3.A Case of Desmoid Tumor

A case report of desmoid tumor(musculo-aponeurotic fibromatosis) arising from sternum and upper part of anterior sheath of rectus abdominis muscle in a one and half years old male child is reported. The patient's parent noticed a protruding hard mass on the lower end of sternal portion two weeks previous to surgery. The mass had dense, grayish white and glistering character invading the upper part of rectus abdominis muscle and cartilagenous portion of sternum including xyphoid process. Wide excision of mass including myo-fascial tissue of the rectus abdominal apponeurosis was performed. The excised mass was 4×4×2cm in size and the microscopic findings were typical desmoid tumor. The operative wound had partial skin necrosis due to poor circulation of skin because of the mass extended up to subcutaneous tissue and biopsy scar but eventually healed. There is no any signs of recurrence of tumor on 7 months postoperative follow up but may need more observation to define it.

4.Two Cases of Gastritis Cystica Superficialis without Previous Gastric Surgery.

Gastritis cystica superficialis (GCS) is a rare lesion which is characterized by glandular hyperplasia with regeneration and degeneration in the mucosa and muscularis mucosa. Recently, GCS is revealed as precancerous lesion, but most report has been associated with those found at the site of a gastroenterostomy. So, we report two cases with GCS who had not previous gastric surgery. A 45-year-old woman visited for epigastric discomfort and another 4Q-year-old woman for epigastric pain. They had not undergone any gastric surgery. The gastroscopy discovered one polyp on anterior wall of greater curvature, upper body and another polyp in the center of the fold of greater curvature, lower body. We removed it by snare polypectomy and the histologic finding showed the character of GCS.

5.Three Cases of Primary Adenosquamous Carcinoma of Stomach.

Adenosquamous carcinoma of stomach is a mixed glandular-epidermoid tumor where both components are neoplastic. Its incidence is extremely rare. The five theories on the origin of squamous components are 1) island of ectopic squamous epithelium in the gastric mucosa, 2) squamous metaplasia of gastric epithelium, 3) squamous differentiation in a preexisting adenocarcinoma, 4) endothelial cell differentiated toward squamous elements, and 5) totipotential undifferentiated cells of the gastric mucosa. We experienced three cases of adenosquamous carcinoma. Case 1 was a 71-year-old female patient; an ulcerative lesion was present in the pylorus, measuring 5 cm in diameter. Case 2 was a 57-year-old male patient; an ulcerative lesion is present in the pylorus, measuring 6 cm in diameter. Case 3 was a 58-year-old female patient; an ulcerative lesion was present in the body and fundus, measuring 10 cm in diameter. Microscopic examination revealed a mixed malignant squamous and adenomatous component.

6.Differential Expression of Glucose Transporter Gene in Mouse Early Embryos.

The uptake of glucose for metabolism and growth is essential to most animal cells and is mediated by glucose-transporter (GLUT) proteins. The aim of this study was to determine which class of glucose transporter molecules was responsible for uptake of glucose in the mouse early embryo and at which stage the corresponding genes were expressed. In addition, co-culture system with vero cell was used to investigate the effect of the system on GLUT expression. Two-cell stage embryos were collected from the superovulated ICR female and divided into 3 groups. As a control, embryos were cultured in 0.4% BSA-T6 medium which includes glucose. For the experimental groups, embryos were cultured in either co-culture system with vero cells or glucose-free 76 medium supplemented with 0.4% BSA and pyruvate as an energy substrate. 2-cell to blastocyst stage embryos in those groups were respectively collected into microtubes (50 embryos/tube). Total RNA was extracted and RT-PCR was performed. The products were analysed after staining ethidium bromide by 2% agarose gel electrophoresis. Blastocysts were collected from each group at 120hr after hCG injection. They were fixed in 2.5% glutaraldehyde, stained with hoechst, and mounted for observation. In control, GLUT1 was expressed from 4-cell to blastocyst. GLUT2 and GLUT3 were expressed in morula and blastocyst. GLUT4 was expressed in all stages. When embryos were cultured in glucose-free medium, no significant difference was shown in the expression of GLUTI1, 2 and 3, compared to control. However GLUT4 was not expressed until morular stage. When embryos were co-cultured with vero cell, there was no significant difference in the expression of GLUT1, 2, 3 and 4 compared to control. To determine cell growth of embryos, the average cell number of blastocyst was counted. The cell number of co-culture (93.8+/-3.1, n=35) is significantly higher than that of control and glucose-free group (76.6 +/- 3.8, n=35 and 68.2+/-4.3, n=30). This study shows that the GLUT genes are expressed differently according to embryo stage. GLUTs were detectable throughout mouse preimplantation development in control and co-culture groups. However, GLUT4 was not detected from 2- to 8-cell stage but detected from morula stage in glucose-free medium, suggested that GLUT genes are expressed autocrinally in the embryo regardless of the presence of glucose as an energy substrate. In addition, co-culture system can increase the cell count of blastocyst but not improve the expression of GLUT. In conclusion, expression of GLUT is dependent on embryo stage in preimplantation embryo development.

7.163 Cases of Decompression Sickness Among Korean Divers.

This study was performed to obtain an useful and basic information of the decompression sickness (DCS) among Korean divers. The medical records of 163 cases of Korean civilian diver, who were diagnosed as DES and received recompression therapy according to U.S. Navy Standard Recompression Treatment Table at Ocean & Underwater Medical Research & Training Center of ROK Navy, for 10 years from Jan. 1983 to Dec.1992. The total of 163 cases of DCS were catagorized into 2 groups as Type I DCS and Type II DGS group. The major results obtained were as follows: 1. 70.5% of total cases were classified as type II DCS, and the ratio of type I to type II DCS was 1'2.5 and the ratio of male to female of DCS cases was about 4:1. 2. About one third of cases occurred in May and June, and 38.7% of cases occurred at Chung-mu and Chin-hae in Kyung-nam province, the Southern Coast of Korean Peninsula. 3. Among the type II DGS group, 85.2% occured in diving for harpshell, and 85.2% with hookah diving gear. 4. There were no cases following the decompression procedure such as well known U.S. Navy's, and 86.7% of cases did not follow the any kinds of decompression procedure for preventing DCS and most of dives were conducted repetitively with 4.2 times on an average. 5. The mean depth between type I(27.3m) and type 11 DCS group(33.3m) showed significant difference statistically (p<0.01) , but both frequences of repetitive dives and bottom times between two groups were not statistically significant (p>0.05) . 6. The rate of symptoms appeared on surfacing and within 10 min. after surfacing of type I and type II DCS were 40.0% and 70.5%, respectively. And 80.9% of type I and 92.2% of type II DCS group were aware of dysbaric symptoms within the first hour after surfacing. 7. The most frequent symptoms and its percentages were "Both limb pain (37.6%) ",Joint pain(27.1%)., and "Scapular pain(22.9%)" in type I DCS group, and "Limb weakness or paralysis (68.7%)" , "Respiratory chokes(27.8%)" and "Bladder dysfunction(22.2%)"intypellDCSgroup. . 8. Among the type II DCS group, 54.8% attempted inwater recompression as a primary management, but 54.2% of type I DCS group did not take any kinds of therapeutic measure after onset of dysharic symptoms till commencing recompression treatment. 9. Recompression treatment was delayed 126.9 hours in type I and 89.7 hours in type II DCS group on an average, and the cases of DCS, who delayed more than two days till visiting the recompression facility, were occupied 52.1% in type I and 29.6% in type II DCS group. 10. Among the total cases, 32.6% were experienced one more of dysbaric symptoms in the past.

8.A Case of Plasma Cell Balanitis Controlled by Fusidic Acid Cream.

In 1952, Zoon described eight cases of benign circumscribed chronic balanitis characterized by an extensive infiltration of plasma cells with no evidence of dysplasia of the overlying epidermis. Plasma cell balanitis can often be confused clinically with other conditions, such as erythroplasia of Queyrat, fixed drug eruptions, secondary syphilis, candidiasis and Reiters disease. We report a case of plasma cell balanitis in a 65-year-old man. He complained of a single, red, shiny and smooth patch involving the glans penis and adjacent prepuce. This patch was unresponsive to systemic and topical steroid treatment. Laboratory studies were negative or within the normal range. Histopathological findings showed a band-like mainly plasmacytic inflammatory infiltrate of the upper dermis. This patient was treated once daily with 2% fusidic acid cream topically for 5 weeks. The lesions resolved and no recurrence was observed during 2 years of follow-up.

9.Effect of Vertical Displacement of Bilateral Horizontal Muscles in AV Syndrome.

We reviewed 20 cases for the surgical effect of the vertical transposition of the horizontal rectus muscle for AV syndrome without significant oblique dysfuction. We performed vertical tranposition of the horizontal rectus muscle with standard recession and resection procedure and followed up at least six months. Sixteen cases were V syndrome and four cases were A syndrome. Medial rectus was moved toward the apex of the A or V, and the lateral rectus was moved to the opposite side of the apex of the A or V. The amount of transposition was adjusted according to the amount of vertical incomitance. If the vertical incomitance was 10delta, 15delta, 20deltaand 25delta, vertical transposition of rectus muscle was 1/4 TW[tendon width], 1/2 TW, 3/4 TW amd 1 TW, respectively. We achieved an average of 12.9deltareduction by 1/4 TW transposition, 14.4deltareduction by 1/2 TW transposition, 13.3delta reduction by 3/4 TW transposition, 19.0deltareduction by 1 TW transposition. We also achieved an average of 5.2delta/mm effect by 1/4 TW transposition, 2.9delta/mm effect by 1/2 TW transposition, 2.4 delta/mm effect by 3/4 TW transposition, 1.9delta/mm effect by 1 TW transposition. The vertical transposition of the horizontal rectus muscle for AV syndrome without significant oblique dysfuction showed good result and the amount of correction per mm of vertical transposition seemed to decrease as the amount of transposition increased, even though the total amount of correction showed a trend toward increasing [Fig.1, 2].
