In previous paper we reported on the radiation spectrum of commercially ava ilable out-door illumination lamp and suggested the use of this lamp as long ultraviolet light source until such time we have adquate medical equipments availahle. We tried to dectermine the erythema dose of above lamps in conjunction with topical psoralen and tar. The skin lamp distance was kept 50cm. The test sites were the backs of 30 young volunters. 200 watt bulbs were used. l. 30 minutes topical application of 0.25% Meladoxen solution required 2 to 6 minutes of exposure to inducs erythema in 48 hours. 2. 6 hours of topical tar application needed 2 to 8 minutes of exposure. 3. With 0.25% Meladoxen solution; 30 minutes of topical application produced more phototoxic effect tnan 2 or 24 hours. 4. With 5% Tar ointment; the longer the duration of topical application the stronger was the phototoxic effect. But topical application time exceeding 6 hours failed to increase phototoxic effect furtber. 5. With 0.3% Meladoxen cream the thicker the applied cream the stronger was the phototoxic effect up to 25mg per cm, over which phototoxic effect decreased, probably due to the screening effect of the cream, It is concluded that in the photochemical treatment of vitiligo and psoriasis psoralen sould be applied for 80 minutes, the tar for 6 hours or ovemight before irradiating the lesions with long wave ultraviolet rays. In this way the time required for each session of photochemical treatment of psoriasis or vitiligo will be greatly reduced while obtaining maximum effect. This is more significant in the view of long term treatmant required for the management of these embarrasing dermatoses.

Korean Journal of Dermatology 1994;32(5):838-847
BACKGROUND: At least 30 per cent of diabetics, which is said to comprise over 3% of general population in Korea, have one or more cutaneous complications during the course of the disease. These skin changes may be the consequences of microangiopathy, arteriosclerosis, direct metabolic disturbances and/or the sequelae of chronic therapy. OBJECTIVE: We tried to observe certain cutaneous and nail changes in diabetics and to relate them with systemic complications, blood sugar level and duration of diabetes. METHODS: During the period of 12 months from March, 1992 to February, 1993, a total of 1858 outpatients diagnosed as diabetes mellitus at Eulji General Hospital were examined. RESULTS: The cutaneous manifestations are observed in 79.5% of diabetics examined and cutaneous infections are the most prevalent(52.3%). Pruritus(p<0.01), anogenital pruritus(p<0.01), peripheral dedema(p<0.05) are more common in females and infections, shin spots, finger pebbles(p<0.01) are more common in males. Systemic complications are observed in 50.7% and the cutaneous manifestations are observed more frequently in patients with systemic complications(p<0.05). Shin spots, diabetic foot, peripneral edema diabetic bullosis are observed frequently associated with systemic microangiopathic complications such as retinopathy, nephropathy and neuropathy(p<0.01). There is a significant correlation between blood sugar level and fungal infections and anogenital pruritus(p<0.01). The frequencies of shin spots, diabetic foot and diabetic bullosis increase with the length of time diabetes has been present. Nail changes are observed in 39.6% of diabetics and onychomycosis is the most frequent(34.5%). CONCLUSION: It is suggested that cutaneous shgns especially fungal infections in diabetics are on the increase in Korea. A variety of skin conditions are encountered with diabetes mellitus and the dermatologist may be able to make a significant contribution in the control of diabetes mellitus by identifying skin signs and conditions as diabetic complications.

7.Economic Benefits of Implementing National Health Insurance by Measurement of Changes in the Consumer's Surplus.
Korean Journal of Preventive Medicine 1989;22(3):398-405
A change in the consumer's surplus was measured in order to evaluate the social benefit to be derived from expanding health insurance to the entire population. The most refined and correct way to measure a project's net benefit to society is to determine a change in the consumer's surplus. Benefits from introducing the health insurance program to the uninsured people can be classified into two elements. The first is the pricing-down effect(E1) which results from applying the insurance price system, which is lower than the actual price, to the uninsured patients. The second effect(E2) is a decrease in actual payment because an insured patient pays only a portion of the total medical bill(copayment). We collected medical price information from the data banks of 93 hospitals, and obtained information of medical utilization by referring to the results of other research and from data published by the Korean Medical Insurance Societies. The total net benefit was estimated as won214 billion, comprising the first effect(E1) of won57 billion and the second effect(E2) of won157 billion. The price elasticity of physician visits is less than that of hospital admissions; however, benefits from the increase in physician visits are greater than those from hospital admissions because there are considerably more of physician visits than hospital admissions. The sensitivity analysis also shows the conclusion that expansion of the health insurance program to the entire population would result in a positive net benefit. Therefore, we conclude that the National Health Insurance Program is socially desirable.

A 36 year old infertile was diagnosed endometrial adenocarcinoma by endometrial biopy due to vaginal bleeding. She was characterized by obesity, hirsutism and infertility. Diagnosis was confirmed endometrial adenocarcinoma(Geade I) associated with polycystic ovarian disease postoperatively. We experienced this case and so report this case with a brief review of literatures.

STUDY DESIGN: This study analyzes the influence of segmental pedicle screw fixation on distal fusion extent of King type I adolescent Idiopathic scoliosis(AIS). OBJECTIVES: To verify the efficacy of segmental pedicle screw fixation in preservation of distal lumbar motion segments. SUMMARY OF BACKGROUND DATA: King type I AIS has usually been treated by fusion to L4. However, segmental pedicle screw fixation may shorten the distal fusion extent by reducing the hopi-Bontal tiIt of the lower end vertebra. MATERIALS AND METHODS: Eighteen King type I AIS subjected to segmental pedicle screw fixation of both curves were analyzed after a minimum follow-up of 2 years. Fourteen were fused to L3(short fusion group, SF) and 4 were fused to L4(long fusion group, LF). They were compared for correction of deformity, spinal balance and L4 tiIt. RESULTS: In SF group, the thoracic currie showed 71.6% correction. Lumbar curve and L4 tiIt corrections were 76.8% and 75.2% respectiveIY. In LF group, the thoracic currie showed 65.2% correction. Lumbar currie and L4 tilt corrections were 76.8% and 78.4% respectively. There was no significant difference between two groups. Spinal balance was maintained in all patients(SF 14/14; LF 4/4). CONCLUSIONS: Segmental pedicle screw fixation may shorten the distal fusion extent of King type IAIS to L3, preserving a distal motion segment with a correction of deformity and spinal balance comparable to fusion down to L4. It is attributed to enhanced segmental control of the system enabling horizontalization of L4.
