1.Thrombosed Capillary or Vein Associated with Angiokeratoma.

Thrombosed capillary or vein is a dome-shaped or slightly lobulated moderately firm, blue black nodule arising either abruptly or gradually. There may be a rim of erythema or brownish pigmentation around it. Its clinical features are similar to a malignant melanoma. We report a case of thrombosed capillary or vein associated with angiokeratoma in 44-year old male patient who has a bean sized, dome-shaped, blue-black nodule and multiple hyperkeratotic pinhead sized dark red papules un scrotum.

2.A Case of Cutaneous Sarcoidosis.

A case of cutaneous sarcoidosis in the 66 year-old Korean female patient is presented. She shows typical skin manifestations of plaque type in the absence of other common objective evidence of systemic involvement. Diagnosis was confirmed by charcteristic histological findings and positive Kveim test with the support of other compatible clinical and biochemical findings. She had been followed up for 3 years since the first visiting. Recently chest X-ray, slit lamp examination and skin biopsies were taken again, but failed to reveal any significant interval changes.

3.Two Cases of Segmental Neurofibromatosis.

Neurofibromatosis(NF) is a disorder characterized by its relative commonness, variability, and heterogeneity. It is usually expressed as a generalized form, but is rarely localized in a limited part of the body as a segmental form. In 1981, Riccardi classified NF into 8 types and a segmental form (type V) was defined by limitation of cafe-au-lait spots and/or neurofibroma on a given unilateral segment of the body. Recently we came across two cases of typical segmental neurofibromatosis. The First case was a 53-year-old woman with localized neurofibromatosis on the right T8,9 dermatome. The Second case was a 34-year-old man with localized neurofibromatosis on the right T5,6 dermatome. There were no cafe-au-lait spots, axillary frecklings, Lisch nodules or a family history in both cases. We report these rare cases with a literature review.

4.A Case of Umbilical Omphalomesenteric Duct Polyp.

A umbilical omphalomesenteric duct polyp is the result of incomplete closure of the omphalomesenteric duct, which connects the midgut with the yolk sac of the embryo. It may be associated with underlying embryologic anomalies such as Meckels diverticulum and umbilical enteric fistula, the complications of which may at times be fatal. This rare malformation should be clinically discerned from persistent granulation tissue or pyogenic granuloma. Histologically, it shows a polypoid lesion consisting of ectopic gastrointestinal epithelium with the appearance of gastric, intestinal, or colonic mucosa. We report a case of an umbilical omphalomesenteric duct polyp in an 8-year-old male patient, who had had a bright-red polyp on the umbilicus from the age of 1 month and had not had any other types of underlying abnormalities.

5.Microcystic Meningioma - Unusual Variant of Meningiomas.

Microcystic meningioma is a rare variant of meningiomas. This unusual variant was originally described by Masson, who labeled it "humid". The computed tomographic scan or magnetic resonance images of these tumors resemble those of a glial or metastatic tumor with cystic or necrotic changes. There is no definitive method for differentiating cystic meningiomas from these more common tumors. But immunohistochemically, they share a similar pattern of positive staining for epithelial membrane antigen and vimentin with other meningiomas. Our case was a 34-year-old woman with a tumor mass on the right frontal area. She was admitted to hospital because of generalized tonic seizure. Grossly all of the tumor could be removed, and histopathologically this tumor was revealed to be a microcystic meningioma.

6.Pubovaginal Sling Using Mesh and Fixed to Cooper's Ligament: Comparison of Efficacy with Anterior Vaginal Wall Sling.

PURPOSE: The results of pubovaginal sling operation using mersilene mesh fixed to Cooper's ligament(Cooper's ligament sling;CLS) were evaluated and compared with the results of anterior vaginal wall sling(AVWS). MATERIALS AND METHODS: One hundred forty patients who had undergone sling operation and had been followed up for more than one year were enrolled in the study. Sixty patients underwent CLS and 80 AVWS. Therapeutic efficacy and complications were evaluated by reviewing medical records and by telephone interview. RESULTS: Mean operating times were 93 and 87 minutes, mean urethral Foley catheter indwelling times were 1.5 and 1.8 days and mean postoperative hospital stays were 5.1 and 5.2 days, respectively, in the CLS and AVWS groups(p>0.05). Recurrence of incontinence was reported by 6 patients in the CLS group and 7 in the AVWS group(p>005). The incidence of significant complication and postoperative urge incontinence did not show a statistically significant difference between the two groups. Suprapubic pain of more than 1-month duration after surgery was found in patients in the AVWS group only, which had disappeared at 6 months after operation. The postoperative satisfaction state was statistically similar in both groups. CONCLUSIONS: Pubovaginal sling operation using mersilene mesh fixed to Cooper's ligament compares favorably with AVWS in terms of incontinence rate and complication. It has the clear advantages of being relatively cheap and devoid of the transvesical suprapubic pain that is seen with AVWS, and, furthermore, has the theoretical advantage of increased durability due to fixation to a fixed portion. Follow up for a longer period of time should be done.

7.Prevalence of Tinea Pedis of College Students and Their Families in Taegu.

BACKGROUND: Tinea pedis is one of the most comrnon dermatoses in the general population. But its prevalence in the general populat,ion was not yet reported in Korea. OBJECTIVE: Prevalence of tinea pedis and associated tinea unguium were checked in the general population. METHODS: Six hundred fifty eight persons from 179 families of the college students were evaluated. Clinical examinatior. for tinea pedis and onychomycosis and collecting scales from the suspicious lesions of the family members were perforrned by educated college students. Then dermatologists exarnined those specimens mycologically by KOH ~tnount and fungal culture. RESULTS: The results were as follows 1. The prevalence of tinea pedis was 41.2% in the studied 658 persons. 2. Among all 271 patients with tinea pedis, 201 strains of dermatophytes were isolated. They were 143 strains of Trichophyton(T) rubrum, 49 strains of T. mentagrophytes and 9 mixed infections of T. rubrum and T. mentagrophytes. In the co existence of i,inea pedis and tinea unguiurn, isolated species were corresponding. 3. Among 179 families, 143 families (79.9%) had at least one patient with tinea pedis. The isolated species of derrnatophytes in a farnily were one organism, 71.4% and two or more organisms, 28.6% of 77 familiys with more than one patient. CONCLUSION: In our study, the prevalence rate of tinea pedis was 41.2%. That suggests that tinea pedis is a wide spr .ad disease in healthy adult in Korea.

8.A Case of Neonatal Graveses Disease.

Neonatal Gaves disease is a relatively rare condition due to transplacental passage of Thyroid-stimulating antibody(TSAb) from a mother with active or inactive Graveses disease or autoimmune thyroiditis. A 11-day-old female newborn was referred to our department of pediatrics from a local clinic because of low level T4(3.55microg/dl) concurrent with high level TSH (501.74uIU/ml) on the 5th day neonatal metabolic screening. But, our repeated laboratory data showed very high serum T4(59.6microg/dl), T3(1,600ng/dl), suppressed TSH(0.43uIU/ml), and the presence of TSH receptor antibody. Her mother was treated with propylthiouracil(PTU) for Graves disease during pregnancy. Therefore, we thought it was a delayed-onset neonatal hyperthyroidism, because the fetal thyroid gland was initially suppressed by antithyroid drug taken during pregnancy. After initiating antithyroid drug therapy for the hyperthyroid nature, TSH levels became elevated again, while thyroid hormone levels decreased. Maternal and infant blood samples at the 23th day after birth were examined for serum autoantibodies directed towards the TSH receptor(Thyrotropin-binding inhibitory immunoglobulin:TBII, Thyroid-stimulating antibody:TSAb, Thyroid-stimulating blocking antibody:TSBAb) and high levels of TBII and TSAb were detected. About 2 months after birth, TBII and TSAb decreased within normal limit, and then we could stop antithyroid medication in safety. We report here a case of neonatal Graveses disease with very high level of T4 and T3, but firstly presented as hypothyroid nature on neonatal screening because of the maternally transferred antithyroid drug, PTU.

9.Three Cases of Cervical Necrotizing Fasciitis.

Cervical necrotizing fasciitis (CNF) is an uncommon infection of the subcutaneous soft tissue and fascia. If proper treatment is delayed, the infection may cause extensive necrosis of overlying skin, extend to deeper planes and produce severe systemic toxicity. So an early diagnosis and a radical debridement of the affected tissue is imperative; however, the onset is often insidious in the form of nonspecific regional neck swelling, erythema and fever, rendering clinical distinction from more benign inflammatory conditions of the neck, such as cellulitis, may be impossible at an early stage. We present a series of 3 patients with CNF, each of whom had different clinical courses. One patient, with significant cormobidity at the time of presentation, died of CNF.

10.Evaluation of Anxiety and Depressive Levels in Tinnitus Patients.

BACKGROUND AND OBJECTIVES: The aims of this study were to evaluate the relationship between tinnitus and the level of anxiety and depression experienced by subjective tinnitus patients, and to determine the effect of the level of anxiety and depression to the results of tinnitus treatment. SUBJECTS AND METHODS: A total of 104 patients were included in this study. All the patients conducted Tinnitus Handicap Inventory (THI), Beck Depression Inventory (BDI), State Trait Anxiety Inventory 1, 2 (STAI 1, 2) to evaluate their psychometric properties. The subjective patterns of tinnitus before and after treatment were assessed through questionnaires. RESULTS: Significant correlation between THI scores and BDI, STAI 1, 2 scores were observed in the moderate and severe tinnitus group. The patients were classified into two groups depending on their THI scores, and also classified into two groups according to BDI, STAI 1, 2 results. Analysis of differences after treatment indicated significantly improved VAS scores after treatment in all groups. CONCLUSIONS: The subjective level of tinnitus is significantly related with depression and anxiety symptoms when inconvenience caused by tinnitus was moderate or over. Assessments on tinnitus after treatment appeared to have improved significantly regardless of the severity of tinnitus, level of depression or anxiety disorder.
