionnaire was completed by the subjects. The anti-Rubella antibodies IgM and IgG were examined and tested by ELISA on all sera from 770 students and the environmental status of class rooms, the dormitory conditions and study rooms were tested. The positive IgM result rate was 17.8% and when the IgM negative was combined with IgG positive the results was 74.9%. IgM IgG negative rates were 7.3%, however, out of a total 770 students. Amongst the older students, the IgM positive rate showed an increase(p<0.01). Out of 137 cases(with a cummulative incidence rate of 71.0%), there were 97 apparant cases(cummulative incidence rate 50.3%) and 40 inapparent cases(cummulative incidence rate 20.7%). Again, the cummulative incidence rate showed an increase in the older students(p<0.04). In the apparent cases, major symptoms included eruption(96.9%), fever(85.6%), lymphadenitis(82.5%), generalized aches and muscle tenderness(76.3%), and a sore throat(61.9%). These symptoms lasted anywhere from 2 degrees =/ 13 days, the average duration being 4 days in length. Initially, there were 9 people on third grade and suspected origin was from more than 2 external sources. It progressed from the upper grades down to the lower grades. In dormitory students, the cummulative incidence ratio was 1.70 and 1.78 amongst study room students.

10.Changes in the Effects of Bilateral Lateral Rectus Muscle Recession According to Preoperative Angle Deviation

PURPOSE: To investigate how the effect of bilateral rectus muscle recession changed by analyzing the effect/dose ratio of surgery according to the preoperative angle deviation.METHODS: We retrospectively studied the medical records of patients from January 2007 to March 2014 who underwent bilateral lateral rectus muscle recession and who visited our hospital for at least 2 years after surgery. We classified the patients into two groups: the preoperative large angle deviation group (35 prism diopters [PD] or more) and the small angle deviation group (20 PD or less). We observed exodrift patterns by measuring distant and near angle deviation according to the preoperative and postoperative times. The effect/dose ratio of recession was calculated at each visit. Surgical success was defined as an alignment between 10 PD of exodeviation and 5 PD of esodeviation, both at distance and at near.RESULTS: Among 165 patients, 84 patients were in the large angle deviation group and 81 patients were in the small angle deviation group. Preoperative angle deviation of the large angle deviation group was 39.34 ± 5.13 PD (range: 35–55 PD) and the small angle deviation group was 19.49 ± 1.62 PD (range: 18-20 PD) (p < 0.001). At postoperative 1 day, the alignments of eyes of the two groups were −8.32 ± 9.31 PD and −13.11 ± 6.94 PD; p < 0.001, respectively. At the date of the final follow-up, the alignments of eyes of the two groups were 4.63 ± 8.94 PD and 1.22 ± 8.01 PD; p = 0.011, respectively, and the effect/dose ratios were 2.25 ± 0.62 PD/mm and 1.94 ± 0.78 PD/mm, respectively, which meant that the effect of correction for the large angle deviation group was larger than that of the small angle deviation group (p = 0.005). Both groups showed postoperative exodrift patterns and similar success rates (75.0% and 80.2%, respectively), which showed no significant difference (p = 0.268).CONCLUSIONS: The small angle deviation group showed a larger effect of correction and high exodrift pattern at the postoperative initial time and the large angle deviation group showed a smaller effect of correction and low exodrift pattern at the postoperative initial time. The preoperative angles of deviation and the operative success rates were not related.
